Job summary
Primary Care Clinical Pharmacist - Market Rasen Surgery
We are seeking an enthusiastic and forward-thinking clinical pharmacist to join Market Rasen Surgery, Lincolnshire. We are looking for a pharmacist to develop and manage all aspects of medicines across the practice network. Working with highly skilled multi-disciplinary teams, we currently have 2 pharmacists and 1 pharmacy technicians.
You will work in a patient-centred role based in the surgeries along with some hybrid working. This is an evolving role with huge scope for development, we will support you with your independent prescribing course if required. Full details can be found in the Job Description and Person Specification attached.
The successful applicant will be a pharmacist who acts within their professional boundaries, supporting and working alongside a multi-disciplinary team in a patient facing role. Please note: this is a hybrid role and you will be required to attend the practice for more than 50% of the time, during the working week, which is essential for this role.
Main duties of the job
The type of duties involved will be:
- Telephone & some face to face consultations and reviews with patients
- Managing repeat prescriptions
- Responding to patient askmygp queries related to medication.
- Long term condition reviews - training will be provided for this if necessary
- Synchronise medication
- Review urgent prescriptions
- Liaise with hospital pharmacists
- Provide regular support and feedback to practice on prescribing action plans.
- Updating patient records
- Ensuring the practice is compliant with CQC medicine-related standards
- Undertaking risk assessments and ensuring compliance with legislation
- Audits

About us
Market Rasen Surgery is a medium-sized dispensing practice based in the centre of town, just on the edge of the Lincolnshire Wolds and within easy reach of hospitals in Grimsby, Lincoln, Gainsborough and Louth. We serve a largely rural community of approximately 10600 patients over approximately 200 square miles.
The practice is contracted by the ICB to provide general medical services to our patient population. As well as general medical services, we also provide many additional services, including minor surgery such as vasectomies, hernias and skin procedures in our purpose-built theatre.
In addition to the 2 GP Partners and 8 salaried GPs, we have 3 ACPs, highly skilled practice nurses who carry out general nursing duties, also manages the care of patients with conditions such as asthma or other respiratory disease, diabetes and chronic heart disease.
We also employ an older adults nurse who carries out reviews of patients and supports them with their day to day activities.
The nursing team is supplemented by trained HCAs who take blood, monitor warfarin levels and take basic data such as height and weight.
We also have a team of attached nursing staff who look after our patients. These include care co-ordinator, physios, OT & community midwife. we also have regular outreach clinics in paediatrics, diabetes and psychiatry for the elderly.
The clinical team is supported by the dispensary and administrative staff.

As Bournemouth's specialist respiratory ward, we care for anyone from younger patients requiring acute illness management, to looking after people with long term conditions such as COPD and providing person centred care in someone's last days of life. We provide exciting opportunities for healthcare support workers to be trained in a range of extended skills, including some that are unique to A11 R such as caring for patients requiring Non-Invasive Ventilation.
Main duties of the job
The ideal recruit would have patient safety and team-working at the forefront of their minds. We believe that working together ensures the best outcomes for our patients, and pride ourselves in maintaining a friendly, fun workforce. We expect healthcare support workers to work closely with the nursing staff, by providing patients with day-to-day care, and always promoting their independence. You would also spend time completing observations and escalating patient's conditions appropriately and documenting any findings.
Although previous NHS experience is an advantage, it is not essential as training will be provided! All we need from you is a compassionate and outgoing personality and a willingness to learn.

We are seeking a Financial Analyst who is looking to develop a career in NHS Finance and work towards becoming a qualified accountant.

We have an exciting opportunity for a Band 6 Community Mental Health Practitioner to join our Adult Community Mental Health Team service, based at Chorley Health Centre, Chorley.

Job summary
An exciting opportunity has arisen to join the Nursing and Quality Directorate in the North East and Yorkshire region as Regional Director of Nursing Leadership and Systems.
Reporting to the Regional Chief Nurse, the postholder will provide senior professional leadership across the region, lead key regional and national portfolios, and support quality improvement, assurance, transformation, and system partnership working. The role will also oversee the regional quality framework and support Integrated Care Systems to maintain a clear and consistent focus on quality, safety, and patient experience.

Main duties of the job
Aligned with the region, the Regional Director of Nursing Leadership and Systems will lead at regional level on:
Regional portfolios

Job summary
An exciting opportunity has arisen for a Registered Mental Health Nurse, Social Worker or Occupational Therapist to joint the Pennine West Home Based Treatment Team. The home is a 5-bed service and is an Edwardian building with lovely spacious rooms including plenty of communal areas where young people and staff can work together. An exciting opportunity has arisen within the Workforce Information Team at Milton Keynes University Hospital NHS Foundation Trust for a Workforce Information Analyst.

Job summary
Kingfisher Ward, St Peter's Hospital in Chertsey, Surrey is recruiting for Senior HCSW. This is a busy acute surgical ward with 31 beds and you would be caring for a variety of different patients providing pre and post operative care . As well as caring for patients requiring close monitoring and specialised clinical care. Main duties of the job

Main duties of the job

Job summary
Due to recent expansion of the chemotherapy unit we have vacancies for Band 6 Chemotherapy Nurses.
The role will involve working with the multi-disciplinary teams, Oncology Consultants, and site specific Clinical Nurse Specialists to provide a seamless service for patients with cancer, their relatives and carers.
To provide a high standard of care to patients who attend the chemotherapy unit for treatment and other supportive therapies.
To ensure that cytotoxic drugs and other treatments are administered in a safe manner in a nurse-led setting. It is expected that the post holder will be responsible for their own decisions regarding care, with the support of senior nurses.
To ensure patients and their carers receive ongoing support throughout their treatment.
The post holder will be expected to work closely with the Clinical Nurse Specialists, the multi-disciplinary teams, Consultant Oncologists, and other appropriate professionals involved in the patient's care pathway.
Main duties of the job
To ensure the effective and efficient care and management of cancer patients.
To facilitate the smooth running of the treatment administration clinics on a daily basis.To assist with the implementation of standard setting, measuring outcomes and taking an active part in clinical audit.
