St Georges Health & Wellbeing Hub brings NHS services, social care, voluntary and community organisations, creative health provision, and community services together under one roof, creating an exciting platform for closer collaboration around the needs of Havering residents. We are seeking an experienced and highly motivated Head of Development & Partnerships to lead the next stage of the Hubs partnership development, integrated services model and future growth. Main duties of the job
You will lead work across partnership and governance arrangements, integrated clinical pathways, community use, and strategic development, while identifying opportunities for innovation, income generation, external funding and long-term sustainability. We are looking for a credible, collaborative, and forward-thinking senior leader with significant experience in partnership development, strategic service development, programme leadership, or business development. You will be comfortable working across organisational boundaries, influencing senior stakeholders and translating complex ideas into deliverable programmes. If you are passionate about integrated care, population health and community development, and want the opportunity to shape an innovative model of neighbourhood health and wellbeing, we would love to hear from you.

NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers, and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Goodmayes Hospital.
Probationary Period This post will be subject to a probationary period.
High Cost Area Supplement – Outer London (B&D, Redbridge, Havering and Waltham Forest, Goodmayes) This post also attracts payment for High Cost Area Supplement of 15% of the basic salary (with a minimum of £4,870 to a maximum of £6,137 p.a. pro rata for part time).
Certificates of Sponsorship Although we are a registered sponsor organisation, we are unable to offer sponsorship for all job roles. Please check your eligibility under the UKVI points-based system. Job summary
The post holder will be professionally and legally accountable for all aspects of own work, including the management of service users in own care. The post holder will perform advanced occupational therapy assessment, providing a clinical diagnosis, to service users who have diverse presentations, complex physical and psychological conditions. The post holder will develop and deliver an individualised treatment programme. The post holder will use evidence-based practice, Occupational Therapy goals and patient centered principles, without direct supervision. The post holder will provide leadership to others through supervision and appraisal.
Main duties of the job
Provide high quality Occupational Therapy clinical expertise and service. Responsible for the delivery a range of evidence-based interventions which promote recovery, social inclusion and normalisation for service users. Manage challenging clinical situations and plan interventions whilst ensuring regular evaluation of the service users care plan. Critically evaluate effectiveness of therapy interventions, in collaboration with service users, carers and other professionals. - Managing new referrals and ensuring information is processed accurately and efficiently.
- Booking and coordinating clinics and appointments, including communicating with patients, parents and carers.
- Maintaining and updating waiting lists and supporting waiting list recovery activity.

Main duties of the job
You will need to be an experienced Paediatric and Adult Cardiac trained nurse who is proactive, self-motivated, and has very strong interpersonal skills, with a vision for service improvement. You will be expected to actively coordinate, support, and assist in the management and delivery of high quality care to patients during the peri-operative phase of their care.
You will be expected to support East Wing Cardiac Theatre in conjunction with the existing band 6/ 7 team, with an expectation of support across the other various specialities within St Thomas Hospital depending on service requirement.

The role of the Nursing Associate/Assistant Practitioner is to work with the Registered Nurse and other members of the multi professional team, providing patient care and supporting with the diverse clinics within the department.

Outpatients is a fast-paced environment centered around scheduled appointments, diagnostic tests, and consultations, the role focuses heavily on patient preparation, clinical procedures, and family-centered support.
Provide support children/young people and their families to improve and maintain their mental, physical, behavioural health and well-being.
Safely undertake procedures to meet the patient's nursing needs for which you have attained competence, for example recording and interpreting vital signs, obtaining bloods, completing ECG.

The Cardiology Investigations Unit (CIU) provides a comprehensive range of diagnostic services, delivering both invasive and non-invasive cardiac investigations for inpatients and outpatients across the Trust.

Main duties of the job
- Analyse and interpret complex and interdependent data acquired during transthoracic echocardiography (TTE) to support accurate diagnosis.
- Following interpretation of images and data generated during TTE, produce a clinical report of the echocardiographic findings.
- Perform diagnostic echocardiography across a range of complex clinical settings, including specialist valve assessment.
- Interpret and report on the complex imaging and data generated during TTE for these patient groups, with senior review as required.
- Perform transthoracic echocardiography in accordance with BSE guidelines and departmental standards.
- Review, maintain and develop agreed standards of documentation in line with BSE guidance and departmental minimum standards, in liaison with the Lead Echo Physiologist.

Job summary
We are currently looking for a part time Band 6 Memory Nurse in the East/North East locality for 28.5 hours a week, working days are negotiable over Monday-Friday. This locality is based near Chapeltown in Leeds LS7 and covers the East/ North East part of the city. We are looking for a committed and motivated practitioner who wants to consolidate their nursing practice in memory services and develop their skills, experience and knowledge. The successful applicant will be expected to demonstrate the capacity to work independently in the assessment for dementia process and offering post diagnostic support.
It is desirable for the candidate to have post registration experience, and experience with memory assessments and evidence based tools used for diagnosis. You will be able to demonstrate an in depth understanding of the needs of service users with dementia and have an ability to support service users and their families/carers practically and emotionally post diagnosis.
You will need to be able to work independently out in the community as well as being part of a Multi-Disciplinary team. The current role within the team does include hybrid working, as well as running a clinic once a week.
Main duties of the job
Undertake comprehensive assessments of individuals referred with memory concerns, including cognitive, mental health, physical health, and social care needs.
Conduct clinical interviews and utilise recognised cognitive assessment tools to support diagnostic decision-making.
Gather collateral information from carers, families, and other professionals to inform assessments.
Contribute to the diagnostic process and present findings within multidisciplinary team meetings.
Communicate assessment outcomes sensitively and effectively to patients and their families.
Provide information, education, and emotional support to individuals living with dementia and their carers.
Monitor the effectiveness and tolerability of anti-dementia medications and undertake medication reviews within scope of practice.
Identify and manage risks associated with cognitive impairment, including safeguarding concerns, self-neglect, vulnerability, and carer stress.
Liaise with primary care, secondary care, social services, voluntary organisations, and community teams to ensure coordinated care.
Support patients through post-diagnostic interventions and signpost to appropriate community resources and support services.
Maintain accurate, timely, and comprehensive clinical records in accordance with professional and organisational standards.
