Job summary
Dorset HealthCare University NHS Foundation Trust are seeking to recruit a Band 5 (developmental) or Band 6 Community Mental Health Nurse to work our well-established Ferndown and West Moors Older Persons Community Mental Health Team.
If you are a RMN with experience working with people suffering severe mental illness whether this be inpatient, or community based we would love to hear from you.
This post is 30 hours per week (non working day must be a Tuesday or Thursday). This post is being extended to allow for applications from newly qualified nurses, or those practitioners currently working at a Band 5 and looking to develop with support towards a Band 6 whilst working in the community.
The Service is a specialist team providing assessment, treatment and support for individuals suffering from organic and functional mental health illness. Experience of working with older adults with organic/functional mental illness in an inpatient or community setting would be an advantage.
The ideal candidate will be enthusiastic, self-motivated and hard-working, with a positive approach to delivering high quality care to patients and be willing to work collaboratively with carers and other professionals. You will carry a defined caseload and manage this with the support of the team leader and the wider team.
Main duties of the job
We work with a multitude of services across Ferndown and West Moors and surrounding areas to ensure the mental health needs of the older adult population is met.
We are a busy CMHT and we are looking for someone who is reliable, committed and motivated with excellent communication and organisational skills and the ability to prioritise their caseload whilst balancing service demands. In return we are ready to offer you comprehensive and structured management supervision, management you can approach any time and peer support.
The post holders will work as part of an established integrated multi-disciplinary team. This team is made up of a Psychiatrists, Occupational Therapists, Clinical Psychologists, Nursing staff, Support Time Recovery Workers and Business Support Team.
The primary role responsibility is to act as dedicated Care Coordinator for a caseload of patients with whom you would provide person centred, holistic care to assist in aiding their wellbeing and recovery.

Job summary

We are looking for a lead clinician to take overall responsibility for the care of patients on the in-patient and community ward(s) at Clacton Hospital, for 5.75 sessions per week.
Main duties of the job
Duties will include:
- Providing exceptional clinical leadership at Clacton hospital
- Overall responsibility for the care of patients on the in-patient ward(s) at Clacton hospital
- Working closely with medical staff at Clacton hospital and fostering the development of effective clinical teams
- Working closely with the ward managers, ACPs and nurse practitioner teams, to ensure that patient care is managed holistically and is of high quality
- Effective use of EPIC and digital patient records, including in basket pools
- Participation in quality initiatives and clinical audit
- In collaboration with clinical colleagues and service leads in primary, community and acute care pathways, ensuring the highest levels of quality care (patient safety, patient experience and clinical effectiveness) across the wards at Clacton hospital and seeking to continually learn and improve
- Creating a patient safety culture by engaging with appropriate governance arrangements, enabling effective management of complaints, incidents and risk management, audits and learning to improve patient care.
- Support the development of learners. For example: GP trainees and nurse practitioners acting as a designated prescribing practitioner (DPP), undertaking supervision of trainee ACPs and GP trainee rotations.

Main duties of the job
- To support the triage and assessments of individuals who referred into the service alongside the wider multi-disciplinary team that includes community mental health nurses, psychology, consultant psychiatrist, occupational therapists, peer support (lived experience) workers, health care support workers and housing and financial advisors.
- You will be responsible for undertaking assessments individually, alongside the wider team and/or alongside our partners in order to establish the person's goals, needs, risks and strengths to generate a formulation based plan of care with the individual and their support network
- You will alongside wider colleagues in the team acting as a point of contact to provide clinical advice and guidance to patients and families where needed, as well as partner services for those being referred to the service
- There will be opportunity where felt appropriate to provide brief interventions to individuals referred into the service to support their recovery and where it may prevent a need for more long term involvement from the service.
- As part of the local mental health alliance network you will have opportunity to attend our weekly meetings to provide opportunity to look at how best as services we can work together to support individuals who are presenting with multiple needs, risks and complexity. Job summary
First Contact Mental Health appointments provided to patients registered at our four practices. Offering initial review, onward referral to external service providers, appropriate prescribing eg SSRIs and follow-up appointments. This is a new position within the PCN structure and would work alongside existing mental health services offered. Although we would welcome candidates with previous Mental Health experience, this is not necessarily required. We would be looking to support this role with appropriate training opportunities, enabling successful candidates to develop specialist skills within this area. There would be regular support from the member practices and the local community mental health trust.
Main duties of the job
Assessing patients experiencing mental health concerns in a primary care setting. This will be face to face in clinic or via telephone consultation but will not include home visits
Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the organisation
Assessing the mental health care needs of patients with undifferentiated and undiagnosed problems
Onward referral to secondary care eg CMHT and/or voluntary sector organisations such as the Recovery Education Centre and Access Wellbeing Hub
Developing care plans for health in consultation with patients and in line with current practice disease management protocols
Providing advice, guidance and health education
Recording clear and contemporaneous consultation notes to agreed standards

The Role
Over the last six years, our Clinical Pharmacy team have become a fundamental part of Mereside Medical Group Practices highly valued and appreciate by out GPs, Practice teams, and patients thanks to a track record of making a genuine difference to patient outcomes and GP workloads.
The team comprises four Clinical Pharmacists and three Pharmacy Technicians, working across three GP Practices - Cathedral Medical Centre in Ely, Haddenham Surgery, and Staploe Medical Centre in Soham.
Each Clinical Pharmacist is offered clinical supervision by a GP, and managerial supervision is offered by the Lead Pharmacist.

- Undertake clinical medication reviews and Structured Medication Reviews, including for care home residents and patients with long-term conditions.

Job summary
We are excited to offer a 0.8 wte qualified Children's Wellbeing practitioner (CWP) the opportunity to join our service on 1 year fixed term contract.
The CWP team sits within our County Wide Mental Health Support Teams (MHST) early intervention pathway. We are looking for practitioners with proactive and positive attitudes to support our waiting lists, so that children can be seen quicker.
The successful candidate will join our CWP team to work with children presenting with mild-moderate presentations of mental health who require Low Intensity- Cognitive Behavioural Therapy (LI-CBT). The role may involve some travel around the county, seeing young people in community and clinic settings and so having a driving license and car will be essential to the role.

Ealing NHS Talking Therapies was one of the first Talking Therapies services to launch in 2008, and is one of the three services provided by West London NHS Trust. We are a large team of over 70 members of staff and are currently working on further expansion. Our main base is in Ealing where we have access to over 16 clinic rooms and two open plan offices. In addition to this, we run clinics at GP surgeries across the borough, covering areas such as Southall, Acton, and Chiswick. Our service is well-established and many of our staff have been with us since the beginning.
Ealing Talking Therapies is passionate about offering a great quality therapy to our patients and we are very proud to be able to consistently offer all the Talking Therapies-approved therapies, such as EMDR, DIT, IPT, BCT, and CfD. Liaising with other services and following up on referrals
Screening referrals as they come in and engaging in clinical discussions and decision making on appropriateness of referrals.
Running and developing specialist safeguarding training with the service.
Reviewing incident reports and providing feedback and learnings to clinicians on these.
Working with clients to resolve concerns or questions they may have about the service to improve treatment outcomes.

We are seeking a dynamic and compassionate Consultant Psychiatrist to lead our Forensic Low Secure Service on Ash Ward and work in partnership with our Specialist Community Forensic Service.
Ash Ward has been a member of the Quality Network for Forensic Mental Health Services since its inception and is part of the South West Provider Collaborative, providing access to shared bed stock, peer networks, forums, and training opportunities.
The ward is co-located with the Specialist Community Forensic Team and benefits from a fully staffed, stable multidisciplinary team, resulting in excellent relational security as demonstrated through regular external Quality Walkthrough Reviews (QWWs).
The post is supported by a shared Specialty Doctor and will include a full-time Core Trainee once a substantive consultant is appointed.
As Consultant Psychiatrist, you will provide clinical leadership, manage a diverse caseload, contribute to service development, and work collaboratively with multidisciplinary colleagues to deliver high-quality, person-centred care.

- Minimum 3-5 years' experience in generalist HR role
- Experience of end-to-end recruitment and onboarding
- Experience managing HR systems and data
- Experience of managing or supervising staff
- Experience advising managers on performance, conduct and capability
- Experience supporting the development of HR policies
- Experience of dealing with difficult and sensitive situations in a diplomatic and professional manner
- Experience with employee engagement surveys and staff feedback systems
- Experience in conducting audits

- Experience of working within a healthcare or primary care setting

- Knowledge of CQC requirements relevant to HR records and workforce compliance

- Formal training in workshop/meeting facilitation
- Training in employment law or HR best practices
