Job summary
You'll support patients through a range of healthcare reviews and clinical procedures, helping them stay well and access the care they need. Based within a skilled and supportive nursing team, you will play an important role in delivering safe, compassionate care and improving patient experience. As a Health Care Assistant, you will work closely with registered healthcare professionals, patients and carers. You will help deliver routine clinical care, support health monitoring, and ensure patient information is recorded accurately. This is a rewarding opportunity to make a real difference while developing your skills within a collaborative healthcare environment.
Main duties of the job
What you'll do:
- Carry out healthcare reviews and clinical procedures such as blood pressure checks, phlebotomy, ECGs and wound care
- Support patients and carers with advice, information and compassionate care
- Record patient information accurately on electronic systems and maintain clinical records
- Prepare clinical rooms and equipment, and help manage stock levels
- Work alongside healthcare professionals to deliver high-quality patient care and support service improvement
- Level 3 qualification in Health and Social Care, or equivalent experience
- Experience working in a healthcare setting and managing your own workload
- Good basic IT skills and confidence using electronic records
- Strong communication skills and the ability to build positive relationships with patients and colleagues
- A caring, approachable attitude and the ability to work both independently and as part of a team

You will join an enthusiastic and motivated dietetic team and will work closely with the other band 5 dietitians and dietetic assistants, while receiving excellent support from our senior Dietetic team.

Job summary
Here at Mid and South Essex NHS Foundation Trust an exciting opportunity has arisen within our Ladybird Nursery.
We are looking for a caring, enthusiastic and self-motivated Nursery Nurse to join the team on a permanent basis.
Although the post is full time, 5 days per week, (37.5 hours), we are happy to consider part time hours for the right candidate . The ideal candidate will hold a recognized Level 3 qualification and have experience of working within the Early Foundation Stage as a key person.
Located in the grounds of the hospital, Ladybird Nursery is available to staff at the hospital as well as the public and cares for babies and young children under 5.

Main duties of the job
A Rehabilitation Assistant in a multi-disciplinary team is expected to work with other health and social care professionals to develop the ethos of multi-disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.As a rehabilitation assistant reporting to a registered health professional, the post holder will implement rehabilitation plans for patients in the community and other settings, observe the patient's condition, providing direct care with a rehabilitation and therapy focus. This may include supporting patients with personal hygiene and activities of a daily living.
Flexible working will be required to meet the needs of the patient group.
The ability to work over 7 days is a requirement of the post. The hours of work will be on a rota covering a shift pattern between the hours of 8.00 a.m. and 20.00 p.m.

We are seeking an enthusiastic and experienced ACP to join the South Manchester and Trafford Urgent Care Hub, supporting Urgent Community Response and Virtual Ward/Hospital at Home services.
The post holder will provide advanced clinical assessment and decision-making for patients with acute and complex health needs in their own homes and community settings. Responsibilities will include comprehensive assessment, history taking, diagnosis, treatment planning, prescribing and evaluation of care, with a strong focus on supporting people to remain safely at home and preventing avoidable hospital attendance or admission.
Working within an innovative multidisciplinary team, the ACP will collaborate across primary, secondary, community and social care interfaces to coordinate urgent and responsive care for people at risk of hospitalisation, as well as those requiring enhanced support following discharge or through Hospital at Home pathways.
Referrals may be received from NWAS, primary care, Emergency Departments, acute hospital teams and other community services. The post holder will work flexibly across South Manchester and Trafford, contributing to Urgent Community Response and Virtual Ward/Hospital at Home pathways and helping shape the continued development of an integrated Community Urgent Care Hub.
Main duties of the job
The post holder will provide specialist triage, assessment and clinical intervention as an autonomous practitioner, developing effective relationships with members of the multidisciplinary team to support patients across a range of healthcare settings, including Manchester Integrated Neighbourhood Teams, Intermediate Care Services, Out of Hours Services, community beds and primary care.
The role will focus on preventing avoidable hospital admissions, enabling patients to remain safely and independently at home, and reducing premature admission to long-term care. The post holder will support patients to proactively manage and anticipate future health needs by developing personalised care plans in collaboration with GPs, medical staff, Allied Health Professionals, nursing and social care teams.
The ACP will promote patient independence, self-management and proactive management of long-term and acute health needs, ensuring appropriate escalation and follow-up where required.
The post holder will lead the development, delivery and evaluation of high-quality, evidence-based, patient-centred interventions in partnership with relevant clinical teams. They will contribute to integrated working across community services and support the delivery of the objectives of the Community Urgent Care Division and Manchester Local Care Organisation, ensuring care is coordinated, responsive and focused on achieving the best possible outcomes for patients in their own homes and communities.

About the role

About us

An exciting development opportunity has arisen for an individual to work within the haematology clinical nurse specialist team . This would suit a nurse with a special interest in haematology and apheresis. The haematology CNS team are committed to providing specialised, excellent care and treatment and wish to continue in our drive to further improve the service to our patients. As a specialist nurse the post holder will be responsible for the delivery of specialist care and clinical management of a variety of haematology related disorders including involvement in the apheresis and transplant service.

Main duties of the job
The post holder will be responsible for providing specialist nursing advice and psychological support to haematology patients and their families.
The post holder will be an experienced nurse who uses specialist knowledge and skills to act as a resource, providing advice and education in relation to all aspects of haematological care to health care professionals across all settings.
Be an active member of the RCHT Haematology Transplant Service, facilitating apheresis services.
The post holder will work under the supervision of the Band 7 Haematology Clinical Nurse Specialists.

This is a post for a Registered Nurse to work on The Headland Unit - a busy Monday to Friday outpatient treatment area delivering Systemic Anti-Cancer Therapies (SACT) including chemotherapy and immunotherapy. Knowledge and Qualifications
Essential
- Educated to GCSE level or equivalent, with Grade C or above in English and Maths.
- Knowledge of administrative procedures, acquired through training and/or previous experience of working in an administrative environment.
- Awareness of the requirement to maintain confidentiality, and knowledge of the Data Protection Act.
Desirable
- NVQ Level 3 in Business Administration, Customer Service, Healthcare Support Services or equivalent qualification.
Experience

- Experience of working in a non-routine environment, with demonstrable evidence of being able to respond to queries and deal with unpredictable situations as they arise (acquired through experience and/or training to Vocational level 3 or equivalent).
- Experience of working in a customer facing environment, with knowledge of a comprehensive range of administrative procedures.
