We have a 8 sessions/30 hours per week vacancy for a Chronic Disease trained Nurse to join our thriving, democratic and enthusiastic team at our surgery located in Redditch Town Centre. Redditch is located 15 miles south of Birmingham within easy reach of Stratford, Worcester and surrounding countryside with good transport links and an increasing and diverse population.
Main duties of the job
Undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs according to policy and protocols and adhere to the Practice policies, protocols and guidelines at all times.
Strive to improve standards of care and be involved in identifying practice population needs.
Knowledge of the GMS Contract is an essential part of the role as well as the mentoring of our Health Care Assistants to assist with achieving the GMS Contract needs.
To maintain a safe and effective level of practice and to bring any deficits in training to the attention of the Practice Manager so that these can be addressed.

Our team consists of 4 GP partners, 7 Salaried GPs, 3 pharmacists, 5 Nurses, a HCA and Phlebotomist supported by a fantastic administrative team. We are passionate about teaching undergraduate students from Birmingham Medical School, FY2 Doctors and GP Trainees.

Main Duties and Responsibilities
Assist GP's to help deliver effective implementation of management of pts in Primary Care.
Support the implementation of evidence-based guidelines in conjunction with the practice nursing team.
Encourage the development of integrated services within Primary Care to include integrated practice based teams and multidisciplinary working.
Promote your own continuing professional development.
Recognize the opportunities for working in partnership with the PCN and Practices involved within the PCN.
Participate in an annual appraisal.
Support the practice in achieving quality indicators and targets of the GMS contract and any relevant National Service Frameworks.
Experience in Chronic Disease Management - Respiratory (Asthma/COPD)
Undertake opportunistic screening when clinically required.
Perform venipuncture and electrocardiography
Undertake wound care/ management/dressing techniques.
Leg ulcer management, Doppler and bandaging.
Perform cervical screening and promote breast awareness.
Administer adult and paediatric injections under an individualised prescription or Patient Group Direction, ensuring safe storage, rotation and disposal of vaccines and drugs.
Provide advice on lifestyle issues, assist and support patients in changing their unhealthy behaviours and refer appropriately.
Provide an adult and childhood immunisation service working to Patient Group Directions - following appropriate training if required.
Deliver travel health care, including vaccinations and advice for patients - following appropriate training if required.
Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours and suicidal tendencies and be aware of local, referral procedures.
Ensure that current evidence based approach to patients are standard practice
Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries.

Job summary
We are seeking a suitably qualified Chief Cardiac Physiologist to join our team at Ashford and St Peters NHS Foundation Trust. We are a very active department providing a wide range of investigations and are looking to expand our team of multi-skilled cardiac physiologists.
The successful applicant will be influential in the direction of the department and development of the EP services that we provide; we are the leading centre in Surrey for Electrophysiology studies and ablation. The applicant will be participating in 24/7 PPCI service.
Applicants should have extensive experience in invasive and non-invasive cardiology, and be specialised in Electrophysiology & Cardiac Rhythm Management. The post holder will still benefit from the opportunity to rotate through all aspects of the department as EP lists run two to three days each week.
The department offers a wide range of invasive and non-invasive investigations. including echocardiography, physiologist-led exercise testing, 24 hour ECG and BP monitoring and event recording. We have a busy Pacemaker/ICD implant and follow-up service; we are the leading implant centre in Surrey for ICDs and CRTs with an extensive remotely-monitored patient population.

Join our East Community Mental Health Team for Older People as a Band 6 Mental Health Practitioner and play a key role in delivering evidence-based, recovery-focused care that empowers adults with complex mental health needs to live independently and thrive. Our Community Mental Health Services are secondary Mental Health Teams who provide assessments for evidence based, time limited treatment and psychological interventions for adults with severe and enduring mental health issues. The focus of our whole service is promoting recovery and independence and developing self-management strategies to encourage people to achieve their potential in the community.
We are a multidisciplinary team who provide advice and support for GPs and work with individuals and their carers and can refer or signpost to a number of statutory, independent and voluntary sector agencies. We seek to work closely with individuals and their families as well as other organisations where consent has been given.
Main duties of the job
To ensure provision of the necessary physical care to help people who use our services i.e.: personal hygiene, diet and fluid intake, clothing and laundry, personal environment, manual handling, elimination, maintaining rest and sleep and activity, physical observations.
To act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs, actively supporting the individual to fulfil these.
To ensure provision of therapeutic activities, emotional support and stability to clients to ensure effective engagement whilst maintaining professional boundaries.
To create, develop and maintain professional supportive relationships with all members of staff with other professionals and agencies to enhance recovery.
To ensure prescribed physical observations are undertaken as follows: blood pressure, temperature, pulse, respirations, blood sugars, urinalysis, weight and height and act upon exceptions appropriately

Job summary
We are a well-established, popular and friendly practice at the heart of the local community in Hendon, London NW4 and are seeking an experienced Prescribing Pharmacist to join our forward-thinking clinical team.
We are a training and teaching practice based on a single, practice-owned site, with approximately 12,000 patients and a full range of clinical and non-clinical team members. Job Summary
St. Georges Medical Centre is seeking an experienced Clinical Pharmacist who is a qualified Independent Prescriber to join our multidisciplinary primary care team.
The post holder will work closely with GPs, nurses, HCAs, pharmacists and other members of the practice team to provide high-quality, safe and effective medicines management and clinical care.
The role will include managing patients with long-term conditions, undertaking structured medication reviews, medicines optimisation and independent prescribing within the post holders clinical competence.
We are particularly interested in appointing a pharmacist with experience or a strong clinical interest in diabetes, who can contribute to the management and optimisation of treatment for patients with Type 2 diabetes and other associated long-term conditions.
Main Duties and Responsibilities
The post holder will: -
- Undertake patient-facing clinical medication reviews and Structured Medication Reviews.

Job Summary
St. Georges Medical Centre is seeking an experienced Clinical Pharmacist who is a qualified Independent Prescriber to join our multidisciplinary primary care team.
The post holder will work closely with GPs, nurses, HCAs, pharmacists and other members of the practice team to provide high-quality, safe and effective medicines management and clinical care.
The role will include managing patients with long-term conditions, undertaking structured medication reviews, medicines optimisation and independent prescribing within the post holders clinical competence.

We are seeking two exceptional individuals to act as Enhanced Care Team leaders, who will deliver effective and efficient operational management and clinical oversight of enhanced care within the organisation to our most vulnerable patients. The postholder will work closely with emergency department and inpatient ward staff, providing expert assessment and allocation of resources, and providing motivational leadership to a team of junior staff, role modelling a psychologically safe and trauma-informed approach.
Main duties of the job
The enhanced care team leader will be responsible for working within the set budget, clinical governance, and staff development and training. The post holder will be expected to provide professional leadership within the clinical area and provide direction, and inspirational leadership to all members of the multi-disciplinary team working alongside the enhanced care team. The post holder will be involved in the development of protocols/guidelines and patient information.
The post holder will be responsible for ensuring that there are systems and processes for the planning, provision and review of enhanced care. The post holder will ensure that the enhanced care team deliver therapeutic engagement and interventions to patients with mental health conditions, cognitive impairment or those experiencing psychological distress whilst working in collaboration with internal and external stakeholders such as the Liaison Psychiatry Service.
They will provide regular reports on the effectiveness of the team in delivering quality care by reducing restrictive practices and incidents of harm.

Job summary
Birmingham Children's Hospital is seeking to appoint a Clinical Fellow in Paediatric Medical Specialties- Hospital at Night to support our dynamic paediatric services.
The post-holder will join a team of medical junior doctors within the hospital.
During the daytime they will be attached to one of the medical teams and participate in the daytime activities of that specialty including ward rounds, clinic, procedures, MDT meetings, departmental teaching, review of Daycase patients and patients attending the emergency department for that specialty.
Out of hours, both posts will contribute to the Hospital at Night rota, covering medical inpatients and new admissions alongside a multi disciplinary team of doctors and nurses to provide effective, safe care.
Main duties of the job
The post-holder will join a team of medical junior doctors within the hospital. Medical specialities at our hospital include general paediatrics, endocrinology, gastroenterology, hepatology, respiratory, neurology, haematology, oncology, rheumatology, inherited metabolic diseases, renal and cardiology. As this is a rotational post, 6 months will be spent with each speciality depending on the training needs of the applicant and the requirements of the various speciality departments. After 6 months the fellow will move to another speciality

Job summary
BPAS are looking for a Surgeon to perform terminations of pregnancy in our Doncaster Clinic. This role would be every Wednesday.
As a treatment doctor/surgeon you will provide safe, effective and comprehensive surgical and medical terminations of pregnancy to BPAS clients.
This role is working at BPAS Doncaster with the option to provide services in other areas nationally if you are able. We welcome candidates who currently operate between 13+6 and 24 weeks, along with those who are keen to train and progress towards working safely and competently at higher gestations within the legal limit.
BPAS is an independent healthcare charity which has been advocating and caring for women and couples who decide to end a pregnancy for more than 50 years. We are the leading specialists of abortion advice and treatment in the UK, taking care of almost 80,000 women each year in over 50 clinics nationwide. Nearly all the women we see have their care paid for by the NHS.
About You
With a Bachelor of Medicine or equivalent and full General Medical Council registration plus a license to practice, you will be passionate and committed to reproductive healthcare. You will ensure that all BPAS clients are treated in a professional and caring way and have unwavering support for a woman's right to choose abortion.
A good communicator you enjoy building strong working relationships across the business and enjoy maintaining such relationships as well.
Main duties of the job
- Carry out termination of pregnancy by the appropriate method to evacuate the products of conception and ensure complete abortion and in accordance with BPAS clinical policies and procedures
- Complete client consultation if needed and complete the statutory documentation
- Assess and provide advice on the management of clients with medical or other conditions which may affect the abortion process.
- Be present in the operating theatre throughout each procedure and remain on site until all clients have been assessed fit for discharge from recovery or passed to the care of another on-site doctor
- Provide support, advice and guidance to nursing staff in the management of unexpected outcomes
We are also looking for:
- Experience of independent practice for any abortion procedures/gestations relevant to the post, or willingness to be trained in-house.

Job summary
Mortuary Services at the Whittington hospital incorporates a busy hospital mortuary as well as a public mortuary for the London Borough of Islington. The mortuary is fully licensed by the Human Tissue Authority, the UK regulatory body that oversees the use, storage and disposal of human tissue. An autopsy service of the highest standard is provided both for the hospital and H.M Coroner, Inner North London District.
The mortuary also offers a CT scan autopsy service to the police and wider community.
An opportunity has arisen within this mortuary for a qualified Anatomical Pathology Technologist to join our team. The individual must hold the Level 3 Diploma in Anatomical Pathology Technology or equivalent. They must be motivated, enthusiastic and flexible and able to work in a challenging and sensitive environment with a calm and professional attitude. Frequent exposure to emotional and distressing situations is required to carry out this role.
Main duties of the job
The successful candidate will also contribute to supporting the hospital Medical Examiner (ME) system.This involves the administration of documentation pertaining to patients that die within the hospital and local community.
The candidate will carry out the technical work of the department with particular reference to the management of the autopsy suite and body handling area as required by the mortuary manager. They will assist the mortuary manager in the monitoring and control of work and supervision and training of junior staff.
The candidate will display good organisational skills, whilst carrying out varied and complex tasks.
The candidate will also be required to assist with the arrangement of 'Public Health' funerals' on behalf of London Borough of Islington.
Close liaison with a variety of other professions including Coroner's officers, police, doctors, nursing staff and funeral directors is therefore an essential function, as is communication with bereaved family and friends.
The candidate will be required to participate in the 'Out of Hours' on call system.
The Whittington mortuary is open 8am - 4pm Monday to Friday but these times will be flexible dependent on the autopsy schedule which often begins before 6am.

Job summary
The post holder will proactively manage a caseload of patients requiring complex case management in order to prevent hospital admissions and facilitate early discharge to improve patient health outcomes. Patients requiring case management in the H@H AND UCR Service will be identified by case-finding in the Ambulatory Emergency Care (AEC) unit, Accident and Emergency Department, Whittington Health wards, North Middlesex Hospital A&E, ULCH @home, LAS, Care Homes, Community Nurses and via GP referral.
Patients will be identified as not requiring hospital admission but need high acuity Community support to go home safely.
Working autonomously, the post holder will have continuing responsibility for proactively monitoring and managing patients on the caseload. The post holder will be expected to develop skills in physical assessment, diagnostic reasoning and nurse independent prescribing and use these both in the community and ambulatory care centre setting. Using these skills will assist in the recognition of early symptoms of disease exacerbation, acute illness and injuries. The post holder will be expected to co-ordinate care and navigate the patient through pathways across the primary, secondary care interface, including the prevention of unnecessary admissions, facilitating appropriate ones through in-reach case finding in both the hospital settings also via Community referral.
Main duties of the job
o Facilitate the early discharge of patients from the Whittington Health and NMUH inpatient wards and complete their recovery in the home environment by working collaboratively with the AEC Consultant.
o Be aware of and utilise appropriate resources within the H@H AND UCR team and in the provision of total patient care. Managing the team on a day-to-day basis, supporting junior staff with decision making and skills development.
o Undertake a comprehensive baseline assessment for each identified patient in conjunction with the AEC consultant, patient, relatives, carers and patient care team.
o Use the assessment to agree and implement a patient held personalised plan of care to be agreed with the patient, relatives and carers and the patient care team.
o Ensure that there are effective communications with other agencies about care plans to support patients at home.
o Undertake prescribing as required and to ensure that medications are reviewed and highlight possible interactions and changes
o Perform and document examinations to support proactive monitoring of chronic diseases and identification of early symptoms of exacerbation and liaise with the MDT
o Work with the AEC consultant, ward consultants and/ or General Practitioner and patient care team to facilitate this and reduce hospital admission.
o Lead by personal example of best infection control practice at all times.

- Nursing Associate on the NMC register.
- OR Assistant Practitioner with a health-related Foundation Degree e.g. Foundation Degree in Health and Social Care.
- OR Overseas nursing qualification without NMC registration at UK Academic level 5 Evidence of numeracy and literacy - minimum national qualification level 2
- Experience of working under appropriate supervision as part of a multidisciplinary team.
- Experience of providing and receiving complex, sensitive information.
- Knowledge of nursing procedures and practices in an acute hospital setting

- Assessed as competent to administer medications
- Acute apprenticeship units as appropriate to UHS Care Group/Divisional requirements.
- Extended clinical skills e.g. venesection/cannulation
