We're looking for a dedicated Community Triage and Allocation Administrator to work within our Community Nursing and Phlebotomy Teams, ensuring patients receive timely and efficient care.

Main duties of the job
- To receive and process referrals and data into the service, using the patient database, distributing information to other services / agencies as necessary
- The post holder will allocate visits to community nursing teams which have been triaged by an appropriately trained colleague within the triage team.
- The post holder will follow a clear process to triage phlebotomy referrals under the direction of a registered clinician. They will review the referral to determine the priority of care, escalating any queries or issues, and processing accordingly.
- The post holder will perform the safety check and data quality check processes on SystmOne.
- The post holder will support with the SystmOne task function, review and allocation.
- This role may require weekend working on a rota basis to support with operating a 7-day service, as required.

Job responsibilities
- To undertake a safe and efficient allocation service in the hub. To allocate patient appointments for the Community Nursing and Therapy clinicians following the set process, from the monthly planner to the holding ledgers, and finally into the live ledgers, whilst utilising relevant data to include geography, skill mix and staffing levels. Scrutiny of appointment detail against care plans that require performing will be undertaken to ensure a correct match and therefore preventing unnecessary visits.
- Support with phlebotomy referrals for all community teams, under the director supervision of a clinician.
- Process urgent and routine referrals for all blood tests and routine observations by following a clear process and using the information received to determine what is required and the priority response necessary. The prioritisation will be supported by a registered member of staff when required.
- Ensure referrals are prioritised according to service targets and patient need.
- Add a phlebotomy care plan on SystmOne and apply them adding due dates and priority.
- Process the information given within the referral, and document using SystmOne triage notes, appropriately ensuring comprehensive detail, using good grammar and punctuation and adhering to NCHC record keeping policy.
- Ensure that the referral details are entered accurately and timely on to SystmOne and the patients record is complete for audit purposes.
- Contact the referrer (e.g. GP / Practice, hospital or patient) to obtain the relevant information. if the referral does not contain sufficient information in order to be accurately processed during the triage process, they will highlight and escalate any potential risk factors to a clinician and document this in the triage notes.
- Maintain patient confidentiality and adhere to the information governance policy.
- Work with all health care professionals, and statutory/non statutory agencies to provide a seamless, integrated service to our service users.
- Identify any improvements / developments which could be made to the service provided to improve the efficiency and / or processes within the Single Point of Contact.
- Adjust communication where there are barriers to understanding, such as hearing impairment, mental capacity impairment and other difficulties in comprehension, in order to gain understanding, consent and concordance.
- Support in the management of the referrers expectations if the service is unable to respond as urgently as requested; remain calm, apologetic and escalate to clinician if required.
- Support the lead clinician with allocation including forward planning of visits and aligning the appropriate clinician with the patient, contacting clinical colleagues when required if there is an urgent visit to allocate and supporting reallocation for short notice absence of team member which is a complex activity.

Job summary
Palliative and End of Life Care Services provide various of levels of support to people with limited conditions and those who are in the final phase of life. The service provides, one to one support as well as advisor capacity to other professionals. The service will work through a central point of referral, working in conjunction with the Community Nursing Teams.
The Palliative Care Team is a health and social care team that provides an expert clinical and social care service for patients with challenging and complex life limiting diagnoses and their families. All senior team members act as clinical or social care experts, leaders and educators for their service areas. The team provides a service in all care settings across the North East Lincolnshire locality.

The role plays a critical part in ensuring Palliative Care Services are delivered safely, effectively and in line with local and national standards, while supporting our strategic ambitions and continuous growth. Working closely with senior leaders, commissioners and partner organisations, you will work alongside the Palliative Care Consultant to provide specialist care to complex palliative patients, support team management and help shape and deliver services that reflect our commitment to high quality care, coproduction and our Commercially Compassionate ethos.
You will demonstrate advanced clinical skills across the four pillars of advanced practice: Clinical practice, leadership and management, education and research working in various care settings include community, hospital and care homes as appropriate

Based at home, with visits to the Doncaster Telemedical Hub, you will undertake telephone consultations with BPAS clients to provide abortion & Sexual Health information, discussing their treatment options & assessing their medical needs. Every client is different, & you will need to adapt your style to meet their requirements.
As a telemedical practitioner you will build a strong rapport with every client you talk to. Your quick thinking & clinical experience will be essential in determining the next steps for the client.
This is a full time role working 37.5 hours per week over Monday-Saturday. Further details to be discussed at interview
Main duties of the job
- To obtain a full medical history from the client, including assessment of gestation from LMP and identify clinical factors which may place the client at risk.
- To establish that the client is sure of her decision to request a termination and ensure that the reason complies with the terms of the Abortion Act.
- To discuss and confirm treatment options with the client if applicable.
- To appropriately complete the clients consent for treatment.
- To discuss, inform, confirm, and consent on the clients future choice of contraception, and provide information regarding access to contraception when relevant.
About you
You will be a registered Adult Nurse Level 1 or registered Midwife (practising) with a valid NMC PIN and at least 12 months clinical practice experience. A competent IT user with the ability to be adaptable and self-motivated, you will work to provide an excellent telephone consultation service to BPAS clients. You'll need to have sound communication and listening skills, whilst also being able to problem solve confidently.
Working on your own initiative, you'll deliver the clients journey of care with empathy and understanding, whilst using your clinical knowledge and skills to provide them with a great understanding of the service that BPAS provides.

Main duties of the job
You will have a key role in a liaison/consultation model to provide early, easy access advice to GPs and the Primary Care Networks to support care of patients in the locality. GPs in the borough will be able to contact you via telephone or email for advice during working hours. You will also provide clinical leadership to the team in clinical multi-disciplinary team meetings and referrals meetings as well as being available for case discussions, advice and supervision within and outside team meetings. You will also have a role in the management of more complex cases, to offer senior assessments and more direct input in particular in risk and/or medication management and diagnostic assessments. This will also include Mental Health Act Assessments, CTO reviews and supporting the duty function of the team with urgent concerns and assessments. You will also share senior medical responsibility with job sharing consultant for the care carried out by the MINT team.

At EKHUFT we are pleased to be offering position of Speciality Doctor in Paediatrics to join a dynamic Child Health Team. The appointee will participate in EWTD compliant full shift Rota of 13 Middle Grades to cover 24 hours, 7 days a week General Paediatrics (number of children admitted, number of children seen in ED) and Neonatal (level 1 SCBU in busy maternity services with 3600 labour a year) acute and emergency care for children and young people age 0 - 16 (18) in various hospital environments specially designated and staffed for children and young people in Queen Elizabeth the Queen Mother Hospital in Margate (QEQM). Attendances to A&E from April 2017: November 2021 57,535 and admissions to Rainbow Ward: 10,934.
Main duties of the job
Our rota supplies two Middle Grades a day from 8:30 AM to 9 PM, two middle grades every night (24/7). Additionally, Monday to Friday, 1 middle grade is dedicated to Neonates 8:30 AM to 5 PM, 1 middle grade dedicated to run an outpatient clinic with a Consultant 9:00am-5pm.
We will work with you to shape the ideal job plan upon successful appointment, and review it with you after three months and then annually - so you can keep the perfect fit.
Middle Grades will receive 0.5 PA of CPD a week in 12 PA full time contract and we will also provide the essential resources to support you in your duties, including administrative support.

Diabetes & Endocrinology Services
A full range of diabetes services are provided at LNWUH including transitional clinic, young adult clinics, diabetes, and insulin pump clinics. There is a strong team of Diabetes Specialist nurses at each site, professionally managed by a cross site Diabetes Nurse Consultant.
Endocrinology clinics are offered on all sites. A wide range of endocrine disorders are seen including benign thyroid disease, thyroid lumps, lipid disorders, calcium disorders, adrenal and pituitary disease. Facilities for investigating endocrine diseases include MRI scanner on site, nuclear medicine scans, access to ambulatory care or endocrine nurse specialist led testing and genetic testing. We have access to regional MDT's in thyroid, pituitary and adrenal diseases.
Main duties of the job
The post holder will undertake 4 clinics per week, with clinic commitments reduced when allocated to covering inpatient care . There is astandardised template for clinics in line with RCP guidelines.
The post holder will attend MDTs either in person or virtually. They will also contribute to the triaging of Endocrinology/Diabetes clinic referrals on the FDP system and to the review of ward referrals to the service.
The post-holder will contribute to the ongoing care of the general medicine patients at Northwick Park Hospital site on a rota basis.
The post holder will contribute to the acute on-call rota covering emergency admissions and ambulatory care.
The post holder will supervise junior doctors and other members of the multidisciplinary team in addition to undertaking undergraduate teaching.

Job summary
Due to continued expansion, internal changes and growth; Newhall Surgery is delighted to announce that we are looking for 2 or 3 motivated, empathetic, customer service orientated people to join our busy Care Navigation Team. These positions are available on a part-time basis.
The post holders will have a positive attitude, good listening and communication skills and be team players. Previous primary care or customer based service experience is essential. This is a varied role and much of the work undertaken will be of a highly confidential and sensitive nature.
Part time hours are offered, however, the successful candidates would need to be available to work the early and late shifts as necessary with daily working hours between 8.00am and 6.30pm. Each team member has an agreed working pattern, these are comprised of both early and late shifts to ensure necessary cover. The working pattern and hours distribution will be agreed with the successful candidates.

Main duties of the job
The post holders will work within our busy Care Navigation Team answering the telephone, manning the front desk, dealing with patient enquiries, booking appointments, undertaking tasks for our clinical teams, signposting patients to internal and external services, handling pathology and other results according to practice protocols and generally providing a high standard of patient liaison.
The post holders will be able to demonstrate excellent verbal and written communication skills, be able to organise and prioritise a busy workload and work to tight deadlines whilst maintaining confidentiality and demonstrating tact and diplomacy. Attention to detail is of paramount importance as there will be a high level of contact with patients and their individual data. Some data will be of a highly sensitive nature.
The post holders will need to be able to work within and understand the importance of policies and procedures around patient information, data sharing, information governance and confidentiality.

Job SummaryDuring the preceptorship period the midwife will have the opportunity to provide care across the perinatal pathway in a variety of settings. The Preceptorship Lead Midwife will provide the post holder with ongoing support with any additional training needs throughout the programme. Every trust across the West Yorkshire and Harrogate LMNS aligns to the National Preceptorship Framework for Midwifery. In terms of timeframes to progress to a Band 6 - this will be dependent on individual progress, contracted hours, and will be decided in collaboration with you, your preceptor and line manager.

Its sees approximately 108,000 new patients a year of which approximately 26% are children under 16 years of age.It is a designated Major Accident Unit and is the 5th largest department in the West Midlands Health Region. With the development of the Trauma Network & the Major Trauma Centres, New Cross Hospital is a designated Major Trauma Unit.

1.Provide administrative support to Consultants, clinical staff and all patients under their care, including diary administration, correspondence support and other administrative duties as required.
2.Use of the Trust digital transcription systems to produce clinical correspondence to inform referrers, patients and other relevant parties of patient pathway progress. In doing so, produce correspondence that is of high quality and conforms to the national target of sending out within 10 working days.
3.Open and prioritise incoming post, ensuring supporting information is available when appropriate and taking responsibility for actioning urgent items in the absence of the Consultant or other clinical staff.
4.Monitor urgent investigation requests to ensure they are processed appropriately.
5.Work closely with all staff in the service to ensure compliance with 18 week referral to treatment (RTT) targets, escalating and intervening where necessary to expedite appointments and procedures to avoid breaches.
6.Maintain an accurate and current filing system, ensuring letters, clinical reports, diagnostic test results and other notes are filed in the correct patient notes, in either paper or electronic record systems.
7.Update patient details on EPR and other Trust and departmental record systems

The Compliance Administration Coordinator provides high-level administrative and coordination support to the Deputy Practice Manager Compliance and Office Managers to ensure the smooth and efficient running of administrative services within the Practice.
The role supports the administration, reception, and secretarial teams through workflow coordination, monitoring of service targets, compliance support, staff Onboarding assistance, stock control, and operational continuity during periods of increased demand or reduced staffing levels.
The post holder does not hold formal line management responsibility but will provide day-to-day coordination and support across administrative functions as directed by the management team.
Main duties of the job
The following are the core responsibilities of the Compliance Coordinator. There may be, on occasion, a requirement to carry out other tasks. This will be dependent upon factors such as workload and staffing levels:
- Support the Deputy Practice Manager Compliance and Office Managers with day-to-day administrative coordination.
- Assist with staff rotas, annual leave monitoring, workload management, and identifying staffing gaps.
- Help maintain efficient administrative processes and contribute to service improvements.
- Support staff onboarding, inductions, training coordination, and development activities.
- Prepare reports, audits, meeting agendas, minutes, and other governance documentation.
- Provide reception and telephone support during periods of increased demand.
- Assist with compliance monitoring, record keeping, and facilities management tasks.
- Support the investigation and documentation of incidents, significant events, and learning events.
- Contribute to staff communications, including meetings and internal updates.
- Help implement new procedures, protocols, and administrative initiatives.
- Assist with ordering and monitoring clinical and non-clinical stock and supplies.
- Build positive working relationships and escalate issues appropriately to the management team.
