- delivers high standards of patient centred care over both sites of the Downend Health Group (DHG).

- have an understanding, acceptance, and adherence of the need for strict confidentiality and clinical governance.

Job summary

Avon and Wiltshire Partnership (AWP) are seeking to recruit individuals dedicated to the community support of people with Learning Disabilities and or Autism that are at risk of hospital admission due to mental ill health.
The Kingfisher Outreach Team is an initiative that will provide specialist advice and support to patients and their care network.
Working as an integral member of the clinical team you will support the implementation of strategies that reduce likelihood of inpatient admission and provide time limited post-discharge follow-up to facilitate the persons return to the community from hospital when admission has been required.
Applicants will demonstrate, through application form and interview, at least two years of relevant experience (Learning Disability/Autism/Mental Health) excellent verbal, non-verbal and written communication skills and an ability to work flexibly to meet the needs of the service. Driving licence and access to own transport is essential for this role.

Main duties of the job
The principal purpose of the job is to provide high levels of support to individuals (and their networks of support) with Learning Disabilities and/or Autism who are at risk of admission to The Kingfisher Acute Psychiatric Inpatient Services and to also support effective discharge following an inpatient stay. The CAMHS Getting Advice Admin team are looking for an enthusiastic and organised lead administrator who will be based at Barton Hill

Job summary
The Specialist Health Advisor (SHAS) for Special Educational Needs and/or Disability (SEND) team support the Bristol, North Somerset and South Gloucestershire (BNSSG) Integrated Care Board (ICB) and Designated Clinical Officer (DCO) for SEND to ensure that they fully discharge all of their statutory duties for children and young people (CYP) with SEND aged 0-25.
The post holder will work within a multi-disciplinary team responsible primarily for providing a health contribution for children and young people with Special Educational Needs and / or Disability (SEND) as part of their Education, Health and Care (EHC) needs assessment.
This role is across BNSSG and the post-holder will be required to maintain accurate records and report on data which will be reported to the ICB at the regular contract performance meetings and which will inform commissioning decisions for the population of BNSSG.
Main duties of the job
To offer clinical expertise within your clinical background area.
To manage and carry own EHCNA health assessment case load.
To be part of an integrated multi-disciplinary team (MDT), which may include Paediatrician's, Orthotists, Nurses, Occupational Therapists, Speech and Language Therapists, and Physiotherapists, CAMHS Practitioner.
To have knowledge of and be able to signpost to suitable resources and services across local authority areas which will support parent, carers, children and young people in relation to SEND.
The post-holder will shadow band 7 colleagues in quality assurance panels where health contributions from each LA area are scrutinised, and contribute with cascading any learning back to the organisation and individual teams involved whilst continuing to champion best practice.

We practice Family Integrated Care and have special interest groups for Infant Feeding, Developmental Care and Palliative Care. HOPE and SHED parent support groups are well embedded on the unit.

Job summary
We are looking to recruit a full-time float Ward Clerk (37.5 Hours p/w - Monday to Friday. 08:00 - 16:00 TBC) on a fixed-term contract of 12 months. Bristol Autism Spectrum Service (BASS) provides specialist assessment, formulation, and intervention for autistic adults and their families/carers across Bristol, North Somerset and South Gloucestershire (BNSSG).

The Corporate Referral to Treatment Validator will assist the Trust and Divisions to deliver high quality, accurate patient data related to 18 week pathways (RTT) and/or average waits.

The Referral to Treatment team is responsible for tracking, reporting and monitoring of patients on their 18 week Referral to Treatment pathway, working with Specialities across the Trust ensuring accurate reporting and assisting with RTT queries.

Job summary
The post holder is a member of the Psychology Services department at Southmead Hospital, located within both the weight management and bariatric surgery teams.
They provide expertise in the assessment and behavioural management of weight and also in supporting patients who are working towards either medical and/or surgical weight management. Responsibilities include assessments of people referred to the service and the provision of psychological interventions in both group and one-to-one format. The post holder is part of multidisciplinary teams made up of staff who are passionate about this area and who aim to offer deep compassion to people who may have experienced high levels of weight stigma.
The main themes of the work include disturbances in relationships with food and eating, body image and self-esteem difficulties, emotion regulation and coping with the long-term psychological effects of trauma. This is predominantly with people attending as outpatients, but there can occasionally be some inpatient-based work as well. The post demands the flexibility to adapt work across these different areas as priorities change, such as with the incorporation of new pathways within the service.
Main duties of the job

To conduct psychological assessments of patients referred to the service who are living with obesity and other long-term health conditions, including the assessment of risk to enable appropriate risk management plans to be drawn up.

To be able to draw together multiple sources of assessment information, identifying critical components, in reaching an interpretation and rationale on which interventions can be planned and carried out.
