Job summary
**The hours for this role are 22.5 per week and the working pattern - 7.5hrs Wed / Thu / Fri**
This is a varied and fast paced role providing all aspects of administrative support to the NHS Individualised Commissioning Team which includes the nursing team, senior managers.

Main duties of the job

- Responsible for maintaining accurate and up to date patient records on the digital referral and case management system

- Accurately code invoices for authorisation. Liaise with service providers and other stakeholders as appropriate to resolve queries without delay
- Responsible for creating or amending meeting room diary bookings for the team.

We are seeking a motivated and organised individual to join our Outpatient Booking Team on a 12-month fixed-term basis during a period of service growth. This post is a Band 7 Enhanced Nurse Practitioner within the Rapid Response Nursing Team in Gateshead.
To work effectively within a multi-disciplinary team to manage patients identified as requiring an urgent community response to prevent a deterioration in their health and wellbeing. In addition, there will be planned activity to prevent avoidable admission to hospital and facilitate early supported discharge, where clinically appropriate.
This post is a senior autonomous clinician who will ensure the fundamentals of care are provided at the point of contact and will ensure delivery of a range of appropriate interventions to be able to meet the needs of the patients, within the community setting. Patients accessing the service will have complex needs and potentially numerous co-morbidities including frailty.
The autonomous practitioner will provide comprehensive healthcare sensitive to the needs of the population, responsible for senior clinical decisions, including advanced clinical assessments, history taking, diagnosing, planning treatment, prescribing and evaluation of care.
Working as part of a multidisciplinary team and across the primary, secondary and social care interfaces in Gateshead and will be responsible for coordinating and meeting the health care needs of people at risk of hospitalisation or requiring a more intensive level of care in the unscheduled, intermediate, palliative and EOL care environment.

Job summary
This role is an exciting opportunity to be an key part of a dynamic team delivering Great Ormond Street's fantastic new building - the Children's Cancer Centre.
The role will be responsible for end-to-end management of the equipping process for all the GOSH-supplied equipment for the new facility - from Imaging scanners and major medical equipment to bedside and waiting room furniture.
The successful candidate will work closely with the GOSH Procurement department, clinical and operational teams and construction partners to ensure the right equipment is specified, procured, and installed in time for the moves in the new building.
Main duties of the job
- Lead procurement of capital equipment for the CCC, including development of specifications and tender documentation, and agreeing evaluation methodologies and contract award processes
- Design, implement and own a controlled information database which meets the requirements of the Procurement and Project teams to manage the procurement of ~10,000 trust-supply items of equipment.
- Develop bills of quantities, procurement packages and room equipment schedules
- Develop and manage detailed equipping plans ensuring that they are aligned with construction milestones and commissioning programmes and are integrated into the wider Children's Cancer Centre programme
- Establish and lead equipment workstreams and delivery groups, ensuring clear governance, reporting and accountability
- Monitor progress against milestones, budgets and quality standards, taking corrective action where required
- Build and maintain strong working relationships with clinical, operational and technical stakeholders across the Trust

We are looking for a Recovery Coordinator for North Wiltshire Intensive Team based at Green Lane Hospital, Devizes. The position is offering full time hours although alternatives would be considered.

We are looking to recruit a suitably experienced/qualified recovery coordinator, to join the North Wiltshire Intensive Team.

The Intensive Team provides assessment and home treatment to individuals and their carers as an alternative to hospital admission, relapse prevention, short term therapy work and help provide facilitated discharge from hospital.
The team operates 0830 - 2200, 7 days per week, 365 days per year, as such the successful candidate will be expected to be able to undertake a shift pattern of working.

In collaboration with the ward MDT and patient's families, carers and support networks, you will take a pivotal role in discharge planning for patients, ensuring a smooth, timely transition back into the community. The role includes some OT assessments in home environments to identify OT interventions, equipment and support needs as part of discharge planning.
Group work is an important aspect of the Therapeutic Programme at Langley Green Hospital. The role includes psychoeducational and therapeutic group facilitation and involvement in the development and coordination of the programme.
You will provide clinical supervision and leadership support to others in the OT team, including band 5 OTs, OT clinical support workers, activity worker support staff, lived experience recovery workers, and OT students, fostering a culture of continuous learning and growth.

Main duties of the job

You will have your own monthly supervision with opportunities to be involved in Quality Improvement and Clinical Audit. Infection Prevention Practitioners are key to the delivery of the infection prevention delivery plan, operational policy and annual programme of work, as well as the formulation, monitoring and implementation of associated policies, protocols and guidelines. They are also responsible for day-to-day surveillance, outbreak prevention and control and advice giving to a range of clinical and non-clinical services in the Trust.
The Infection Prevention team provides a 7-day service.
Main duties of the job
The post-holder will be expected to participate in the delivery of a trust-wide delivery plan for the prevention and control of infection.
The post-holder will be expected to contribute to the formation, review and evaluation of the annual programme of work for infection prevention and control and the associated annual report.
Take responsibility for communicating developments, best practice and infection prevention priorities to a designated group of clinical and non-clinical areas and the whole multidisciplinary team.
Be expected to formulate, monitor, deliver and evaluate projects aimed at the achievement of local, regional and national priorities relating to infection prevention.
Be expected to scrutinise local and national surveillance data, guidance and expert opinion to identify areas for local improvement and to facilitate the implementation of related improvements to practice/the environment.
Be expected to maintain an advanced knowledge of infection prevention and control and, where appropriate, implement evidence-based practice (including in the development of Trust policies) and outbreak control measures.
Work flexibly to provide a 7-day service including bank holidays as agreed with the Matron Infection Prevention.

Main duties of the job

Job summary
Are you an experienced registered Mental Health Nurse looking to secure the next step in your career or to take on a new challenge within Older People's Mental Health Services? If so, we are currently looking for an enthusiastic Senior Practitioner to join our fantastic team.
You will have specialist knowledge of the care of older people with mental health conditions and an understanding of the challenges that older adults face and how these impacts on their well-being. You will hold a small caseload of complex cases and lead by example demonstrating exemplary clinical and management standards.
As a member of the wider OPMH Service, you will be the main point of contact with local multidisciplinary and multi-agency Locality Access Point (LAP)/Cluster meetings and support formulation within the meetings. The purpose of these meetings is to deliver an integrated approach to assessment, care planning and decision making for frail patients with long term conditions, in line with the Integrated Care Decision Making (ICDM) initiative of the Frimley Integrated Care System. You will help to identify the appropriate level of psychosocial support for individual patients. To achieve this, you will need to be aware and understand the full range of support available locally, from third sector through to secondary MH provision. You will also be required to develop close relationships with other Integration Leads across the Frimley system and promote integrated working.
Main duties of the job
- To undertake comprehensive initial assessments in a range of settings following acceptance of referral, considering the persons physical, psychological, social and spiritual needs.
- To develop and implement an agreed plan of care with the service user and where appropriate, include the views of families and carers.
- To identify and select from a range of health, social and voluntary agencies which could assist and improve the care of individuals and their families.
- To represent BHFT mental health services at locality LAP/Cluster meetings.
- To provide clinical leadership and offer mental health expertise & guidance to professional colleagues and assist in formulation to support integrated decision making and care provision.
- To utilise advanced clinical skills and expert knowledge to deliver evidenced based person-centred interventions to a small caseload of people with complex mental health needs and offer practical advice and emotional support to their family carers.
- To contribute to and influence the development and improvement of anticipatory care planning.
- To have knowledge of a variety of therapeutic interventions including short term problem solving, crisis intervention and basic CBT techniques.
- To manage complex caseloads and maintain responsibility for clinical decision-making within professional accountability frameworks for patients requiring secondary care mental health support
