An exciting opportunity has arisen for a full-time Senior Care Act assessor within a well established and dynamic initial response team.
The position is within the older persons with physical disability team which is a community-based team working with Adults over 18 years of age with a range of physical and Mental health needs.The successful applicant will hold a level 3 diploma in adult care or lead adult care/ social care worker and or level 4 diploma in adult care or lead practitioners in adult care level or be willing to complete/ achieve this if a level 3 qualification is already held. The successful candidate will also have experience working within a relevant field.
This is a dynamic post that requires the post holder to work closely with multi-disciplinary teams/agencies in addressing a range of care/support issues in the promotion of independence and improving the health and wellbeing of the population served.
Main duties of the job
The post holder will be expected to access Social care and health records to gather information, therefore it is important for the post holder to be able to use a computer and have an understanding of the importance of good recording. You will require effective communication and written skills and be willing to work as part of a team to promote best outcomes for individuals. You will be required to be self-motivated and organised in order to manage any competing demands and be provided with all relevant training and support in the role.
Midlands Partnership Foundation Trust is dedicated to ongoing Social Care development, and you will have access to the Social Work learning academy in order to promote professional development. You will also receive the support of your team and your line manager who will provide regular supervision both on a formal and informal basis.

You will be working in the Respiratory/Critical CareTherapies Team which consists of Occupational Therapists, Physiotherapists and Therapy Assistants. You must be a fully registered Nurse, Social Worker or Allied Health Care Professional with appropriate mental health experience to apply. Working 37.5 per week you will be required to cover a 24 hr rota on a shift basis.
Main duties of the job
- Provide specialist assessment and care planning in conjunction with service users and carers, evaluation of individual treatment plans and discharge planning.
- Contribution to the delivery of home treatment through nursing care to an identified community client group and within a multi-disciplinary team.
- To provide an active involvement in the assessment, development, implementation and evaluation of service user care which focuses on ensuring that service users are supported to remain in their own homes wherever hospitalisation can be safely and appropriately avoided.
- To contribute to the clinical service development as part of the 0-25 Emotional Health and Wellbeing Service.
- Triage and assess referrals received by service in liaison with multi-disciplinary colleagues as appropriate. Formulate and action appropriate response based on need, risk and urgency.

Main duties of the job
The appointed person will conduct a varied range of administrative duties and act as a focal point for communication with patients. Excellent interpersonal and communication skills (both written and verbal) are essential, as is an ability to organise and prioritise their own workload and to react quickly to changing situations. Strong IT skills are vital so applicants should evidence their experience of using data basis and Microsoft Outlook, Excel and Word on their application.

The service works core 9-5 hours but offers therapy opportunities for clients between the hours of 8am-8pm and in the future, on Saturday mornings. The successful candidate should be prepared to work flexibly to meet service needs within these hours.

Main duties of the job

To offer an extended service to ensure an appropriate assessment of mental health need for CYP in mental health crisis and in need of appropriate intensive support ensuring the delivery of safe and effective practice as part of the CYP wellbeing and recovery action plan. The post holder will work directly with children and young people with mental health needs and their families, carrying out assessments (including risk management) and effective treatment and/or interventions, with the aim of helping children and young people participate in normal family and community activities and to facilitate positive outcomes of care in relation to young people's mental health needs. The role will also facilitate hospital admission avoidance and seamless transitions between services where appropriate.
They will also be expected to work flexibly within the Team to support a crisis, brief intervention and intensive support function. Potholder will provide assessments and interventions for CYP as part of the MDT for those experiencing severe to moderate mental health conditions.

Main duties of the job
- Provide a confidential, quality administrative service to an identified pathway within the Care Group.

Job summary
The successful candidate will have experience of working in acute or community mental health and be able to contribute to the assessment, implementation and delivery of high-quality therapeutic mental health patient care within the Home Treatment Team. This will include delivering mental health care in line with coproduced care plans under indirect supervision of qualified members of staff. There is a requirement to work a shift pattern that provides a service 7 days a week 365 days of the year.
