Main duties of the job
As a Senior Primary Care Nurse your responsibilities will vary. You will
- Work within a multi-disciplinary team to deliver a nurse led service and prevent hospital admissions
- Undertake medication rounds, dispense controlled medication, attend emergency response situations, assess patients, and undertake reception screening
- Use your skills and knowledge to deliver nurse led clinics to your patient group whilst being supported by a multi-disciplinary team.
- Provide outstanding evidence based care to our patients, seeking out, listening to and acting on their feedback, so that care is personalised and informed by what matters to them.
- Deliver non-judgemental care that makes a real difference
- Inspire excellence by forging strong links with our partners and stakeholders
- Promote best practice, clinical supervision, evidence based care and continuous quality improvement, embedding a culture of shared learning.
- Share knowledge, skills and expertise to ensure safe care, building strong multi-disciplinary teams.

Job summary
We have an exciting opportunity for someone to join our Waiting List Team, based at the Queen Elizabeth Hospital, Gateshead. As a member of the team, you will assist with managing waiting lists for elective surgery and working in accordance with Government Access targets to ensure patients are treated in line with Trust Access policies.
This postis varied and includes contact with patients and many Health Care Professionals associated with admitting patients for surgery.
We are looking for an individual with excellent interpersonal/communicationskills and the ability to work in an environment, where demands and priorities change quickly.
We are looking for someone with a minimum of one year's experience working in administration within an NHS patient/customer focused environment with excellent verbal and written communication skills to ensure accurate and timeous information sharing across the multi-disciplinary team.
An awareness of Government Access targets would be advantageous but not essential. You must be well organised, flexible and able to work on your own initiative and demonstrate confidence in independent decision making.
The successful candidate for the post will work Monday to Friday 9am-5pm, although you will need to be flexible to the needs of the service and be committed to high quality patient care. You will be based at the Queen Elizabeth Hospital site.
This job may close early if large amounts of applications are received.
Main duties of the job
- To take a lead role in co-ordinating the waiting lists of a group of Consultants in accordance with Trust Policies and government wait time targets for elective surgery, both urgent/cancer and non-urgent, adding to waiting lists/ensuring VBC compliance and calculating pathway dates in line with local and national targets ensuring these targets are not breached.
- To co-ordinate admission and associated pre-operative assessment appointments, ensuring all key hospital staff are aware of any organisational arrangements.
- To coordinate surgeons' theatre lists, including selecting appropriate cases in clinical/ chronological order and with appropriate attention to medical requirements of the procedures ensuring theatre lists are utilised to their full potential.
- Schedule surgery and any other related appointments across surgical specialties, monitor progress, identify potential breaches & liaise with clinician/other parties to resolve, plan and manage effective utilisation of theatre lists.
- To arrange interpretation services for any relevant appointments.
- Have the ability to manage the demands of a high-pressure, fast-paced environment to ensure service continuity.
- Provide information to assist performance reporting and resolution of queries or complaints (e.g. Inphase) as required

In MPFT Children and Families Mental Health Function, we work with young people who have a wide range of needs using an inclusive, person-centred approach. The Children's Learning Disability team offers: support and intervention for young people with emotional and physical healthcare needs who may be at risk of placement breakdown or hospital admission.
You will be responsible for the management of a complex caseload, which

conjunction with service user and carer, evaluation of individual treatment
plans for CYP with learning disabilities.

Our purpose built Diagnostic Ultrasound department has a fleet of flag ship GE equipment, a successful, more than 10 years, rolling Sonographer training program, 7 day affable Radiologist support and dedicated Interventional Ultrasound facilities.
Our refurbished Antenatal clinic is based adjacent to the antenatal clinic, providing Antenatal care to approximately 3500 women a year and has state of the art Canon and GE equipment in a 4 bed unit, providing an interdisciplinary fusion of Radiographer, Nurse and Midwife Sonographer led services.
Main duties of the job
As an Obstetric Sonographer, you will be an integral part of the maternity service and work closely with the Obstetric teams to provide exceptional care to all patients. This includes working alongside the ANC Screening team and FMC to deliver the FASP & achieve all KPI's within your practice.
As a General Medical Sonographer, you will be a strong team player, working as part of team undertaking a wide range of examinations. The Radiologists at MPH are approachable with an eagerness to facilitate learning and development to providing exceptional service to all patients.
You'll perform and report ultrasound examinations across both disciplines to a high standard in a professional manner, working within departmental guidelines whilst being accountable for your own practice. Reports are expected to be of high quality providing advice on future patient management to other clinicians.
Excellent communication skills are essential within both spheres, you will be expected to adapt your style of communication to meet the needs of a variety of situations, some of which can be distressing.

Main duties of the job
As Harm Reduction Team Leader, you will provide operational leadership, supervision and support to a team of Harm Reduction Workers delivering services across Hampshire.

Job summary
This is an exciting opportunity for an experienced Registered Mental Health Nurse (RMN) to join the psychosis team, in the assertive outreach pathway as a Band 6 Community Mental Health Nurse. The Psychosis Team is inclusive of The Psychosis Pathway, Assertive Outreach, At Risk Mental State, and Early Intervention in Psychosis. This role will be primarily based in the assertive outreach pathway, however depending on service need, work across the psychosis team may be required.
Main duties of the job
The successful candidate will provide a comprehensive Community Mental Health Nursing Service across a whole range of clients experiencing mental ill health within the assertive outreach pathway. The service will be delivered in collaboration with representatives from other agencies and therefore the ability to work independently and with partner agencies is essential.
It is essential that applicants can demonstrate post registration experience as an RMN, which may include inpatient and community settings.

We are looking to recruit existing or new graduate Band 5 Occupational Therapist onto our physical specialities rotation at South Tyneside and Sunderland Foundation Trust. This is a great opportunity to develop clinical skills and develop knowledge of a wide range of conditions within several different teams between Sunderland Royal Hospital, South Tyneside Hospital and Recovery at Home. The rotation covers the following specialities:

Main duties of the job
Our physical specialities rotation provides the opportunity to experience therapy intervention with patients throughout the whole acute patient journey. Our well-established Interface Team work into ED and EAU to help prevent hospital admission and also to facilitate safe and timely discharges from our short stay areas; our inpatient OT teams assess patients for discharge from base wards and our Recovery at Home Therapy Teams assess and work with patients at home on discharge from hospital, as well as responding to situations to help prevent admission to hospital.
With each rotational area, you will hold your own caseload of patients and will be responsible for assessing the functional needs of your patients to either prevent admission to hospital or facilitate discharge from hospital. You will also be assessing for and prescribing aids, equipment and wheelchairs, and liaising with partner agencies whilst adapting your core skills to the patient group or speciality you are working in. You will also have the opportunity to develop your manual handling skills and your knowledge of specialist equipment.
Due to the need to visit patients homes as part of assessment and treatment interventions, you will need to have a valid UK driving licence and be able to drive our department vehicles in the acute teams at Sunderland Royal, and have access to a vehicle for work purposes for the Recovery at Home rotations.

We are looking for a highly motivated Registered Nurse to join our Specialist Nurse Service. The team provides a 24/7 Specialist Stroke Nurse service based at the Calderdale Royal Hospital. Are you looking for a role which brings a real challenge every day?
Hello, I'm Lisa and I am looking for Urgent Care Practitioners to join the Mental Health Liaison team who can bring knowledge and expertise to this dynamic area of mental health service provision.
You will be working at the interface between physical and psychological health and bring your knowledge and expertise to your general hospital colleagues. You will undertake specialist and timely mental health assessments and treatment for patients attending the General Hospital. We support patients with a wide range of problems, including self-harm, adjustment to illness and physical and psychological co-morbidities. As there is no entrance requirement for A&E and people's perceptions of what constitutes a mental health emergency or crisis can vary, our team are referred people with all sorts of problems, including those people presenting to mental health services for the very first time.
This post offers the Trust's £2,000 Golden Hello or Relocation Incentive for Band 6 Nurses & Practitioners, (pro rata and subject to terms and conditions; please see attached Protocol Document for full details).
Main duties of the job
I am looking for practitioners who are confident in the management of common mental health problems and who can bring their knowledge to their general hospital colleagues both informally and by undertaking teaching sessions. We have recently merged with our Older Persons Liaison Team so are now providing assessments and support to anyone over the age of 18.
You will need excellent communication skills and be able to foster excellent relationships with patients, carers, general hospital colleagues and other parts of the health and social care system.
This is a challenging but rewarding role, offering an exciting opportunity to nurses, occupational therapists or social workers who have strong assessment and therapeutic skills. You will need the ability to work autonomously whilst still contributing to the team as a whole. If you enjoy the challenge and stimulus of working under pressure and are able to remain calm in quickly changing circumstances, you will be well suited to this role.
About us
As our Coastal Urgent Care Pathway moves towards CORE 24 standards, the post-holder will work flexible hours to support delivery of 24/7 services. It is essential that you are able to work on a shift pattern to include nights.
This role will support the work of and across Urgent Care Teams (Crisis Resolution Home Treatment Team and The Haven) and as such the post holder will be expected to work across these teams as required. Our three services are integral to our Urgent Care Pathway and ensure people presenting to acute mental health services receive a compassionate, person- centred assessment and timely care plan whether this occurs in an acute setting such as The Haven; the General Hospital or in their home.
