Job summary
An exciting opportunity has arisen to make a positive impact within The Inclusion Bases within the London Borough of Islington. This newly created post will support a caseload of Primary Aged Children with complex learning needs, the majority of whom have a diagnosis of Autism.
The post holder is an autonomous practitioner and is responsible for organising and managing a defined caseload of children and young people (CYP). They are responsible for delivering a comprehensive and highly specialist service to CYP who have special and complex needs that may include autism, down's syndrome, complex learning difficulties and social and emotional behaviour difficulties. Some of these CYP will require support around developing access to AAC systems.
Intervention includes assessment, differential diagnosis and therapy for individual children or groups of children and working with the MDT to provide support to parents and professionals. The post holder will devise and provide individualised care plans that are evaluated via outcome measures. They will employ specialist communication strategies used with the client group as appropriate.
Main duties of the job
The post holder will take a lead on a clinical speciality within the team. They will provide clinical supervision and support to less experienced colleagues both within the team and the wider service; this will include the provision of second opinions and advice / support around AAC.
They will represent the Speech and Language Therapy Service at multi-disciplinary / multi-agency meetings as necessary.
The post holder will have excellent communication skills and the ability to empathise in order to support and counsel parents to accept the implications of their child's special needs. They will be able to communicate complex information effectively taking into account the barriers to communication presented by the diverse caseload. They will be skilled in working in and liaising within a multi-disciplinary and multi-agency environment. The clinician's level of high level of expertise in the field will ensure that they can reassure and instil confidence in clients, carers and other professionals. They will demonstrate good negotiation skills in working with other team members, parents/carers or other professionals. They will contribute to policy development and the development of clinical protocols.
The post holder will be involved in developing and delivering training for school staff and parents. They will provide clinical education for more junior staff in within IANDS.

Main duties of the job

We are looking for an enthusiastic, compassionate and motivated Band 6 Podiatrist to join our Brent and Harrow Specialist Care Team. This is an exciting 12-month fixed-term opportunity, based at Alexandra Avenue Health and Social Care Centre, Harrow, HA2 9DX.
This role offers an excellent opportunity to develop your clinical expertise within a supportive specialist community service. The Mildred Creak Unit is a children's inpatient unit caring for young people aged 7 to 13 with a range of severe and complex mental health difficulties.
Our supportive team will allow you to develop your experience and knowledge of child and adolescent mental health. Working across disciplines to support our patient group with complex difficulties and supporting young people with activities of daily living, you will facilitate activities and therapeutic groups while using your expertise to create and deliver individualised care plans. With weekly team and monthly MDT education opportunities alongside a bespoke professional and academic development pathway which aligns with your goals, you will be part of a team which strives to provide young people with greater collaboration and empowerment in their treatment journey.
Main duties of the job
The role involves working as a member of the nursing team to provide care to the young people. You will facilitate their daily routines and attendance of the therapeutic timetable in a nurturing and supportive manner. The role includes various duties such as supporting young people to eat, helping young people with their physiotherapy programmes, providing a space for young people to talk about their feelings as well as clinical tasks such as administration of medication, nasogastric feeding, physical observations and the facilitation of therapeutic groups. The role also involves aspects of housekeeping and admin.

Job Purpose
The role of a Cardiac Physiologist Associate requires the post holder to undertake a broad range of ambulatory techniques, including highly skilled recording, measuring, and monitoring of physiological changes. Responsibility for effectively interpreting results and providing factual reports on investigations to enable clinical diagnosis, aiding patient treatment and outcomes. You should be able to accurately identify situations that need urgent medical attention and escalate as appropriate, using best clinical judgement.
To organise and assist with the ambulatory Electrocardiogram (ECG) training of new Healthcare Scientists, student Cardiac Physiologists and medical students, ensuring staff exposure to a range of patients and clinical environments/situations during their training period. You shall effectively assist the Cardiologists in Cardiology clinics and provide support for the Cardiac Physiologists in the provision of clinical care to patients within medical services, this is inclusive of administrative and clerical responsibilities.

The post holder will work as part of the Discharge Teams to facilitate transfers from hospital and community beds, working in partnership with colleagues across health and social care to reduce length of stay and improve outcomes for patients and carers.

The key aims are as follows :
- Excellent communication and IT skills are required to triage patients to the right team and services.
- Delivering a consistent, co-ordinated approach to assist patients to identify potential needs at the point of discharge to avoid unnecessary delays in transfer of care
- Working collaboratively with external and internal partners for the benefit of the patients to ensure smooth transition from hospital and reduce risk of readmission

The post holder will provide specialist palliative care advice and support for patients receiving palliative and end of life care who have complex symptoms associated to their life limiting condition. Caseload Coordinator - Urgent Community Response & Virtual Ward: Permanent (30 hours)
This position is for 1 part time 30 hours per week post, some weekend working is required.
This is an exciting time to join our team, supporting us to care for patients safely in their own home avoiding unnecessary admissions and facilitating early discharges from hospital.
UCR/HaH is a multi-disciplinary team that provides a two-hour urgent response to patients in their own home that are experiencing a crisis. The service can clinically monitor patients remotely via Virtual Ward using enhanced technology.
This role requires a highly organised person, with ability to prioritise their workload and work in a calm and effective manner and communicate well at all levels.

Children's Support Worker - Continuing Care Schools: 35 Hour Term Time Only Position
The Children's Continuing Care Schools Team are looking for a motivated and dynamic Children's Support Worker to work alongside the School Nurse and provide care for children with complex health needs, within the school setting.
The successful candidates will be expected to work between the hours of 08:30 -16:00 Monday - Friday during term time only.
Your working day will start at the child's own home, where you will be responsible for escorting the children and young people to school, on school transport.
Once you arrive at school you will be responsible for the care and support of children and young people with complex health needs within school and perform any health interventions that the children or young people may require
There is a school nurse on site who will provide support as required. In addition to this you will be expected to support the child to access their educational curriculum helping them to fulfil their potential.
The post holder will work under the direction of the team leader and registered nursing staff to set standards and plan of care. They will carry out specialised care in an unsupervised capacity.
The successful candidate will be put through a comprehensive training programme to ensure they are competent and confident whilst delivering a high standard of care.
Main duties of the job
The post holder will either be responsible for the one to one care and support of an individual child or will assist the allocated School Nurse with healthcare related tasks for all children and young people in school.
Depending on the role, the post holder will start their day at the school base or at an identified child's own home. You will be expected to escort the child into school on school transport and remain with them throughout the school day providing specialised support in the classroom. The working day will finish once the post holder has escorted the child back home on school transport.
The children will have complex health needs, and require bespoke and individualised plans of care. The post holder will be responsible for medications, suctioning, manual handling, tracheostomycares, gastrostomy cares, and other health care related needs.
The provision of robust training and regular competency reviews will ensure care is provided safely and in line with all policies and guidelines.
The aim is to enable the children to achieve set targets and goals, and also make the school day enjoyable whilst maintaining safety at all times.

The Ear, Nose and Throat Outpatient Team would welcome applications from a Healthcare Assistant who enjoy working independently and working within a team in being instrumental in providing patient focused care.
This post is a full time position, however to support flexible working, part time hours can be discussed at interview (minimum of 21 hours per week)
The successful candidate will join a well-established team and will need to be flexible and adaptable, a good team player with the ability to work well on their own initiative and a willingness to take on a variety of duties, whilst having the ability to maintain attention to detail.
You will ideally have knowledge or experience within a surgical ward environment and previous experiencing of nursing patients with ear, nose or throat conditions.
Within this role, there are numerous learning opportunities. We offer a supportive working environment, and we are committed to the development of our staff.
We ask that candidates applying for this post have a current NHS Care Certificate and a Health and Social Care Qualification at Level 3 with current secondary care experience.

Main duties of the job
Highlighted below are some of the nursing skills needed for a fast-paced working area such as Outpatients:
- The ability to work both independently and as part of the wider Multidisciplinary Team
- Supporting and assisting your consultant colleagues in the smooth running of their clinics.
- Acting as a chaperone.
- Being able to undertake baseline measurements; BMI, blood pressures, blood sugar monitoring, ECG's, Urinalysis as well as venepuncture and/or cannulation.
If you do not already possessall required skills, additional training will be given to support you in this role, but this may initially be at a Band 2 until all Band 3 competencies have been achieved.

Job summary

Main duties of the job
The role involves supporting scientific staff in the receipt, analytical preparation, labelling and data entry of patient samples & requests, maintenance and quality control of specialised analysers and other technical equipment and general laboratory support duties. Previous experience in an NHS clinical laboratory would be an advantage, but not essential as full training will be provided.
The Microbiology department operates 24 hours a day, 7 days a week service provision.
