Job summary
The Pharmacy Technician will play an important role in complementing the Pharmacy Team and other members of the practice. Pharmacy Technicians work under supervision to ensure effective and efficient use of medicines. The responsibilities of a Pharmacy Technician cover clinical, and technical and administrative categories.
The successful postholder will be responsible for encouraging the development of better understanding of the principles of medicines optimisations throughout the practice team and promote good practice in line with therapeutic developments which will involve national requirements, NICE implementation, and utilisation of medicines optimisation initiatives.
Main duties of the job
- Undertake patient facing and patient supporting roles to ensure effective medicines use
- Carry out medicines optimisation tasks
- Support medication reviews and medicines reconciliation
- Support the PCN Clinical Pharmacist with PCN Workstreams
- Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients
- Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing locally.

The Practice operates from 2 sites in the North of Preston (Fulwood and Ingol) and we care for over 11,500 patients.
We have a strong clinical and administrative team in place and promote a positive working environment. We have 5 GP Partners, 4 Salaried GP's, 1 Advanced Care Practitioners, 4 Practice Nurses & 3 Health Care Assistants. We assist with the training of FY2, ST2 and ST3 doctors and we also host medical students from Manchester University.
The Practice is part of and works closely and collaboratively with the Greater Preston Primary Care Network.

Main duties of the job
- Provide a range of medical services as defined in the core PMS/APMS contract, additional and enhanced services where appropriate.
- Make professional, autonomous decisions in relation to presenting problems whether self-referred or referred from other healthcare works within the organisation.
- Assess the healthcare needs of patients with undifferentiated and undiagnosed problems
- Screen patients for disease risk factors and early signs of illness.
- Develop care plans for health in consultation with patients and in line with current practice; disease management protocols, provide counselling and health education

- Be available and accessible to patients
- Maximise effective use of current clinical computer systems. Record clear and contemporaneous consultation notes to agreed standards
- Compile and issue computer generated acute and repeat prescriptions (avoiding hand-written prescriptions whenever possible).

An opportunity has arisen to join the specialist nursing and midwifery team within the acute Gynaecology service at PAHT. The role supports delivery of care to service users experiencing acute Gynaecology symptoms and pregnant people with complications of early pregnancy as part of the Same Day Emergency Care pathway. The post holder will work closely with medical colleagues and the main ultrasound department as well as the wider Trust to ensure service users receive timely, compassionate and person centered care.
There is a requirement for the postholder to support the Consultant team in delivery of the Pregnancy Choices service on a rotational basis.
The post is offered part time for 22.5 hours a week.

Main duties of the job
- Deliver hands on patient care to service users referred to the acute Gynaecology service
- Triage patients referred to the acute Gynaecology service via telephone and face to face and independently initiate an appropriate plan of care

The health board operates over four sites in west Wales. This post will be covering Bronglais Hospital in Aberystwyth while also supporting the wider team.
Our team of 7 consultants provide a comprehensive haematology service in the four hospitals in an area of outstanding beauty. This post will provide support for a wide range of patient groups. This hospital is our smallest of the four and the team are spread between the four sites. Support for the post includes Specialty Doctor, Haematology CNS team and Specialist Haematology pharmacists.
Our staff are at the heart of the quality of our service and are looking to add to our numbers. We are looking for someone to bring additional expertise and a passion to provide excellence.
Main duties of the job
Clinical - The appointee should be able to demonstrate experience in the management of all Haematological conditions. The successful candidate will be able to develop a subspecialty across the health board and working alongside the other consultants to contribute to subspecialisation teams.
Inpatients - The appointee will be expected to consult on all inpatients with the local medical team looking after them. Additional teaching on management of haematology patients will be essential. Liaising with the Acute Oncology Service which covers haematology in-patients
Outpatients - The appointee will be expected to participate in 3-4 clinics per week. Consideration for other subspecialist workload could be considered, depending on applicant's experience to date. There an increasing number of patients returning from the tertiary centres following treatments that return home between treatment cycles that need additional input from the local haematology team. BMA's are currently done weekly. Phone clinics are encouraged given the rural area and inability to travel for our patients.
Laboratory - The appointee will be expected to help out with advice and input on blood films and reporting bone marrow reports
There is a weekly health board wide MDT and opportunities to join other MDTs from all Wales MDTs (Lymphoma, Myeloma, Myeloid).

Job summary
The appointees will be expected to participate in full within the unit and take an active interest in promoting vascular surgical services within and outside of the Trust. The successful applicants will have a broad base in conventional vascular surgery as well as endovascular techniques. High Lane Medical Centre is looking for an experienced receptionist/administrator to work in a busy GP surgery based in High Lane. Previous experience is desired along with experience of practice IT systems; EMIS, DOCMAN, ACCURIX and TQuest.

To act as a Technical Pharmacy Advisor to Practice Clinicians across Sound Primary Care Network (PCN).
To advise Practice Management and other non-clinical staff concerning administrative aspects of medicines management as appropriate
To optimise medicine management, resolve queries where possible and produce prescriptions for clinical review and signature
To work within own competencies as a Pharmacy Technician and ensure that registration and CPD (Continuing Professional Development) is kept up to date.
To address the following areas without the need to consult a GP/Clinical Pharmacist in each case (although clinical support and advice will always be available if required):
1.Update regime review dates when conducting level 1 and 2 medication reviews
2.Medicine reconciliation of discharge summaries and other clinical documents.
3.Optimisation of repeat medication, cost effective switching, commencing repeat dispensing and synchronisation of medication
4.Ordering of required monitoring i.e., blood tests, blood pressure, height, weight etc. for medicine/condition monitoring.
5.Addition/removal of medication for Nursing Home patients under GP/Pharmacist Prescriber guidance
6.Co-ordinate District Nurse Medication and Dressing requests for the practice population
7.Co-ordinate medical device prescription requests, including requests for catheters, nutritional supplements/feeds, and stoma care products.
8.Work alongside colleagues or independently to deliver project work relating to medicine optimisation, an example of this may be deprescribing Gabapentanoids or Bisphosphonate reviews
9.When requested by a clinician to be able to have appropriate conversations with patients and initiate medications such as statins, Vitamin D, Folic Acid, iron supplements etc as per PCN protocols.
To work in conjunction with the nominated Clinicians who support Nursing Homes in the area

Ward 5 is a busy environment consisting of 18 short stay acute surgical patients. The ward is complemented by an established SEAC Unit which offers rapid access to the assessment and diagnosis of a range of surgical conditions.
About us
We operate from three main hospitals - Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, as well as a number of community health care premises including Millom Hospital and GP Practice, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
FGH and the RLI have a range of General Hospital services, with full Emergency Departments, Critical/Coronary Care units and various Consultant-led services.
WGH provides a range of General Hospital services, together with an Urgent Treatment Centre, that can help with a range of non-life threatening conditions such as broken bones and minor illnesses.
All three main hospitals provide a range of planned care including outpatients, diagnostics, therapies, day case and inpatient surgery. In addition, a range of local outreach services and diagnostics are provided from community facilities across Morecambe Bay.

Job summary
We are looking for an enthusiastic, highly motivated band 4 Community Nurse Associate to join the Furness District nursing RELIEF team on a fixed term contract of 30 hours per week to cover maternity leave. The successful applicant will be committed to working closely with health and social care colleagues to provide high quality care to patients with a wide range of medical conditions in the community setting. The role requires excellent communication skills as you will be undertaking complex nursing assessments and developing and delivering programs of care for patients and carers. The district nurses work a 24hr service and regular weekends, bank holidays, evening shifts and l nights will be part of your rota. You will need to work collaboratively with other professionals, voluntary sector and informal carers to promote excellent care to patients in their own homes and community setting. We are very patient focused and staff development is high on our agenda. As part of our team we want to develop all staff to the highest standard.
Main duties of the job
The post holder will work as a registrant within Community Services, accountable for providing evidence based, holistic nursing care of patients. The service covers a 24 hour period 365 days a year. It is the expectation of this post that the post holder will work weekends, bank holidays, evenings and night shifts as required.
