We are recruiting for two Evaluation, Research, and Insight Officers to join our team in Practitioner Performance Advice.

0.6 full time equivalent (22.5 hours per week), permanent contract
Main duties of the job
You will be working with a range of senior specialists across the Advice service and external stakeholders including health care organisations and national bodies.
What you'll be doing
The post holder will support a number of work areas, which may include but are not limited to:
- Evaluation and research Insights publications
- The Advice services equality, diversity and inclusion agenda in respect of our service users
- Policy, guidance and systems
- Other key innovation and development activities with the Advice service.

Job summary
We are currently recruiting a Band 3 Healthcare Support Worker, 23 hours a week, to be part of our Stroke & General Rehab Inpatient Unit at Evesham Community Hospital. The candidate will work closely with the multidisciplinary team (MDT) to achieve specific rehabilitation goals. This will be a challenging and exciting opportunity and will enable the individual to develop new and existing skills.
Evesham Community Hospital plays a critical role in the delivery of local NHS services, providing more care and treatment closer to peoples homes. Emphasis is to reduce length of stay whilst ensuring excellent care which is safe, effective, personalised and compassionate and to the standards you would expect for your family and friends.

We will provide you with resources to develop and progress a long-term career as well as flexibility, continual professional development as well as a wide range of wellbeing and support.

Main duties of the job
The main duties of a healthcare support worker are:
Assist with admitting patients to the ward and the care of their relatives
Undertake clinical procedures following assessment of competencies by qualified senior members of staff
Carry out delegated nursing care
Act as health
Provide safe and effective care
Identifies areas for the improvement
Assist patients with attending to the activities of daily living and support the therapy team with the rehabilitation of the patient.
Assist patients to sit up or out for breakfast.
Serve and prepare meals/breakfast and drinks, help to assist patients when required. Complete appropriate paperwork i.e food diaries and fluid balance charts.
Assist patients with morning personal care and dressing.
Mobilise patients using correct mobility equipment when required/liaise with physiotherapy team.
Liaise with nursing staff for information regarding Observations, catheters, lab samples, blood sugar monitoring, dressings and further health checks.
Carry out patient weekly assessments which involve weighing, clean and check mattress, catheter bag change when required.
Stores to be restocked daily.
Assist patients with toileting.
Assist to support patients to bed/ attach catheter night bags when required.
Comply with policies and procedures of the ward.

This senior specialist role is responsible for the management, development and optimisation of the Trust's Radiology digital systems, including PACS, RIS and associated imaging technologies. Job summary
This is a role within our Plus Team which provides an enhanced service to all children and families subject to Safeguarding Plans.
You will be a Specialist Community Public Health Nurse (SCPHN) Health Visitor (HV) within the integrated 0 to 19 Torbay Partnership and provide the expertise, knowledge and resources to deliver the Healthy Child Programme (HCP) to families living within Torbay.
Working with the 0 to 19 Torbay workforce to ensure pathways are followed in supporting children, young people, families and carers. The SCPHN-HV works autonomously and collaboratively to deliver a family centred public health service, taking responsibility for a defined population in partnership with the wider multi-disciplinary and inter-agency teams.

Main duties of the job
- The post holder will be flexible when providing care to meet the needs of clients

The Specialist Stop Smoking Practitioner will support the delivery of evidence-based stop smoking services aimed at reducing health inequalities across the borough of Sefton. The role will include delivering stop smoking and tobacco dependence interventions throughout multiple venues across Sefton including Community centres, GP practices and workplaces. The postholder must be able to work flexibly, independently, and collaboratively as part of a team and be willing to work one evening per week to meet the needs of the service.
Part of this role will also involve occasional work with hospital settings. This will include reviewing and updating referral pathways to make sure patients are referred to the service promptly when they are discharged from hospital.

Main duties of the job
Working under the line management of the Head of Service, you will play a key role in delivering and developing high-quality smoking cessation services across the local community. You will work collaboratively with partner organisations and services to promote the service, increase referrals, and embed smoking cessation into routine client and patient care. You will motivate and empower individuals to make positive, sustainable changes, providing evidence-based stop smoking interventions through a range of delivery methods, including one-to-one appointments, drop-in and group sessions, telephone and online support, in line with the NCSCT Standard Treatment Programme. You will provide evidence-based education and ongoing motivational support, work with clients to set realistic goals, and lead peer support sessions for successful quitters to help prevent relapse. The role will also involve interpreting health, equality and service data to understand local needs and inform service delivery, while ensuring a high-quality, efficient and cost-effective service across diverse community settings. Main duties of the job
The post holder will be providing direct hearing managementof the adult population. This includes diagnostic assessment, speech testing and rehabilitation.
Apart from the clinical duties the post holder will also be expected to provide leadership to junior staff, by doing regular 1:1 meetings, clinical supervision and appraisals.
The post holder will take an active role in areas of clinical audit, research and clinical governance of the Audiology service. They will be expected to design and lead of audits within the service and participate in data collections for larger projects.

The post holder will support clinical activity within Sexual & Reproductive Health, Contraception and Termination of Pregnancy, Women's Health services.
This Band 3 Pregnancy Advisory Service (PAS) support worker post will play a key role in providing high quality compassionate care to individuals accessing PAS and Termination of Pregnancy (PAS) services within CTM UHB.
Working as part of a multi-disciplinary team the post holder supports smooth running of the service whilst ensuring the patients receives safe, confidential and person centred care throughout their TOP pathway.
The role involves supporting patients before, during and after consultations and procedures, assisting clinical staff in outpatient settings and Manual Vacuum Aspiration (MVA) services. Undertaking delegated clinical duties within the competencies, preparing clinical environments and equipment, maintaining accurate documentation and providing emotional support to patients during what can be a difficult time.

Job summary
This role offers a unique opportunity to join a supportive and welcoming Speech and Language Therapy team working within community integrated stroke services and acute SLT services. You will be based Withybush General Hospital, Haverfordwest.
This post is ideal for a clinician who enjoys variety in their day, values collaboration, and wants to make a tangible difference in patients' recovery journeys. You'll work closely with our MDTs on the wards at Withybush General Hospital and community services, supporting patients with communication and swallowing needs post stroke as they transition from hospital to home.
Given the community aspect of the role, the ability to travel across Pembrokeshire efficiently is important, as you will be visiting patients in their own homes as part of their ongoing rehabilitation and support.
You will be part of a service that values both clinical quality and staff wellbeing. We're a small team that offers regular supervision, peer support and opportunities to learn from others.
Main duties of the job
To contribute to the provision of speech and language therapy for the population of Hywel Dda University Health by providing high quality, efficient and effective speech and language therapy services, within available resources and in accordance with the Registration Codes of HCPC and RCSLT.
Work as part of the Community Integrated Stroke Service and Acute SLT services, supporting timely, safe, and person-centred discharge from hospital to community.
Liaise closely with a range of professionals contributing to holistic care planning.
Deliver rehabilitation-focused interventions in hospital and community settings, adapting your approach to suit a wide range of needs and environments.
Support patients and their families through education, advice and collaborative goal setting.
Actively contribute to MDT meetings, case discussions, and service development.
Maintain accurate and timely clinical records in line with professional and local standards.
To make specialist clinical decisions following assessment, including referrals to other services, professions as appropriate eg FEES, VF, dietetics.
