Job summary
An exciting opportunity has arisen for an enthusiastic and motivated Practice Nurse to join our vibrant, forward-thinking practice. The successful candidate will be an integral part of the nursing team and will be responsible for participating in the delivery of high-quality patient care to our registered practice population.
Working alongside our Nurse Manager, three Practice Nurses, and two GPAs; we are ideally looking for chronic disease experience in Diabetes Management.
This post is a permanent position at the Practice, 34 hours per week.
Main duties of the job
The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition, and the provision of preventative health care to the practice population.Youwill deliver care, focusing on supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively within the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

- To be responsible for diagnostic procedures for example, venepuncture, blood pressure, height/weight, ECG and any specimens.
- Work with other health care professionals to diagnose, monitor, manage and treat long-term conditions (inclusive of diabetes management), including non-drug-based treatment methods using a management plan, and in line with national and local policies and practice needs
- To advise patients on general health care and promotion of healthy lifestyles.

- To administer immunisations for example, influenza, pneumococcal, tetanus and childhood immunisations.
- Support and manage health needs of women presenting for family planning and cervical cytology consultations
- Advise, support and administer vaccinations where appropriate for patients travelling abroad
- Assist in providing minor-surgery sessions.
- Apply infection-control measures within the practice according to local and national guidelines

The post holder will support the delivery a high quality, comprehensive mental health service for children and young people with emotional health/mental health difficulties. As a Domestic Facilities Assistant your role is integral in keeping our hospitals clean and safe for our patients, visitors and staff, the NHS couldn't function without you.

We are seeking a Porters to join our team, working at our Hartlepool hospital site. Typical Day of Working in the role of a Porter:
Responsible for the movement of patients by bed/trolley/wheelchair between Wards and Departments
Collection and Delivery of laundry, pharmacy boxes, bedding and furniture throughout the hospital
Exchange of medical gas cylinders
Collection and Delivery of specimens, blood bags and medical records
Cleaning of wheelchairs and trolleys after every use.
Potentially provide driver cover for vehicles including vans and the shuttle bus, and hospital tugs.

Main duties of the job

- Maintains equipment, building fabric and utility services of Trust property.

To provide specialist clinical care and preventative treatment without direct supervision to patients attending the specialist Dental Hospital services for secondary or tertiary care. To carry out dental treatment on these patients to a level of competence as directed by the Dental Act and its amendments.
Main contact is with hospital staff and patients. Patient groups as a priority are adults and children with special needs, including medically compromised patients, anxious patients and patients with challenging behaviour.
Main duties of the job
- Carrying out the full range of treatments currently permitted under the General Dental Council regulations.
- Specific duties may only be undertaken if trained and competent to do so.
- Treatment may be carried out within hospital dental departments and wards or by domiciliary treatment visits, if necessary
Treatment is provided following a prescribed course of treatment as directed by a registered dentist. Any combination of the following duties may be performed on a daily basis.
Permitted duties of the Dental Hygienist:
Education of patients on the causes and prevention of periodontal diseases and dental caries.
Identification of risk factors for the dental diseases, including medical and social risk factors and the provision of appropriate counselling advice, including:-
Medical history
- Diet analysis and nutritional guidance
- Advice in the use of fluoride agents
- The benefits of smoking cessation and accessing cessation service
Motivate patients to maintain a high standard of oral hygiene.
Monitor patients' progress using appropriate indices:-
- Bleeding and plaque indices

- Furcation index
Perform supra- and subgingival scaling of teeth including polishing, allowing removal of hard and soft deposits and stains.
The provision of comprehensive specialist periodontal invasive root debridement therapy using Gracey area curettes and high-speed ultrasonic device.

We are looking to employ two Band 4 Registered Nurse Associates in our Special School Nursing Service, working in a service that covers 30 hours per week, Monday - Friday.
The Service provides care in the school to ensure children with complex care needs are met
These post is citywide and staff can expect to be moved to different areas to gain a wealth of knowledge and experience in supporting Children, Young People and Families.
You will work alongside a multi-disciplinary team that includes Community Paediatricians, Paediatric & Learning Disability Nurses, Speech and Language, Physiotherapists, Education Staff and Disabled Children's Social Care.
You will be required to either be a car driver or be able to evidence how you can travel across the city as you may be assigned to several care packages in one day to attend and support staff.
Main duties of the job
You will support families at Schools to ensure health needs are met and Child, Young Person & Family are supported through Early Help, Education & Healthcare Plans, SEND. You will be responsible for the holistic, comprehensive assessment of more complex community patients, delivering clinical care and contributing to the evaluation of person centred care plans.
You will be required to support staff in the service
The role requires excellent communication skills, IT skills, and the ability to be adaptable, reliable and highly organised and to prioritise your workload effectively.
Experience of care in a community setting would be an advantage but not essential. You must be able to travel around the city during the working day.
You must be a registered Nursing Associate registered with the NMC - or in the process of registering.
You will have access to ongoing training and development, annual appraisals and clinical supervision.

To undertake the following tasks as directed by the Office Manager: -
- To assist in the provision of a meet and greet service, to monitor and maintain the reception area, ensuring a polite, efficient and professional introduction is given to all patients, staff and other visitors, maintaining a signing in/out book.
- Provide flexible administrative and clerical cover as required to
- To accurately access the patient administration system and all patients related information as required whilst assessing any risks to patient confidentiality and maintaining confidentiality
- To update the patient administration system for the purpose of extracting, amending, and in-putting data as directed in above.

- GCSE English & Maths or equivalent
- Word Processing Qualification RSA I/II or equivalent

- Ability to maintain office based systems, i.e. filing, ordering and recording.
- IT literacy - use of MS Office Packages including Word, Email, Internet, Excel and Access
- NHS experience
Skills/knowledge

- Knowledge of databases and spread sheets and able to input information accurately.
- Excellent communication skills, written and verbal
- Ability to work to strict deadlines
