Band 2 Occupational Therapy Assistant (Elderly Care)
Exciting opportunity to join a compassionate and aspirational Therapy Team at the Royal Berkshire Hospital.
The role will entail supporting older people who are admitted in to an acute hospital to regain function and independence. A big part of the role involves supporting to get patients home safely.
The successful candidate will work with qualified Occupational Therapists, Physiotherapists and other member of the multidisciplinary teams to deliver a high quality service for patients.
In this role you will work in line with the trust's CARE values to deliver person-centred care to ensure the best outcomes for patients in our care.
You will work with the Occupational Therapy team to assess and treat patients. You will work with the MDT and community partner organisations to ensure patients receive the right care, in the right place, at the right time whenever possible.
Main duties of the job
- Face-to-face contact with patients, completing 'initial interviews' and information gathering with patients, their next of kin and other organisations such as social services.
- Delivering Occupational Therapy, working with the guidance of an Occupational Therapist to rehabilitate patients through practicing and supporting activities of daily living such as, washing and dressing, meal preparation, transfers.
- Supporting the completion of home visits with patients along side a qualified therapist.
- Liaison with internal teams to promote smooth transition for patient from hospital to community.
- Manual handling.

Job overview We are delighted to be able to offer an opportunity for a highly motivated, ambitious individual to join the Intensive Care Medicine team at The Princess Alexandra Hospital NHS Trust. Job summary
We are looking to recruit a Receptionist/Care Navigator for our busy GP Surgery in Southampton.
You will be working as part of a busy team where you will be the first point of contact for all enquiries. You will be answering the phone to patients, professionals and clinicians and will be a key link within the team. The duties are many and varied and include answering the telephone, covering reception, booking appointments, sorting mail and using Systm1 (an electronic patient record), and working with the workflow and back office teams.
The role requires a high level of inter-personal skills and personal confidence. Having a respectful and empathetic attitude will be essential, with the capacity to manage sensitive, and confidential information. We are looking for people who have a 'can do' attitude.
This is a 6 months fixed term post. We have full time (37.5 hours per week) and part time (18 hours per week) contracts available. We require somebody with the ability to work a mixture of early shifts with a 7.45 a.m. start and late shifts with a 6.30 p.m. finish. Ensuring all new patients complete relevant paperwork, details are inputted onto TPP Systm1 computer database correctly, and that correct procedures are followed regarding patient notes. Deducting patients as appropriate, printing out summaries and photocopying paper notes.

Job summary
The Critical Care Transfer Service Operative is responsible for safely transporting critically ill patients between intensive care and other treatment centers. The role involves driving specialist vehicles, including under emergency conditions using blue lights and sirens, while supporting the clinical team with patient care, monitoring and reassurance.
The post holder works alongside trained clinical and transfer team staff, follows relevant London Ambulance Service policies and UK Ambulance Services Clinical Practice Guidelines, and ensures patients are transferred safely, efficiently and with dignity. Main duties of the job
This role offers the opportunity to be at the heart of Primary Care Network development, supporting projects that make a real difference to patients, practices and the wider community. With a varied workload and no two days the same, it is ideal for someone who enjoys building relationships and contributing to innovative healthcare services.
As a Care Coordinator, you will work with PCN leaders, practice teams and partner organisations to improve patient care and access to services. You will support patients with personalised care needs, helping them navigate health and community services and access the right support at the right time.
You will help promote the work of the PCN through social media, patient engagement activities and health campaigns, raising awareness of local services, screening programmes, immunisations and wellbeing initiatives. You will also support the Patient Participation Group, gather patient feedback and contribute to service improvements.
Working closely with the PCN Manager, Clinical Director and partner organisations, you will support quality improvement projects, audits and healthcare programmes while developing partnerships that enhance integrated patient care.
