Job summary
HASP PCN is a network of five GP practices across Hull. We are looking for a Community Paramedic, Nurse Practitioner or Advanced Nurse Practitioner to join our acute care team. The role is primarily to provide home visits to our housebound patients and support care home residents.
During periods of high demand there will also be a requirement to provide acute clinics at our practice sites.
Main duties of the job
The Community Paramedic/NP/ANP will work as an integral member of the HASP PCN multidisciplinary team, delivering high-quality, patient-centred care within patients' homes, care homes, GP practices and community settings. The post holder will provide assessment, diagnosis, treatment and management of patients with acute, chronic and complex health needs, helping to reduce avoidable hospital admissions and improve access to primary care services.

The Cancer Psychological Care Team is now a permanently established service at King's College Hospital, originating from a programme of work in partnership with Macmillan and in relation to the Kings cancer priorities to improve cancer patients experience. Job summary
We have an exciting opportunity to join North London NHS Foundation Trust as a Band 5 Medicines Management Pharmacy Technician (MMPT), working at the heart of our inpatient pharmacy service.
This is a varied, patient-facing role where you will work directly with patients and multidisciplinary teams to improve medicines safety and optimise treatment. You will have the opportunity to use and develop your skills in medicines reconciliation, patient consultation and counselling, discharge planning and medicines optimisation.
The post is a cross-site role working across St Ann's Hospital and Chase Farm Hospital, with an initial indicative allocation of 60% St Ann's Hospital and 40% Chase Farm Hospital. You will be expected to work flexibly across NLFT sites according to service need, and the balance between sites may change in response to operational requirements and future service developments. The role may also develop into a rotational post across medicines management services and/or Trust sites as the service evolves.
We are looking for an enthusiastic and proactive pharmacy technician who enjoys working with patients, takes ownership of their role and wants to contribute to the continued development of pharmacy technician-led services.
Main duties of the job
No two days are quite the same. As an MMPT, you will:
- Work directly with patients to understand their medicines and identify medicines-related issues.
- Undertake medicines reconciliation and detailed medication histories.
- Assess patients' own drugs (PODs) and help ensure medicines are available when patients need them.
- Provide high-quality medicines counselling and support patients to understand and manage their treatment.
- Play an active role in discharge planning, including DMS referrals and improving continuity of medicines care after discharge.
- Work alongside pharmacists and multidisciplinary teams to identify and resolve medicines-related problems.
- Provide ward-based medicines management and promote the safe and secure use of medicines.
- Use your accredited checking skills to support safe and efficient pharmacy services.
- Work flexibly across Trust sites and provide cross-site support according to service need.
- Support and develop pharmacy trainees and colleagues.
- Get involved in audits, quality improvement, medicines safety and the development of new ways of working.

The specific clinical and managerial responsibilities of the post-holder will be as follows:
To provide advice and support to local GPs, and other practice staff, regarding the assessment and management of the mental health difficulties of patients registered with their practices. This may be done through case discussions, joint consultations and bespoke training, as appropriate.
To provide consultant input and clinical leadership to the local Intensive Outreach pathway within the Core Community Mental Health Teams.

Job summary
To provide effective and timely care within a multidisciplinary team setting for Older People who present with functional and/or organic mental health problems.
To assess care needs, devise care plans based on dialog+, carry out risk assessment and implement and evaluate these in collaboration with individual service users and also their carers, where appropriate
To support and mentor less experienced staff, support workers and teach students from related professional backgrounds who are on placement with the CMHT.
To undertake duties designed to extend clinical and managerial expertise as delegated by the Deputy CMHT manager when required. This will include deputising in the absence of the Principle Practitioner.
Main duties of the job
1. Carry overall responsibility for the management of a caseload in the role of Key worker /care manager, as defined by the Dialog+ approach and other government and local directives. Ensure that the service of the CMHT i.e. nursing, social care, occupational therapy and psychology is carried out effectively within a multidisciplinary framework and in collaboration with other agencies. Maintain contact with service users on their caseload during periods of admission, including attendance at MDT meetings. To take lead in the planning and implementation of the client's re-integration back into the community.
2. Provide service users and their carers with a service that is culturally sensitive and gives due consideration to any special needs
3. Provide therapeutic interventions by means of assessment and ongoing treatment in various settings. Design care programmes in line with risk assessment and psychosocial needs. In consultation with MDT colleagues discharge service users in line with Trust and CMHT policies and procedures
4. Participate in the Duty System as the Duty Officer or 'Backup' providing a Direct Access mental health service to the community. Be responsible for the screening of all new referrals, arrange assessments, provide advice and refer to other agencies as appropriate.

Job summary
Two exciting trainee posts have arisen in our Tri-borough Under-5s Team: one on the IEYP Parent-Infant pathway (under 2s), and one on the Early Childhood pathway (3-5 yos). This is part of the NHSE funded Children and Young People Psychological Training Programme.

We are seeking candidates with a strong interest in the mental health and relational development of children aged 0-5. You will be enthusiastic about developing skills in assessment and intervention with infants, young children and their families, and able to apply learning from the training programme in a busy CAMHS service.
You will bring experience of working with the development and wellbeing of children aged 0-5 and their families, an understanding of safeguarding and multi-agency working, and the ability to communicate sensitively with young children & parents/ carers. We are particularly interested in candidates who can reflect on their own practice, work well in teams, manage the combined academic and clinical demands of the post, and contribute to inclusive, evidence-informed care.
Main duties of the job
This full-time training will begin in January 2027 with around 2.5 days a week for academic work (teaching and personal study), and 2.5 days based at the service . Trainees will be both an employee of the service and an enrolled student with King's College London. Upon completion, qualified trainees will receive a KCL Postgraduate Diploma.
At your placement in our welcoming CAMHS under-5s team you will receive regular supervision, exposure to our multi-disciplinary team forums, and lots of opportunity for clinical work. You will benefit from working in a team dedicated specifically to provision of CAMHS care to under-5s.
This is a dual application process. In addition to applying for this position with us, you must also complete a UCL application for the Course Team to review. The Course Team will liaise with us on whether you meet the academic requirements for the course during the recruitment process. Job summary
An exciting opportunity has arisen to join NLFT CAMHS Single Point of Access (SPOA) as a Clinical Psychologist. This role is suitable for someone who is passionate about providing high quality mental health care to support children and young people and their families.
The purpose of the Single Point of Access (SPOA) is to assess children and young people being referred in to the CAMHS Division to ascertain the most appropriate service to meet their needs. The post requires significant clinical and organisational skills. You will adhere to the goals of 'No wrong Front Door' and support the operations of the Access Team who seek to identify the right service for a young person at the right time. We have developed a system of triage to help identify needs, assess risk and if appropriate offer initial assessment and brief intervention, using the iThrive model. As SPOA work closely with the other CAMHS teams, it is likely that there will be opportunities for work across teams in line with the post holder's interests or skills.
Main duties of the job
Coordinate the day to day running of the SPOA team, including screening new referrals, allocating these to the access clinicians.
Complete telephone triages/screening for young people who are referred to our service, making contact with families, allied professionals, the young person themselves.
Complete over the telephone risk assessment, management plans.
To provide (specialist) psychological assessments to children who are referred to the using a variety of sources including psychological tests, self-report measures, rating scales, direct and indirect structured observations, semi-structured interviews with clients, family members and others involved in the child or young person's care.
To provide brief interventions for young people referred to SPOA or other CAMHS teams
Help to manage and maintain clear waiting lists for the clinical teams that sit within the CAMHS Division.
To provide advice and on clients' psychological care.
To work within Trust and professional guidelines, to a clear CAMHS job plan.
To support and promote the use of measures to evolve clinical practice and enhance user experience within supervision, team meetings and other forums.
To utilise research skills for practice evaluation and service development, as required by the service and set out in the job plan.
To be involved in co-production, to support the development of services.
