Job summary
Working with a supportive community nursing team, you'll provide high-quality care to people in their own homes so they can stay as independent, safe and well as possible. Based on the role of Community Staff Nurse, you will assess needs, plan care and build trusted relationships with patients, families and other health and care professionals. You'll play an important part in delivering person-centred care, supporting people with a wide range of health needs. The role offers opportunities to develop your clinical skills, work autonomously and contribute to service improvement while being supported by experienced colleagues within the wider community nursing team.
Main duties of the job
What you'll do:
- Carry out holistic assessments and contribute to personalised care planning
- Deliver safe, evidence-based nursing care in patients' homes
- Work closely with multidisciplinary colleagues and partner organisations
- Maintain accurate patient records and support high standards of care
- Communicate sensitively with patients, families and carers, including in complex situations.
- A Registered Nurse qualification or nearing first-level registration
- Understanding of community nursing and person-centred care
- Experience working in a healthcare setting and as part of a team

Job summary
Post holders will be expected to undertake training to ensure a basic knowledge or level of competence in sexual health appropriate to the role and to be able to work across all sites of the integrated service according to service needs. Flexibility in working hours is expected including weekend & evening working.
This role is eligible for visa sponsorship under the UK Visas & Immigration (UKVI) Skilled Worker route. Sponsorship can only be provided where applicants meet the specific requirements set by UKVI, including the relevant skill and salary thresholds, and any role-specific criteria. For example, applicants for the Senior HCA role must be able to evidence a minimum of 2 years' experience in the same role to qualify for sponsorship.
If you are applying using a Graduate visa as your right to work, you must have a minimum of 18 months remaining on your visa at time of application.
Main duties of the job
Another exciting opportunity has arisen to join the busy and dynamic Nursing team as a Clinic Support Worker within the Sexual Health and Contraception Service in Brighton. The Integrated Sexual Health Service in Brighton operates across three sites, with outreach to other parts of the city. We are a multi-professional progressive team providing a comprehensive, evidence based and patient focused service for Contraception and Sexual Health.
Experience in venepuncture and/or diagnostic microscopy is desirable but not essential as all necessary training will be given to the successful candidate/s. However, excellent interpersonal and communication skills are imperative as the role requires a sensitive, confidential and non-judgemental manner at all times. Evidence of previous team working and flexibility is essential.

For this role it is essential that you have previous NHS or NHS primary care experience with a Care Certificate from an Ofqual regulated awarding organisation. Or that you are an internationally educated nurse/midwife/doctor who has worked in an acute NHS Trust within recent years with evidence of your degree verified by an ENIC report.

Job description
Job responsibilities
- The post holder will be responsible for delivering planned, high quality nursing care that supports the integrated service (the service) aim of providing effective care that encompasses and responds to the sexual health and contraception needs of patients attending the service.
- The post holder will (after in-house training and successful completion of competency based assessments) provide evidence-based, clinical patient care that supports holistic sexual and reproductive health care of patients attending the service. This will include venepuncture, assisting medical and nursing staff performing intimate genital examinations, specimen collection and delivering contraception care , acting as a chaperone, performing diagnostic microscopy, facilitating asymptomatic patient pathways, recording patient observations (blood pressure, weight, height) and sexual health discussion / health promotion (including C-Card and condom provision.
- The post holder will be responsible for the preparation of the clinical area and assisting colleagues in the genital biopsy, Horizon, IUD / IUS and implant clinic.
- The post holder is required to work in an autonomous role when performing venepuncture and diagnostic microscopy and will be required to make clinical diagnosis based upon their microscopy assessment
- The post holder will work across hub and spoke sites across Brighton and Hove, and is required to work some evenings and weekends.

Job responsibilities
- The post holder will be responsible for delivering planned, high quality nursing care that supports the integrated service (the service) aim of providing effective care that encompasses and responds to the sexual health and contraception needs of patients attending the service.
- The post holder will (after in-house training and successful completion of competency based assessments) provide evidence-based, clinical patient care that supports holistic sexual and reproductive health care of patients attending the service. This will include venepuncture, assisting medical and nursing staff performing intimate genital examinations, specimen collection and delivering contraception care , acting as a chaperone, performing diagnostic microscopy, facilitating asymptomatic patient pathways, recording patient observations (blood pressure, weight, height) and sexual health discussion / health promotion (including C-Card and condom provision.
- The post holder will be responsible for the preparation of the clinical area and assisting colleagues in the genital biopsy, Horizon, IUD / IUS and implant clinic.
- The post holder is required to work in an autonomous role when performing venepuncture and diagnostic microscopy and will be required to make clinical diagnosis based upon their microscopy assessment
- The post holder will work across hub and spoke sites across Brighton and Hove, and is required to work some evenings and weekends.

Main duties of the job

Provide professional advice and leadership
To actively work as a member of the multi-professional team to provide high quality therapy intervention to patients.
Manage a caseload of patients on a daily basis including patients with complex needs and multi pathology.
Clinicians working at this level will be expected to evaluate their outcomes, review non-responding cases either with their peers or clinical specialists and refer on as appropriate using documented clinical reasoning.

"Every patient admitted to a mental health hospital is screened for their smoking status and offered brief smoking cessation support by 31st March 2024."
Now, we're looking for a Clinical Lead to develop, implement and deliver a new Mental Health (MH) Tobacco Dependence Treatment Pathway. In this exciting and strategic role, you'll play a key role in supporting the Trust's ambition to be a lead for reducing health inequalities.
Initially, this will support adults with severe mental illnesses who have tobacco dependency based within MH inpatient wards and from April 2024 the Community Mental Health Teams too.
You will lead a new team, with Tobacco Dependency Advisors, alongside people with lived experience, Stop Smoking Champions and support from Public Health smoking cessation support offers. Quality Improvement approaches will support implementation and ensure robust metrics are in place to measure impact.

An exciting opportunity has arisen for a highly organised and detail-oriented individual to join our Stroke Services Team as a SSNAP Stroke Data Administrator at Musgrove Park Hospital working 30 hours per week Monday - Friday.
The successful candidate will play a vital role in supporting the collection, validation and submission of stroke patient data to the Sentinel Stroke National Audit Programme (SSNAP). A key responsibility is supporting the development and continuous improvement of practice learning environments. Assistant Psychologist - South Warwickshire Community Mental Health Team

An exciting opportunity has arisen for an Assistant Psychologist to join our South Warwickshire Community Mental Health Team. The service provides biopsychosocial interventions for individuals with a wide range of mental health presentations. The post will primarily have a focus on working with people with a diagnosis of psychosis as part of the psychosis pathway within the CMHT. However there may also be opportunities to work with people with other mental health presentations.
Main duties of the job
The post holder will join a multi-disciplinary team and work across both of the teams bases located in Leamington Spa and Stratford Upon Avon.
The post holder will support the qualified psychologists with psychological assessments and provide low intensity psychological interventions under the supervision of a qualified psychologist.
Psychological Professionals staff from the South Warwickshire Service have close links with other psychological professionals in the Trust through peer supervision, service development meetings, specialist interest groups and CPD events.
The service regularly offers core and specialist trainee placements as it has strong links with the Coventry and Warwickshire Doctorate in Clinical Psychology Course.
