Job summary
iGPc has an exciting opportunity for an experienced, self-motivated, hard working, and patient focussedCare Coordinatorto join our team, working closely with our 8 member GP practices and other primary care professionals within the PCN. We recognise the value that Care Coordinators can bring to our practices and our patients and aim to provide exemplary patient care; finding innovative solutions in general practice to deliver the best care we can to our patients.
This role is offered on a full-time basis, for 37.5 hours per week. With possible weekend/evening work as and when required.
Main duties of the job
As a PCN Care Coordinator you will
work closely with teams to help and support the multi-disciplinary team (MDT) meetings offering a personalised care approach, and the ongoing patient case management making sure that appropriate support is made available to them and their carers, and ensuring that their changing needs are addressed. This will involve linking with GP surgeries and a range of Community Health Services and VCSF services.
work closely with the GPs and other members of your assigned practice(s) and PCN teams to ensure the coordination and management of identified patients at all points of their care journey.
assist with the co-ordination of Network clinics, services and projects
liaise with the additional roles staff in the PCN to meet any unmet patient needs, for example Social Prescribing Link Worker
Deliver against a thematic area of the PCN work

The collaboration of our member GP Practices is designed to address the healthcare needs of the local community by making full use of the resources and hands on experience of our Practices, as well as other healthcare providers and partners.

Liverpool General Practice Collaborative are looking for a part-time (22.5 hours) experienced Business and Development Manager to help design and implement the foundations required for long-term success, increase the visibility and influence of General Practice within the wider health and care system, and support the Collaborative in realising its vision of stronger primary care, improved population health and better outcomes for Liverpool's communities.
Main duties of the job
The Business & Development Manager will play a pivotal role in establishing and developing Liverpool GP Collaborative,supporting nine Primary Care Networks to create a sustainable, provider-ready organisation. The role is responsible for developing governance arrangements, organisational infrastructure, policies, business plans, risk registers and operational processes that support the Collaborative's long-term ambitions.
Key responsibilities include delivering strategic and operational plans, identifying opportunities for collaboration across member PCNs, developing future service models, securing funding, preparing business cases and supporting income generation. The postholder will coordinate programmes and projects, monitor delivery against agreed objectives, manage risks and performance reporting, and ensure effective programme governance.
The role acts as a key link between member PCNs and wider system partners, building productive relationships with General Practice, the Integrated Care Board, NHS Trusts, Liverpool City Council and voluntary sector organisations. Working within a developing organisation, the postholder will help translate strategic priorities into practical delivery, strengthen the voice of Primary Care, support neighbourhood-based working and contribute to improved health outcomes for Liverpool residents.

Our shared ambition is to put General Practice in the strongest possible position to improve population health, reduce inequalities and help shape the future of integrated neighbourhood-based care across Liverpool.
By working collaboratively, we aim to create a collective voice for Primary Care, influence system decision-making, strengthen partnerships, and develop innovative models of care that deliver better outcomes and experiences for local communities. The Collaborative is committed to reducing duplication, sharing expertise and enabling Primary Care Networks to achieve more together.

Liverpool General Practice Collaborative is not yet established as a formal legal entity and is currently developing the governance arrangements, leadership structures, operational processes and organisational infrastructure required to support its future ambitions.
