Job summary
This is an exciting opportunity for registered Nurses, Social Workers, and Allied Health Professionals to join the Trust Child and Adolescent Mental Health Services Tri-Borough Single Point of Access (SPoA), covering Barnet, Enfield and Haringey. This post will primarily be based in Haringey
The post holder will be based within the SPoA service and will work across the referral pre-screening and triage function and the Initial Assessmentpathway. The successful candidate will play a key role in ensuring that referrals are reviewed promptly, allocated appropriately, and discussed within multidisciplinary team meetings where required. They will support the delivery of a seamless referral process, ensuring that young people are either directed into the most appropriate clinical pathway or discharged from the service when indicated.
The role includes oversight of the administrative and clinical processes associated with assessment and discharge, ensuring that IA care plan letters, discharge notifications, and all relevant documentation are completed accurately and shared with young people and their families in a timely manner
We are seeking enthusiastic, motivated clinicians with a passion for working with CYP, their families/carers. Applicants should have an interest in supporting CYP, presenting with a wide range of complex mental health needs. The successful post holder will work within a balanced job plan that incorporates both clinical leadership and clinical intervention
Main duties of the job

The postholders will offer supervision and appropriate level of senior clinical oversight for a range of bandings within the team. The post holder will support specialist ND triage advice where necessary, guidance, and support for CYP, their families and other professionals working with CYP. The role includes having a caseload providing therapeutic interventions with an opportunity to lead and develop new interventions at the front door, as well as deputy leadership and management functions as and when necessary, as part of the SPOA leadership team. The postholder will assist in implementing and reviewing systems, policies standards and procedures that contribute to the delivery of high-quality service within the frameworks agreed by the Trust and the Commissioners.
The service will have a centralized collation of outcome measures and scoring used to determine pathways in conjunction with shared decision making.
You will lead on trauma informed assessment and care, with effective signposting where indicated. You will have close ties to other health and social care providers and education services. The role includes having a clinical caseload providing expert therapeutic intervention, as well providing senior clinical support to team members.

Job summary
We are seeking a skilled and compassionate CBT Practitioner with specialist expertise and experience in working with people experiencing psychosis. You will work in our community based mental health team in S Camden. You will provide evidence-based psychological assessment, formulation, and intervention for individuals presenting with psychosis and related difficulties, delivering person-centred care that promotes recovery, resilience, and improved quality of life.
In addition to the specialist psychosis work, the successful candidate will undertake comprehensive CBT assessments and develop formulation-driven treatment plans for individuals experiencing common mental health difficulties, including anxiety and depression.You will work within a strong psychology team. We hold regular meetings and are an integrated part of our MDT team and the wider psychology team across the three core teams of South Camden, Kentish Town and North West.

Job summary
The South Camden Community Team (SCCT) supports the emotional well-being needs of young people (0-18 years) with assessment and evidence-based therapeutic clinical work. It is a rich multi-disciplinary team made up of a range of clinical disciplines to include: Child Psychotherapy, Clinical/Counselling Psychology, CBT Therapy, Family Therapy and Psychiatry. The team works closely with partners in the borough to include: schools, local authority and the voluntary sector with a view to meeting the diverse needs of the young people and families.
Main duties of the job
The post holder will assess, plan, implement and evaluate of programs of evidenced based care to service users, whilst being conscious of their changing healthcare needs and varying levels of complexity, working collaboratively, with other clinicians and stakeholders. They will offer short, medium and longer-term interventions, including parent-work and group-work if appropriate. The post holder will hold a highly complex caseload.

The Mental Health Support Team [MHST] are a service that work within Camden primary and secondary schools, they offer direct interventions to CYP within the school context in a 1:1 and group format. They also support schools with a whole school offer, that can typically support educational staff. MHSTs work closely with other CAMHS colleagues in the Camden CAMHS Unit as well as external services (statutory and VCSEs).
Main duties of the job
The candidate should be able to demonstrate the ability to work as a member of a functional, task-based team, and can ensure they can communicate and liaise with administrative colleagues, patients, family members, external professionals and with clinical practitioners.

Job summary
The successful post holder will be joining Camden's Mental Health Support Team (MHST). Camden was a national trailblazer site for the mental health support team (MHST) in school initiative. We are a well-established multi-disciplinary team that provides high quality support to children, young people, and families, presenting with mild to moderate concerns. Support to schools includes advice and sign posting, consultation, delivering brief evidence-based interventions through universal (e.g. whole school approach), targeted (e.g. class or small group intervention) and individual one to one therapeutic support.
Main duties of the job
The role of Specialist Clinician and Supervisor includes consultation, assessment, therapy/intervention, supervision as well as service development, evaluation and training. This will be a community-based post, with most of the work taking place in school settings or other community settings. The post holder must be able to work collaboratively with other members of the team, staff from other teams as well as with our allied partners across education, health, social care and the charity sector.

Job summary
Our Training Practice is looking for a six-session a week GP, to join our team. Set in the Community of Seven Sisters, Dulais Valley Primary Care Centre would welcome a GP who would like to develop their own interests and skills within this rural locality.
You will be joining our four existing GPs, pharmacist, two practice nurses and two Health Care Support Workers serving approximately 6000 patients which includes a very low percentage home visit requirement.
Our 3 GP partners are experienced trainers; they would encourage you to engage in the in the teaching opportunities to continue to develop your own learning and skill set.
We currently train GP trainees and Swansea University GEM students. In future we have ambitions to increase our training scope to include pharmacy students/pre-registration.
Main duties of the job
We are fortunate to have, Health Visitor Services, District Nurses and various health board and Cluster services being held at our practice, services include; retinal screening, physiotherapy, social prescribing, heart failure clinic, breast screening and Aneurysm screening. We currently offer Implant and coil fitting and we are in the process of setting up joint injection Clinics, scheduled to commence in the Autumn. We are open to exploring opportunities to expand our additional services for future growth of the Practice.
Ideally, we would require you to work, Monday Wednesday but there is some flexibility. This post would attract 6 weeks annual leave and a week of study leave per annum. All bank holidays are pro-rata. Partnership is something that we would consider with the right candidate.
We have a lovely purpose-built building with very spacious consultation rooms, comfortable relaxation areas and staff facilities. We have ample staff and patient parking, we are situated just a twenty-minute commute from junction 43 of the M4 or the A465 at Glynneath, which makes it easily accessible.

Job summary
An exciting opportunity has arisen for a Social Worker within the Secure Care Division of Mersey Care NHS Foundation Trust. We are looking to recruit a dynamic and skilled Social Worker into a band 6 Forensic Social work post at Ashworth High Secure Hospital. Main duties of the job
Ashworth Social Work Department provides a service to patients, their families and carers and routinely works in collaboration with them, MDT colleagues and external stakeholders from the point of pre-admission to discharge. The service strives to deliver the Trust's vision of Perfect and Trauma informed care to promote recovery and support service users to move on and manage risk.
Social care activities range from pre-admission referral assessments, Mental Health Review Tribunal work, Safeguarding Strategy & Section 42 enquiries, Social Supervision, Discharge Coordination, Victim Communication & Liaison, MAPPA, Appropriate Adult (PACE), Care Reviews & Discharge Planning, CPA (Care Programme Approach), CTR (Care & Treatment Reviews) and Social History reports.

Job summary
An exciting opportunity has arisen for a part time Trainee Advanced Clinical Practitioners (apprentices) to join the ACP Team within the Division of Urgent and Emergency Care at DBTH.

Successful post holders will undertake the MSc Advanced Clinical Practice masters degree programme (level 7 apprenticeship)) run in conjunction with Sheffield Hallam University and candidates must be committed to completing both the academic, clinical and leadership requirements of the programme. You will be required to fulfil the full apprenticeship and commit to three years of study.
Pay will commence mid-point Band 6; on successful completion of academic modules year 1&2 and completion of core competencies, you will then progress to band 7. Completion of programme expected within 3 years, unless exceptional circumstances

Main duties of the job

Job summary
The postholder will lead on robust preassessment screening and risk management across Quays neurodevelopmental services, ensuring timely, safe, and appropriate access to care. This includes undertaking referral triage, identifying clinical and safeguarding risks, and using both validated and non-validated screening tools to inform early decision-making, prioritisation, and care pathways.
The role will incorporate the use of digital tools to enhance patient access, streamline screening processes, and support remote engagement alongside traditional face to face delivery. The postholder will also deliver accessible, low intensity health clinics in community settings, including basic physical health checks such as blood pressure, pulse, height, and weight, to inform holistic treatment and care plans.
Working within a skill mixed multidisciplinary team, the postholder will contribute to safe caseload management, patient consultation, safeguarding processes, and continuous service development. The opportunity
East Cornwall Primary Care Network is looking for an experienced Frailty Practitioner to join and strengthen our reactive frailty service.
This is a patient-facing role supporting GP practices across the Saltash and Torpoint, and Liskeard and Looe Integrated Neighbourhood Team areas. The postholder will work on a hybrid basis between home, GP practices and other health and care locations across these areas. Regular travel throughout the locality will be required.
You will work with people living with frailty whose health, independence or functional ability has deteriorated, providing timely assessment, intervention and care coordination.
The service supports practices through daily multidisciplinary team meetings, helps prevent avoidable hospital admissions and supports patients to return home safely following discharge.
We welcome applications from registered nurses, occupational therapists and physiotherapists. Professional background is less important than strong frailty knowledge, sound clinical judgement and the ability to coordinate care across organisational boundaries.
Main duties of the job
You must be registered with the Nursing and Midwifery Council as a registered nurse or with the Health and Care Professions Council as an occupational therapist or physiotherapist.
You will also need:
- Substantial post-registration experience working with people living with frailty, multimorbidity or complex needs.
- Experience of holistic assessment, care planning and clinical or functional risk management.
- The ability to recognise deterioration and make safe, timely decisions within your competence.
- Experience of effective multidisciplinary and cross-organisational working.

Core Responsibilities
- Develop, implement and evaluate a seamless Frailty support service across the PCN, working with community and secondary care where appropriate, and aimed at continuously improving standards of patient care and wider multi-disciplinary team working.
- Where appropriate, support the development of discharge and contingency plans with relevant professionals to arrange on-going care in residential, care home, hospital, community settings and at home;
- Periodically review, evaluate and change rehabilitation programmes to rebuild lost skills and restore confidence;
- Advise on home, and workplace environmental alterations, such as adjustments for wheelchair access, technological needs, and ergonomic support;
- Advise patients, and their families or carers, on specialist equipment and organisations that can help with daily activities;

We are looking for an enthusiastic and patient focused Registered Nurse to join our growing Clinical Homecare Nursing Team as a Community IV Nurse.

We have an excellent opportunity for a Client Referral Co-ordinator to join our Customer Service Centre in Stockton-on-Tees.
Our Customer Service Centre operates 365 days a year and is the first point of contact for people exploring treatment as a self-paying client, as well as NHS professionals seeking emergency bed placements across our national network of hospitals.
As a non-clinical team, youll help people understand their available treatment options and take their first steps towards the most appropriate pathway. This is a rewarding role where your communication skills, empathy and ability to listen will make a real difference.
Location: Customer Service Centre, Ground Floor, Admiralty House, 9 Fudan Way, Stockton-on-Tees, TS17 6EN
Main duties of the job
Youll provide a customer-focused service, handling inbound calls and emails from people making initial treatment enquiries. Youll listen to their needs, explain available options and help them navigate the next steps.
Youll accurately record client information in line with GDPR, liaise with internal and external stakeholders to support access to treatment, and recognise and respond appropriately to vulnerability and safeguarding concerns.
Youll also tailor available services to individual needs and support the arrangement of funding for NHS services.
