Job summary
The Patient Navigator plays a vital role in ensuring patients receive timely, coordinated and compassionate support throughout their care journey. Working as part of the practices multidisciplinary team, the postholder will help patients understand their care pathways, access the right services, and overcome barriers that may prevent them from receiving the care they need.
This role is designed to improve patient experience, reduce unnecessary delays, and support clinicians by guiding patients to the most appropriate point of care. The Patient Navigator will provide personalised assistance, whether helping a patient prepare for an appointment, supporting digital access, or proactively following up on outstanding actions.
Main duties of the job
The Patient Navigator will support patients in accessing the right care at the right time by providing clear guidance, proactive follow-up and personalised assistance throughout their healthcare journey. Key duties include:
Fielding incoming patient calls throughout the shift, providing clear advice, resolving queries, and directing patients to the most appropriate service or clinician.
Helping patients understand their care pathway, including appointments, referrals, investigations and followup actions.
Proactively contacting patients to ensure outstanding tasks (blood tests, monitoring, reviews, medication queries) are completed in a timely manner.
Assisting with digital navigation, such as online consultation tools, NHS App functions, and remote monitoring platforms.
Reducing barriers to care by identifying patient needs, resolving practical issues, and escalating concerns to clinical or administrative teams when required.
Working closely with clinicians and administrative staff to streamline patient flow, reduce delays, and improve overall patient experience.
Maintaining accurate records of patient interactions, actions taken and outcomes, ensuring safe and effective communication across the team.

Main duties of the job
The post holder is a pharmacist, who acts within their professional boundaries, supporting and working alongside a team in general practice. In this role they will be supported by a senior clinical pharmacist who will develop, manage and mentor them and by a GP who will provide clinical supervision.
To provide clinical leadership on medicines optimisation and quality improvement as well as managing aspects of medicines safety and antimicrobial stewardship.
To work as part of a multidisciplinary team in a patient facing role and take responsibility for areas of chronic disease management within the practice and undertake clinical medication reviews to proactively manage patients with complex polypharmacy.
The post holder will provide primary support to general practice staff for prescription and medication queries. They will help support the repeat prescription system and medicines reconciliation on transfer of care and systems for safer prescribing, providing expertise in clinical medicines advice while addressing both public and social care needs of patient in the GP practice (s).
The post holder will ensure that the practice integrates with community and hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure better access to healthcare and help manage workload.

An exciting opportunity has arisen for a skilled, enthusiastic and motivated Assistant Practitioner to join the Integrated Reablement Team, incorporating the Urgent Community Response (UCR) Service, Pathway 1 Discharge to Assess (D2A) and Reablement Services across Nottingham City.
Working within a dynamic multidisciplinary team of Advanced Clinical Practitioners, Physiotherapists, Occupational Therapists, Nurses, Pharmacy Technicians, Assistant Practitioners, Rehabilitation Support Workers and Administrative Staff, you will support patients to maximise independence through rehabilitation and crisis intervention within their own homes. The service aims to prevent unnecessary hospital admissions and facilitate timely, safe hospital discharges.
As part of the UCR service, you will support clinicians in responding to urgent referrals from emergency and out-of-hours services, providing a rapid two-hour response to crises such as falls, functional decline and acute illness. Comprehensive in-house training will be provided, including falls recovery and safe assistance from the floor.
The successful candidate will plan, deliver and evaluate person-centred, goal-focused rehabilitation programmes to adults with physical and mental health needs.
Main duties of the job
The successful candidates will be expected to work holistically alongside the multidisciplinary team. Our team provide comprehensive assessments to patients who are at risk of hospital admission, along with facilitating discharges from the Acute hospitals.
You will need to be able to use your own initiative within your own limitations to support the clinicians to provide gold standard quality care to our patients within the local community.
The ideal candidate would be someone with experience of working within the health and social care sector, who can demonstrate they have the skills and qualities to work holistically in a fast paced environment. You will be expected to complete holistic assessments, order and assess equipment and minor adaptations within your scope of practice, assess, review and set goals for patients in their own homes.
In return you will be offered a comprehensive induction, access to internal and external training, exposure to a range of clinical expertise within our team, along with regular supervision. If you do not hold the Foundation degree you will be supported to enrol on the Assistant Practitioner foundation degree to aid your future development.

About the Department
An opportunity has arisen for two pharmacists to join our expanding, friendly, diverse and inclusive pharmacy team at United Lincolnshire Teaching Hospitals NHS Trust.
The Pharmacy Departments within ULTH provide ward based, Dispensary, and Aseptic Dispensing Services to a range of general and specialist areas including Admissions Unit, General Medical and Surgical, Haematology and Oncology and Critical Care wards. We have a good integrated team of Clinical Pharmacists, Pharmacy Technicians and Support Workers and there is a supportive Pharmacy Education and Training team.
The posts are Trust-wide, although the successful applicant/s will be based primarily at either Lincoln County Hospital or Pilgrim Hospital Boston - preferences will be discussed during interview.
Main duties of the job
Our progressive department is in the process of a period of rapid transformational change to ensure that it is a service fit for the future and is at the forefront of the Hospital Pharmacy Transformation Programme.
We are dedicated to ensuring that medicines optimisation is embedded across all aspects of care within the Trust to deliver safe, efficient and cost-effective use of medicines.
We take pride in developing our pharmacists through formal and informal study, clinical supervision and training. We will support you to provide high quality services to patients and healthcare professionals alike. This includes offering opportunities to complete an accredited post-graduate clinical pharmacy diploma and pharmacist independent prescriber qualification.
We support progression from band 6 to band 7 upon completion of a post-graduate clinical diploma or equivalent experience. You will also work with lead Pharmacists to review and develop policies and contribute to monthly directorate medicines optimisation reports.

The role will involve travel across all sites in the United Lincolnshire Teaching Hospitals NHS Trust, therefore, the post holder must be able to meet the travel requirements of this post.

An exciting opportunity has risen in the Clinical Haematology Directorate for a Reception Supervisor. The post holder will be supervising a busy reception team in the Catherine Lewis Centre, responsible for co-coordinating all appointments for the service, thus will be required to work as part of a multi-disciplinary team alongside consultants, nurses and the wider team.
The post holder will work closely with relevant team members to ensure pathway pathway are coordinated effectively, booked into appropriate clinics and that a high quality customer service is provided.

The successful candidate will have significant experience and knowledge of working clinically in an operating theatre environment as well as managing large teams and budgets. Main duties of the job
This role is accountable through their teams for :
- Ensuring that all staff in areas of responsibility practice in accordance with statutory and corporate policies, including those specific to operating departments including relevant day case wards and recovery services.
- Overseeing and monitoring the quality and timeliness of patient care in accordance with relevant professional standards
- Ensuring that the environments in which patients receive care and treatment are fit for purpose, clean, well ordered and maintained.

We are looking for enthusiastic, motivated Radiographers who enjoy the challenges a Major Trauma Centre (MTC) will offer within a friendly multidisciplinary team in a central London location.
St Mary's Hospital is one of four Major Trauma Centre's (MTC's) in the London region with a dedicated Acute Imaging Centre containing a recently upgraded CT scanner and a separate Paediatric A&E.
The post will give you the opportunity to build on your existing clinical skills in all areas of the General Imaging Department including A&E across a range of specialties and developing new skills in CT and Angio. Education and training are key elements of the department.
The Imaging Department at St Mary's Hospital performs a comprehensive range of procedures over all modalities. Being flexible and innovative in your ways of working will be fundamental to your role and good communication skills are essential. We also teach radiography students from a number of London Universities.

Main duties of the job
- To carry out radiographic examinations across a range of modalities to the agreed standard of the department.
- To maintain a high standard of patient care and comfort and be patient focused at all times.

Job summary
To perform the full range of hearing and audiological assessments necessary for the diagnosis of hearing loss including (pure tone audiometry, tympanometry, play audiometry, otoacoustic emissions, speech testing) and fit hearing aids to deaf and hearing impaired children to national standards
Appropriate training and support will be provided to help the postholder progress towards becoming competent in a Band 6 role via the Higher Training Scheme. This is a Band 5 developmental post. The postholder will be supported to develop paediatric audiology competencies through structured supervision, training and clinical exposure.
Progression to Band 6 is not automatic and is subject to:
- Demonstration of competencies aligned to Band 6 practice
- Completion of relevant training (e.g. Higher Training Scheme)
