Job summary

We are recruiting to a Band 7 Fixed Term Contract position within our Community Speech and Language Therapy (SLT) service in Hertfordshire. This is a part time opportunity - 28.5 hours per week. We welcome applications from qualified Speech and Language Therapists who are committed to delivering high-quality care and are interested in working in a community setting.
Our dedicated and supportive team sits within the Community Adult Health Services provided by Hertfordshire CLCH Trust. We are a community-based therapy team providing care to the Hertfordshire residents within their own homes, clinics, bedded units and care homes.
In this role, you will work with a general adult caseload, carrying out assessment, treatment and support to those experiencing swallowing difficulties and communication impairments. You will collaborate closely with our neighbourhood colleagues across multidisciplinary teams, primary and secondary care services, voluntary and third sector organisations.
As this is a community-based role, with travel between patient homes, there is a requirement to hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010).
Main duties of the job
Key duties within this role include:o Take an active role within the clinical team, contributing to the delivery of a comprehensive, highly specialist Speech and Language Therapy service.o Provide skilled dysphagia and communication assessments and treatments to the general adult community population of Hertfordshire.o To be proficient in managing a complex caseload of a variety of long-term conditions including dementia.o Work in line with professional standards and clinical guidelines, supporting best practice across the service.o Carry out all aspects of clinical care, including assessment, treatment planning, and maintaining accurate, timely clinical records. You will work independently with your caseload, with appropriate support from the wider team.o To share responsibility for delegated operational management for a defined aspect of the service.o To provide effective leadership, management and be responsible for the career and personal development for a defined number within the service workforce.o To deliver a high performing service with effective demand management systems.o To provide evidence based advanced SLT clinical care and/or education programmes to patients and their relatives and carers within a defined patient cohort across the SLT service.

We're delighted to offer a brand-new 30-hour Band 6 Speech and Language Therapist opportunity within our welcoming Harrow Adult Community Speech and Language Therapy (SALT) Service.
In May 2024, our team achieved Quality Development Unit (QDU) status, recognising our commitment to delivering high-quality, patient-centred care and continuous service improvement.
In this role, you'll provide domiciliary and outpatient assessment and therapy for adults with dysphagia and communication difficulties, including those with acquired and progressive neurological conditions. You'll manage a varied specialist caseload, contribute to service development projects, and support the supervision and development of junior colleagues.
Based across Honeypot Lane Health Centre and Alexandra Avenue Health Centre, you'll work within a supportive multidisciplinary team with strong links to our Acute Speech and Language Therapy Service, Harrow Dietetics Service and wider Rehabilitation Team, offering excellent opportunities for collaboration and professional development.

Main duties of the job
The successful post holder will be part of the SLT team that will offer support and supervision, but they will be expected to work autonomously in client's homes, nursing homes and outpatient clinics.

- We will provide a lot of opportunities to engage in service development, including research, audits and QI projects.
- A framework of regular supervision including 1:1, peer supervision, and appraisal is in place.
- We will also support your continuing professional development with training opportunities both internal and external.

We are seeking an experienced and motivated Physiotherapist to join our Unplanned Care Service in Hertfordshire on a temporary bank basis, working 24 hours per week, Monday to Friday.

Job summary
To provide a specialist speech and language therapy service as part of the Community Neurological Rehabilitation Team for adults within the tri-borough area of Hammersmith & Fulham, Kensington & Chelsea, and Westminster. The post holder will work in collaboration with the community teams within CLCH, other health care professionals, statutory and voluntary agencies to deliver excellent standards of patient care.
Main duties of the job
o The post holder will be based within one of the three MDT neurorehabilitation teams, and provide specialist assessment and treatment of clients with a wide range of acquired neurological conditions with swallowing and/or communication impairments. The post holder is also part of the Early Supported Discharge Team (ESD) for Stroke. This includes the opportunity to work as a service coordinator, triaging and managing ESD referrals from acute site partners.
o The post holder will be an integral member of the community neuro-rehabilitation team based in the specified borough, acting as a resource to other medical, nursing, therapy, social services and voluntary agency staff. They will build and maintain key relationships with referrers to the service (including GPs, acute hospitals, bedded services, Care or Neuro navigators) to facilitate appropriate referrals through promoting understanding of the role of the speech and language therapist within the multi-disciplinary team (MDT).
o The role covers a variety of settings including clients' homes, out-patient clinics, residential and community facilities. The post holder would be expected to provide advice, training and support to nursing home staff, students, therapy assistants and others.

Job summary
This is a Band 6, Speech and Language Therapy clinical role within the Merton Community Team (37.5 hours per week).
The post holder will provide a specialist clinical speech and language therapy assessment, diagnosis and treatment service as part of the Merton Community Neuro Rehab / Community SLT Team on a virtual and / or domiciliary basis across Merton.
The service includes clients with a wide range of acquired communication and swallowing difficulties, such as stroke and head injury, and those who have a degenerative condition and need support to manage disability and quality of life. The service also provides input for clients who have cognitive-communication and vocational rehabilitation needs.
The post holder will be an integral member of the multidisciplinary neuro rehab team, in addition to the wider speech and language therapy team across CLCH, acting as a resource to other medical, nursing, therapy and social services staff and voluntary agencies and as an interface link for Speech and Language Therapy with acute providers.
The post holder will undertake teaching and research in relevant specialist areas for a wide range of other professionals, as appropriate.
The post holder will support self and service development through active participation in continuing professional development and by participating in any audit or research being undertaken by the service.
Main duties of the job
- Provide a specialist speech and language therapy clinical role within the neuro team.
- Co-develop and provide appropriate specialist speech and language therapy assessments and interventions.
- Facilitate group work and peer mentoring for people affected by a neurological condition, alongside the MDT.
- Carry out specialist physical examination of the orofacial process including the mechanism of eating, drinking and swallowing.
- Monitor all aspects of the interaction of significant others with clients.
- Make appropriate clinical decisions following assessment, using evidence-based practice and outcome measurement to provide the most effective treatment to clients and their families within clearly defined care plans.
- Use clinical judgement and specialist skills to evaluate deterioration in clients' health status and report same as appropriate to other health services (in particular, regarding dysphagia).
- To adapt practice to meet individual client's and families' circumstances, including due regard for specific clinical conditions affecting their speech and language and / or cognitive, cultural, religious and linguistic differences.

This is an exciting opportunity for an experienced administrator to join our newly formed administration team and the well-established HARI service supporting people with complex medical and therapy needs.

Main duties of the job
- To give skilled nursing care, based on principles of best practice and clinical evidence. This includes: Chronic disease management, end of life care, continence promotion and wound management. Specific skills include phlebotomy, administration of medication via syringe drivers and intravenous therapy, PEG management, urinary catheterisation and tracheostomy suctioning.
- Act as a Named Nurse with the support and supervision of the DN Team Lead. To take continued responsibility for the holistic health needs assessment including care planning, evaluation and appropriate implementation of health care and ensuring continuity of care following referral from acute care, rehabilitation units, GPs and other agencies.
- Undertake comprehensive assessment of the physical, psychological and social care needs of patients using the Single Assessment Process and including the needs of carers. To provide individualised and person centred care plans that provide monitoring and promotion of self-care to prevent unnecessary hospital admissions
- Act as the patient's advocate and to facilitate the patient's own choices with regard to nursing care, promoting independence and self-care, as appropriate.
- To identify and record new problems and other relevant information reporting back to the DN Team Lead as appropriate.
- Maintain accurate, comprehensive and contemporaneous records in accordance with Trust and NMC guidelines.

You will work with patients throughout their rehabilitation journey, carrying out comprehensive assessments, delivering evidence-based interventions and supporting individuals to achieve their rehabilitation goals and maximise their independence.
Main duties of the job
As a Band 5 Physiotherapist, you will play a key role in delivering high-quality rehabilitation services as part of a skilled multidisciplinary team. You will:
- Deliver a comprehensive physiotherapy service to patients across the Inner North West London Boroughs.
- Work autonomously within professional standards and clinical guidelines, managing your own clinical caseload while contributing to the wider multidisciplinary team.

The Home First Service supports timely and safe discharge from acute and urgent care settings, helping patients to recover and remain independent within their own homes. Main duties of the job

An exciting opportunity has arisen for a Bank Band 3 Temporary Staffing Booking Officer within the People Division.
This role plays a vital part in supporting the Trust by coordinating the supply of temporary Bank workers to cover vacancies and unplanned absences across all staff groups. You will help deliver a high-quality, customer-focused service, ensuring all staffing arrangements are in line with Trust policy, best practice, and are cost-effective.
You will work as part of a supportive and collaborative team, building strong working relationships with colleagues across the Trust, including HR Operations, Payroll, Workforce teams, the wider People Directorate, and Divisional Managers and Leads.
Shift patterns vary week by week and will primarily include evenings, weekends, and bank holidays.

Main duties of the job

Main duties of the job
o Support and deliver a high quality clinical pharmacy and medicines optimisation service to patients in Community Hospitals.
o Ensure an integrated approach to medicines management and continuity of pharmaceutical care for patients admitted from primary and secondary care into Community Hospitals.
o Contribute to the Trust's clinical governance through interventions that optimise medicines management across all relevant areas of practice.

We are pleased to offer a part-time (22.5 hours per week) Band 6 permanent post within the Adult Community Speech and Language Therapy (SLT) in the London Borough of Brent.
We are a dynamic and welcoming team, focused on providing high standard patient-centred care with emphasis on continuing service development and excellent links with our community multidisciplinary team and the local acute SLTs. We are looking to recruit an enthusiastic and innovative Speech and Language Therapist to complete our Adult SLT Team.
This is a newly established Speech and Language Therapy (SLT) post, providing specialist domiciliary and outpatient assessment, management and intervention for adults with acquired communication and/or swallowing impairments, with a particular focus on clients residing in community care homes.

Job summary
Join the CLCH Academy as a Practice Development Nurse and help shape learning, development and high-quality care across our 0-19 Children's Services.
Based in Richmond, South West London, you will work with clinical teams, the Academy and Locality Manager to create positive learning opportunities that build staff knowledge, skills and confidence. You will use your experience as an SCPHN Health Visitor to support practice development, teaching, supervision and assessment.

Main duties of the job
As a Practice Development Nurse, you will work closely with the Academy team and Locality Manager to plan, deliver and evaluate learning and development across designated services.
Key responsibilities include:
- Identify learning and development needs and help teams respond to them effectively.
- Plan, deliver and evaluate education and training, considering local and national priorities.
- Facilitate induction, education and continuing professional development for registered and non-registered nursing staff across community nursing teams.
- Provide teaching, supervision and assessment that supports staff to develop their knowledge, skills and confidence.
- Work alongside clinical teams to embed learning, development and evidence-based practice into everyday care.
- Promote high standards of care by sharing clinical knowledge, expertise and best practice.
- Help create an inclusive, supportive learning environment where staff feel valued and encouraged to develop.
- Contribute to the ongoing development of practice, learning and workforce capability across the service.
- Share learning and contribute to wider practice development priorities within CLCH.

Join our Merton Enhanced Home-Based Rehabilitation Team (MEHBRT) as a Specialist Occupational Therapist and help people make meaningful progress towards recovery and independence in their own homes.
Working as part of a multidisciplinary team, you will provide timely, person-centred occupational therapy to patients transitioning from hospital who have ongoing health or social care needs.
This is an opportunity to combine specialist clinical practice with leadership and service development. You will:

Manage your own caseload, including complex cases, with access to senior support when needed.
