The Enhanced Outreach Team are looking to recruit to 2 full-time band 5 Community Mental Health Practitioners.
The successful candidates will:
Act as a named nurse / care co-ordinator, to be responsible for the assessment of needs and the planning, delivery and evaluation of care for these patients and to deliver specific interventions according to assessed need.
Oversee the nursing care process and to provide direction and supervision to junior staff in meeting patient needs.
Provide supervision and leadership in respect of maintaining and developing standards of practice and implementing change.
Main duties of the job
To act as named nurse / care coordinator for a group of patients ensuring that appropriate interventions are planned and delivered according to assessed need. To promote a positive nurse patient relationship and to engage with carers / relatives as appropriate. To have responsibility for documenting the plans of care and ensuring that plans remain current and that standards of record keeping are adhered to.
To administer and manage medicines according to policy and local protocol. To assess patient response and provide advice and education to junior staff and both patient and carers in respect of prescribed medicines.
Provide support and guidance to junior or less experienced staff in the planning and delivery of nursing care for individuals for whom they are named nurse / worker. To provide support to patients within this group at times when the named nurse / worker is unavailable. E.g. periods of annual leave sickness etc. To take an active role in auditing and maintaining standards of record keeping for this group of patients.

Main duties of the job
Deliver high quality CT and/or MRI imaging, ensuring safe, efficient and patient centred examinations across both Musgrove Park Hospital and the brand new Bridgwater Community Diagnostic Centre.
Rotate through our state of the art CDC, working with the latest scanners and streamlined pathways to support faster diagnosis and improved patient access.
Use advanced clinical skills to plan, perform and adapt a wide range of cross sectional imaging examinations, making real time decisions to optimise image quality.
Cannulate patients and administer contrast safely in line with PGDs and Trust protocols.
Prioritise and coordinate daily workflow, ensuring smooth patient throughput and effective use of scanner time.
Work autonomously within a supportive team, contributing to extended hours and oncall services as required.
Support and mentor junior radiographers, students and support staff, helping to build skills and confidence within the team.
Communicate clearly and compassionately with patients, relatives and colleagues, ensuring excellent patient experience throughout the imaging pathway.
Contribute to service development through audit, quality improvement projects and the implementation of best practice.

Bud Flanagan East Ward is a 16 bedded unit caring for patients with Leukaemia, Myeloma and Lymphoma throughout their treatment pathway. Main duties of the job
This role will lead the Trust's costing workstreams including the annual National Cost Collection submission and Service Line Reporting using benchmarking information alongside PLICS to support service delivery.
Reporting to the Head of Strategic Finance you will be responsible for managing the Costing and Sustainability Assistant and will work closely alongside colleagues in planning, contracting, and the wider finance team, as well as clinical specialists.
You will play a key role in supporting colleagues to understand their responsibilities in relation to the statutory returns as well as highlighting areas within services which may require further investigation. Your input will be essential in embedding the principles of costing within the Trust and promoting savings opportunities.

Main duties of the job

These include stock receipt, stock counting & ordering (top-ups), medicines order assembly, and distribution. Stock control procedures and stocktaking. The cleaning of aseptic facilities and the disposal of pharmaceutical waste. Assembly, packaging and over-labelling pharmaceutical products. There are no line management responsibilities. All duties are performed working to procedures, mostly under direct supervision. The jobholder will work as a member of a team of Support Workers. To promote a friendly and supportive atmosphere for parents and relatives in which they can learn to care for their babies. To utilise a variety of strategies to communicate effectively: with the neonate's family/carers, the multidisciplinary team, other departments within the Trust and pertinent agencies outside the organisation. Support the nursing team in proving advices on breastfeeding, developmental care, family integrated care and parent education.

You would be joining a comprehensive and dynamic CAMH Service, delivering a substantial, stepped model of care following the needs-led i-Thrive framework. TEACHING AND ASSESSING

Main duties of the job
We are looking for someone with a compassionate nature who can communicate successfully with people who have different abilities and presentations. They will be a team-player who can undertake some duties independently and at times act upon their own initiative. Knowledge of social inclusion and housing and benefits would be an advantage, alongside having excellent workload management skills.
The occupational therapy team consists of qualified Occupational therapists, Occupational therapy assistants, and Recovery Support Workers, all of who focus on enabling patients to achieve optimum independence as part of recovering their ordinary lives. The senior staff offer regular clinical, professional and line management supervision as well as opportunities to attend internal and external training.
Working directly with service users for at least 70% of the time, this post holder will support occupational therapists to achieve this aim through supporting the assessment and analyse of strengths and needs, providing interventions, reporting on progress, and supporting service users to care to achieve their personal goals and aspirations.
Service requirements may involve some flexibility of work hours and the post holder is expected to respond to service user need relating to this.

Main duties of the job
The successful applicant will provide essential support to Biomedical Scientists within the Microbiology laboratory, assisting with the preparation of biological samples and the operation of laboratory analysers. They will have responsibility for supervising and supporting the development of Senior Associate and Associate Healthcare Scientists (Bands 2 and 3), including training and day-to-day oversight.
The role offers opportunities for further development, with additional training provided to enable the post holder to take a lead on defined aspects of service delivery, such as analyser management, Health & Safety, and Quality Management. The post holder will also contribute to more complex scientific investigations and undertake basic interpretive tasks in support of Biomedical Scientists. A commitment to Continuous Professional Development (CPD) is expected to ensure ongoing competence and professional growth.
The laboratory operates 7 days per week, with core working hours of 08:30 to 17:00. The post includes participation in a rota covering a range of shifts, including late shifts up to 19:00, 1 in 4 weekends, and bank holidays. We are happy to consider flexible working arrangements within core hours.

The Orchard is a stand-alone unit based in Lancaster. It is a 16 bedded male acute unit that specialises in the assessment and treatment of mental health conditions. This enables people to return to their communities and allows them to regain their independence.

The successful candidate will bring excellent role modelling skills, a passion for supporting and developing junior colleagues, including student nurses and preceptors, and they should have a keen interest in quality improvement projects that can lead to improvements in patient care and staff wellbeing.
You should bring with you experience of nursing people with acute mental health needs, as well as some experience of providing high-quality supervision, an awareness of not just how to complete an audit but what to do with the data an audit provides us with, and be keen to develop your own skills once in post.

Inpatient wards and units are designed to meet the acute holistic needs of service users and patients and carers, in a recovery focussed environment. This working environment is conducive to staff development and wellbeing.
