Job summary
To work within the Home First pathway with a view to ensuring that patients remain in their home environment.
To work in the community with patients who have long term conditions and/or palliative care needs.
To assess and treat patients whilst following the Occupational Therapy process.
Main duties of the job
To develop and deliver specialist Occupational Therapy for people with long term conditions and palliative care needs within the community.
To work within the Doncaster Community Therapy Service to deliver the Home First model to people in the Doncaster area, to prevent hospital admissions and accelerate hospital discharges.
To assess individual patient's needs, set and implement treatment plans and long term condition management for patients linking with primary, secondary and tertiary care, using a coordinated approach.
To contribute to the development of the Occupational Therapy role within the Occupational Therapy Service by working across boundaries and inter agency working.
As part of a team, initiate research based practice and monitor the quality of the service being provided.

Job summary
An exciting opportunity has arisen for a motivated and enthusiastic Audiologist to join the Audiology Department at Kettering General Hospital. While the department provides services for both adults and children, this role has a primary focus on paediatric audiology, supporting children and families across University Hospitals of Northamptonshire.
This is an excellent opportunity for an experienced Paediatric Audiologist or an Adult Audiologist looking to develop specialist skills within paediatric services. The successful candidate will deliver high-quality, patient-centred audiological care, undertaking assessments, hearing aid fitting and verification for children while working closely with families and the wider multidisciplinary team. Support will be provided through structured training and professional development, enabling the successful candidate to enhance their clinical expertise and progress within a supportive and forward-thinking service.
You will contribute to the continued development of audiology services, ensuring children and their families receive safe, effective and compassionate care throughout their hearing healthcare journey.
Main duties of the job

Conduct age-appropriate audiological assessments for children aged 6 months to 18 years using a range of behavioural and objective testing techniques.

Lead Visual Reinforcement Audiometry (VRA) clinics, accurately interpreting results and making informed clinical decisions.

Undertake paediatric hearing aid fitting, verification and validation to ensure optimal hearing outcomes for children.
-
Assess hearing needs and recommend appropriate management plans in line with local and national guidance.
-
Communicate assessment outcomes, treatment plans and recommendations to parents, carers and families with empathy, clarity and sensitivity.

Provide support and advice to patients, families and other healthcare professionals regarding hearing loss and hearing management.

Work collaboratively with multidisciplinary teams to ensure coordinated, child-centred care.

Maintain accurate and contemporaneous clinical records and documentation in line with professional and Trust standards.

Contribute to the delivery of high-quality paediatric audiology services and support service development initiatives.

Participate in continuing professional development activities and maintain clinical competencies.

Provide support to adult audiology services as required, including hearing assessments, hearing aid fittings, repairs and ENT clinics.

Main duties of the job
You will provide senior clinical and operational leadership across clinical pharmacy services, with specific responsibility for delegated Care Groups, ensuring services are safe, effective, responsive and patient-centred.
A key focus of the role will be driving clinical service performance, using KPIs, data and service intelligence to improve patient flow, productivity, quality, medicines safety and workforce performance, while identifying variation and addressing areas of underperformance.
You will oversee workforce deployment, rota management, capacity and service resilience, supporting Clinical Lead Pharmacists and wider teams to deliver consistent and effective clinical pharmacy services.
You will lead and support service transformation and medicines optimisation, including the development of pharmacist prescribing, digital solutions and new models of clinical pharmacy practice, while contributing to financial sustainability and efficient use of resources.
As a senior pharmacy leader, you will contribute to clinical governance, quality improvement, workforce development and strategic planning, deputising for the Associate Chief Pharmacist - Clinical Services as required and representing pharmacy at relevant Trust and system-level forums.

Main duties of the job
The Pathology Directorate is looking to appoint one 37.5 hour Band 6 Chemotherapy nurse to the existing service. The post holder will be based at the Prince Charles Hospital and required to attended Royal Glamorgan Hospital. The post holder will be part of a nursing team supporting the Haematology Day Unit Manager to deliver a safe and efficient Haematology service within Cwm Taf Morgannwg University Health Board (CTMUHB), working closely with the multidisciplinary team. The position will involve 2/3 days developing and streamlining the nurse led Hemochromatosis service and the remainder of the time on the Haematology day unit.
The role will include providing expert advice, education to all levels of staff to improve the safety and effectiveness of the practice throughout the CTMUHB, in accordance with NPSA notice 18 and ensure cancer standards are adhered to as in Cancer Delivery Plan (2016-2020). Amongst other aspects of the role the post holder will be expected to perform local audits, incident investigation and subsequent reports to the relevant forums.

Main duties of the job
As a Senior Mental Health Practitioner, your responsibilities will vary. You will
- Responsible for assessment and ongoing management of our patients mental health needs.
- Work collaboratively with the multi-disciplinary team as an autonomous practitioner managing a caseload of patients, assessing their physical, social and psychological needs to develop a holistic health care plan.

The Pharmacy Departments within ULHT provide ward based, Dispensary, and Aseptic Dispensing Services to a range of general and specialist areas including Admissions Unit, General Medical and Surgical, Haematology and Oncology and Critical Care wards. We have a good integrated team of Clinical Pharmacists, Technicians and Support Workers and there is a supportive Pharmacy Education and Training team.
Main duties of the job
This post will be based at Grantham and District Hospital but the successful applicants may need to travel to other sites within the Trust. The successful applicants will be expected to participate in the existing Weekend and Bank Holiday Rotas within the Pharmacy Dispensary.
You will need to have excellent interpersonal and communication skills and be confident speaking to patients, nursing staff, prescribers and other health care staff. You will need to have experience working as a pharmacy technician in a hospital or community setting. Experience of using pharmacy specific computer systems such as Ascribe or JAC is desirable.

The Trust are seeking one qualified Level 3 Joiner to join the Estates Maintenance Team working at our Rotherham Doncaster and South Humber sites. You will enjoy working with all our clinical staff, nurses, doctors, and corporate staff to support them in their work.
Main duties of the job
To carry out the repair, maintenance or installation of all plant, equipment, systems and building structure/fabric associated with all Trust properties. This will consist of planned preventative maintenance (PPMs), testing and calibration, fault finding, emergency response, defect correction and minor works.
You will be employed to work appropriate to your core skills around building and engineering plant and equipment maintenance. You will progressively be trained, where required, to acquire other skills with the intention of reaching levels of multi trade flexibility and competence. You will work closely with the Senior Technicians to improve planned maintenance.

We are currently recruiting a Band 3 Healthcare Support Worker, 23 hours a week, to be part of our Stroke & General Rehab Inpatient Unit at Evesham Community Hospital. The candidate will work closely with the multidisciplinary team (MDT) to achieve specific rehabilitation goals. This will be a challenging and exciting opportunity and will enable the individual to develop new and existing skills.
Evesham Community Hospital plays a critical role in the delivery of local NHS services, providing more care and treatment closer to peoples homes. Emphasis is to reduce length of stay whilst ensuring excellent care which is safe, effective, personalised and compassionate and to the standards you would expect for your family and friends.

Main duties of the job
The main duties of a healthcare support worker are:
Assist with admitting patients to the ward and the care of their relatives
Undertake clinical procedures following assessment of competencies by qualified senior members of staff
Carry out delegated nursing care
Act as health
Provide safe and effective care
Identifies areas for the improvement
Assist patients with attending to the activities of daily living and support the therapy team with the rehabilitation of the patient.
Assist patients to sit up or out for breakfast.
Serve and prepare meals/breakfast and drinks, help to assist patients when required. Complete appropriate paperwork i.e food diaries and fluid balance charts.
Assist patients with morning personal care and dressing.
Mobilise patients using correct mobility equipment when required/liaise with physiotherapy team.
Liaise with nursing staff for information regarding Observations, catheters, lab samples, blood sugar monitoring, dressings and further health checks.
Carry out patient weekly assessments which involve weighing, clean and check mattress, catheter bag change when required.
Stores to be restocked daily.
Assist patients with toileting.
Assist to support patients to bed/ attach catheter night bags when required.
Comply with policies and procedures of the ward.

Job summary
This is a role within our Plus Team which provides an enhanced service to all children and families subject to Safeguarding Plans.
You will be a Specialist Community Public Health Nurse (SCPHN) Health Visitor (HV) within the integrated 0 to 19 Torbay Partnership and provide the expertise, knowledge and resources to deliver the Healthy Child Programme (HCP) to families living within Torbay.
Working with the 0 to 19 Torbay workforce to ensure pathways are followed in supporting children, young people, families and carers. The SCPHN-HV works autonomously and collaboratively to deliver a family centred public health service, taking responsibility for a defined population in partnership with the wider multi-disciplinary and inter-agency teams.

Main duties of the job
The post holder will be providing direct hearing managementof the adult population. This includes diagnostic assessment, speech testing and rehabilitation.
Apart from the clinical duties the post holder will also be expected to provide leadership to junior staff, by doing regular 1:1 meetings, clinical supervision and appraisals.
The post holder will take an active role in areas of clinical audit, research and clinical governance of the Audiology service. They will be expected to design and lead of audits within the service and participate in data collections for larger projects.

We are looking for a Senior Community Support Worker to work within the Ellesmere Port and Neston Community Response Hub, providing an early supported discharge service to patients from the Countess of Chester Hospital & Ellesmere Port Hospital. Main duties of the job
- To work flexibly as part of the ward team, using their initiative and experience to ensure all their incoming work is appropriately prioritised and actioned.
- To be responsible for the maintenance of the ward's administrative processes and systems, contributing to their on-going development and improvement.
- To receive/welcome new patients onto the ward, checking particulars on their records and ensuring notes and necessary paperwork is collated and in order; ensuring identity bracelets are available for nursing staff; compiling sets of required blank documentation for emergency patients; informing the nurse in charge or deputy of any query new patients may have.

Based on their own knowledge, to assess when to personally answer the question or action the request and when to refer it to a more senior member of staff, ensuring the caller is informed of the action to be taken

- To provide professional/clinical supervision to Registered Practitioners and Assistant Practitioners along with other professional/clinical supervisors in the team.
- To support the EMHPs, Registered Practitioners and Assistant Practitioners to liaise with other education, health, social care and voluntary community sector staff from a range of agencies, in the care provided to children, young people, their families and professionals.
- Support EMHPs in the co-delivery of training sessions, group and individual interventions in schools.
