Job summary
This is an exciting opportunity for an experienced and compassionate Diabetes Specialist Nurse to join Modality East Surrey Medical Practice and help shape the continuing development of specialist diabetes care within primary and community care.
We support approximately 45,000 to 46,000 patients across our sites in Horley, Smallfield and Townhill in Caterham. You will join an established multidisciplinary team and work closely with GPs, nurses, healthcare practitioners, clinical pharmacists, healthcare assistants, community services and Consultant Endocrinologists.
Our existing Diabetes Specialist Nurse is moving on to pursue a different venture, creating an opportunity for someone new to build on the strong foundations already established while bringing their own expertise and ideas to the service.
The role combines autonomous specialist practice with clinical leadership, education and service improvement. You will support adults with Type 1, Type 2 and complex diabetes, including patients requiring treatment intensification, insulin management, diabetes technology or coordinated multidisciplinary care.
Previous general-practice experience would be advantageous but is not essential. We welcome applications from experienced diabetes nurses working in community, integrated or secondary-care diabetes services. Appropriate induction, supervision and development will be provided to support a successful transition into primary care.
Main duties of the job
The Diabetes Specialist Nurse will manage an appropriate specialist caseload and provide expert assessment, treatment, care planning and follow-up for patients whose needs extend beyond routine long-term-condition review.
You will:
- Assess and manage adults with Type 1, Type 2 and complex diabetes within your professional competence and agreed clinical pathways.
- Develop clear, personalised and evidence-based management plans, with appropriate monitoring, follow-up, safety-netting and escalation.
- Initiate, optimise and monitor diabetes treatments within your competence and prescribing arrangements.
- Support insulin initiation, dose adjustment and complex insulin management.
- Review and support the appropriate use of contemporary diabetes treatments, including GLP-1-based therapies and SGLT-2 inhibitors.
- Interpret capillary glucose profiles, flash glucose monitoring and Continuous Glucose Monitoring data.
- Run or contribute to specialist clinics involving CGM, complex insulin management and diabetic-foot assessment.
- Support patients using insulin-pump therapy within demonstrated competence and agreed shared-care pathways.
- Recognise and respond appropriately to hypoglycaemia, hyperglycaemia, sick-day risks and potential diabetic emergencies.

- Use clinical data and audit to identify patients with poor glycaemic control, uncontrolled hypertension, recurrent hypoglycaemia, high cardiovascular or renal risk, or gaps in essential diabetes care.
- Contribute to the development of clinical pathways, policies, templates and service-improvement initiatives.
- Work with the Clinical Director, Medical Director, Head of Operations and wider leadership team to improve and develop the patient journey.
A V300 Independent Prescribing qualification would be desirable but is not essential. Candidates who do not already hold the qualification should be willing to work towards it within an agreed timeframe where required for the role.
Primary-care experience, community diabetes experience, insulin-pump knowledge, diabetic-foot expertise, teaching qualifications and experience of service redesign would also be advantageous.

Job summary
PCN Care Co-ordinators play an important role providing extra time, capacity and expertise to support patients to be actively involved in managing their care and supported to make the choices that are right for them. Working closely with GPs, GP practice teams and other health and care professionals and organisations involved in care, they act as a central point of contact for patients.
Please note that the role of a care coordinator is not a clinical role.
Main duties of the job
The following are the core responsibilities of the PCN Care Co-ordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
- To identify cohorts of patients and signpost to the relevant services.
- To support the coordination and administration of all Meridian clinics i.e. Weight Management, Menopause & Pain Management sessions.
- Adhoc administration tasks and clinic booking for GP practices.
- Ensuring patients records are updated and correctly coded.
- Support the coordination and administration of MDT meetings, taking and distributing minutes, bringing information and feedback to others where needed within Meridian PCN.
- Administrative support for all PCN Projects.
- To reduce administrative burden on GPs and other clinicians.
- To attend the MAF (Multi Agency Forum) meetings when needed and have information ready for this

Main duties of the job
Salisbury ED currently has a spacious 3 bedded Resus, 14 bedded Majors, Paediatric assessment area as well as a newly built minors department. It has further plans for redesigning the current work space.The Department covers a large geographic area and sees over 48,000 new patients per year. It receives the full range of Emergency patients, with Paediatric patients making up approximately 25% of the total. Salisbury is a designated Trauma Unit linked to UHS as our nearest MTC. There is a large military population utilising the Salisbury Plain Training Area and living in the area with their families. There is an experienced and supportive cohort of nurses with Sisters and Senior Sisters in charge of each shift and 3 ENPs through the day until midnight responsible for the Minors area. The hospital has a range of specialities including acute medicine based on AMU, paediatrics and daytime interventional cardiology. Overnight surgical work is covered by a pooled junior doctor rota supported by critical care outreach. Anaesthetics, medicine, O&G and paediatrics all have registrar presence overnight. An exciting opportunity has arisen for a full-time senior clinical fellows within the department of Obstetrics & Gynaecology at Salisbury NHSFT. Whilst this has a core component of service provision, there will opportunity to choose a specific special interest in order for you to develop a specific skill during your time here. There is opportunity to undertake training in one of the following: infertility, outpatient hysteroscopy, menopause or vulval skin. If the candidate has an interest in obstetrics leadership, there is the option for developing this area of expertise.
The applicant must be the equivalent of ST4+ on the RCOG matrix and be able to independently run the labour ward. You will be expected to be flexible and partake in the fair share of all duties (as above). Salisbury District Hospital is a medium sized district general hospital with approximately 2500 deliveries per year.
The post requires some service provision including being part of a 1:8 out of hours on call rota for Obstetrics and Gynaecology. During the day (up to midnight) you will be supported by a FY2/ST1/ST2. There is a non-resident consultant out of hours.
You will also be required to support the service in antenatal clinic, labour ward day duties, gynaecology clinic and will have a minimum of one regular special interest session. Lee Mill Low Secure Unit is a 12-bed male inpatient ward just outside of Plymouth. We provide specialist mental health care for men who have often been involved in the legal system due to their mental health needs. The staff at Lee Mill are passionate about providing the highest level of patient centred care to an often challenging and disadvantaged patient group.
As a valued member of the team, you will receive regular supervision and support from psychological services along with continuing professional development. We offer training in relevant therapies and approaches for this client group including essential training for staff working within secure services.
Due to the nature of the role, all staff are required to attend a physical interventions training session to ensure we can meet the needs of our patients and remain effective in maintaining the safety and wellbeing of all. This post covers a 24/7 shift pattern.
Main duties of the job
You will be responsible, for the assessment, planning, implementation and evaluation of programmes of care using the Electronic CPA process and will be required to act as named nurse for identified individual clients. The post holder will be required to take charge of the unit on a regular basis.
The post holder will have the ability to sustain respectful, consistent and reliable therapeutic relationships with clients that enhances their ability to build social networks and diminishes their social exclusion, maximise their strengths and interests and increase their participation in meaningful activities.
The post holder will contribute to and be an effective member of the Multidisciplinary team. Which requires the attendance and contribution to MDT meetings, to be involved in the assessment of new referrals and to attend and contribute to reviews, clinical meetings and case conferences.

Job responsibilities
Clinical/Professional
1. To act as care co-ordinator or named nurse for an identified group of clients following the procedures of the Electronic Care Programme Approach (eCPA).
2. To deliver a high standard of individualised care, which will include the assessment, planning, implementation, review and evaluation of care following the eCPA process. This should be in partnership with the client, carers and multi-agency professionals as appropriate. This requires a complex level of communication in the receiving and transmitting of information to clients carers, colleagues, the wider service and the general public on an individual or group basis. This will be over a wide range of complex and sensitive issues and will need to take into account barriers to understanding.
3. To ensure comprehensive risk assessment, history, planning and management is recorded and communicated to the team and the wider service if appropriate, for individual clients. This will require the electronic recording of risk and relapse indicators and warnings as per the Livewell Southwest SystmOne policy. This may require sharing of information, negotiating care and monitoring of Risk in the wider context of a multi-agency approach eg. with probation and with the co-ordinators of both vulnerable adult and child protection.
4. To contribute to and be an effective member of the Multidisciplinary team. Which requires the attendance and contribution to MDT meetings, to be involved in the assessment of new referrals and to attend and contribute to reviews, clinical meetings and case conferences. It is the role of the care co-ordinator to convene necessary meetings, invite the relevant stakeholders, communicate and record outcome and be responsible for the allocation of individual aspects of care.
5. To work in partnership with carers of clients, offering support, advice and reassurance on often quite sensitive and emotional issues and at times of high levels of distress. Where ever possible involving carers in planning care as appropriate. Offer advice on where best to seek expert support from organisations both within Livewell Southwest and from local and national voluntary agencies.
6. To direct and support junior staff in the delivery of high standards of individually planned care. To create and maintain a learning environment for the development of junior staff and be able to mentor/educate qualified and unqualified staff, including pre and post registration students.
7. To maintain accurate, legible and contemporaneous clinical notes, CPA and other relevant paperwork in line with the NMC guidelines and with the Livewell Southwest SystmOne and eCPA policies.
8. Maintain and regularly update knowledge of The Mental Health Act 1983, and be aware of own responsibilities and clients rights under the Act. To work at all times within the Code of Practice and Livewell Southwest policies.
9. To be responsible and accountable for the safe storage and administration of medication as per national and Livewell Southwest policies and procedures. To educate clients and carers regarding the potential benefits and side effects of medication and feedback information to the medical staff/community pharmacist.
10. The post holder is required to comply with all infection control policies and guidance, attend relevant updates and report issues of concern to their immediate line manager (if no action or explanation received, then it is the individuals responsibility to escalate to Director of Operations/Chief executive Office.)

1. 2. To encourage staff development using all training resources available and demonstrate work activities to new or inexperienced staff. Provide advice and informal clinical supervision for staff and direct to appropriate resources which facilitate evidence based nursing practice.
3. To be aware of and appropriately implement national and Livewell Southwest policies and procedures governing a wide range of clinical and non clinical practices ie. Clinical guidelines such as the safe administration of medicines, NICE guidelines, CPA etc. Non clinical policies and procedures such as Health and Safety legislation, COSHH, Fire regulations and Food Hygiene. To comment on and propose changes to draft policies and procedures.

To be responsible in attitude towards economy of resources and care of equipment and property including; the day to day financial management of petty cash; safe storage and recording of patient monies; stock control of all supplies ie stationary, medicines, food office supplies etc and the health and safety of the environment.
5. To be responsible for the safe use of unit vehicles and the accurate recording of mileage. To be responsible for carrying out risk assessments and planning to reduce risk when transporting patients.

Job summary
We are delighted to offer an exciting opportunity within the Mood, Emotions and Relationships Service (MERS) in North Devon to join our team in a permanent role and undertake fully funded Cognitive Behavioural Therapy (CBT) training through a one-year programme with the University of Exeter.
During the training year, the postholder will develop the knowledge and skills required to practise as a CBT Therapist.
Following successful qualification, they will become a valued member of the wider MERS team, working across all areas of the service to support children and young people. This may include tasks such as undertaking mental health assessments, supporting duty and screening functions, delivering evidence-based interventions, and contributing to the holistic care and treatment provided within the pathway.

Clinical
Assist in and perform routine tasks related to patient care as directed by Lead Nurse and GPs. Advising and escalating any potential problems, issues, or errors in a timely manner.
Undertake chronic disease management including monitoring progress and response to treatment, identifying where changes to medication may be necessary and seeking senior support to implement, processing referrals where required, working to reduce unnecessary hospital admissions, and promoting the preventative aspects of nursing practice. Chronic diseases include but are not limited to:
o Asthma
o Diabetes
o COPD
Once trained and competent work within your scope of practice, undertake womens health management including but not limited to;
o Cervical Cytology
o Contraception including reviews, depo injection, pill checks and coil removal (where the practice is registered to provide this regulated activity).
o HRT reviews
Work to recognise changes in a patients condition and take appropriate action.
Undertake wound and ulcer care including suture/clip removal, dressings, and assessment to include doppler studies.
Undertake diagnostics tests including but not limited to:
o Urinalysis.
o Taking swab samples.
o Request pathology tests, for example urine culture.
o ECG and BP testing including 24hr ambulatory blood pressure monitoring.
o Pulse rate and rhythm.
o Venepuncture
o Blood Glucose monitoring.
o Weight management including BMI.
o Height measurement.
o Peak flow readings.
o Spirometry (where ARTP registered).
Administer adult and child immunisations, vaccinations and injections including travel vaccines, under a recognised legal framework.
Ensure safe storage, rotation and disposal of vaccines and drugs.
Chaperone patients to their healthcare appointments when requested.
Assist with minor surgery and invasive procedures when required, this includes but is not limited to:
o Coil insertions
o Joint injections
o Minor surgical procedures
Monitor and maintain stock of vaccinations, emergency drugs and wound dressings, keeping accurate records and ordering low stock items when required.
Ensure appropriate health promotion literature is available.
Maintain clinical equipment efficiently and ensure servicing/calibration is undertaken, updating the calibration records as necessary.
Prepare and maintain environments and equipment before, during and after patient care in line with infection prevention and control guidance.
Undertake housekeeping duties including, general tidiness and cleanliness of equipment, nursing rooms and treatment rooms ensuring room and equipment logs are kept up to date and available for auditing upon request.
Attend and participate in any MDT and practice meetings when required.
Ensure practice fridges are checked daily, temperature recorded, cold chain maintained and reporting any issues where relevant.
Keep up to date with any changes or advances within the Nursing profession, completing annual competency reviews and necessary training.
Communicate effectively, sensitively, and compassionately with colleagues, patients, carers recognising needs for alternative methods of communication.
Liaise and communicate with multi-agency partners and third-party organisations as appropriate, adhering to organisational and national information, sharing guidance at all times.
Administration
Accurately input, update and extract information onto/from the Practice clinical system when required.
Record patient information clearly and concisely, adhering to national and local guidance.
Liaise with and feedback effectively to the nursing team and other members of the multi-disciplinary team (MDT).
Infection Control
Support the Infection, Prevention and Control lead through the following measures:
Follow procedures aimed at reducing the risk of passing on the organisms that can cause infections.
Demonstrate a duty of care in following best practice in adherence to guidelines which is a fundamental requirement in underpinning the management of Infection, Prevention and Control.
Apply infection control measures within the practice according to local and national guidelines. This includes:
o Dealing with blood and body fluid spillages
o Decontamination of instruments and clinical equipment
o Reporting and treatment of sharps injuries
o Segregation and disposal of waste materials
o The collection and handling of laboratory specimens
o Universal hand hygiene precautions
Monitor and change sharps bins within the practice when requested or identified.
Assist patients and colleagues in adopting recommended infection control measures and health and safety measures.
Effective cleaning and sterilisation of clinical equipment and working surfaces, preparing treatment/consulting rooms prior to use.
Support the Lead Nurse in the completion of relevant Infection, Prevention and Control audits.
Professional Registration
Work in accordance with Nursing and Midwifery Council Code of Professional Conduct ensuring NMC registration is valid.
Ensure you comply with requirements for revalidation and maintain the knowledge and skills you need for safe practice.
Continue professional development (CPD) to supplement existing knowledge to aid further autonomy within the role.
Disseminate updates in current evidence to all in practice, particularly new learning from study.

Main duties of the job

The role requires excellent organisational, communication and teamwork skills.
