Job summary
To support the nursing team within the Children and Young People's Continence Service to deliver treatment and intervention for children with bladder and bowel difficulties. To work within a clinic environment, home setting and special school environment supporting the continence pathway set out by the nursing team. You will carry out a range of clinical and non-clinical healthcare, or therapeutic tasks, under the direct or indirect supervision of the registered healthcare practitioner, in accordance with the Trust values. There will be opportunities to develop your knowledge and skills, specific to your needs. Training will be provided to ensure you have the required core skills and skills set specific to your area of work. You will also be supported to obtain your care certificate if you do not have this qualification.
HCT will prioritise recruitment of workers living in England under the new immigration rules for care workers effective from 9 April 2025. Employers must first prove that they have attempted to recruit a worker from within England who needs new sponsorship. This ensures that those who came to the UK to pursue a career in adult social care can do so.

Main duties of the job
Provide continence care and advice to CYP in homes, clinics, and community settings, following the care plan under the direction of the Senior Community Nurse.
Support and encourage CYP and their families with continence management and toilet training as outlined.
Assistwithin the Continence Clinic, including setting up andclosing down, preparing necessary equipment, and ensuring the comfort of CYP during appointments.
Carry out and reassess continence pad needs as delegated by the Senior Community Nurse, andparticipate in training school staff and professionals in the use of continence products.
Undertake clerical and administrative duties andmaintainingaccuraterecords in line with data protection policies.
Monitor andmaintainequipment lists, stock levels, and ensuretimelyservicing for the Children and Young People's Continence Team.
Work collaboratively with CYP, their families, and the multi-disciplinary team, alwaysdemonstratingsensitivity, professionalism, andmaintainingconfidentiality.
Promote a safe and comfortable care environment, responding to changes in the CYP's health and reportingconcernsas necessary.

An opportunity has arisen for a qualified Counsellor to work in our Occupational Health and Wellbeing Service based at Rotherham NHS Hospital. Main duties of the job
The postholder will be expected to:
Deliver short term counselling interventions to NHS staff for a range of presenting issues.
Contribute to the development and delivery of psychologically informed groups to promote staff wellbeing.
Provide initial assessment of clients presenting with multiple and complex issues; select appropriate therapeutic interventions, make decisions regarding referrals to specialist services and facilitate these as needed.

Job summary
This is an exciting opportunity to join a relatively new team, which aims to help young people at risk of developing psychosis who are experiencing an at-risk mental state. The Therapeutic Interventions for Distressing unusual Experiences Service (TIDES) is a team working therapeutically with young people across Norfolk & Waveney.
**If successful at interview you will be allocated to a locality. This will be Central or West Norfolk. There is an expectation that case managers split their time between the central bases.
We are accepting those with core professions from: mental health nurses, occupational therapists, and social workers. (You will require an up-to date registration).
We are a supported team currently made up of clinical psychology, systemic family therapy, cognitive behavioural therapists, mental health practitioners, support workers and a vocational worker, all of whom are from a variety of professional backgrounds. Our team will be offering young people evidence-based interventions such as psychoeducational work, individual CBT and family therapeutic work aimed at helping our young people and their families to manage their unusual experiences. TIDES practitioners work with small manageable caseloads and hold an integrated approach to the care we provide.
Phone calls, teams calls or visits to meet the team and manager are welcomed and encouraged.
Main duties of the job
You will be responsible for the care coordination of a caseload of people, working alongside the current MDT to deliver NICE concordant care.
When you join us, you can expect a thorough induction and ongoing support.
You'll be joining a team with extensive experience of working in a psychosis pathway, and we are passionate about helping our growing team to develop their skills and understanding in this area through ongoing CPD . You will be joining a supportive environment, with regular clinical team meetings, reflective practice and opportunities to work with the wider MDT, as well as working independently.
The ideal candidate will be a team player, have a good interpersonal skill, is flexible in their approach, and is enthusiastic about working with young people and young adults, to help live the lives they want to.

You will join a small, close team of dietitians providing specialist services for community learning disabilities and mental health in-patients within Bradford District Care NHS Foundation Trust (BDCFT). Experience of working within learning disabilities and/or mental health, whilst beneficial, is not an essential requirement as you will be part of a large dietetic team covering multiple specialities and your learning and development needs will be supported.
Main duties of the job
You will work across both specialities, managing an inpatient and outpatient caseload with a wide range of clinical areas including nutrition support, home enteral feeding, weight management and gastrointestinal problems. Our learning disabilities work is delivered as part of a specialist multidisciplinary team of nurses, speech and language therapists, occupational therapists, physiotherapists and psychologists.
The role involves developing and delivering training and education for staff and service users, collaborative working, project and audit work, policy development, coordinating care across services, supporting our catering team and leading on nutrition for the service.
The post holder will be expected to participate fully in the training of 'A', 'B' and 'C' Placement student dietitians.
The team is fully integrated within the BDCFT multi-disciplinary team as well as Bradford Teaching Hospitals NHS Foundation Trust's (BTHFT) Specialist Outreach dietetic team. Main duties of the job
As a Senior Psychological Wellbeing Practitioner (SPWP) you will support the Service Lead, Professional Lead & Team Managers to develop the Step 2 service provision across the organisation.
This role provides an opportunity for experienced PWPs who are committed to developing their careers to share their interest and passion and to develop these qualities and skills in others. In addition to maintaining a PWP (Step 2) caseload, the post holders will work within the Talking Therapies service providing a range of more senior duties including:

We are looking for a qualified Clinical Associate Psychologist to join the East CAMHS Team

Main duties of the job
As a CAMHS Nurse /Practitioner you will be responsible for providing full assessments, formulation and evidence based interventions to children, young people and their families, who are experiencing mentalhealth difficulties.
You will be responsible for maintaining an individual caseload, from referral to discharge, conducting routine and high priority assessments and interventions as appropriate and exercising autonomy.
You will be liaising and providing advice, information and consultation to colleagues and other agencies as well as undertaking service governance activity, including research and audit as appropriate.
Successful candidates will provide regular and ongoing consultation, joint work, training and advice to other professionals in relation to child and adolescent mental health to ensure the best outcomes for children and young people.

Hospice at Home Team at Trinity Hospice requires a Healthcare Assistant to provide flexible provision of the service 7 days per week 9pm 8am
At Trinity Hospice, we support people to live as well as possible during the last year of life, surrounded by the people, comforts and familiarity of home. We are looking for a caring, compassionate and dedicated Healthcare Assistant to join our Hospice at Home team, helping deliver exceptional end of life care to patients and their families across the community.
The role is working 35 hours per week.
As a Healthcare Assistant within our Hospice at Home Service, you will work alongside Registered Nurses and other healthcare professionals to provide practical, emotional and psychological support to patients whose preferred place of care is home. You will play an important role in helping patients remain comfortable, safe and supported while ensuring families and carers receive guidance, reassurance and compassionate care throughout their journey.
No two days are the same. You will provide hands-on personal care, support symptom management under the direction of registered professionals, contribute to care planning and work closely with GPs, District Nurses and wider community teams to ensure patients receive coordinated, person-centred care.
