Job responsibilities
We have an exciting opportunity for an independent, autonomous Prescribing Clinician to play a key role in our Care Coordination and Integrated Urgent Care services across the Bath, Swindon and Wiltshire locality on a permanent basis. We can support both part-time and full-time working arrangements on a fixed rota pattern basis.
This role offers a unique blend of clinical assessment and system navigation, supporting patients to access the right care, in the right place, at the right time, helping to reduce unnecessary hospital admissions and improve patient outcomes.
You will work across our Care Coordination service in hours and to support our Urgent Care service out of hours. Our Care Coordination shifts are based at our Chippenham Head Office and our out of hours sessions will be based at one of our Primary Care Centres.
The Role
As a Prescribing Clinician within our Care Coordination function, you will:
- Act as a senior clinical decision-maker, supporting the triage, assessment and redirection of patients across the wider health and care system
- Work collaboratively with ambulance services, primary care, community services and secondary care, ensuring patients are safely managed in the most appropriate setting
- Provide real-time clinical support to ambulance crews, identifying suitable alternative pathways to avoid unnecessary conveyance to Emergency Departments
- Deliver clinical assessment, diagnosis and prescribing, where appropriate, through a combination of remote consultations, face-to-face care, and home visiting
Alongside care coordination responsibilities, you will also support our wider urgent care service through a variety of shifts, including:
- Triage shifts based at Head Office, reviewing NHS111 and system referrals
- Face-to-face shifts at Primary Care Centres (Chippenham, Salisbury, Keynsham and Swindon)
- Mobile/home visiting shifts, supporting patients unable to travel, with access to a fully equipped vehicle and driver
Key Responsibilities
- Deliver safe, effective clinical triage and prioritisation, managing undifferentiated presentations across all age groups
- Support system-wide patient flow, ensuring timely access to appropriate services and reducing pressure on urgent and emergency care pathways
- Provide clinical advice and decision support to colleagues and partner services, including ambulance teams
- Undertake remote consultations and care planning, facilitating onward referral where required
- Contribute to the development of an integrated whole system approach to care delivery
- Support the ongoing improvement of care coordination pathways and clinical effectiveness
Please note that remote senior clinical support is always available during your shift.
About You
You will be a registered nurse, paramedic or allied health professional with a non-medical prescribing qualification, able to work confidently within a broad scope of practice.
Skills & Experience
- Experience working within urgent care, primary care or emergency care pathways
- Strong clinical reasoning and triage skills, with the ability to manage risk and uncertainty
- Ability to work autonomously and make complex decisions under pressure
- Understanding of system pressures and patient flow within urgent and emergency care
- Experience supporting or working alongside ambulance services or care coordination functions (desirable)
- MSc in Advanced Practice (achieved or working towards), or equivalent experience
- Minimum of 2 years prescribing experience
- Completion of a recognised clinical reasoning module (e.g. PADRIP or equivalent)

Primary Care GP
Brighton | 46 sessions per week | £84,660 per annum based on 6 sessions
Help shape how primary care is delivered in your Brighton community.
At IC24, we're building something ambitious in Brighton - and we're looking for GPs who want to help shape it.
Over the last two years, our Brighton practices have been on a significant transformation journey. We've made substantial changes to the way we work and are now bringing our Sussex practices much closer together, creating stronger links between teams and sharing expertise, resources and support across our services.
We're now taking the next step.
Allied Medical Practice, Church Surgery and School House Surgery are coming together as part of a wider vision for primary care in Brighton, with plans to move into purpose-built practices next year. This will give us the capacity to significantly grow our patient population and create a modern, sustainable primary care service for the future.
We're looking for Primary Care GPs who want to be part of that journey.
This isn't simply an opportunity to walk into a finished model and deliver the same clinics in the same way year after year.
It's an opportunity to bring your clinical experience, ideas and judgement to a service that is evolving - and help shape what it becomes.
If you enjoy improving things, working collaboratively and having a genuine influence on the way care is delivered, we'd like to hear from you.
What you'll focus on
Delivering high-quality General Practice
You'll provide comprehensive primary care through face-to-face and virtual consultations, clinical administration and home visits where appropriate.
You'll have the clinical autonomy to manage your patients effectively, while being part of a supportive team that values good medicine and good patient care.
Building relationships with your patients
We're looking for GPs who value continuity, take the time to understand their patients and enjoy the breadth and variety that General Practice offers.
You'll care for a diverse population with a wide range of clinical, psychological and social needs.
Working as part of a multidisciplinary team
You'll work alongside experienced GPs, Practice Nurses, pharmacists and the wider multidisciplinary team.
We're increasingly connecting our Brighton practices and wider Sussex primary care services, creating opportunities to share expertise, learn from one another and provide more joined-up care.
Bringing your clinical interests
Our practices support a broad range of clinical needs, including long-term conditions, mental health, sexual health, HIV care and women's health.
We're particularly interested in hearing from GPs with experience or a clinical interest in Women's Health, but we're equally interested in what you can bring to the wider service.
We want our GPs to be able to develop areas of interest and use their expertise to benefit both patients and colleagues.
Improving the way we deliver care
This is where your influence can really make a difference.
As our services develop, there will be opportunities to contribute to quality improvement, clinical audits, health promotion, QOF, locally commissioned services and service development.
We're looking for people who don't just identify what isn't working but are interested in working out how to make it better.
Helping shape the future of the practices
We're ambitious about what our Brighton services can become.
You'll have opportunities to work with clinical and operational colleagues to develop new ways of working, improve patient pathways and help shape the model of care as our practices grow.
You won't be expected to simply fit into the way things have always been done.
We want you to bring your experience, challenge constructively, share ideas and help us build a service that works better for patients and clinicians.
This is a role for a GP who likes to build
We're deliberately looking for people who are comfortable with change and motivated by improvement.
You may be at your best when:
- You're given a challenge and enjoy finding a solution.
- You like having a voice in how a service develops.
- You're comfortable when not everything is perfect yet.
- You enjoy working collaboratively rather than operating in isolation.
- You can adapt when priorities, processes or services change.
- You see opportunities where others see problems.
- You enjoy sharing your experience and learning from others.
- You care about the wider patient population, not just the individual consultation.
- You want to leave a service better than you found it.
If you're looking for a completely established practice where your role is simply to deliver clinics within a fixed model, this probably isn't the opportunity for you.
But if you want to help build the future of primary care in Brighton, influence how care is delivered and see the impact of your ideas, this could be a very exciting time to join us.
Who this role suits
You'll be a GMC-registered GP included on the National Performers List who enjoys working collaboratively and wants to play an active role in developing primary care.
You'll bring:
- Experience or a strong clinical interest in Women's Health
- A broad interest in General Practice and confidence managing a varied caseload
- Experience caring for patients with complex clinical needs, including mental health, HIV and gender reassignment
- Strong clinical judgement and a commitment to evidence-based care
- A genuine commitment to continuity of care and building long-term patient relationships
- Excellent communication skills
- The ability to work effectively as part of a multidisciplinary team
- Flexibility to work across our Brighton practice sites
- A positive and adaptable approach to change
- A commitment to high-quality, compassionate patient care
Most importantly, you'll be a GP who cares about how primary care works, not just the care you personally deliver.
Let's be honest about the opportunity
General Practice is rewarding, but it can also be demanding.
You'll care for patients with a broad range of health needs, balancing face-to-face consultations, telephone appointments, clinical administration and occasional home visits. Some clinics will be busy, and you'll regularly make complex clinical decisions while managing competing priorities.
You'll also be working across practice sites and within a service that is continuing to change and develop.
We don't want to pretend everything is already perfect.
We're building and improving - and that's precisely why this is an interesting time to join us.
You'll have the support of experienced colleagues and a wider IC24 primary care community, but you'll also have the opportunity to contribute to how the service develops.
If you're a GP who is resilient, adaptable, clinically curious and motivated by making things better, you'll have the opportunity to make a real difference here.
Why IC24?
We are Integrated Care 24 (IC24), the leading not-for-profit Social Enterprise providing innovative and patient-focused primary care services.
IC24 is committed to improving access to health and social care for our patients and reducing demand on secondary care services. We provide services to over six million patients, including GP Out of Hours, NHS 111, Primary Care and Integrated Urgent Care services.
Every day, our people help patients access the care they need, when they need it most.
Our Primary Care teams are supported to work collaboratively, develop specialist interests and contribute to service improvement while maintaining high clinical standards.
You'll join an organisation that invests in its people, encourages innovation and recognises the importance of creating an environment where clinicians can build rewarding, sustainable careers.
You'll also be part of a wider organisation providing NHS 111, GP Out of Hours, Primary Care and Integrated Urgent Care services, giving you the opportunity to work within a broad clinical community and benefit from experience across different parts of the NHS.
What's in it for you?
- Salary of £84,660 per annum based on a 6-session week.
- £14,110 per session annually, including paid administration time between clinical sessions.
- Opportunity to join the NHS Pension Scheme.
- 6.6 weeks annual leave plus bank holidays.
- Enhanced maternity, paternity and adoption leave and pay.
- Employee Assistance Programme including free 24/7 counselling and occupational health support.
- Professional development and continuing education opportunities.
- Access to our employee rewards and discount platform.
- Blue Light Card and NHS discount schemes.
Location: Allied Medical Practice School House Surgery & Church Surgery, and Ship Street Surgery, Brighton (cross-site working required)
Hours: 46 sessions per week (28.5 hours for 6 sessions), preferably Monday to Wednesday but there is flexibility here for the right person.
We celebrate brave ideas and brave people.
Due to the nature of this position, employment is subject to proof of eligibility to work in the UK, a satisfactory enhanced DBS check with barred list checks and satisfactory references.
We have a duty to safeguard, protect and promote the welfare of those who use our services and our employees. This means we take every step we can to protect people from harm, abuse and neglect.
We are committed to providing equal opportunities for all and encourage applications from people of all backgrounds, including ethnic minority communities, disabled people, LGBTQ+ communities, neurodiverse individuals and other under-represented groups. We're committed to creating an inclusive workplace where everyone feels they belong.
