Job summary
We are looking to recruit a friendly and flexible domestic assistant to join our small team at Blyth Community Hospital.
Hours of work for this position are:
16 hr post - predominantly to assist with cleaning office and clinic areas in late afternoon / early evenings.
Other hours may also be available.

Main duties of the job
Quite simply, the NHS could not function without our Domestics! Every single service in the Trust relies on domestics to keep the place clean and safe for patients, visitors and staff. You'll be part of a team striving to maintain the highest cleanliness standards in wards, departments, corridors and offices, and in doing so making a real difference to the NHS.
The successful candidate will maintain the standard of cleanliness as specified throughout the designated work area by means of approved cleaning methods.

Job summary
We have a new vacancy for an outstanding person to work within the team at Coombewood in Rayleigh. You will manage the coordination of care needs, planning and implementation of evidence-based care to a defined group of service users and their carers, within a defined catchment area/locality.
You will provide professional advice and support to other agencies and members of the multidisciplinary team and act up for Team Lead in their absence as appropriate. You will act as a role model and resource for junior/less experienced staff.
The successful candidate will:
Provide supervision to junior staff.
Will act as an independent practitioner within a defined community setting, while liaising with other professionals as necessary.
Will manage a defined caseload and provide supervision to others as appropriate.
Will be responsible for the assessment planning implementation and evaluation of nursing care within agreed parameters, and have a working knowledge of local, national policies and legislation.
Will be responsible for ensuring effective communication pathways exist.
Will be responsible for ensuring the promotion of equality diversity and rights in accordance with good practice and legislation.
Will be responsible for contributing to the training needs necessary in providing quality evidence-based care as appropriate
Valid UK driving license and access to a vehicle essential for this post, along with previous care coordination experience.
Main duties of the job
- To be responsible for the assessment, planning implementation and evaluation of the evidence based care required, including health promotion for a named service user/group..
- Responsible for the admission, case conference/reviews and discharge in collaboration with members of the multi-disciplinary team.
- Responsible for coordinating the care and assessment under the Care Programme Approach of a designated caseload
- Responsible for ensuring the postholder understands and meets their professional responsibilities under Safeguarding Vulnerable Adults and Children Legislation.

This is an exciting opportunity for a Band 8 a or band 7 to Band 8a psychologist/psychological therapist preceptorship to work in a primary care hub partnership delivered by Oxleas NHS Foundation trust , SEL Mind and Bridge Mental Health.
The mental health hub offers a collaborative and holistic approach and range of psycho social interventions in order to prevent major mental health problems. Psychologists develop and oversee psycho education /well being programmes and evaluate these as well as advising primary care staff on complex cases. The service is delivered in collaboration with psychological therapists working in the mental health hubs in Bexley and Bromley and will be linking with new Integrate Neighbourhood teams as these are set up.

Main duties of the job

An exciting opportunity has arisen for an Advanced Speech and Language Therapist to join our service, providing input to inpatient wards across North Cumbria.

This post forms part of the Speech and Language Therapy Inpatient Team, which supports both working-age adult and older people's mental health wards across Cumbria, Northumberland, Newcastle, Gateshead, and Sunderland. Main duties of the job

Main duties of the job
Older People's Specialist Mental Health Services in Mid Essex consist of locality based community mental health teams (including memory assessment and support services), urgent care home treatment teams and an inpatient ward. Each post will be based primarily in one community MDT or the inpatient ward, whilst working across the pathway to enable greater continuity of care for service-users.
You will provide direct psychological assessment(inc neuropsychological assessment) and interventions for older people with complex mental health difficulties and adults with neurodegenerative dementia(and their families and wider systems) across community settings and inpatient settings. You will provide group interventions, attend multi-disciplinary meetings/ward rounds and work collaboratively with our multi-disciplinary colleagues, providing training, consultation and reflective practice groups. You will supervise an assistant psychologist and trainee psychologists.
You will receive supervision from a Principal Clinical Psychologist and peer support from colleagues across Trust Older People's Psychological Services. Clinical research, teaching and specialised interests are actively encouraged and you will have opportunities to contribute to service improvement and develop your clinical leadership skills.

You will play a key role in supporting the implementation of Nervecentre ePMA as part of the Trust's strategic implementation of the wider Nervecentre Electronic Patient Record (EPR).
This role offers the chance to be at the forefront of a major digital transformation programme, helping to improve prescribing, medicines administration, clinical workflows, data quality, staff experience and, most importantly, patient safety.
These posts are offered as a23-month fixed term opportunity and will be based within the Pharmacy Department at HDFT.
Main duties of the job
You will:
- Act as a key point of contact for ePMA and digital medicines queries from pharmacy, medical, nursing and other clinical staff.
- Support the implementation, optimisation and safe use of Nervecentre ePMA and related digital medicines processes.
- Contribute to system build, configuration, testing, validation, user acceptance testing and change control.
- Support go-live activity, upgrades and optimisation, including floor-walking, issue logging and user support.
- Help design, deliver and evaluate ePMA training for pharmacy, nursing, medical and other clinical staff.
- Develop and maintain user guidance, quick reference guides, SOPs, FAQs and other support materials.
- Support medicines safety, governance and incident review relating to ePMA and digital medicines systems.

Job summary
Humber Health Partnership is implementing a major new Electronic Patient Record (EPR) that will transform digital care across our hospitals and services. To support this, we are recruiting a highly skilled Deputy Solutions Architect Integration to lead development of the Trust's new Data Warehouse and interfaces essential to a safe, modern EPR ecosystem.
This senior role is central to ensuring the Group's data architecture, warehousing capability and system interfaces are robust, future-proof and aligned to national and internal digital priorities.
You will:
- Lead the design, development and evolution of the Group's data warehouse, reporting infrastructure and interface solutions supporting the new EPR.

Job summary
We hope to appoint a chaplain of any faith or belief to join our team. This post will have particular responsibility for supporting families of children who are palliative or who have been bereaved. The post will be based in the SWFT chaplaincy team but will work closely with the community children's nursing team and will be part of their MDT. They will also work closely with the acute paediatric nursing team, based in the hospital.
They will provide holistic, emotional, pastoral, spiritual and sometimes religious care to families who are dealing with intense challenge and distress. They will help families to find meaning in what is happening for them and their children and ensure parents have a voice in decision-making about their child's care. They will support staff who care for very sick and dying children, and provide training for bereavement support, compassionate conversations, and staff wellbeing. They will help staff in the identification of spiritual and religious need.
They will build links with local charities and with faith communities and will signpost families to relevant support. They will help families to make meaningful rituals before or after their child's death. They will support with funeral planning, and with memorials.
For children who die in the care of the CCNT, they will act as keyworker for the child death review process, whereby every child's death is reviewed.
As part of the chaplaincy team, they will take part in the out of hours on call rota

The team chaplain and keyworker is to work with the whole Chaplaincy Team and with other healthcare professionals in the delivery of holistic, pastoral, spiritual, emotional and religious care to families whose children are palliative or who have died, in the community and in the hospital.

Main duties of the job
You will:
- Work as part of a multi-disciplinary team within Allkind Mental Health to increase and improve emotional and mental health support for children and young people (typically aged 818) attending or linked to St. Andrews Youth Club.
- Work in partnership to support children and young people experiencing mild to moderate mental health difficulties, as well as their parents/carers and Youth Club staff, in the self-management of presenting difficulties.

We are looking for enthusiastic receptionist/admin(Docman) assistants to complete our primary care administrative team.
The rota for the post is set at 37 hours per week. Monday to Friday from 10am - 6.30pm ( This shift pattern is required to ensure adequate cover is available within the reception team)
