Job summary
We are recruiting passionate and proactive Neighbourhood Case Managers to join the Hertsmere Neighbourhood Proactive Care Service. This is an exciting part time opportunity (26.25 hrs per week) to play a key role in a new team delivering community-based, person-centred care for residents. The focus will be on holistic assessments, care coordination, addressing health inequalities and delivering on the strategic ambition of a 'neighbourhood health service'.
You'll work as part of a multidisciplinary team supporting a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year, linking them to health, social care, and voluntary sector services.
This service will see the shift from reactive care to proactive identification by using risk stratification searches within Primary Care records. The searches will identify a cohort of patients who would benefit from targeted intervention to improve outcomes, their overall experience and reduce the likelihood of further unplanned admissions. The service will operate alongside a number of acute and community services, with the ability to step patients up into appropriate services - including the highly successful virtual ward models in South and West Hertfordshire.
As this is a community-based role, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
The post holder will be responsible for:
Overseeing care provided in the Proactive Anticipatory Care Service. This will include line management of the Neighbourhood case coordinator, ensuring proportionate and holistic patient assessment is completed, developing integrated care plans which anticipate potential future needs, using an effective multidisciplinary platform for discussion of complex cases, and maximising the value added by the role of senior clinicians.
This will include:o Provision of a proactive, personalised and multidisciplinary case management approach to "high risk" patients identified through a risk stratification search in the Neighbourhood. This includes those with end stage disease who are approaching end of life, have moderate to severe frailty or dementia and have had 2 or more unplanned hospital admissions.o Facilitation of timely, effective, and purposeful multidisciplinary discussion and decision making, resulting in coordinated and integrated care plans which can effectively anticipate and respond to changing patient needs.o Undertaking comprehensive assessment of complex care needs using tools and templates available within the electronic clinical record.

Main duties of the job
This Neighbourhood Case Coordinator role is a hybrid clinical/administrative role which would suit a person with experience working as a health care or therapy assistant in community settings who also has good administration skills to support with producing clinic letters, MDT meeting minutes, appointment bookings, etc.
Clinically you will be visiting patients at home or seeing them in clinic based settings to carry out assessment or monitoring of their long term conditions, providing advice and support to enable them to live well and avoid unnecessary hospital admissions, and liaising with health and social care professionals to ensure that care is well coordinated.
Within this role, the post holder will be responsible for:
o Working under delegated authority from a registered nurse or allied health professional (Neighbourhood Case Manager) to support patients on the Proactive Anticipatory Care Service, with the aim of reducing unnecessary admission to hospital by the completion of preventative comprehensive care plans and care coordination across a range of services.o This support may take the form of virtual consultation and review, face to face assessment or care delivery, professional liaison, and arranging and attending meetings with colleagues from across health and social care.o Work may be delivered in patient's own homes, in clinic, or other community settings.

The Logistics & Post Department is looking to recruit an enthusiastic individual to fill the position of Logistics & Post Assistant in our busy department.
Logistics duties will include receiving, unpacking and checking deliveries from suppliers. To order (to agreed stock levels), unpack and put away stock on wards/departments. These services are provided over the Wonford, Heavitree and Nightingale Exeter sites.
Duties also include picking and packing items in our offsite warehouse.
Post room duties include delivering and collecting post throughout the main Wonford Hospital site and franking outgoing post.
Experience of working in an NHS Logistics/supplies/Post environment will be an advantage, but not essential as full training will be provided.

An exciting opportunity has arisen to join the Chief Nurse team as the Lead Nurse Mental Health for UCLH. The Acute Paediatric Liaison Health Visitor is a Registered Nurse with Specialist Community Public Health Nursing (Health Visitor) qualification who works within the acute hospital setting at Croydon University Hospital.
The post holder provides a safeguarding and public health interface between Emergency Department, paediatric and maternity services, and community 0-19 Public Health Nursing services. Job summary
The Community Radiology Service in East Sussex is seeking to appoint a highly motivated radiographic assistant to join the team at Lewes Victoria Hospital. The role will be a clinical and administrative one (40/60 split).
The main tasks will include ensuring that referrals are administrated according to local processes and appointments for imaging are made within national waiting times. To ensure the post holder gets the most out of the job, communication and organisational skills are key and will be evidenced at interview. Previous experience working in an NHS setting will be essential. The clinical aspects of the role will include chaperoning for ultrasound scans and assisting in the x-ray room when required and according to patients needs.

We are looking to recruit 3 housekeepers to work the following hours:
- 1 housekeeper working 3 days out of 7 on a variety of morning and afternoon shift working 20 hours a week
- 1 housekeeper working 3 days out of the 7 on a variety of morning and afternoon shift working 18.75 hours a week
- 1 housekeeper working 2 days out of the 7 working afternoon only shifts on a 12 hour a week contract
The housekeeper will enhance the patient's experience by ensuring that the clinical environment is presented and maintained in a way which maximises the patient's sense of comfort, safety and well-being.
The successful candidates will have the ability to work on their own initiative and as part of a team, with the ability to communicate effectively to patients, staff and visitors.
Main duties of the job
To ensure the environment is cleaned in line with national cleaning standards.

Job summary
We are seeking an experienced Physiotherapist with a passion for providing high quality, patient-centred care for the medical and frail patient cohort in the acute hospital setting, to join our Medical Rehabilitation Therapy team.
They will primarily provide clinical and leadership support to the Medical Rehabilitation Therapy team on the discharge ready unit (DRU) and play a vital role in supporting patient flow and timely discharges. They will also support across the team's medical, care of elderly & frailty wards as required. The post holder will be expected to help other therapy teams as, and when required.
The ideal candidate will have a broad range of clinical experience in core areas including acute inpatient wards, critical care or respiratory, neurology and orthopaedics. Additionally they should have some experience of leadership- either through previous experience of supervising junior staff or students and being involved in service development projects.
As part of the role, the post holder will be expected to undertake respiratory on-call shifts and work as part of the weekend rota (which may include cover between the respiratory, orthopaedic and ED and Acute Medicine weekend rotas. This also includes Bank Holiday cover).
Main duties of the job
The post holder will manage a clinical caseload on our medical and frailty wards and discharge ready unit wards whilst also providing direct line management to junior Physiotherapists and Therapy Assistants. They will also be expected to take on students and share their expertise with the wider multi-disciplinary team as part of the in-service training rota. They will be expected to support rehabilitation of patients, prevent deconditioning of patients, as well as be involved in discharge planning of patients.
Additionally, the post holder will work alongside their Occupational Therapist colleagues to support the service and staff development across the acute medical therapy service.
As part of the role, the post holder will be expected to undertake respiratory on-call shifts and work in the respiratory and orthopaedic weekend rota (which includes Bank Holiday cover).

Elton Rehabilitation Unit is a 8 bedded facility that is located in Clevedon, North Somerset. The unit is one of a number of community units providing support and active rehabilitation for those people being supported on Discharge to Assess pathway 2 (please see below).
Following a stay in the acute setting, the ethos for all people being discharged is 'home is best' and a large number of people are supported to return home utilising the robust and established ~Discharge to Assess Pathway 1. For those people that are identified as having on-going rehabilitation needs, they are considered to 'stepdown' into a Pathway 2 Rehabilitation, Reablement and Recovery bed at the Elton Rehabilitation Unit.
On the Unit, they will receive therapy intervention and support around complex discharge planning, to enable a timely and safe discharge to their usual residence or to a more suitable location if appropriate.We actively promote a multi-disciplinary model on the Unit, where nursing staff work closely with therapist, social workers, GP's with Special Interest in Older People's Care as well as other Acute and Community colleagues, to ensure the person we are supporting is kept at the heart of every decision

The Health Care Assistant role compliments and forms a key part of the multi-disciplinary healthcare services within the organisation, implementing and providing high standards of individualised care for the service user. You will be part of an integrated multi-disciplinary team (MDT), which includes nurses, occupational therapists, physiotherapists and other allied healthcare professionals.o You will provide holistic, person-centred care and support for service users of all ages in a variety of settings. The opportunity has arisen for a motivated Speech and Language Therapist with an interest in working with people with a learning disability to take a Band 6 specialist role in the Sirona Adult Learning Disability Health Service (SALDHS).
This is a full-time position based in North Somerset, with opportunities to also work with colleagues covering South Gloucestershire and Bristol. The service hours are Monday to Friday, 9am to 5pm.
You will work within a locality multi-disciplinary team, with the opportunity to work with specialist SLTs across Bristol, North Somerset and South Gloucestershire delivering person centred care, and training to care and support staff.

Main duties of the job
You will hold a clinical caseload of people with learning disabilities who have communication and/or eating and drinking needs. Our service users have a broad range of communication needs including supporting the implementation of AAC, the use of signing, Talking mats, Social Stories and Visual support strategies. Management of complex dysphagia, including triage, prioritisation, assessment, risk assessment and the development of individualised care plans as well as care and support staff education to support understanding and implementation of guidance given.
Your work will be in the community seeing people in their homes or other familiar and appropriate environments. You will also be working collaboratively within our multi-disciplinary team which provides a holistic approach to meet the service users health needs. You will receive support from clinical specialist SLTs and the SLT Clinical lead to develop in this role We actively encourage skill development and case discussion to ensure the team deliver a high-quality service that is innovative, safe and continuously improving for a positive and meaningful service user experience.
Alongside your clinical caseload there will be opportunities to provide advice, education and training to health staff, social care workers and family carers. and information in a range of formats to meet service user needs.

Come and join our team! We are seeking to employ enthusiastic and motivated Band 3 clinical support workers for Ward L19, located on F floor Jubilee Wing at the Leeds General Infirmary and Ward J11, Located on Level 7 Gledhow wing at St James University Hospital.
Main duties of the job
The successful candidate will have a proven track record of providing safe and compassionate care of their patients. This includes providing high standards of basic care and supporting and guiding new unregistered staff members. Excellent inter-personal skills are essential. A positive and flexible style and approach is necessary and you must be a team player, who supports your colleagues and works well within the team and also independently. The successful candidate will be required to work as part of a friendly and supportive team who are creative and enthusiastic, with a track record of continually improving performance. We need someone with a passion for interventional, acute and chronic nursing to support the delivery of a high quality service that enhances patient safety and the patient experience. It is an extremely fast paced service and we care for both chronic and acute patients. J11 strive to be dedicated to patient care and aim to maintain patient advocacy and voice in what they would like and how they would like to be discharged. The team work in conjuction with the respiratory discharge team and together we aim to build a bridge between the hospital and community services with patient voice at the forefront. If providing outstanding patient care, working alongside a busy and dedicated team of which is continuously growing and developing then this is the job for you.
