In this role, you'll work closely with an experienced Medical Secretary Team, providing vital administrative support in a fast-paced and dynamic environment. This is a great opportunity for someone who enjoys variety, teamwork, and making a real difference in the smooth running of our services.
You'll be working alongside a supportive and enthusiastic team, liaising with colleagues across all levels. Strong administrative skills, the ability to manage competing priorities, and a proactive, flexible approach are essential. You'll be expected to maintain high standards of accuracy and confidentiality in all aspects of your work.

Main duties of the job
In this key role, you will:
- Provide a complete medical administrative service including Referral to Treatment (RTT), patient correspondence and editing and formatting letters.
- Liaising with Nurses, Doctors and other members of the multidisciplinary team including inpatient and outpatient bookings teams to provide a quality service for the patients.
- Working as part of a team, sharing responsibility for the smooth running of the administrative service to deliver safe patient care
- To provide a first point of contact, answering all telephone enquiries, taking and relaying messages in a polite and helpful manner, and using own judgement to establish the priority of issues and taking action as appropriate.
You don't need NHS experience - just the ability to adapt quickly in a fast-paced environment and a commitment to high standards. We offer flexibility in working arrangements and a chance to be part of a team that truly makes a difference.

About this Role - The Integrated Transfer of Care (ITOC) Lead provides visible, senior operational leadership across acute, community, social care and VCSE partners. You will lead the ITOC hub as a single integrated function, ensuring consistent processes, shared priorities and a unified approach to discharge flow and admission avoidance.
Drawing on real-time intelligence, frontline insights and strong cross-organisational relationships, you will coordinate partners to activate community pathways, prevent unnecessary admissions and support timely, safe discharge home. You will act as the consistent point of escalation for the locality - unblocking delays, resolving operational pressures and aligning decision-making across organisational boundaries.
This is an integrated post, funded by five organisations offering a unique opportunity to influence system-wide improvements whilst maintaining strong operational presence within acute and community settings.
Main duties of the job
Key responsibilities
- Provide visible, senior operational leadership for discharge flow and community coordination across all partners
- Lead the ITOC hub as one integrated service, ensuring shared priorities and consistent standards
- Maintain a strong on-site presence across acute and community settings to support real-time problem solving
- Act as second-line escalation for complex discharges, working with senior managers across organisations
- Coordinate cross-system responses to operational pressures, including during critical incidents
- Use data, dashboards and intelligence to identify trends, variations and bottlenecks, driving improvements
- Lead targeted transformation to strengthen Home First pathways and reduce delays
- Support workforce leadership, culture and well-being across the integrated hub
- Ensure compliance with national discharge guidance and contribute to system-wide planning and assurance

We have an exciting opportunity for a registered mental health nurse to join our Early Days in Custody Team (EDiC) at HMP Elmley.
The EDiC team will act with the following principles:
Holistic - the EDiC team is multi-disciplinary, drawing on expertise from the other teams within healthcare to provide a knowledgeable and skilled service with the ability to draw upon specialist input rapidly when needed.
Curious - the EDiC team will engage with patients with clinical curiosity - drawing on all the information available to them to understand patient needs. The team will engage where possible with patients' families/carers to fully understand patient's needs and assist in helping maintain family relationships.
Prioritising Safety - Keep prisoners safe by identifying and addressing immediate risks for that patient including self-harm, risk of death by suicide, and unmanaged substance misuse dependencies.

Main duties of the job
As a EDiC Practitioner you will:
- Ensure that new prisoners are supported during their first 14 days in the prison to access relevant healthcare offers and manage their general health and wellbeing whilst in prison.
- Identify the wider needs of patients, through the completion of a comprehensive assessment and working in partnership with them to co-produce a written care plan that addresses their health and social care needs.
- The role will include covering some work for the mental health in-reach team. For example:
- Care planning
- Caseload reviews
- Administer antipsychotic depot medication.
- Collect and synthesize all relevant clinical history to ensure safety, continuity of care and minimal re-assessments for the patient and ensuring that the records held for patients are up to date and accurate.
- Ensure ongoing care for every person requiring it, with clear pathway management in place that is understood by the patient and their families /significant others.
- Information is shared where appropriate and consent has been given to ensure that care plans and risks for patients are widely understood by all staff supporting them, including HMPPS staff where appropriate. This will be done with full patient consent.

- The post holder will be responsible to take part in reception screening for new prisoners on the day of first entry into prison. This will identify immediate health, risks, and safety needs.
- To be able to develop initial mental health crisis plans for patients who are presenting as particularly at risk of a mental health crisis.
- The post holder will be responsible for carrying out comprehensive health and social care assessment (secondary screening) within seven days for all prisoners who are willing to undertake this assessment.

Job summary
An exciting opportunity has arisen for a Junior Clinical Fellow ST1-2 (CT1-2 Level or equivalent) to join our Surgical Department.
The post is based at Bournemouth Hospital, and is suitable for candidates who ideally will already have at least 12 months' pre-registration experience, who would like to build on their experience of general surgery as well as emergency surgery.
Main duties of the job
The post will be integrated into the elective and emergency workload of the department. The successful candidates will work alongside the FY1/2, nurse practitioners, and Middle Grade and Consultant teams covering elective and emergencies in the surgical department.
Successful candidates will have full support from their teams and supervisors so they can gain the appropriate experience and knowledge in elective and emergency surgery and progress into the next step of their careers. This will include access to clinics, theatre, on calls and ward rounds in both elective and emergency settings. The full educational programme will be discussed and adjusted at the beginning of the placement to meet the post takers' training needs. Educational supervision will be available and study leave provided to support the development of your career.

Job summary
The Shrewsbury and Telford Hospital NHS Trust is looking for a Consultant Radiologist with specialist interest in Upper GI Imaging for a busy and expanding Department, predominantly based at The Royal Shrewsbury Hospital (RSH), with responsibilities across both hospitals' sites. The Upper GI Team cover the Cancer MDT with links to regional meeting and deliver an inflammatory bowel MDT and imaging.
A second interest can likely be accommodated. Fluoroscopic skills would be an advantage in this role and experience in ultrasound biopsy and drains would be welcomed. The posts will include sessions at the two main hospital sites within an overall flexible group job planning format, with an agreed overall sessional make up as described below.

Main duties of the job
- Share in the general imaging workload and collaborate with colleagues to support tumour-specific and other sub-specialty reporting, using PACS.
- Contribute to general work of the department, including reporting of the general radiographic examinations carried out within the main sites, as well as in the various community hospital locations in Shropshire and the CDC.
- Maintain a general Radiology practice while contributing to sub-specialty areas, as per departmental needs.
- To work flexibly according to changes in work patterns and requirements. Participate in the general on-call rota. On call overnight is outsourced from 8.30pm to 8.30am and weekdays /weekends after 5.30pm to provide a more sustainable work pattern. On site attendance will occasionally be necessary during on call periods.
- General radiological sessions to include in and outpatient cross section as well as plain films and body ultrasound. General skills must be maintained for on call work.
- Addressing GP and clinician queries and vetting are core to the role.
- To be involved in the organisation and administration of the Radiology Department and teaching of other clinical staff including participation in regular clinico-radiological meetings with other Consultants.
- To be involved in the training of junior Radiology staff, as well as support the informal education of a wide range of multidisciplinary colleagues, including foundation doctors, advanced clinical practitioners, and rotating registrars from Stoke.

We are actively seeking three locum consultant trauma and orthopaedic surgeons. The department would expect appropriate fellowship training or previous experience in the areas of major trauma, orthopaedic trauma and elective orthopaedic surgery.
You will join a dynamic team at the Peninsula Trauma Centre which is part of an expanding university teaching hospital in Britain's Ocean City. This is an opportunity to live and work in one of the most attractive parts of England. There is easy access to both coasts of Devon and Cornwall with beautiful beaches and we are in close proximity to Dartmoor National Park, the Tamar Valley and South Hams. These posts are an additional appointment and the successful candidate would contribute to the major trauma and orthopaedic trauma services.

The Musculoskeletal Service Line
The Service Line has responsibility for all aspects of Orthopaedics, Trauma and Orthogeriatrics services. The Directorate Clinical Director and Directorate Manger have responsibility for the department and are supported by a Matron.
Derriford Hospital has a regional Emergency Department and provides tertiary services including Cardiothoracic Surgery, Neurosurgery and Plastic Surgery in addition to its' status as the Major Trauma Centre for the South West Peninsula.
Scope of the service
The Orthopaedic Department provides emergency and elective inpatient, day case and outpatient services to the catchment population of the Trust, which includes Plymouth and the surrounding area (South Hams and East Cornwall). Most outpatient services are delivered from Derriford Hospital, with outreach clinics being held in community hospitals. Elective operating is delivered at Derriford Hospital. The Paediatric Orthopaedic team offer outreach and in-reach paediatric services for North Devon and Cornwall and have been actively developing a "hub and spoke" model for the delivery of care.

This is a full-time Consultant post within the Older Adult Functional Inpatient Mental Health Service, based at Ruby Ward at The Crystal Centre, Broomfield Hospital. Ruby Ward is a 17-bed functional older adult unit serving Chelmsford, Maldon and Braintree. The ward provides assessment and treatment for older adults, typically aged 70 and over, or for adults with significant frailty requiring inpatient care for functional mental illness.
Main duties of the job
The post holder will have responsibility for the clinical care of those patients admitted to the in-patient unit on the ward. The post holder will also be expected to work closely with the team and the ward manager to ensure the provision of an effective service.

We have a 10,500-patient list size in a diverse suburban patient population with wide ranging health needs. The post holder will manage a diverse suburban patient caseload with wide ranging health needs, ensuring the highest standards of care.
We have partnered with Doctaly Assist (https://doctalyassist.com/) to develop digital pathways to improve access for our patients and we feel this role would suit Portfolio GPs who are looking to develop their leadership skills and get involved in service transformation. As part of this role you will develop a deep understanding of how General Practice contracting works, learn about Health Technology development and support digital transformation. One of our partners is a former Primary Care Chief Clinical Information Officer for SEL ICB and will be on hand to provide support and mentorship.

Job summary
Our Training Practice is looking for a six-session a week GP, to join our team. Set in the Community of Seven Sisters, Dulais Valley Primary Care Centre would welcome a GP who would like to develop their own interests and skills within this rural locality.
You will be joining our four existing GPs, pharmacist, two practice nurses and two Health Care Support Workers serving approximately 6000 patients which includes a very low percentage home visit requirement.
Our 3 GP partners are experienced trainers; they would encourage you to engage in the in the teaching opportunities to continue to develop your own learning and skill set.
We currently train GP trainees and Swansea University GEM students. In future we have ambitions to increase our training scope to include pharmacy students/pre-registration.
Main duties of the job
We are fortunate to have, Health Visitor Services, District Nurses and various health board and Cluster services being held at our practice, services include; retinal screening, physiotherapy, social prescribing, heart failure clinic, breast screening and Aneurysm screening. We currently offer Implant and coil fitting and we are in the process of setting up joint injection Clinics, scheduled to commence in the Autumn. We are open to exploring opportunities to expand our additional services for future growth of the Practice.
Ideally, we would require you to work, Monday Wednesday but there is some flexibility. This post would attract 6 weeks annual leave and a week of study leave per annum. All bank holidays are pro-rata. Partnership is something that we would consider with the right candidate.
We have a lovely purpose-built building with very spacious consultation rooms, comfortable relaxation areas and staff facilities. We have ample staff and patient parking, we are situated just a twenty-minute commute from junction 43 of the M4 or the A465 at Glynneath, which makes it easily accessible.

Role Overview: The post holder will provide specialist psychiatric assessment, management, and advice for patients admitted to YDH who require mental health input during their inpatient stay. Main duties of the job
Assessment and Management - Support existing liaison staff in the assessment, diagnosis, and management of adults presenting with mental health problems
Medical Leadership - Provide senior medical expertise to YDH teams caring for patients with functional or organic mental health conditions. This includes supervising the clinical caseload of liaison team members and overseeing the work of any trainee doctors attached to the service.
Clinical Decision Making -- Work with the liaison team to determine the most appropriate interventions for referred patients. Responsibilities include facilitating safe and timely discharge planning and ensuring effective communication with Primary Care, Community Mental Health Teams (CMHTs), and Inpatient Services. The Consultant will provide clinical leadership, particularly for patients with complex needs or elevated risk.
Responsible Clinician Duties - Act as the Responsible Clinician (RC) under the Mental Health Act for patients detained at YDH. During periods of annual or study leave for Musgrove Park Hospital (MPH) consultants, RC responsibility will transfer to this post.
Liaison with Acute Hospital Teams - Develop and maintain effective working relationships with medical, surgical, nursing, and allied health professionals within the acute hospital. Provide advice on the application of the Mental Health Act and Mental Capacity Act where relevant.

Job summary
This is an exciting time to be involved in CLL/Lymphoma management, with major advances in diagnostics and target therapies, including cellular immunotherapy. We are seeking to appoint a clinical research fellow on a fixed term basis for 12 months. The post will support the lymphoma service primarily at the Sutton site. The fellow will attend the major Wednesday CLL/Lymphoma clinic in Sutton and there may be instances when the candidate is required to cover the clinics in Chelsea. An important part of the role will be to support the clinical trial activity and it is anticipated that the candidate will review the patients in early phase clinical trials under the supervision of the Consultants. It is also anticipated that the postholder would carry out a clinical research project with an aim to publish results in peer reviewed journals. The post would not form part of study for a higher degree (PhD or research MD) although there may be opportunities to move into a higher degree following the post. The job would be ideal for a candidate who wanted to gain a deeper experience of CLL/Lymphoma management and to learn about clinical trial set up and delivery, including phase I/II trials, at a leading tertiary centre.

Applications are invited for a replacement, full-time Specialty Doctor in Dermatology appointment in response to service requirements.The post is for 10 PAS at the Norfolk and Norwich University Hospital NHS Foundation Trust. However, you may be asked to work at other sites as we are in partnership. This post is based at the NNUHFT and Cromer Hospital.
The NNUH Dermatology team includes 17 substantive consultants, 3 nurse consultants and 3 locum consultants.

Main duties of the job
The post involves 10 sessions of which 6-7 will be outpatient clinics. These clinics vary from general dermatology, skin cancer clinics, minor surgery and specialist clinics. Dermatology does not have dedicated inpatient beds but dermatology patients may be on other wards. The duties of the SAS post also include working with the Core trainees in looking after inpatients. The SAS are on regular on call/ acute dermatology rota along with the ST3+ resident doctors (21 annualized sessions) holding the emergency phone between 8am-6pm (weekdays), dealing with calls from GPs, ward patients and A&E.
The Dermatology department is also involved in the teaching of undergraduate medical students for the University of East Anglia. Duties of the SAS include teaching of medical students at the bedside and as part of seminars.
The appointee will be encouraged to undertake an audit and annual appraisal.
This post is designed for a doctor looking to gain further experience and a career in dermatology. Applicants will be encouraged to register for e-portfolio and to undertake work based assessment in line with those performed by trainees and we also support CCT via the portfolio pathway route. Job summary
This exciting appointment is for a newly developed Consultant Psychiatrist (10 PAs) post working within the Herefordshire and Worcestershire CAMHS Learning Disability Service. The post-holder will provide a consistent and child centred provision into the CAMHS Learning Disability Service.
The post is substantive with 10PAs per week being offered. The service operates Monday to Friday, with working hours primarily between 9am and 5pm. There is an on call rota associated with this post.
The CAMHS LD service is a friendly, multi-disciplinary team that includes specialist LD/CAMHS clinicians, psychologists and medical colleagues.
The base for this position will be discussed and agreed at interview and could be anywhere across the two Counties including Hereford, Worcester, or Kidderminster.

Main duties of the job
- Who is keen help young people to develop a better understanding of their own emotions and behaviours, where appropriate and is skilled at adapting their approach to meet individual needs.
- Who prioritises delivering a quality clinical service and strategic development within the service, in collaboration with key team members and wider Trust priorities.
- To work as part of a multidisciplinary team and liaise with statutory and voluntary agencies.
- To manage a case load consisting of clients for whom you will either be the lead healthcare professional (HCP) and have responsibility for managing their needs (low risk patients who do not require multidisciplinary input and only require medical outpatient management alone), or allocated medic where you will be responsible for the medical management of patients managed under CPA (or similar framework) and shared with other MDT professionals.
- To conduct outpatient clinics consisting of routine and urgent new patient assessments and review/follow up appointments.
- To carry out home visits and telephone/remote consultations with service users as appropriate.
- To participate in appropriate multi disciplinary team meetings such as MDT meetings & CPA reviews.
- To maintain electronic health records in accordance with Trust policies.
- To actively participate in clinical governance activities.
- To work within the duties and responsibilities of the Mental Health Act.
