Main duties of the job
We are seeking an experienced and motivated Transition Lead Nurse (Band 7) to lead a high-profile programme focused on improving the experience and outcomes of young people transitioning from children's to adult health and care services. This is an exciting opportunity to drive transformational change across a complex system, working in partnership with health, social care, education, voluntary sector and independent organisations. As the Transition Lead Nurse, you will be responsible for developing, implementing and embedding integrated transition pathways, policies, Standard Operating Procedures (SOPs), and best practice guidance that promote consistent, safe and person-centred care. The post holder will work collaboratively with a wide range of stakeholders to reduce variation in practice, strengthen partnership working, and ensure young people receive coordinated support as they move between services. We are seeking an experienced and motivated Transition Lead Nurse (Band 7) to lead a high-profile programme focused on improving the experience and outcomes of young people transitioning from children's to adult health and care services.

Job summary
The role of Community Nurse is to work in partnership with service users in the designated care group with complex health presentations, developing their care plans and recovery plans to enable them to lead fulfilling lives. Main duties of the job

About the unit
Our refreshed clinical model includes purposeful admissions, structured care with early review points, strong links with COAST (Crisis Outreach Acute Support Team), and a 7-day therapeutic timetable including evenings and weekends.
You will work as part of a highly skilled multidisciplinary team, supporting young people, families and carers through assessment, treatment and discharge planning in a therapeutic inpatient environment.
This role is ideal for an experienced registered nurse ready to take the next step in leadership within CAMHS inpatient care.
Main duties of the job
- Undertake comprehensive mental health and risk assessments for young people with complex needs
- Act as named nurse and care coordinator, contributing to care review processes and mental health tribunals
- Deliver and evaluate person-centred, trauma-informed nursing interventions
- Provide clinical leadership and act as a senior decision maker during shifts
- Regularly take charge of the ward, coordinating care delivery and staffing

At Wokingham Hospital, we have two wards within our Inpatient Unit, caring for adults of any age with a wide mix of health care needs. We are a 24 hour service 365 days a year, mostly nursing led with support from the medical and therapy teams. The role aims to serve our patients, promote their independence, and support the service and team.
In addition to ensuring you develop the skills needed to deliver a high standard of personal care to the patients on our wards, BHFT is keen to provide access to any additional training. A number of our current HCAs have progressed to achieve qualifications including NVQ, sponsored Nurse Associate Training, and overseas Nursing adaptation programmes. We recognise not all staff wish to pursue these qualifications, so will also provide access to shorter non-academic courses that will enhance the care you deliver on a daily basis too.
The unit is based in Wokingham close to shops and public transport for easy commuting, and Wokingham Community Hospital staff are eligible for 'fringe London weighting' under agenda for change, which equates to an additional 5% of your basic NHS salary, and free staff parking on our Hospital site
Main duties of the job
- Work closely as part of the Multi-Disciplinary Team to support qualified nurses in the care of patients requiring medical and rehabilitation support.
- Responsible for the assistance of patient's care needs, such as washing, dressing, mobilising and promoting their independence in all activities of daily living.

Job summary
An exciting opportunity has arisen for an enthusiastic, organised and motivated Medical Secretary to join our Faster Diagnostic Service.
The service provides timely assessment, investigation and diagnosis for patients referred with potential cancer symptoms. The service supports patients presenting with a wide variety of symptoms and works closely with multidisciplinary teams across primary, secondary and diagnostic care to ensure patients receive high-quality, patient-centred care throughout their diagnostic journey.
As a Medical Secretary, you will play a vital role in supporting the clinical team and ensuring the smooth administration of the patient pathway.

Main duties of the job
*To provide a complete confidential general and medical secretarial service for the Consultant(s) and their medical team. To assume responsibility for the daily management of the medical team diary. All work to be carried out on own initiative, without direct supervision and to tight deadlines, in order to achieve Trust standard and targets.
*To touch type from audio and / or shorthand the following documents: clinical letters; results letters. This requires an understanding of medical terminology, phraseology and medications, as well as high levels of concentration. Many documents may be of a complex or distressing nature.
*Knowledge and use of various Microsoft Office Software programmes including Outlook, Word, PowerPoint, Excel, Access (database) to produce clinical letters and other relevant correspondence.
*To provide day to day supervision to secretaries/clerical officers, allocating and monitoring work on a daily basis.
*Provide and be responsible for the training of Medical Secretary trainees.
*To sort, distribute and prioritise incoming mail, using judgement and experience to decide which documents are to be passed to the Consultant(s) and their team for urgent action, and which may be passed directly to other areas for action and information.
*Provide reciprocal cover for administrative Team during annual leave and sickness absence.
