This is an exciting opportunity for an experienced Advanced Clinical Practitioner (ACP) to join our Vascular Service at Sheffield Teaching Hospitals NHS Foundation Trust. Main duties of the job
As an Advanced Clinical Practitioner within the Vascular Service, you will work autonomously as a senior clinical decision-maker, providing expert assessment, diagnosis, treatment, and management of patients with a wide range of vascular conditions. You will play a key role across all four pillars of advanced clinical practice: clinical practice, leadership and management, education, and research.
Working as an integral member of the multidisciplinary team, you will contribute to service development, quality improvement, workforce education, and the delivery of high-quality, patient-centred care.

Job summary
Discharging an individual on to the right care pathway when they no longer need to remain in a hospital or community bed requires a whole system approach. Every local health and care system has a transfer of care hub where (physically and virtually) relevant community services across sectors (e.g. health, social care, housing, voluntary sector) are linked together.
The hub coordinates care for people who require care and support after discharge from hospital, and any support for unpaid carers providing care. This covers any patient discharged from hospital or community beds regardless of the area they are resident in. The hub is staffed by a small team, dedicated to ensuring people are discharged from hospital on the right pathways, with the right discharge information, and that they get the right onward care and support if needed.
The hub supports safe discharges through close working with the acute and community wards, quality assurance of information and practical support, including proactive early identification of those who may become ready for discharge, supporting planning as early as possible following admission. This role within the Care Transfer Hub will provide the skillset of an Occupational Therapist who will take a lead in supporting patient discharges that have complexity around function and / or cognition.
Main duties of the job
Accountable for the delivery of high quality therapy care within a defined clinical area or pathway.
Responsible for providing expert clinical practice with advanced clinical reasoning and support to staff.
Provide clinical leadership to develop staff.
Lead evaluation and development of clinical practice within the specialism ensuring compliances with National Frameworks.
Assist and support Clinical Service Managers and Lead Therapists in the service in the improvement of AHP services within STH.

Job summary
Within the Care Transfer Hub, the Occupational Therapy skillset takes a lead in supporting patient discharges that have a complexity around function or cognition or where there is lack of clarityaround which pathway is the best fit for a patient leaving the hospital.
The Therapist problem solves where there are challenges around discharge in 4 main areas that require a holistic approach and knowledge of overcoming functional or cognitive issues whilesupporting reablement and maximizing independence.
Main duties of the job
Assisting in discharge planning where there are complex moving and handling needs. Particularly where patients have specific equipment requirements to support reablement at home. For example, bariatric patients where knowledge of equipment and function isneeded to support optimal independence. This patient group may include identifying those who fit the criteria for proportionate care.
Overseeing discharges where cognitive impairment requires problem solving to facilitate pathway 1 back home. For example, when delirium and or dementia presents and re orientation at home is likely to improve function.
Where pathway 3 is not suitable but concerns remain from family and MDT. An understanding and support of ongoing needs and risks is required. Managing discharges where functional challenges could be overcome to facilitate pathway 1. This can then beexplained to patient family and carers to gain understanding and offer reassurance and support.
Early identification of patients with pre-existing complex challenges at home. Including those with previous extensive community involvement. They may have multiple health and or social needs. Where a sound understanding of the previous function and communicationwith community services is needed. Using clinical reasoning and problem solving to implement early discharge planning.

We are looking for a keen, enthusiastic , and experiences Specialist Biomedical Scientist with experience in Blood Sciences and Microbiology, and an interest in developing services to help us continue to deliver exceptional, high-quality integrated care to our patients. WVT is an excellent place to train and develop new skills, and further study will be supported.
Main duties of the job
The post holder will provide operational, scientific and strategic advice on decisions for POCT as a member of the POCT department Management Team. This will include day to day management of POCT making efficient use of available resources to maintain a cost efficient and high standard of service in conjunction with the POCT Manager.
They post holder will be responsible for the implementation and adherence to policies to meet the requirements of ISO 15189, reporting on a regular basis to the POCT Manager and departmental Governance meetings.
The post holder will be required to deputise for the POCT Manager in periods of absence and may be required to provide support outside of routine hours.

Job summary
We are looking for an individual who has a special interest in Upper GI cancer care. An individual who is dynamic, organised, flexible, reliable & compassionate to support patients and their families /carers with an UGI cancer. This would involve patients with an oesophago-gastric cancer diagnosis, through treatment pathways.
The post holder will be an effective core member of our Upper G.I Multidisciplinary team (MDT) demonstrating the highest standard of advanced supportive care in the management of patients with an UGI cancer.
They will be responsible for working as an autonomous expert practitioner with support of the senior CNS team, able to make critical holistic clinical decisions through assessment, monitoring and planning. Facilitating timely investigations, coordinating care & providing information to enable patients to make informed choices.
A high proportion of our patients diagnosed with an UGI cancer have a palliative diagnosis therefore the post holder will need to be prepared for the distressing nature of this role and have mechanisms in place to manage their psychological wellbeing.

Main duties of the job
- Work as part of the Children and Young Peoples Integrated Therapy team under the direction of a Band 7 Operational Lead. You will function as an autonomous clinician with a role in supporting the delivery of the CYPIT service.
- Deliver a high standard of speech and language therapy care to children with developmental and complex needs. This could be in a range of settings, including clinics, educational settings, homes. You will evidence a high level of problem solving, reasoning skills and independence of judgement, using evidence-based practice and person-centred principles.
- Undertake clinical sessions, and manage a caseload as required, offering direct assessment, advice, signposting, intervention and training and support as required by the integrated therapy model.
- Investigate, interpret and analyse clinical and non-clinical facts to form accurate clinical diagnosis, evaluation and prognosis as part of the comprehensive assessment of patients, particularly those with a highly complex presentation, and to recommend the best course of intervention.
- Function as an autonomous clinician, supporting other more junior staff and accessing high level, specialist advice in highly complex cases, including child protection and safeguarding.

The Children's Wellbeing Practitioner (CWP) Service at Hertfordshire Community NHS Trust (HCT) has an exciting opportunity to train a new cohort of 2 trainee Children's Wellbeing Practitioners (CWPs), who will cover a variety of areas in Hertfordshire.
Post holders will undertake training with University College London (UCL) alongside working in the HCT CWP service . Main duties of the job
You will study at University College London/Anna Freud National Centre for Children and Families, with tutors that are renowned for their great work with children and families in the area of emotional wellbeing and mental health. The Children's Wellbeing Practitioner (CWP) Postgraduate Diploma will begin in January 2027 and the course lasts 1 year. As part of your training you will use the theory that you learn to deliver low intensity mental health interventions to adolescents and the parents of children presenting with mild anxiety, low mood and behaviours that challenge.
In service, you will initially shadow intervention and assessment sessions; shadow and take part in triage discussions and partake in clinical skills development and supervision. You will then be expected to complete initial assessments and deliver interventions for anxiety, low mood and behaviours that challenge with the support of your supervisor. Additionally you will attend weekly supervision and training sessions; engage in service development activities and deliver workshops and groups with qualified members of staff.

We are looking for dynamic, enthusiastic and creative Therapy Assistant Practitioners to be a part of our well-established Occupational Therapy (OT) and Physiotherapy (PT) team. Main duties of the job
Children's Occupational Therapy and Physiotherapy is based on a 3 Tier model of service delivery, with provision at Universal, Targeted and Specialist tiers. We offer assessment and treatment to children and young people with a wide variety of conditions, including cerebral palsy, autistic spectrum disorder and developmental coordination disorder. We also carry out home assessments and advise on equipment, housing adaptations and rehousing. The team works closely with colleagues in the wider multiagency team supporting children, young people and their families to achieve their maximum potential.
Continuing Professional Development is actively supported through access to training, regular supervision, peer support meetings and staff appraisal scheme.

We are seeking an experienced and motivated Cardiac Physiologist specialising in cardiac rhythm management to join our busy friendly team.
In this role you will lead by example to provide an excellent service for patients of all ages who have received, or are about to receive, an implanted cardiac device. As a leading Cardiothoracic centre we are exposed to a vast array of complex medical conditions and the opportunity for you to grow within the role is extensive.

Main duties of the job
The Experts at Hand model is a key part of the local SEND Reform Plan, which focuses on providing earlier specialist advice, consultation and intervention through an integrated multidisciplinary model. Our Band 7 SLT roles will help to enable the service to continue to shape the support offered to children under 5 years who are at risk of/present with SLCN.
As an Early Years specialist, you will:
work collaboratively with Early Years settings, strengthening relationships with education professionals (within Hertfordshire DSPL areas)
work in partnership with our SLT Education Leads, Clinical Leads and Clinical Quality leads, to help develop and shape early years interventions
develop networks with our colleagues in Family Centres, and continue to support delivery of our universal and targeted offer
seek regular feedback from Early Years settings and be responsive/make changes to service provision that will benefit the wider Hertfordshire system
work with therapy teams and settings to provide training and specialist advice and support
support the senior management team at a strategic level

We have a very successful model of SLT Education leads across each of our 9 DSPL areas in Hertfordshire, who provide systemic support to schools. Our CYP Therapies service is jointly commissioned by health and education, allowing us to provide a seamless pathway of care from early years through to schools, specialist provisions and further education settings. We have excellent joint working relationships with our partners in education and a wide range of stakeholders, and we work in partnership with them to deliver interventions at Universal, Targeted and Specialist levels across Hertfordshire.

The Experts at Hand model will help our Childrens SLT Service to continue to build on our strengths and the excellent support services we already have in place. We have a very successful model of SLT Education leads across each of our 9 DSPL areas in Hertfordshire, who provide systemic support to schools. We currently have Early Years Band 7 SLTs in post currently who are offering fantastic support to our wider childrens workforce but we want to build on our model and create more roles for Early Years so that we can continue to focus on creating the best outcomes for children.

Job summary
This is a front line position and you will often be the first health care worker that the patients see in our department so it is therefore essential that you are a good communicator and can greet each individual with respect and dignity.
As an overview of the predominant tasks include:
- Meet and greet patients upon arrival, making them feel safe and secure in our care across the two acute hospitals in our Trust.
- prepare patients for their examinations/procedures. This includes providing clear instruction,assisting with changing, cannulation and radiation protection screening.
- perform administrative and house keeping tasks to facilitate effective service delivery
- monitoring patients for the duration of their visit, including vital signs observations as required
-support the nursing team in providing quality patient care in two Interventional Day Case Units across sites.
- Transfer of patients to and from the department as well as in and out of examination rooms and onto specific imaging tables.
- contribute to a clean and safe department, complying with policies and reporting faults quickly
- working within a multidisciplinary team to provide essential services to almost every patient that visits the hospital
Additional working pattern details: Mon - Fri shift patterns 8.00-16.00
Main duties of the job
- To deliver direct patient care in line with Trust values caring, listening and excelling and how this relates to yourself and patients.
- Support the nursing team to delivery high quality care for patients in Day Case Units and the IR labs.
- Undertake patient observations and documentation in line with the NEWS (national early warning score) system. Respond to signs of a deteriorating patient and alert the appropriate team if a need for emergency intervention is required following the departmental protocol.
- Perform Cannulation and removal of cannula after appropriate training.
- Support the interventional procedures in all modalities through competency in sterile trolley preparation and decanting and assisting as required.
