Job summary
This is an exciting opportunity to work within the Harrogate and Ripon Community Treatment Team based in Harrogate.
The post would be an ideal developmental post for someone looking to develop their community assessment and intervention skills. Anyone applying from a different environment that is looking to develop their skills would be supported by the leadership team members to ensure their wellbeing was maintained.
As a senior practitioner you will have a genuine interest in collaborative working with people of varying ages and complex needs, including service users, Carers, families and our partners including statutory and voluntary agencies.
The successful candidate's role will include triage, complex assessments and risk management, deliver evidence-based interventions training will be provided to support a purposeful service user journey whilst with the team as well as provide ongoing coaching support, clinical and caseload supervision to junior colleagues and wider members of the MDT
Candidates must hold 18 months post qualifying experience including sign off of appropriate preceptorship or competency framework at the point of application.
Main duties of the job
This is an ideal opportunity for someone who wants to develop their leadership and clinical skills in a community setting. As a core member of the team, you will also be responsible for your own caseload, mostly providing assessments, care co-ordination and delivering intervention in the community with people who have complex needs ranging from mild to moderate level of need to severe and enduring level of need and complexity.
You will demonstrate excellent assessment and analysis skills, interpersonal, communication and team working skills. You will need to be committed to working collaboratively with a range of statutory and third sector agencies to promote holistic wellbeing and recovery focused care.
The Integrated Community Team is a lively, dynamic, and supportive multi-disciplinary team committed to collaboration and joint working. You will support the team's improvement activities for co-creation, aiming to deliver care that is highly personalised to each individual's needs and working closely with families, paid and unpaid carers.
You will be encouraged and supported to bring your skills and experience to contribute to our team and locality's service development and continuous improvement and innovation endeavours.

Main duties of the job

An Exciting opportunity has arisen to join - and help develop - the Gastroenterology service at the Barnet Hospital. We are looking for an applicant who has experience in Gastroenterology and Endoscopy to support the provision and development of service across both hospital sites. The successful applicants will work within the multidisciplinary establishment with outpatient gastroenterology clinics and endoscopy lists at both the Barnet Hospital and Chase Farm Hospital. This post offers 10 PAs of combined direct clinical and supporting programmed activity. The successful applicants will form part of the out of hours GI bleeding service, contribute to the provision of an inpatient, outpatient GI service and Endoscopy sessions.
The applicants will be encouraged to contribute to the audit, research as well as under- and post-graduate teaching within the department.

Job summary
The Directorate of Women's Services comprises the clinical departments of Maternity (including obstetrics) and Gynaecology at GSTT and the academic department of Women's Health at King's College London. St Thomas's Hospital hosts the largest single site maternity service in London. The state of the art maternity unit includes a midwife led birth unit co-located to a Hospital Birth centre. A 19 bedded antenatal ward and a 38 bed postnatal ward. The Trust delivers 6000 to 7000 women annually, with a 3% homebirth rate. The birth centres are supported by the busiest maternity day assessment unit in London, an obstetric high dependency unit and an active ultrasound and Fetal medicine unit. Most antenatal care occurs in the community.

The post holder will work in parallel with the Medical Examiner's Office and work alongside the Medical Examiners Officers and Medical Examiner to provide a service.
Main duties of the job
Liaising with both internal and external users of the service, by phone and in person.
To take part in daily meetings with the bereavement supervisor to discuss daily deaths.
To assist the bereaved by answering any questions they may raise, arranging for them to speak or contact the most appropriate person or agency. To deal with bereaved persons who may have difficult family situations or circumstances.
Liaise with the Anatomy office, regarding donation of bodies left to medical science.
Liaise with the hospital chaplaincy team for spiritual support for the bereaved.
Liaise with the bereavement midwife, delivery suite and SCBU following the death of a baby.
Liaise with funeral directors or authorised persons
Ensure cultural and religious beliefs are respected and complied with at all times.
Notify General Practitioners of deaths.
To be familiar with Q-Pulse quality system and ensure all documents are read and acknowledged when reviewed and updated.
To be familiar with Eden and ensure that all communication is recorded on Eden.
To take part in the writing of condolence cards and ensure these are sent to families within 4 weeks of the death.
To take part in arranging and facilitating visits with family members.

An exciting opportunity has arisen for an enthusiastic individual to join the Trust within the Quality and Risk team as a PALS officer.
This role involves providing support, guidance and advice for people who have concerns about various aspects of care or who want general information about our Trust services. Main duties of the job

We have an exciting opportunity within our department. This post provides a broad range of specialised, complex scientific and technical services and computing support within Leeds Teaching Hospitals Trust and, as contracted, to other providers. The post holder will apply a broad range of skills and experience in Radiological Physics to ensure the efficient and effective delivery of physics services. This will require the post holder to work closely with medical, scientific and technical staff. The post holder will ensure the individual health needs of Children in Care are met to enable access to appropriate health advice and services and to encourage the engagement in a healthy lifestyle.
Engage with the relevant multidisciplinary services to ensure service delivery and clinical standards are maintained and provide information, advice and practical support in all areas that impact on a child and young person's health.
Main duties of the job
- Under the guidance of the Lead Specialist Nurse, be responsible for assessing, planning and reviewing the health needs of Children in Care. Identifying outstanding health needs and supporting young people to address their health requirements
- To support the Lead Specialist Nurse and be professionally and legally be accountable and responsible for all aspects of own work and caseload.
- To assist and support the development and innovation of the service within, to enable the team to provide provisions in a timely and up to date evidence based manner.
- To support children, young people, their families and carers to implement appropriate health plan and work effectively within residential settings, providing advice, guidance and support as appropriate.
- To manage a variety of complex and sensitive patient casework commensurate to the nature of the post. This will include exposure to a variety of conditions, overseeing clinical functions, managing distressing situations and undertaking administrational activities to ensure records and information is uploaded onto systems in a timely and accurate way, to ensure effective integration of practice.
- To undertake patient health assessments and assess health needs holistically to develop a health plan that relates to the individual children and young person. Taking into account their physical, emotional, behavioural, social and developmental needs, undertaking regular analysis of complex information in accordance with clinical procedures.

Pharmacy experience is desirable, but full training is provided. We can offer plenty of opportunities for development and progression to those who have demonstrated excellence in the service of our patients. Experience in customer services, stock and inventory management or medicines procurement is desirable, but the ability to learn quickly and work accurately important.
You must have a good level of education, having a minimum of a GCSE (or equivalent) in Maths, English and Science at Grade C or above.These posts will be based at our Clatterbridge Cancer Centre Liverpool and Alder Hey Children's Hospital sites, but you may be required travel to work at other PharmaC sites as required.
Main duties of the job
Pharmacy Assistant 40 hours per week at PharmaC
- Perform all aspects of medicinal stock management duties, including ordering, receiving, distribution, dispensing, selling, stock taking, date checking and destruction.

In return, you will receive full support, training, and guidance to enable you to undertake a range of cleaning duties to include, general cleaning tasks (bathrooms, kitchens, wards, clinics, theatres, office areas, etc.) as well as specialised cleaning processes to include machines, with some specific tasks being tailored to the needs of a particular service and around patient care.

Job summary
We are excited to recruit an enthusiastic Band 5 Occupational Therapist to join our 16-bed Higher Dependency Rehabilitation Unit (HDRU) in Doncaster, providing specialist mental health rehabilitation for adults from Rotherham, Doncaster and North Lincolnshire.
The HDRU supports adults with complex mental health needs who require intensive rehabilitation within a structured, therapeutic environment. Patients typically remain on the unit for up to 12 months, working with the multidisciplinary team to develop the skills and confidence needed to progress to less restrictive settings and successful community living.
Occupational Therapy plays a central role in the rehabilitation pathway, delivering recovery-focused, person-centred interventions through meaningful occupation. Group-based interventions are a key part of the role, and you will be expected to plan and deliver a varied programme of occupation-focused groups alongside individual interventions.
You will work within a supportive multidisciplinary team including Nursing, Psychology, Psychiatry and AHP colleagues, embracing our "One Team" approach to delivering high-quality care.

Main duties of the job
- Undertake comprehensive Occupational Therapy Assessments using the Model of Human Occupation (MOHO) and other evidence-based assessment tools.
- Develop, implement and evaluate person-centred intervention plans that promote recovery, independence and meaningful occupational engagement.
- Plan, deliver and evaluate a structured programme of occupation-based group interventions, using meaningful activities to promote recovery, skill development, social participation and independence.
- Support patients to establish healthy routines, develop life skills and build confidence in preparation for discharge to less restrictive environments.
- To assist in the supervision of support staff and students and contribute to the development of a range of inpatient based therapeutic activities in partnership with service users, carers and staff.
- To develop links with statutory and voluntary sector community resources and ensure that on-going occupational needs are addressed as part of a safe effective discharge and the occupational therapy pathway process.
- The service is new and will be in Doncaster, High Dependency Rehabilitation Inpatient Unit staff may also be required to work in a variety of other settings dependent on patient needs, e.g. patients' own homes, community settings
