Job summary
Calling all Registered Nurses (Adult, Mental Health or LD) or Allied Health Professionals registered with the HCPC who have an interest in Adults Continuing Healthcare and Funded Nursing Care, including Fast track. The Practitioner role is part of the All Age Continuing Care Team in the cluster ICB of NHS Dorset, NHS Somerset, NHS Bath, NE Somerset, Swindon & Wiltshire. The role operates Monday to Friday core hours, with no night shifts or weekends, with a hybrid approach of office work, remote working and face to face assessments with individuals across the cluster footprint. Main duties of the job
This role is for a Practitioner, with a requirement for cross cover across the Cluster. Although there will be a primary allocation to a Place team, the expectation is that we will be moving towards one ICB and flexibility is required. Please refer to the Job description for the full role duties.

The post holder will play a key role in supporting the All Age Continuing Care (AACC) across the organisational footprint. Working within the remit of The National Framework for NHS Continuing Healthcare and Funded Nursing Care (FNC), the post holder will deliver an effective and competent level of service and consistently delivering a client-focused service. The post holder will undertake a range of tasks within workstreams including Duty, Care Coordination, reviews and assessments and the reporting and analysis of information to support delivery of the AACC service.
Key accountabilities:
Undertake CHC, FNC and other AACC assessments in line with the National Together across NHS Bath and North East Somerset, Swindon and Wiltshire, NHS Dorset and NHS Somerset Framework.
Complete reviews, including 3month and annual reviews, ensuring decisions remain appropriate.
Gather and analyse clinical and social care information to support robust eligibility recommendations.
Contribute to Decision Support Tools (DSTs) and provide clear, evidencebased rationales.

We have an exciting opportunity for a motivated, forward thinking Occupational Therapist to join the Mental Health Complex Care Service for a 12 month secondment.
The service incorporates inpatient rehabilitation, community rehabilitation and the assertive outreach teams.
You would be based at Nightingale House which is a level one, mental health rehabilitation unit based in Westbourne, Bournemouth.
This is a full time post, 37.5 hours working across Monday - Friday.

The team is based at James Cook University Hospital but also covers the Friarage Hospital, Northallerton. Main duties of the job
To provide advanced clinical and professional Occupational Therapy to patients on the acute Trauma and Orthopaedic wards. The successful candidate will work within the MDT, providingsupport to the senior specialist Occupational therapist and AHP Service Lead, to ensure that the wider Orthopaedic Team contributes to the Strategic plan for University Hospital Tees NHS Trust.
A key role is to undertake advanced occupational therapy assessments of patients with diverse and complex conditions and to develop and delivery individualised treatment programmes.
The post holder will be expected to have experience and knowledge of the occupational therapy management of musculoskeletal/orthopaedic conditions and experience of managing patients post surgery is desirable. Successful candidates will be able to provide comprehensive assessments and treatment plans to these patient groups. Relevant post graduate study, leadership training and experience is also desirable.
This post will offer development in clinical skills and the post holder will be supported to attend relevant post graduate courses. We have an established training programme and our staff are encouraged and supported in their CPD through workshops, clinical supervision and the appraisal process. There is also an expectation to actively participate in audit and research and the Hospital has close links with both the University of Teesside and Durham University.

As a RMN/RNLAD Nurse, you will work directly with young people and their families providing a range of assessments and therapeutic work, adopting the Thrive framework. Our aim is to provide outstanding care, promoting recovery and wellbeing in a modern, purpose-built setting to make a difference and have a positive change on the lives of young people. We invest in supporting and developing our nurses. Supervision from a Clinical Team Lead will be provided and training will be arranged where needed.

Main duties of the job
The role will be based in the Adolescent In-patient Unit providing individual care to those within the unit in accordance with the integrated care programme approach.
You will work in accordance with the Trust Clinical Governance programme informing Senior Clinical Nurse when changes occur in circumstances that affect standards of care.
As part of the role, you will be assisting in the organisation of outside activities in line with the Clinical Risk policy and be responsible for the safety and wellbeing of young people on external activities.
The role will allow you to develop effective liaison and communication with all disciplines, agencies, parents/carers and service users, giving sound advice and support to service users and their carers as appropriate in line with professional practice.
You will be required to undertake a Prevention and Management of Violence and Aggression course and it is a condition of your employment that you successfully complete all aspects of this course; if this is not achieved you will not be able to commence working in the post.

An exciting opportunity to join the West Dorset inpatient Therapies Team at Forston Clinic. Your work will aim to encourage engagement in meaningful and purposeful occupation with people whose lives have been affected by occupational disruption as a result of mental illness.

We are looking for a Band 5 Biomedical Scientist to join our Microbiology Team on a full time basis.
Biomedical Scientists (BMS) are an integral part of the laboratory team responsible for the diagnosis of infection and disease processes by the application of latest scientific knowledge, coupled with the most up to date scientific methods and use of traditional methods where required. BMS carry out, and interpret the results of, laboratory tests on biomedical samples that will aid clinical staff in the diagnoses of illness and evaluate the effectiveness of treatment. Main duties of the job
- To interpret clinical details and patient demographic data for the allocation of tests to samples.

- To accurately interpret and report timely complex analytical results, and authorise Microbiology reports according to laboratory policy.

This is an exciting and challenging opportunity for an experienced administrator to join our new CAMHS Inpatient Ward Seastone CAMHS High intensity Environment.

Main duties of the job

You will be working as part of the multi-disciplinary team, which will involve developing knowledge of the processes and the electronic systems used to ensure that patients needs are met. You will take a lead role in providing secretarial support to accurately pre-populate forms and document the meetings held on the ward into the patient records. For which experience of audio typing and medical terminology will be an advantage.
Other responsibilities will include organising the Ward office, by managing the reception area, visits, and telephone; keeping on top of the paperwork to include scanning and uploading patient documentation and liaising with patients and staff.
You will be required to undertake a Prevention and Management of Violence and Aggression course and it is a condition of your employment that you successfully complete all aspects of this course; if this is not achieved you will not be able to commence working in the post.

We are looking to appoint a reliable and motivated receptionist to join our dedicated administration team to support the Community Mental Health Teams at Kings Park Community Hospital. Job summary
The Royal National Orthopaedic Hospital (RNOH) is an international centre of excellence with an esteemed reputation for diagnosis, assessment and treatment of neuro-musculoskeletal conditions.
We are the largest specialist trust in the United Kingdom and amongst the largest orthopaedic specialist hospitals in Europe. The theatre complex at the Royal National Orthopaedic Hospital consists of 10 operating theatre, a plaster theatre 4 new operating theatres in the near future. In addition to the wide range we also perform minor urology case support general anaesthetic provision and recovery for the CT/MRI diagnostic scanning and CT guided procedures.
Furthermore we do major revision work not undertaken in other centres. The recovery areas currently consist of 15 bed spaces and other recovery spaces in the Surgical Admission Procedure Unit for chronic pain procedure. The Recovery has a well-established team where excellent standards of care are delivered.

Main duties of the job
The band 5 role is open to learning and development of new knowledge and skills in theatres recovery environment. We are seeking a Band 5 recovery practitioner who is passionate, dynamic, forward thinking, to join the team. Adult trained Candidates should be aware that it would desirable for them to be considering working a paediatric second registration in post anaesthetic role. As the department seeks to increase the paediatric recovery staff. They will be an expectation for the provision of high standards of care that is update and informed and best practice.
You will be expected to maintain a high standard of clinical work when carrying out recovery / post anaesthetic procedures. Responsible for the safe custody of medication, including checking and administration. Ensure compliance with our clinical governance framework, you will motivate colleagues, champion service user safety and dignity and drive continuous improvement. Our Neonatal Unit provides specialist care for premature and sick newborn babies and their families across Swansea Bay and the surrounding region. Main duties of the job
The post holder will support the Neonatal Unit by undertaking a range of essential non-clinical duties that help maintain a safe, efficient and well-organised environment. Responsibilities include maintaining stock levels, organising supplies and equipment, and supporting the effective use of resources across the unit.
You will clean and prepare incubators, cots, patient areas and parent accommodation, ensuring bed spaces are ready for new admissions. The role also includes cleaning clinical equipment and ward areas in line with infection prevention and control standards, helping to maintain a safe and hygienic environment for babies, families and staff.
Working closely with the multidisciplinary team, you will assist with stock management, equipment movements, record keeping and monitoring designated equipment. Excellent communication skills are essential, as you will liaise with colleagues, parents and visitors in a professional and compassionate manner.

We are looking for two enthusiastic and experienced Nursery Nurses to work within our dynamic East Dorset 0-19 Public Health Team, as Public Health Nursery Nurses, working with children, families, and children 0-11 years. We have a 22.5 hour vacancy in Wimborne/Corfe Mullen and a 26 hour vacancy in Ferndown.
You will work within the Children Young Person Public Health (CYP PH) Service to support the delivery of the Healthy Child Programme and targeted interventions to children 0 to 11 and their families, as delegated by and under the supervision of the SCPHN/TC/ 0-19 CYP PH clinical Lead, whilst working closely with all members of the 0-19 PH team.

The team is friendly, supportive and works collaboratively to optimise patients' level of independence and find innovative ways to enable them to increase their functional performance.
You will work as part of the Integrated Community Team (ICT) pathways to prevent inappropriate hospital admissions and to provide therapeutic holistic assessment, support, treatment and rehabilitation of patients in their own home. We are developing closer working relationships with the Primary care networks with their frailty pathways and with other statutory services.
Main duties of the job
You will provide specialist occupational therapy intervention for your patients and able to delegate and support the OT skills of our therapy technicians
You will be able to manage a clinical caseload and to support patients with goal setting and supporting their functional performance through activity

We are seeking a Band 5-6 Dietitian to join our supportive team on a full-time basis of 37.5 hours per week.
You will be providing specialist community dietetic care to adults receiving enteral nutrition in their own homes or care settings. This role offers a fantastic opportunity to develop your expertise in home enteral nutrition (HEN) while working closely with a multidisciplinary team, including Nutrition Specialist Nurses and acute care colleagues.
As this is a developmental role, it is suitable for a newly qualified dietitian who would be supported to work through objectives with an aim to progress to a B6 level. If you already have relevant experience, this role could be offered as a B6 role.

Job responsibilities
- Vacuum/mop sweep/damp mop/spot clean/spray buffing of floor surfaces, using machinery as required.
- High and low dust (including skirting boards/window ledges/curtain rails/notice boards/picture frames/furniture/doors and door stays)
- Vacuum carpeted areas, daily removal of spots or stains, and shampooing of carpets using machinery on housekeeping supervisor instruction.
- Collection and tagging of all rubbish, removing bags, clinical waste bins, sharps bins and flattened cardboard containers to designated collection points.
- Ensure waste is correctly segregated at the collection point, and placed into locked containers, as required by Trust policy.
- Replace bags on stands/containers, with the correct colour code, and ensure that containers are clean.
- Damp dust lockers, over-bed tables, chairs, beds, ledges, over-bed lights, pipes, radiators (remove cages as instructed) bed and locker wheels.

Job summary
The Hepatology Clinical Nurse Specialist (CNS) will play an integral role in the development of a quality driven service to all patients at/or admitted to THHFT with Liver disease. The CNS has a leadership role within the Trust and is responsible for the development of innovative nursing practice within Hepatology. The post holder is expected to facilitate the development of other nurses to benefit patient care and to be an educational resource to all members of the Trust wide multi-disciplinary MDT regarding practice provision of care. The CNS will help to promote and co-ordinate a seamless pathway of care for patients with liver disease, from diagnosis through to treatment and outpatient management.
The post would be Band 6/7 dependent upon experience. For Band 6 applicants, Band 7 would be applied when the post holder undertakes clinics independently, with supervision.
Main duties of the job
The role will involve independently screening patients for liver fibrosis using Fibroscan (training provided), carrying out comprehensive face to face and telephone assessment of patients and providing health education to patients and their families/ caregivers through supervised, nurse led clinics. The post holder will manage a caseload of a range of patients with liver disease and will be expected to work independently within the remit of their role, education, support and supervision will be provided. The CNS will be required to liaise closely with members of the MDT to progress patient care and maintain safety at all times. High quality IT skills are essential for this role to assist with data collection, audit, analysis and potential opportunity to present data at regional and national meetings.

Main duties of the job
The purpose of the role is to ensure that those requiring complex rehabilitation and care are assessed appropriately and accepted onto an NHS funded pathway.
