The post holder will assist in assurance that the Community Nursing Service are compliant with statutory and mandatory training as well as wider clinical skills relevant to roles and service. We use electronic clinical records through EMIS and a work IPad and IPhone will be provided.
Main duties of the job
We are seeking a highly motivated and ambitious Registered Nurse to join the wider Community Nursing Team which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates and Healthcare Assistants.
The post holder will ensure a high quality, safe, efficient, and clinically effective locality based adult community nursing service that meets the needs of patients and carers. To provide highly visible clinical and professional skills within the community nursing service. To support the community nursing service to develop and maintain specialist skills relevant to their role to provide a safe and high-quality Community Nursing service, maintain evidence and assurance of this at service and individual level. To support the completion and assurance the Community Nursing service are compliant with statutory and mandatory training.
Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Job summary
We are delighted to announce an exciting opportunity for a permanent Principal Practitioner Psychologist (Clinical, Counselling, Forensic) to join our Oldham Crisis Resolution and Home Treatment Team (CRHTT), based at Royal Oldham Hospital.
This is a 0.8WTE Band 8b position, offering a key leadership role within our dynamic and supportive service. The vacancy comes at a time of significant new investment in the crisis pathway, supporting the transformation of our existing Home Treatment Teams into CRHTTs. The successful candidate will join a 0.8WTE Highly Specialist Psychologist as part of expanding psychology provision.
Our CRHTT provides intensive home-based assessment, intervention, and support as an alternative to hospital admission. The team works with adults experiencing acute mental health crisis, often presenting with high levels of distress, risk, and complexity. The service plays a crucial role in admission avoidance, early intervention, and supporting safe discharge from inpatient care.
The successful candidates will join committed and experienced MDTs, working closely with medical, nursing, OT, and support staff to deliver psychologically informed crisis care.
This is an excellent opportunity to contribute to a growing and evolving service, influence the development of psychologically informed crisis pathways, and make a meaningful difference to service users and their families during some of the most challenging moments in their lives.
Main duties of the job
As outlined in the job description, the main duties of the job include:
- To provide consultation and formulation to colleagues within the CRHTT.
- To provide a qualified specialist psychology service for people referred to the CRHTT care pathway.
- To provide specialist assessment, formulation and interventions for a clinical caseload.
- To provide psychological leadership to the CRHTT care pathway and work systemically across services (e.g., acute services and secondary care) to develop a psychological understanding and context for other professionals, service users and carers.
- To lead the psychological CRHTT provision within borough and support the Trust wide acute pathway when needed, to help ensure equity in provision and service.
- The post holder will propose and implement evidence-based practice, utilising research skills for audit and service developments.
- The post holder will enhance psychological understanding and skill set through the delivery of training, supervision and reflective practice. This will include within the current CRHTT team but also making links with local universities and providing placements for trainee Psychologists.
- To be an integral part of local and national initiatives to improve care quality. Forexample (but not limited to) inpatient transformation, community transformation carein the community and reducing admissions.

Job summary
This post consists of 10 Programmed Activities (PAs), annualized as 8.5 Direct Clinical Care (DCC) and 1.5 Supporting Clinical Activities (SPA). This is an appointment by the Hillingdon Hospital NHS Foundation Trust with license to attend at Imperial College NHS Trust. The post holder must hold full MRCP and a certificate of completion of training (CCT) in Rehabilitation Medicine. We welcome application from trainees in Rehabilitation Medicine who are approximately 6 months from CCT.
Main duties of the job
The appointee will join two consultants in Rehabilitation Medicine and a team of neurologists as part of a friendly and reputable service that works across Hillingdon Hospital and Imperial College Healthcare NHS Trust to provide seamless transition for patients from tertiary care into secondary care and community services. This post provides an excellent opportunity to provide high quality neurological rehabilitation care for patients with spinal cord injury, in a busy District General Hospital or in a tertiary setting at Imperial. 6 of the DCC PA's will be based at the Hillingdon site and 4 PA's will be based primarily at Imperial.
The post holder will provide consultant input to the London Spinal Cord Injury Network and the Major Trauma Network in North West London. The post holder will be responsible for improving the care of spinal cord injury patients needing rehabilitation in the North West London sector. There is scope to develop integrated care across secondary and community services in partnership NW London ICB. This will involve linking with community teams and outreach visits to community settings.

This is a full time Consultant post in General & Adult Psychiatry in the community covering the area of Chelmsford West.
This post has been formed following the transformation of services whereby community mental health teams have been re-established and the teams are closely aligned to the primary care networks. This post is based at the C&E Centre in Chelmsford.
This post provides community services for patients referred to the secondary mental health services from GP surgeries in the Chelmsford west Area of 5 GP's -Chelmer Medical Partnership (formerly Melbourne House Surgery, Humber Road Surgery and Tennyson House Surgery) Dickens Place Surgery, The Writtle Surgery. The post holder will work with Chelmsford community Team. The team covers the Chelmsford area.
There are two other Consultants who work with the Chelmsford team. It is expected that the three Consultants will work together to provide clinical leadership to the team as well as promoting true multidisciplinary team working.
Main duties of the job
The consultant will have responsibility for the clinical care of the patients who are deemed to require secondary mental health intervention and will be expected to work closely with the team manager to ensure the provision of an effective service.
The PCN teams are also being established and the consultant will be expected to provide input into the team to support them.
The post-holder is required to work independently and accountably as part of a team of clinicians offering Mental Health services. Working collaboratively with key partner organisations, service users, carers, clinicians and other practitioners within the multi-disciplinary team in delivering services; providing a 7 day a week, 24 hours a day service for 365 days a year.

Main duties of the job
You will be working on varied roles but your main duties include the following:
Type patient letters relating to clinic appointments, test results, referrals, discharge summaries from digital audio dictation, long hand, or verbal dictation. Aiming towards turning typing around in 48 hours.
Cover the medical secretaries absences (sickness and annual leave) taking telephone messages and undertaking any urgent typing and tasks for the consultant and team.
To assist the medical secretaries in managing patients through their 31/62 cancer pathway and 18 week pathway. To validate pathways and amend them on ConnectEPR when typing. To escalate any possible breaches to Line Managers.
Assisting medical secretaries in the management of in-patient waiting lists ensuring that Trust and National waiting list policies and procedures are adhered to, tracking the patients through their cancer pathway and 18 week pathway for their in-patient stay.
Management of patient investigations using Trust computer systems.
Dealing with telephone queries from patients/other healthcare workers.

Main duties of the job
The post holder provides medical input to an existing Mental Health Liaison team to provide medical leadership, Team advice and guidance and medical consultation to our service users. Both full time and part time applicants are encouraged to apply.
As part of STP investment, a 24/7 telephone access to a trained mental health crisis response team and Core 24 service was established which will offer access to people of all ages to enquire about support for themselves or for family and friends if they are in crisis. Some of these crisis presents themselves at A&E which is managed by Mental Health Liaison Team.
The team is an all-age team, with additional Consultant medical input for under 18 pathways. The post holder will be providing support to adult patients and 18 plus pathway.
The post holder is appointed as a senior professional to provide advanced professional expertise and clinical leadership. As such, the post holder is always expected to conduct him/herself in such a manner as to be demonstrably helping the Trust to deliver its strategic and business objectives, and to act as an ambassador at the interface with external partners and stakeholders. At no time must the post holder bring the Trust into disrepute.
Additionally, the post-holder, as is expected from the holder of a senior public position, must always comply fully with the Nolan Principles and the Standards of Business Conduct.

Main duties of the job
Provide near patient medicine management services as part of our ward-based team across 7 days ensuring cost effective and efficient services are maintained throughout our patients stay. Additional duties include; the assessment of patients own drugs, audits, medication ordering, dispensing and checking of supplies. We are looking for a new Trust Grade doctor to join us at NUH within our Stroke team, at CT3+ level . The post is available for 12 months initially with the possibility to extend if mutually agreed.
This role is for you if you have full MRCP plus experience in specialities including Geriatric Medicine, Neurology, Rehabilitation Medicine, Clinical Pharmacology and Therapeutics or General (Internal) Medicine and to those who intend to make a career in Stroke Medicine.
There will be opportunities to undertake audits, to be involved in governance activities and to take part in service improvement projects to enhance your CV. There is also the opportunity to get involved in seeing patients in the TIA clinics and receiving potential Stroke patients in EDThe post will be working across all stroke areas particularly on the hyper-acute stroke admissions ward, providing support and advice to junior doctors, as well as the ED team.
NUH stroke services also provides 5 days a week 0800-1700 hrs Mechanical Thrombectomy service for the East Midlands .

The Stroke service is across the two sites, both City hospital & QMC and consists of a hyper-acute ward, and acute ward based at QMC and rehabilitation ward at City campus, as well as a daily Fast Track TIA clinic. We are keen to appoint a Specialty Doctor to join our team at 80, St Stephens Road, Norwich, NR1 3RE (Dragonfly Unit). This post is located in the Central Locality, offering services to younger adults between the ages of 18 and 25 years, living in the southern part of the locality. The post is based in the city of Norwich, alongside colleagues covering the northern part of the locality at the main base of the Youth team in the locality.
Main duties of the job
To work with the multidisciplinary team, including Psychologists, Nurses, Therapists and Allied Health Professionals in the assessment, diagnosis and treatment of referred patients; take part in team meetings, supervision and the development of the team. Working within the South Central Norfolk Youth team, the post holder will undertake, exceptionally, to perform additional duties in emergencies and unforeseen circumstances and to be available for such irregular commitments outside normal duties as are essential for continuity of patient care which will on occasion involve seeing people at home and allowing time for 'hard to engage' children and families. The post has 5 PA's made up of 4.0 Direct Clinical Care PA's and 1.0 SPAs. The job plans and working patterns of doctors reflect the functional requirements of the Locality and the National Service Framework for Mental Health and are not determined by catchment area.

The successful candidate will be providing cover across all areas of the Hospital, which will change daily, so a flexible approach to working is essential.
You will be working as part of an established Team who provide a full Hotel Services function across the Hospital.Right to Work in the UK: This position is not eligible for Skilled Worker visa sponsorship. Main duties of the job
Catering - The handling, reheating, and service of cook-freeze meals. Making light snacks including salads, sandwiches and beverages. Washing up and associated cleaning within the ward kitchen and dining room.
Cleaning - Cleaning all areas of the Hospital, including bedrooms with en-suite facilities, bathrooms, day rooms, lounges, corridors, offices and other areas of the Ward.
Other duties - Handling and disposal of waste, curtain changing and laundering.
