Job summary
The PCN Neighbourhood Development and Service Improvement Coordinator will provide dedicated operational capacity to support the development, coordination, and improvement of services across Darlington PCNs 11 member practices and its developing neighbourhood arrangements.
Interviews will be held on Thursday 12 November 2026.
Main duties of the job
Working under the direction of the PCN Business and Governance Manager, the postholder will coordinate agreed Neighbourhood and Integrated Neighbourhood Team (INT) workstreams, support engagement between member practices and partner organisations, undertake structured reviews of PCN-funded and commissioned services, and coordinate the implementation and monitoring of agreed service improvements.
The postholder will work with a high degree of organisation and initiative within clearly defined delegated authority. The role will identify issues, analyse information, coordinate actions, and develop recommendations, while escalating matters requiring strategic, contractual, financial, workforce, or governance decisions through the appropriate PCN arrangements.
The post is an ARRS-funded role and will be delivered in accordance with the approved scope of the role and applicable Network Contract DES, and local commissioning requirements.

Service Review and Improvement
Coordinate a structured programme of reviews of PCN-funded and commissioned services.
Review available information relating to activity, demand, capacity, utilisation, access, outcomes, variation, and stakeholder feedback.
Identify gaps, duplication, operational barriers, and opportunities to improve pathways, integration, productivity, and patient access.
Work with relevant leads and stakeholders to develop proportionate improvement plans with clear actions, measures, and timescales.
Track agreed service improvements to ensure actions are completed and promptly raise any delays, risks or problems.
Support evaluation of implemented changes, including whether intended benefits and outcome measures are being achieved, and capture learning to inform future service development.
Project and Programme Coordination
Coordinate agreed PCN projects from planning through implementation and review.
Develop and maintain project documentation including objectives, milestones, actions, risks, issues, dependencies and outcome measures.
Support the identification of intended benefits and proportionate outcome measures for improvement projects, monitoring progress against agreed measures.
Coordinate input from practices, PCN services and partner organisations, ensuring agreed actions are followed up and identifying where delays or decisions in one area may affect progress elsewhere.
Identify risks or barriers early and escalate appropriately with options or recommendations where possible.
Support with producing clear reports, briefings, and presentations.

To work as part of the multi-disciplinary team to provide high quality care to patients attending the Accident & Emergency department by assisting the nursing and medical staff in performing clinical investigations set out in this job description. Reporting to the Shift leader, the Emergency Department Technician will be allocated tasks by the area coordinator.
Main duties of the job
Use effective communication skills to interact with colleagues, patients and visitors entering the department promoting the core values of the trust.
Be aware of and utilise various tools for communicating with patients to alleviate anxiety and to overcome language barriers.
To gain appropriate consent from patients when carrying out procedures within role.
To assist nursing and medical staff with the provision of health promotion, such as advice following procedures on discharge.
Utilise Customer Care and Information Technology (IT) training to facilitate effective communication with patients and members of the multi-disciplinary team
Clinical Duties
Carry out routine observations; blood pressure, temperature, pulse, respiration rate on patients attending the Accident & Emergency department, documenting all patient observations and PAR scores in accordance with department protocols.
Carry out manual & machine analysis of urine samples, gaining consent for urinary HCG readings as required.
Perform Cannulation & Venepuncture on patients, as per Aseptic Non-Touch Technique (ANTT) protocol as directed by the Registered Nurse or Medical staff.
Carry out treatments such as primary wound closure by Steri-Strip, skin-adhesive and application of Plaster of Paris splints in accordance with ANTT protocol.

With an ageing population and pressures on Acute and Community Health and Social Care, enabling people to stay at home and live as independently as possible is the great healthcare challenge of our time.
The Discharge 2 Assess Pathway 1 Service was created to support any person at home following discharge from acute hospital or from a community health bed. The service provides short-term Assessment, Recovery, and Rehabilitation support within the patient home, working collaboratively with Lincolnshire County Councils commissioned reablement provider with the goal of moving people to being as independent as possible. This will aim to enable people to return to their own home sooner from a hospital stay.
This service will follow the 'Discharge to Assess' (D2A) principles where a person is assessed for their long term needs in their own home or place of residence.
This service covers the whole county although people will generally work within a more locally based teams. This means that roles are available across Lincolnshire. The hours of the service are set to be 0800 to 2000, 7 days a week. We are able to consider part time working.
Main duties of the job
The post holder will provide rehabilitation and reablement to patients immediately following hospital discharge in their home environment. To promote independence and contribute to the assessments required to determine a person's future needs.

Main duties of the job
The post holder will support users of the Trust's key electronic patient record systems, undertake critical system management tasks, and carry out ongoing configuration to ensure systems continue to meet the needs of the Trust. They will work closely with the wider team to deliver an effective and high-quality service
The post holder will use a service desk system (currently Fresh), to resolve IT incidents, to provide ongoing support and advice, to resolve data quality issues, and to identify and make configuration changes to SystmOne. The post holder will also have the opportunity to work on, and support wider SystmOne improvement projects where required.

We are currently seeking enthusiastic, passionate, and proactive Sonographers, to join our Ultrasound team at Wirral University Teaching Hospital (WUTH).
You will become part of our supportive and dynamic team of sonographers, who take pride in delivering excellent patient care and clinical standards. We have recently developed a new preceptorship programme, for both our newly qualified students and any newly qualified sonographers joining our trust.
We provide a 7-day service, across 4 key sites on the Wirral Peninsula. As part of this role, you may be required to work across all 4 sites (Arrowe Park, St Catherine's, Victoria Central and our new Clatterbridge Diagnostic Centre). The service covers both obstetric and general ultrasound departments as well as a variety of sub-specialties including head/neck, MSK and paediatrics. We actively encourage career development and have regular training opportunities available.
Applicants must hold a recognised ultrasound qualification (PgCert, PgDip or Direct-Entry Masters) and be eligible to work in the United Kingdom with HCPC/NMC/ Equivalent registration. Nuchal Translucencyaccreditation is desirable for obstetric sonographers, although training will be provided if required.
Typical full-time hours are 08:00 - 18:00, 4 working days (working days to be negotiated, over a 7-day service). However, part-time roles, weekend only and alternative working patterns can be discussed at interview.
Main duties of the job
Applicants will be expected to have experience in one or more: Obstetric, General Abdominal, Pelvis and small parts scanning. Experience in MSK and paediatric scanning is desirable.

Main duties of the job
Using EMIS, you will consult via telephone, video or face to face, according to patient needs. (Full training and support for remote consultations will be available if required). The clinical team includes GP's, Practice Nurse, Pharmacist, and HCA. You will possess leadership qualities and provide support to the wider team, act as a positive role model and proactively identify areas for quality improvement. You will proactively contribute to improving health outcomes for patients by identifying new initiatives / ways of working and as such promote health improvement and disease prevention.
In addition, you will assist to further improve standards of care and, achieve QoF targets as well as KLOE objectives, as outlined by CQC.

Clinical Practice
- Assess and holistically clinically examine patients with both minor and complex needs through history taking, physical examination and appropriate use of investigations to confirm diagnosis.
- Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
