We are looking for an Administrator to join our Integrated Neighbourhood Team (INT) in Woodspring!
The INT is a multi-disciplinary team who assess and manage a caseload of patients with varied and complex needs. You will support clinicians in a wide range of administrative tasks. The post holder will offer administrative support across the locality and will need to be flexible with regards to working across bases.
The post holder must be able to communicate effectively with service users and colleagues alike and able to express yourself equally well verbally and in writing. This is a busy role with a need for excellent IT skills and the ability to learn new IT systems efficiently.
You must be adept at multi-tasking and excellent at managing your time and prioritising tasks to facilitate the smooth running of this key team function.
As part of your role, you will be expected to work some weekends and bank holidays. This vacancy is aimed to cover our late shifts finish at 8 PM.
Travel to other bases within our locality in north somerset will be required.

Main duties of the job
- Provide administration support to the Integrated Network Team and Administration Leads as required
- Use Community Electronic systems i.e. EMIS, ESR, Health Roster
- Manage telephone and e-mail queries
- Booking clinician appointments and clinics
- Place orders, manage deliveries and replenish stock cupboards
- Support with sickness logging and reporting
- Assist with booking/authorising bank shifts
- Maintain spreadsheets and electronic documents
- Prioritise workload, work without direct supervision
- Reserve meeting/clinical rooms
- Complete regular audits and stock checks
- Attend meetings and training as required
- Circulate information to staff as directed by the management team, ensuring distribution lists are kept up to date
- Report any Health and Safety issues to Admin Lead
- Record Compliments/Complaints and Adverse Events

Main duties of the job
This is a unique opportunity to help build a clinical service rather than inherit one. Working closely with our wider clinical and operational leadership teams, you will help shape the physiotherapy provision at Active Neuro Manchester, contributing to service development, clinical pathways, quality improvement and the delivery of excellent outcomes for our clients.
As Clinical Lead Physiotherapist, you will provide clinical and professional leadership to the physiotherapy service, ensuring the delivery of safe, effective, evidence-based and person-centred neurological rehabilitation. You will maintain your own clinical caseload while providing specialist assessment, advice and treatment, and will play an important role in ensuring consistency and quality across assessment, rehabilitation planning, goal setting, outcome measurement and clinical documentation.
You will also have responsibility for supporting and developing the physiotherapy team, providing clinical supervision, guidance, training and mentorship to junior physiotherapists, Therapy Assistants and students as appropriate. Working as an integral member of the interdisciplinary team, you will contribute to team meetings, client reviews and service development, helping to establish collaborative ways of working across the service.

An exciting opportunity has arisen to work as a Senior Heart Failure Specialist Nurse in the Sirona Community Heart Failure Service.
We are looking for a motivated and passionate clinician to join our team.
Our community-based specialist service works to improve outcomes for patients living with heart failure. By working as part of a wider Multi-Disciplinary Team we strive to deliver best evidence care that is individualised for our patients. We work closely with our partners in Primary and Secondary Care, providing early treatment intervention and supporting the transition from hospital to community management. The service also offers an advice and guidance to other Health Care Professionals across Bristol, North Somerset & South Gloucestershire.
There will be a requirement to work across BNSSG to meet service delivery, but we look to deliver clinical caseload elements of the service within geographical locations with bases currently within Bristol and North Somerset and South Gloucestershire.
This role is open to clinicians currently working at Band 7 level looking for a change in role, or as a career development opportunity for Band 6 clinicians. All applicants should have experience working within cardiology or heart failure pathways.

Are you a Nurse, Therapist or Paramedic passionate about supporting people to return to their own homes following acute illness or injury?
Bristol NHS Foundation Trust have a Transfer of Care Hub at Weston General Hospital where the aim is to support patients with discharge planning from admission through leaving hospital and into the community. The Transfer of Care Hub brings together the Acute Trust, Community Partners (Sirona), Local Authorities and Voluntary Sector colleagues across BNSSG to support complex discharge planning.
Sirona Community Transfer of Care Hub (CToCH) are the community experts in discharge planning working together with the Transfer of Care Hub partners to navigate patients to the most appropriate community discharge pathway.
As a case manager you will need to have experience working in a community setting and an understanding of the levels of clinical acuity that can be supported in peoples own homes. Alongside this you will have an understanding of rehabilitation pathways, discharge planning and mental capacity issues, excellent interpersonal and negotiation skills.
In return, we will provide you with regular training and supervision and enable you to become involved in future service developments.
This role is based mainly in Cadbury Garden Centre Offices with inreach days into Weston General Hospital.
Main duties of the job
- Be a key member of the multi-disciplinary team, responsible for providing person-centred, holistic assessment to facilitate discharge from the acute hospitals to the right discharge pathway.
- Responsible for triaging referrals received by CToCH. This may include liaising with the patient, family/carers and ward staff to identify the most appropriate discharge pathway.
- Support the CToCH to discuss complex cases with all partners in the Transfer of Care Hub to agree a joint action and discharge plan.
- To utilise multi-disciplinary assessment skills, supported by sound clinical reasoning to facilitate timely hospital discharge to the most appropriate environment
- To be able to effectively triage and prioritise a large caseload and be able to adjust daily work plan in response to system-wide pressures and escalation. Pro-actively optimising the usage of community capacity on a daily basis.

Main duties of the job
Based at various locations around the BNSSG region, you will be responsible for a busy physiotherapy case-load, working within experienced and supportive teams.
Your responsibilities will include:
- To act as an autonomous Physiotherapist practitioner, providing assessment and treatment, and managing your own patient caseload within a speciality team, with the support and mentoring of Senior Physiotherapists.
- Rotate to different clinical areas across the region. Each rotation will have clear learning objectives to consolidate and build on graduate skills and academic knowledge.

The Childrens Specialist Safeguarding Team - CSST is a highly skilled team working with children who have disabilities and, or complex health needs.
We provide a wide range of services for children with additional needs and those who may be more vulnerable. This includes delivering the councils statutory responsibilities for disabled children in need within the community, such as care packages, practical support and access to short breaks.
The team brings together an eclectic mix of expertise across different service areas, covering a broad spectrum of work with children with disabilities, Occupational Therapy, and Short Breaks, including Larchwood, our dedicated short break respite unit.
Social workers in CSST focus on disabled children's lived experiences and take time to understand their specific needs. As a result, they are able to identify neglect early and use practical toolkits to support parents to make positive changes. When risks escalate, decisive action is taken, meaning you are well supported in your role to keep children safe. The Urology Centre is a modern out-patient and diagnostic department specialising in urology and uro-oncology. The department offers Lithotripsy treatment for the treatment of urinary stones. Main duties of the job
The Advanced Practice (nurse) role is for a qualified autonomous practitioner with a caseload within a team structure. The post holder will be given an opportunity to utilise existing clinical skills in Urology and to develop new specialist skills. The development of new skills will be acquired with the support and supervision of experienced specialists working in the area. We are hoping to recruit a qualified practitioner (Nurse) who can communicate well and can adapt to an ever changing environment. Urology nursing experience is essential. The role will mainly be based at Epsom Hospital, but there will be some requirement to work at St Helier Hospital.
