Home First Planner and Administrator
Gloucestershire Health and Care NHS Foundation Trust View all jobs
Posted 7 hour(s) ago
Reference: 2415584
Job summary
Be part of an exciting new service that launched on 1 September 2026.
The Home First service is transforming the way support is delivered to people following discharge from hospital, helping them regain independence and remain safely in their own homes. We are seeking a highly organised and proactive Planner and Administrator to play a vital role in the success of this new countywide service.
Working at the heart of the operational team, you will be responsible for coordinating patient visits, managing service capacity and demand, and ensuring resources are deployed effectively to meet the needs of the people we support. You will also provide comprehensive administrative support to the multidisciplinary team, acting as a key point of contact for staff, patients, carers and partner organisations.
This is an excellent opportunity for someone who thrives in a fast-paced environment, enjoys problem-solving and wants to make a direct contribution to improving patient outcomes and service efficiency.
You will be part of a weekend rota, working 1 in every 5, and based at the Home First Hub in Brockworth.
Main duties of the job
As a Home First Planner and Administrator, you will be responsible for coordinating and scheduling patient visits to ensure care and support plans are delivered safely, efficiently and in line with individual needs. Working closely with the Home First Leads, Therapy Leads and multidisciplinary team, you will proactively manage service capacity and demand to maximise productivity and support effective patient flow.
You will be responsible for accurately recording and maintaining information on clinical systems, creating and updating schedules, monitoring workforce capacity and responding to changing service demands. You will act as a key point of contact for the service, handling enquiries from patients, carers, staff and partner organisations professionally and efficiently.
The role also includes providing a comprehensive administrative service, including diary management, meeting coordination, minute taking, maintaining records and databases, ordering supplies, supporting invoice processes and ensuring information is recorded accurately and confidentially.
You will work collaboratively with colleagues across Home First, Integrated Community Teams and partner organisations to support safe, effective and responsive service delivery. The successful candidate will be well organised, adaptable and able to prioritise competing demands within a busy operational environment while maintaining high standards of customer service and attention to detail.
About us
We have a skilled and dedicated workforce of over 5000 colleagues working in a diverse range of services over 55 sites and within people's homes. We strive to enable a welcoming workplace culture that builds and celebrates civility, inclusivity and diversity, while providing a sense of belonging and trust.
Annual staff surveys, regular Pulse surveys and other engagement opportunities provide our people with lots of opportunity to tell us about their experiences of working with us. In the latest staff survey, 61% of colleagues gave us their views. It was great to hear that:
- 72% of colleagues would recommend the Trust as a place to work, ranking us 1st for Provider Trusts in the South West region on this question.
- 76% would recommend the standard of care provided in our services if a friend or relative needed treatment, also ranking us 1st in the South West region.
- 81% said that care of patients and service users is the Trust's priority, compared with an average in comparable NHS Trusts in England of 64%.
This high-level overview shows we are in a healthy position, with higher scores than average for comparable organisations, alongside a great response rate, indicating good staff engagement. However, we also know we have plenty of room for improvement in many areas. To that end, we continue to prioritise and invest in our commitment to genuinely becoming a Great Place to Work with consistent top-quartile performance in the annual staff survey and Pulse surveys.

Managing the nursing care of more than 18,000 patients across three sites, alongside leading a nursing team of 13, requires a strong clinical strategist, an inspiring people leader, and a highly organised, detail-focused completer-finisher. As a dynamic practice, led by four partners we view our Lead Nurse as a core member of our operational and clinical leadership team.
We are looking for a Lead Nurse who will play an active role within our leadership team, combining strategic thinking with effective operational delivery. The successful candidate will be visible, approachable and trusted, with strong communication skills, a clear sense of ownership and accountability, and the confidence to use data to inform decisions, challenge constructively and support the development of others.
The successful candidate will lead by example, maintaining a visible presence across all three sites and building strong, collaborative relationships with staff, management and GP Partners. They will uphold high clinical standards, proactively address issues, support staff through change, and identify opportunities for improvement rather than simply responding to problems as they arise.
You will work directly with our Partners and management team to design, standardise and deliver a high-standard nursing care model across our entire practice.
Main duties of the job
Role Purpose and Key Objectives
The Lead Nurse will provide strategic and operational leadership to our nursing and frailty teams, ensuring that patients receive safe, effective, evidence-based and consistently high-quality care.
Key objectives include:
- Providing visible and effective clinical nurse leadership across all three sites.
- Promote evidence-based nursing practice.
- Supporting and developing the nursing workforce.
- Ensuring high standards of clinical governance and patient safety.
- Promoting consistency of clinical practice while recognising site-specific needs.
- Leading workforce planning and professional development.
- Provide clinical mentorship, supervision and professional support.
- Identify opportunities for service improvement.
- Ensure nursing practice reflects current guidance and professional standards.
- Supporting effective use of clinical data, templates and recall systems.
- Driving quality improvement and service innovation.
- Supporting the practice to meet its regulatory and governance responsibilities.
About us
Culverhay Surgery began as a single-site practice in Wotton-under-Edge. The organisation subsequently expanded to incorporate our Berkeley site in 2021 and our Frampton-on-Severn site in 2023. We now look after over 18,000 patients, and employ 86 members of staff.
Although we operate across three physical locations, we function as one integrated practice, with:
- A unified patient list whereby patients can book appointments and be seen at any site.
- A shared management and leadership structure.
- Fully integrated SystmOne.
- Staff working across sites where appropriate.
- Shared clinical policies, processes and governance arrangements.
- A strong emphasis on collaboration and resilience across the organisation.
Our three sites work closely together as one surgery in three locations. This model provides significant benefits, including greater workforce resilience, shared expertise and the ability to develop services across a larger patient population. It also creates unique leadership challenges, particularly around clinical standardisation, communication, workforce planning and recognising differences between individual sites and patient populations.
