Job summary
We are looking for an enthusiastic and highly motivated and experienced physiotherapist join our inpatient therapy bank.
We are looking for candidates with a minimum of rotational band 5 experience in a hospital-based environment, with strong communication and teamwork skills. You will ideally be respiratory competent and have experience in managing acute medical, orthopaedic and /or neurological conditions. You also need to be self-motivated, adaptable and dynamic, and able to work autonomously. There may be opportunity for weekend working.
We are committed to your training and development of acute specialist skills and clinical expertise and will provide opportunities to participate in training, service development, and clinical supervision.

Main duties of the job
-
- To be professionally and legally accountable for all aspects of your own clinical practice,
- To undertake a comprehensive assessment of patients including those with diverse or complex presentations/multi pathologies; use advanced clinical reasoning skills and physical assessment techniques to provide an accurate diagnosis.
- To formulate and deliver an individual physiotherapy treatment programme based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills and knowledge of treatment skills.
- To formulate soundly reasoned physiotherapy prognoses and recommend best course of intervention, developing negotiated goals leading to comprehensive discharge plans. Where appropriate develop these goals with the multidisciplinary team.

The clinical caseload will include providing a high quality, evidence-based dietetic service to a range of adult wards including general medicine and surgery where you will be a valued member of the multidisciplinary teams. In addition, you will manage an outpatient caseload, which includes weekly diabetic and general clinics. You will have the opportunity to develop your skills in areas such as ICU, gastroenterology, surgery and neurorehab. The post holder will also be involved in staff and patient education programmes. The post holders will be involved in clinical audits, department wide projects and service improvements.
The department is committed to continuing professional development, with one of the Trust's key priorities being to develop a quality workforce through an effective appraisal system. There is a well-established borough-wide Dietetic network which offers peer support and there are good opportunities and guidance for ongoing personal development. The post holder will be supported by a Senior Dietitian involving regular clinical supervision.

The senior nurse plays a key role in supporting the transition of care within 24/7 PCH care and/or partner organisations, ensuring unplanned needs are managed out of hours and an appropriate follow up plan is in place. In addition to clinical work, the senior nurse will have supervisory and line management responsibility, supporting the development of a highly experienced and well-embedded out of hours team. The senior nurse is accountable for the effective delivery of high standards of evidence based nursing care, including through the use of audits and supervision. The service uses SystmOne as a clinical IT system to support mobile working.
Applicants will be required to participate in working weekend and bank holidays on a rotational basis.
A mobile phone, laptop and pool car are provided for use when on duty.

Job summary
An exciting opportunity has arisen for a for a highly motivated Band 6 Specialist Occupational Therapist to join our inpatient acute service in Warrington.
Sheridan Ward is a 16 bedded female inpatient acute ward, at Hollins Park Hospital, Warrington.
The post holder will be required to undertake holistic multi-factorial evidence-based Falls assessments, in a clinic setting. They will provide the MDT and their patients with Specialist OT knowledge to be be able to identify and reduce falls risks on the ward through the use of equipment, adaptations and education ensuring that the needs and safety of their patients are met.
Main duties of the job
The post holder will be expected to effectively manage a caseload of patient's comorbidities as an autonomous practitioner, using evidence based/ patient centred principles to assess, plan, implement and evaluate interventions of the patient group. The post holder will provide leadership for junior staff when appropriate, through supervision and appraisal.
The post holder take a holistic approach in the assessment and treatment of service users / patients, being responsible for the provision of appropriate, high quality care to a defined caseload which may be of a complex nature, using the appropriate model of care and evidenced based practice.
Work in partnership with other professionals to enable patients / clients to be maintained in an appropriate environment, thus avoiding hospital admission.

Job summary
- Work as part of the multidisciplinary team delivering care to patients/service users experiencing mental health needs within inpatient services with support and supervision provided by a registered health care professional.

Main duties of the job
Undertake care as directed by the registered healthcare professional, following agreed care plans, providing information to the registered healthcare professional that will enable the monitoring and amendment of care plans.

Demonstrate care and compassion to patients, carers and relatives to help ensure high-quality person-centered patient care is delivered at all times.
Provide personal care to patients/clients, including using the shower or bath, washing facilities, dressing, supporting oral hygiene needs, enabling patients to meet their hygiene needs, accessing, and using toilets, collecting, and disposing of bodily waste, ensuring that privacy, dignity, and security is maintained. This may involve using hoists, wheelchairs or other mobility aids.
Support people to eat and drink through choosing appropriate food and drinks and enabling them to prepare for eating and drinking, assisting them where appropriate.

Job summary
We are looking for a dynamic and enthusiastic team player to provide secretarial support to the Gastroenterology department as a Medical Secretary.
This is a key role in a busy department that works closely with the Medical Teams, Nursing Team, Outpatients Department, the endoscopy unit and the wider hospital teams.
You will be part of a friendly patient focussed team, delivering high quality care to patients referred to the Gastroenterology team in Ipswich.
Main duties of the job
You will be responsible for ensuring that the administration needs of the department are completed in a timely way.
You will ensure that timeframes are met for the management of patient pathways in accordance with Referral to Treatment targets.
You will support team members by having a flexible approach to both workload, additional tasks and working patterns.

An exciting opportunity has arisen for a patient focused Call Handler to work within the Newcastle & Gateshead Children and Young Peoples Service (CYPS) based at Bensham Hospital, Gateshead. Post is full time 37.5hrs per week Monday to Friday covering shift patterns of 8am to 4pm and 9am to 5pm on a rota basis
The Call Handler role is extremely busy and involves assisting clients who may be experiencing a range of mental health conditions. You will act as a liaison between the patient and the clinical team to support all patients in receiving a positive, high quality and seamless experience throughout. Postholder provides a telephone call reception service for all enquiries and referrals into the service and supports the team by engaging with service users, carers, and professionals to ensure that all calls are dealt with efficiently and signposted appropriately.
Postholder will be required to utilise a high level of sensitivity and professionalism whilst providing call handling duties. The call handler will ensure all calls are responded to in an appropriate time frame and that all clinical information is effectively communicated and documented within the electronic patient records system as per agreed CNTW Trust standards whilst always maintaining confidentiality. This is a unique opportunity for an experienced nurse to work autonomously while delivering high-quality clinical care outside of a hospital setting.

Key Details:
Location: Working with trusts and patients in Plymouth and Surrounding areas.
Working hours: Monday to Sunday, 40 hours over 5 days per week on a rolling rota.
The shifts are structured as split shifts to accommodate patient needs. This means you will typically work around 5 hours in the morning to disconnect patients, and another 5 hours in the evening to reconnect them, with a break in the middle of the day. Flexibility is essential; some days may be longer or shorter depending on patient visits, but overall, average out to 40 hours per week.

- To administer prescribed IV therapies and associated care and support to patients who are unable to be trained to independence.
- To provide training and education to support patients/carers in the connection and disconnection of Parenteral Nutrition and any other prescribed therapies to independence.

- To support the provision of a phlebotomy/cannulation service for patients.

Job summary
The post holder will be responsible for the assessment of the individuals in the community/ home/ nursing homes and hospitals to determine eligibility to receive NHS Continuing Healthcare or Funding Nursing Care. The assessor will act as a member of the multidisciplinary team to co-ordinate the process, establish eligibility and assist in identifying a suitable care package. The assessor will be responsible to ensure the review process for all NHS Funded Care is undertaken in accordance with current guidance.
Main duties of the job
The practitioner will utilise the national assessment tools and adhere to the required timescales, co-ordinate, participate and support the assessment process with the multidisciplinary team. The role will be part of the MDT and determine the individuals eligibility in line with the NHS Framework for NHS Continuing Healthcare and Funded Nursing Care. This will encompass explaining the process to patients and families; health and social care colleagues and care providers. Ensuring that the process is transparent and clearly understood by all parties.
