Main duties of the job
The post-holder will be expected to undertake all of the normal duties and responsibilities associated with a GP working in primary care. In accordance with the practice timetable the post-holder will make themselves available, as agreed, to undertake a variety of duties, including surgery consultations, telephone consultations and queries, visiting patients at home, checking and signing repeat prescriptions, resolving medication queries, and completion of paperwork and correspondence in a timely fashion. The post-holder will ensure the highest standards of care for all registered and temporary patients.

Job summary
The Respiratory Nurse Specialist will provide high-quality, patient-centred care to adults with acute and chronic respiratory conditions. The post-holder will work autonomously and as part of the multidisciplinary team to deliver expert assessment, clinical interventions, education and support across inpatient, outpatient and community settings. They will play a key role in the management of conditions such as asthma, COPD, interstitial lung disease, bronchiectasis, oxygen therapy management and respiratory failure.
The role includes supporting early discharge schemes, reducing hospital admissions, leading nurse-led clinics, and promoting self-management. The post-holder will contribute to service development, clinical audit and education of staff, patients and carers.
Main duties of the job
Provide specialist respiratory assessment, diagnosis support, and evidence-based interventions for patients with complex respiratory conditions.
Lead and manage nurse-led clinics (COPD, asthma, drug trials, 6MWT.).
Deliver timely interventions to support early discharge and prevent avoidable admissions.
Carry out advanced respiratory assessments, including spirometry, peak flow monitoring, oxygen saturation assessment, and inhaler technique reviews.
Initiate, titrate and evaluate oxygen therapy in line with national and local guidelines.
Provide expert advice on the management of acute exacerbations and chronic disease symptoms.
Support patients with long-term condition management, self-management plans and personalised care planning.
Work collaboratively with the wider MDT including respiratory consultants, physiotherapists, pharmacists and community teams.
Educate patients and families regarding disease management, inhaler use, lifestyle changes and treatment plans
The role also includes outpatient clinics for nebulised antibiotics, and biologic therapies for severe asthma, as well supporting with our sleep service.

Job summary
An exciting opportunity has arisen to appoint a new Lead nurse for SACT (chemotherapy) within CDDFT. We need someone who can demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families and carers.
Practising as a Chemotherapy Lead Nurse within the Integrated Medical Specialties Directorate acting as an autonomous practitioner, working independently and in conjunction with other healthcare professionals. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate.
Support new ways of working that emphasises a more efficient and patient focused service, and will ensure the safe treatment, referral and discharge of patients with undifferentiated and undiagnosed presentations in their area.
Responsibility for leading the on-going development of clinical practice and standards of care within the service, including the development of policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues.
Deliver clinical components of their role and support appraisal, clinical audit, teaching, continuous professional development and research.

Main duties of the job
As the chemotherapy lead nurse you will provide expert knowledge in order to address the needs of the chemotherapy service. They will lead the development of initiatives and strategies utilising a high degree of personal autonomy.
The post holder will provide clinical leadership and act as an expert role model to advise and support colleagues at an individual, team and organisational level by utilisation of their advanced knowledge and skills. Provide and support training to staff on chemotherapy pathways, passports and ensure all nursing standards are maintained.
Works closely with visiting Consultants and Trust Consultants as well as the Cancer Networks and share best practice and new initiatives across the trust
Actively supports and acts as an advocate for patients and their families.
