Main duties of the job
- Deliver diabetes reviews and ongoing management.
- Provide patient education and lifestyle advice.
- Monitor and optimise diabetes treatment in line with national guidelines.

You will work closely with the Associate Director of Operational Productivity, representing BI as a key stakeholder the programme team. Main duties of the job
Work as a key member of the Business Intelligence Team providing expert analytics in relation to: Model Service planning
Utilisation of Capacity and Demand Modelling tools
Utilisation of benchmarking tools to drive capacity and demand improvement
Delivery of informatics to inform the Annual planning cycle
Provide expert interpretation of complex data to drive performance, quality and service improvements
Provide capacity and demand expertise and direction to the Divisional Analyst Team
Provide proactive support to Divisional teams working to build effective working relationships
Act as a role model within the BI team, supporting and developing colleagues to maximise their learning and potential
Work proactively with colleagues in BI to develop reporting solutions that meet the needs of Divisions

Main duties of the job

This is a 6 PA substantive Consultant Psychiatrist in Community Old Age Psychiatry: Newcastle North OAP CTT.
This service operates at the St Nicholas Hospital in Gosforth, There are close links with the other Newcastle older adults' community mental health teams and inpatient services, as well as the large all-age liaison psychiatry services at RVI, and the Universal Crisis team.
You will join an experienced and well-established OAP CTT service which provides assessment and treatment for older adults with both functional and organic mental illnesses.
You will join a skilled multi-disciplinary (MDT) workforce, including Consultant colleagues, Specialty doctor, resident doctors, non-medical prescribers, Nursing colleagues, Psychologist and Occupational therapist.
As a Consultant, you will know what it means achieve this by leading a team. Striving for a better service leads to a better you.
Working 6 PAs a week over 3 days alongside the multidisciplinary team, including SAS doctors, resident doctors, and wider clinical team, you will be the clinical expert from a medical perspective to ensure best practice care is delivered and support the team.

We are looking for a proactive and dynamic physiotherapist to join our expanding paediatric physiotherapy team at North West Anglia NHS Foundation Trust, to lead and develop the Cystic Fibrosis Paediatric Physiotherapy service.
North West Anglia NHS Foundation Trust is a shared care centre for CF who work together with the Tertiary Centre at Addenbrooke's Hospital to provide in-patient, out-patient and home care to children and adolescents living in the Peterborough City area. As a Paediatric Clinical Specialist Physiotherapist in CF you will work closely with the CF MDT to deliver physiotherapy support to children and their families across the acute and community environment. CF care has dramatically changed in recent years and it is therefore essential that you have experience working with patients who have chronic respiratory conditions and knowledge of exercise/health prescription.
The post holder will be required to participate in our on call rota (adults and children). The successful candidate will be required to be able to work flexibly to meet the needs of a growing service, which will also include working on our children's ward.
Interviews for this role will be conducted in person to assess relevant skills effectively.
An element of remote working would be considered for the right applicant.
Main duties of the job
The Highly Specialist physiotherapist working with the Paediatric Therapy Team develops the quality of the clinical service and staff that deliver it. The post holder will work using an enhanced level of practice applying specialist knowledge and skills in the field of paediatric cystic fibrosis. Working within the cystic fibrosis multi-disciplinary team they will assess and treat a specialist complex caseload of patients and make clinical decisions for treatment plans.
They will act as the profession specific lead to support the training and development of the clinical staff within the Paediatric therapy team and wider multidisciplinary team. To lead on service development, audit and assist in research initiatives to deliver on best practice evidence based care in line with trust and national priorities. They will be responsible for the delivery of key quality performance indicators linked to providing high quality care.

An exciting opportunity has arisen for Trainee Psychological Wellbeing Practitioners to join the NHS Talking Therapies Service delivered jointly by Coventry and Warwickshire Partnership Trust (CWPT) and Coventry and Warwickshire Mind. The successful candidates will be joining a well-established service that continues to offer high quality; evidence based psychological interventions for our identified populations. The posts are for the Coventry, North Warwickshire & Rugby & South Warwickshire.
The PWP training programme is a 12-month course. The programme combines clinical competency development with academic and work-based learning through close collaboration between Coventry & Warwickshire Partnership Trust and Coventry University. Successful applicants will need to commit to the academic and competency development requirements of the course.
IMPORTANT:
If you are shortlisted for the trainee role, you will be entered into a two-stage recruitment process:

Job summary
We are looking for dynamic, visionary and hard-working candidates to join our expanding Respiratory Department. We are advertising a full time post for excellent candidates to join the COPD Team. The team consists of Band 6 and 7 Respiratory CNS's and Respiratory Specialists Physiotherapists. The team will be responsible for identifying and assessing inpatients in hospital. Suitable in-patients with an exacerbation of COPD who are from the Swansea Bay area will be supported at home by this team. The team also works with patients ,GPs and Virtual Wards to prevent avoidable admissions .The successful candidate will also support patients requiring Non Invasive Ventilation ( NIV ) in the Community
Main duties of the job
The successful candidates will be expected to work with colleagues to deliver a unified respiratory service across Swansea Bay UHB.
Wards are visited daily to identify and support patients who are suitable for an early discharge and patients are then followed up at home
The team manage a caseload of patients to provide education, teach self management skills and monitor recovery from acute exacerbations of COPD in patients homes
Education is encouraged in the team by formal education and an in house education programme.
The successful applicant will be supported via an agreed bespoke induction plan and at regular MDT meetings.
Support patients with COPD requiring NIV when new starts and during exacerbations. Previous experience of COPD and NIV is essential

Flexible cystoscopy (with state-of-the-art Olympus flexible cystoscopies), uro-dynamics, nurse-led and Consultant andrology clinics and intra-vesical chemotherapy clinics.
ESWL is provided by the regional mobile supplier and overseen by a Nurse Specialist with an associated Stone MDT.
Outreach clinics are held in the surrounding towns of Newmarket, Thetford, and Sudbury. The new appointment would be expected to undertake outreach clinics to fit with the rest of the department after consultation.
The eight-bedded intensive care unit has recently been substantially enlarged and upgraded. A high dependency facility has recently been opened. There is a 24-hour recovery unit attached to theatres.

Band 3 Health Care Assistant Children's Complex Care
An exciting opportunity has arisen to recruit Band 3 Health Care Assistants. The team supports children with complex health needs in their own homes and schools. We are looking for confident innovative HCA's to join our Children's Complex Care team working across Birmingham.
You will participate in the provision of basic nursing care and specialist care for children who have a range of complex disabilities due to chronic disease, congenital abnormalities, trauma, or who are terminally ill and at the end of their life. The post holder will work as part of the nursing team and supporting the other members of the multi-disciplinary Team. The post holder may be required to work within the service user's home / a hospital / hospice / school, or day centre environment depending upon their individual needs at that time
Main duties of the job
The role involves delivering care to children in a variety of settings, including home and school. The successful candidate will be trained in a variety of clinical skills such as gastrostomy feeding, suction, oxygen therapy using a competency based framework. You will received continued support in the role.
You will need a good level of basic education including computer skills and at least NVQ level 3 in a child or young adult related subject or equivalent level of experience related subject. Experience working with children who are ventilated or have complex health problems would be beneficial but not essential. It is also important the successful candidate is innovative in providing play and learning opportunities for the children.
You will need to be flexible in the community setting and a good team player and be able to develop collaborative working relationships with other healthcare professionals, services and agencies.

The Hotel Services Catering Assistant is responsible to the Hotel Services Supervisor to undertake a range of catering duties, which include the preparation of beverages, snacks, salads, sandwiches, buffets and cooking of hot meals for both patients and staff within the Health Board. To carry out other relevant duties associated with the catering operation and periodically those as directed to support the wider Hotel Services Team e.g. cleaning or porter tasks.
Main duties of the job
We are looking to recruit to a Hotel Services Catering Assistant at Machynlleth
You will support the local delivery of hotel services catering and associated facilities activities, ensuring that they are delivered at the levels agreed and that they meet with full compliance with all appropriate legislation and local policies and procedures. You will need to be self-motivated, have a flexible approach to work, able to work as part of a team and have the ability to work on your own initiative. Previous experience is an advantage, but not essential, as full training will be given. You will be working as part of a rota that can include evenings and weekends.
We would particularly welcome applications from Welsh speakers; but even if you don't speak any Welsh, or want to develop your skills, don't worry: the health board will support you with training.

Job summary
Are you an experienced diabetes nurse specialist looking for a next-step senior leadership role in a genuinely new model of care? A rare opportunity has arisen to join the Tier 3 Clinical Oversight Group of the Diabetes Neighbourhood Health Programme as Diabetes Matron a Leeds system role hosted by South and East Leeds General Practice Group.
This role sits within the Tier 3 Clinical Oversight Group alongside the Consultant Diabetologist, Consultant Pharmacist, Highly Specialist Pharmacist and Clinical Psychologist. The Tier 3 group provides advanced clinical decision-making, MDT leadership and specialist oversight across the multi-neighbourhood footprint interfacing closely with Tier 2 Neighbourhood Support Teams, Practice Diabetes Leads at Tier 1 and specialist Tier 4 services at Leeds Teaching Hospitals.
Working pattern: this is a 0.4 WTE post 15 hours per week based across the multi-neighbourhood footprint with flexibility to be agreed at appointment. There is genuine scope to shape how the Tier 3 nursing offer is delivered and evaluated over the three-year development period. This is not a ward-based Matron post it is a system-facing, MDT-embedded senior nursing role designed to work across organisational boundaries.
Main duties of the job
Main duties
You will provide senior nursing leadership within the Tier 3 Clinical Oversight Group of the Diabetes Neighbourhood Health Programme. Your focus is advanced clinical decision-making for complex and atypical diabetes presentations, MDT leadership, and workforce development across the multi-neighbourhood diabetes nursing offer.
The day-to-day mix includes weekly Tier 3 MDT attendance (combined with the existing LTHT community diabetes MDT), advanced clinical assessment for complex or atypical presentations, contribution to step-up and step-down decisions across the tiers (including new CKD Year 2 step-down and stable Type 1 Year 2 step-down), and clinical supervision of the Tier 2 Clinical Lead Nurses for Diabetes.
You will provide advanced nursing input to complex case sign-off, including insulin regimen optimisation, pre-operative optimisation, hypoglycaemia unawareness work-up, and management of atypical presentations. You will lead the interface between the community programme and Tier 4 services at LTHT, working across the acute, community and primary care boundary.

Job summary
The main purpose of the MDT Co-ordinator role is to support the clinical team and patient pathways at the Cancer MDT Meeting to ensure that the Trust meets the requirements of the national Cancer Waiting Time Standards. This will involve collecting data from a variety of sources and entering the data items into the Trust's cancer database Somerset. The post holder will need excellent organisational and communication skills and a good attention for detail to be successful in the role.
This role is primarily based at West Middlesex Hospital, with cross-site working at Chelsea & Westminster hospital as/when required
Main duties of the job
The post holder will be supporting the cross cover of all tumour sites with dedicated time to support the Gynaecology MDT meetings where required.
They will work with the clinical teams and liaise closely with their band 4 tracker to ensure all patients receive the upmost care. The post holder will ensure that all patients progress through their pathway post MDT, ensuring all treatment plans and next steps are in place or escalated. The aim of an MDTC is to provide the highest quality of care to patients and ensure they are being processed effectively and efficiently through the cancer pathway.

The MDT Coordinator must review data uploaded on Somerset to ensure accuracy and completeness and arrange to meet weekly with the MDT lead and/or lead CNS to validate this.
Circulate and collect an attendance register for the MDT meetings and maintain an electronic record of attendance for core members.
Ensure a clinical member of staff is documenting outcomes from the start of the MDT
Ensure accurate minutes of the MDT are circulated to the distribution list post MDT.
Ensure that every patient with a cancer diagnosis is discussed at an MDT meeting.
Ensure delays in patients pathway are escalated to services so they can be resolved, for example: diagnostic tests.
Ensure actions are raised on the Federated Data platform (FDP 360) Tool for next steps required and updates are tracked tracking accordingly.
Ensure actions not responded to on the Federated Data platform (FDP 360) Tool are escalated appropriately to the next level.
Develop and maintain effective working relationships with counterparts in other local Trusts.
Ensure clinician completes a referral proforma and liaise with the appropriate MDT Co-ordinator for patients requiring discussion at a specialist MDT at another hospital (Imperial, The Royal Marsden & Chelsea & Westminster). Ensure that any imaging and pathology slides are sent to the other hospital in time for the MDT meeting.
Monitor the generic inbox and ensure any patient requiring upgrade is added to the Somerset Cancer Registry (SCR) database in line with the process
Correct and update any validations in relation to COSD and other required data monitored audits.
