Main duties of the job
You will be a Nurse working within a multi-disciplinary team and carry out a diverse range of duties. As a Band 5 Nurse within the team you will be regularly supported and supervised by a nominated Band 6 Nurse Specialist and you will be encouraged to take personal responsibility for clinical supervision and your continued professional development plan.
You will undertake a period of preceptorship overseen by a named Band 6 Nurse Specialist within your team and engage on the trusts Preceptorship programme.
You will actively promote teamwork, respect, integrity, accountability, excellence and compassion within the clinical team in accordance with the Trust values and Code of Conduct. You are required to maintain links with your professional body and provide evidence for the revalidation process.You will provide nurse leadership within the multi-disciplinary team and ensure the effective assessment, formulation, planning and monitoring of care given to service users.
You will be required to offer clinical and managerial leadership to junior Health Care Staff and Student Nurses as part of your role within the team. You may be required to take on tasks that require you to operate electronic staffing record systems, authorising periods of leave, sickness and absence for junior nursing staff.

Job summary
An opportunity has arisen to join Secure Services Guild Lodge as a Forensic Discharge Pathway Facilitator (Inpatient Social Worker). The post holder is required to be a qualified Social Worker with Social Work England registration.
The Discharge Pathway Facilitator (Inpatient Social Worker) is an integral member of the Multi-Disciplinary Team. The Post Holder will work in close partnership with a range of agencies and organisations internal and external to LSCFT to offer social care input. They will take a lead role from pre-admission by completing a holistic social work assessment, identifying any social work needs, undertake family work, and support with discharge planning. This includes early identification of barriers to discharge, taking action to prevent any unnecessary delay to discharges.
Secure Mental Health inpatient care for Lancashire and South Cumbria is provided at Guild Lodge, Whittingham. The service has steadily expanded from it's inception in 1999 and currently has a range of inpatient and community services.
Guild Lodge has 144 beds and is made up of 3 Care Pathways:
Men's Care Pathway - Men's Medium secure, Low secure and step down wards
Women's Care Pathway - Women's Medium and Low secure wards
Forensic Brain Injury Service - Men's Medium and Low secure wards
Guild Lodge hospital site is situated in a beautiful rural area, yet close to motorway links from all directions of travel
Main duties of the job
This post will be suited to a Social Work England registered Social Worker with an interest and knowledge of Secure Mental Health and Offender Health.
The Post Holder will work in close partnership with a range of professionals, agencies and organisations both internal and external to LSCFT to ensure effective social care provision and discharge processes within Guild Lodge.
The Post Holder will take a lead role in providing a social care service to patient at Guild Lodge and facilitating discharge planning. This will include:
Demonstrates specialist skills to ensure all necessary assessments for a complex patient group for example Health and Social Needs assessments are completed in a timely fashion.
Demonstrates knowledge of complex mental, physical health, and Learning Disability and Autism needs.
Support with identifying the need for capacity assessments and liaise with the Local Authorities or the Decision Maker about this.
Attend Multi-Disciplinary meetings for example, Care Team Meetings, Care Programme Approach and Best Interest's meetings, and MAPPA meetings.
In-depth knowledge of individual patient's needs, requirements and wishes which will be used to formulate in depth assessments to facilitate funding applications and discharge into the community.

Bayman ward is a 25 bedded ward providing a service supporting stroke rehabilitation. The ward is nurse led, supported by Medical Practitioners and provides a progressive service which promotes multidisciplinary working. Main duties of the job
The successful applicant will be required to integrate flexibly across the ward with a particular focus on communication with the MDT to support a team approach to care. A key requirement of the role will be to deliver safe high quality care that meets the needs of increasingly complex patients in the community wards. This will involve joint working between all of our inpatient units and supporting community services to manage patient pathways safely and effectively and ensure that robust discharge plans are in place that support patients to live their lives as independently as possible on discharge. There are close working relationships with medical practitioners and MDT which encourage a seamless and proactive approach to planning successful discharge.
The successful candidate will also need to have an academic qualification of NVQ Level 3 or higher and with previous Health Care experience and need to demonstrate that they are highly motivated and able to provide high quality patient care. Experience of team working is also essential.
You will be required to have excellent communication skills. Previous experience of nursing in an elderly care / rehabilitation environment desirable.

Job summary
We are looking for an experienced Counselling/Clinical Psychologist/Psychological Therapist to join our Trust-wide Specialist Eating Disorders Services (CEDS) for Adults. This post will be based at least part of the working week in North Devon (Barnstaple) and also at our central team base in Exeter so the post holder will need to be prepared to travel and be flexible to meet these requirements. The successful candidate will work as a senior member of the clinical team to support effective care for clients often with complex eating disordered presentations.
CEDS is a fully integrated multi-disciplinary team which includes medical staff, senior mental health professionals, psychological therapists, practitioner psychologists, assistant psychologists and dietitians. It is a caring and supportive team with regular MDT meetings, reflective practice and service development team days. Our vision is to provide the best treatment possible for those with eating disorders through the provision of treatments recommended by NICE, and by working together across inpatient, outreach and community services to provide the most appropriate care.
As a senior member of the clinical team the post holder will be involved in all aspects of assuring the delivery of the psychological therapies provision: we would particularly welcome someone with previous training and experience in CBT-E or MANTRA but there are opportunities for funded therapy training available for the right candidate.
Main duties of the job
You would be providing assessments for clients referred to service, particularly where there is comorbidity and complexity, in order to determine the most appropriate treatment pathways.
You will be responsible forproviding specialist psychological interventions to clients with eating disorders and their families/carers
You would be offering clinical supervision to psychological therapy colleagues, trainees and assistant psychologists and there may be opportunities to line manage staff as part of career development.
The role involves working together with multi-disciplinary colleagues, offering specialist clinical supervision and consultation to the MDT and across the wider mental health service.
Prior experience in working with people with severe and complex eating disorders would be highly desirable and a strong commitment to increasing access to evidence based psychological interventions.
The candidate will have a degree in psychology and doctoral level training in applied psychology (or its equivalent for those who trained before 1996 or outside the UK) as approved by the Health Care Professions Council (HCPC) or will be a senior experienced psychological therapist with appropriate accreditation with a relevant professional body e.g. HCPC, BABCP, UKCP.

Due to the new national review of POSCU services we have the exciting opportunity to expand our team. The Royal Surrey NHS Foundation Trust is moving towards being an Enhanced Level A POSCU for Children and Teenagers with Cancer. To support the development of this service, we have an exciting opportunity for an experienced nurse to undertake the role of Paediatric Oncology Sister for Children and Teenagers with Cancer.
You will need to be an RN Child and have a high level of up-to-date relevant clinical knowledge and experience in paediatric nursing at senior band 5 or band 6, a 1st Level Nursing degree and experience of caring for children and teenagers with cancer and their families. An accredited post registration chemotherapy qualification in this specialist area of nursing practice is desirable or the willingness to undertake the training.
Main duties of the job
The post holder will work closely with the Lead Nurse and Clinical Nurse Specialist for Children and Teenagers with Cancer to continue to develop a high quality service for children and teenagers with cancer and their families. You will work as part of the team to ensure the safe care of paediatric oncology patients on the Children's Ward and administration of chemotherapy. You will have a key role in the development and delivery of teaching programmes for staff.
Excellent communication skills will be needed to work across both professional and organisational boundaries in order to ensure co-ordinated patient care for these children, teenagers and their families. You should also be confident and self-motivated utilising the benefit of your clinical knowledge and experience and be able to act as a resource to all levels of healthcare professionals and support staff.

We are looking for an experienced Band 6 Occupational Therapist to join our Integrated Discharge Team (IDT), working across A&E and other acute assessment areas.

Main duties of the job
You will assess working-age and older adults presenting in crisis, considering the physical, functional, environmental, social and psychological factors affecting their ability to return home safely.
You will work closely with A&E, acute medicine, acute frailty, adult social care, Hospital at Home and community services to develop person-centred plans and support positive risk-taking.
We particularly welcome applicants with experience in A&E, SDEC, acute assessment, acute frailty, front-door therapy, admission avoidance, UCR, or Hospital at Home.
You will work autonomously within a supportive team, with responsibility for clinical decision-making, supervision of junior staff/students and contributing to service development.
The service is supported by highly specialist Band 7 OTs and an experienced team lead, providing excellent opportunities to develop your clinical reasoning and front-door assessment skills.

We are recruiting IT Service Desk Analysts to complement our existing team, ensuring that we can respond to the demands our busy hospital services across Guildford and the surrounding areas.Our Digital Service Desk Analysts are the first point of contact between Trust users and the IT Department and are responsible for handling all 1st line technical support issues. Duties primarily include the logging, triaging and progressing of calls right through to resolution, ensuring our customers' needs are satisfied at every opportunity. Could this be you?
Whilst an understanding of NHS environments and systems is desirable, on-the-job training will be given to the successful candidate. Currently based in Guildford, Surrey the successful candidate(s) will have a flexible attitude and be expected to cover the core hours 8.00am to 5.00pm, Monday to Friday, as part of a shift pattern to a maximum of 37.5hrs per week.
Main duties of the job
o To provide high quality 1st line technical support to all users connected to the IT environment using diagnostic tools and fault-finding techniques to identify root causes. This may be by telephone, email, remote support or face to face.o Troubleshoot authentication, hardware & software issues for a variety of clinical and business applications & devices.o To fix approximately 80% of incidents by telephone and to escalate the ticket to specialist teams in a timely manner when not resolved in 1st line.o To gain accurate information from callers, using effective communication skills to assess the impact of the reported incidents or requests, recording this information on the Service Management tool.o Make use of internal and external resources (knowledge bases, support websites, vendors) to answer questions and resolve service issueso Contribute to the creation, facilitation and maintenance of FAQ documents, knowledge articles and user guideso To Create and maintain user network accounts in Active Directory and other supported applicationso Ensure the Incidents and Service Requests are managed effectively on behalf of end-users throughout the full lifecycle and responding to status enquiries when appropriate.o Be responsible for managing own workflow through the department using the IT Service Management tool, ensuring incidents and Service Requests are resolved within the agreed SLA.

As an Infection Prevention Specialist, you will provide expert IPC advice, support and education across Provide Group, working with clinical and non-clinical teams to reduce healthcare-associated infections, manage risks and improve patient safety.
Key Responsibilities- Provide specialist IPC advice and support across services.
- Support national and local IPC policies, standards and surveillance programmes.
- Undertake audits, analyse data and identify risks, trends and improvement opportunities.
- Support infection incident and outbreak investigation and management.
- Deliver IPC education, training and awareness programmes.
- Contribute to the annual IPC Programme, improvement plans and assurance reporting.
- Undertake clinical reviews, walkrounds and audits.
- Work with multidisciplinary teams to identify and mitigate IPC risks.
- Promote evidence-based practice, including hand hygiene, PPE and antimicrobial stewardship.
- Challenge poor practice and support regulatory compliance.
- Champion patient safety, learning and continuous improvement.
You will be a registered healthcare professional with an interest in IPC, patient safety and quality improvement. You will have strong communication, influencing and analytical skills, with the ability to work collaboratively, interpret data, provide evidence-based advice and support sustainable change. Experience in IPC, audit, surveillance, education or quality improvement is desirable.
