At Minehead Medical Centre, we are looking for a dedicated Advanced Clinical Practitioner to join our multidisciplinary team working 20 hours per week, delivering high-quality, patient-centred care to our local community.
Based in the coastal town of Minehead, Somerset, and on the doorstep of Exmoor National Park, this is an opportunity to combine a rewarding career in primary care with an exceptional place to live and work.

Main duties of the job
As an Advanced Clinical Practitioner at Minehead Medical Centre, you will provide expert, autonomous assessment, diagnosis, treatment and management for patients presenting with a range of undifferentiated and complex health conditions.
You will act as a first point of contact, managing acute presentations, routine appointments and telephone or remote consultations. You will undertake and interpret diagnostic investigations, initiate appropriate management plans and manage both acute and chronic conditions.
As an Independent Prescriber, you will prescribe and review medications safely and effectively in line with current legislation and clinical guidelines. You will also manage long-term conditions, support patients with self-management and lifestyle modification, and respond appropriately to urgent or emergency situations

Job responsibilities

We are looking for a psychologist with an interest in holistic work and strength-based approaches to bring psychology to all aspects of a new paediatric respiratory service based at St Mary's Hospital, London W2. You will work together with an 8a psychologist who brings expertise in the wider service to develop the pathway, offer clinical interventions and be part of the MDT.
Main duties of the job
To offer psychological thinking to all aspects of a child and family's journey in this health setting.

We are a small, experienced and supportive CAMHS team based in the community hospital next to the beautiful Savernake Forest.

The Family Therapist role will deliver a high-quality, comprehensive mental health service for children, young people, and their families experiencing mental health and eating disorder challenges with and alongside the multidisciplinary team. We are looking for an experienced professional, who embraces technology, is able to meet the CAMHS core clinical standards, connect with the team and be able to think outside the box, to support and engage children, young people, and their families.
We have good professional support in our service and regular professional regional meetings. You will be supported by the Clinical Lead for Systemic Therapy and have the opportunity to connect with your professional colleagues.
Main duties of the job
Assessment and provision of NICE concordant treatments, including family-based treatment including FT-AN. The MDT consists of clinicians from a range of specialisms including psychology, nursing, occupational therapy, social work, psychotherapy, and psychiatry. In this role, you'll hold your own case load.
Key tasks: The assessment and treatment of young people with a range of mental health and eating disorder presentations
- Delivering family-based treatment and family and systemic psychotherapy to young people and families
- Maintaining a systemic perspective of the young people we see in context and using systemic skills such as systemic formulation to aid the evidence-based treatment we offer.
The role of systemic family therapist in accordance with the iThrive model is located predominantly within the 'Getting More Help' pathway and offers the opportunity to make second order change, enhancing the life trajectory of many young people, their family/carers who come to our service. You would work in collaboration with and alongside your team colleagues and other agencies and support networks. Our service has an exciting opportunity to join our team as a CYP-PT: Therapy Trainee [ASC-LD], part of the NHS England (NHSE) funded programme in partnership with The Anna Freud Centre, University College London, and Kings College London. The post-holder will be training on the Children & Young People Mental Health (CYP-MH/CYP IAPT) Therapy Course and carrying out clinical work in our service to complement their studies and use their newly developed skills. We are seeking highly motivated and enthusiastic mental health practitioners to work and train within our service. The training post will equip the post-holder to provide interventions for children, young people or families using evidence-based intervention consistent with the course they are attending. The post holder will work with people with different cultural backgrounds and ages, using interpreters when necessary and should be committed to equity, diversity and inclusion.
Main duties of the job
This is a fixed term post with this training year funded by NHSE. After successful completion of the course the fixed term training contract will end however applicants would be encouraged to apply for any vacant posts within the service.
This role will be best suited for applicants that are able to balance a very busy work and study schedule. You will be an employee at the service but also enrolled on a Postgraduate Diploma course. You will be required to study and perform at PG academic level. Applicants must be prepared to work towards their studies outside of protected study time.Applicants must be willing to travel into London to attend their lectures.

Main duties of the job
The Band 6 Nurse for the Oxfordshire Assertive Outreach Team (AOT), working across Oxford City, is responsible for delivering high-quality, recovery-focused and evidence-based nursing interventions to adults with severe and enduring mental health difficulties who experience significant challenges in engaging with services.
The Team will be a fully integrated multidisciplinary service comprising a Consultant Psychiatrist, Clinical Team Manager, Psychologists, Occupational Therapists, Nurses, Support Workers and Administrators.
You will:
- Work as an autonomous practitioner an active participant of the multi-disciplinary team undertaking specialist nursing assessments in collaboration with the patients and their families/carers and presenting back to the wider MDT.
- Work in partnership with patients and their families/carers to ensure the development of a treatment plan to facilitate their safety, promote their well-being and support their independence and inclusion in the community. This may include working closely with partner agencies.
- Deliver evidence-based interventions following NICE guidelines to promote recovery and relapse prevention in a variety of settings.
- Regularly review care and outcomes, supporting service users to complete clinical outcome measures and monitoring progress and adapting treatment options as required.

Working within Oxfordshire MHST, your role will be to supervise EMHPs / CWPs and help them to develop their role. Supervision consists of both Case Management Supervision and Clinical Skills Supervision, following a low intensity model.
As a supervisor for the Mental Health Support Team, your main duties will involve:
- Offering caseload management and clinical skills supervision to qualified and trainee EMHPs / CWP in the service.
- Holding a caseload of young people, offering clinical assessments and low intensity interventions within MHST Schools
- Triaging and allocating referrals via agreed protocols within the service, with support from the Senior Management Team.
- Providing signposting and self-help advice, or refer onto other services, where referrals are not appropriate for MHST
- Supporting EMHP trainees using role modelling to assess and support children and young people with a common mental health problem in the self-management of their recovery.
- Supporting EMHPs / CWP in delivering Whole School Approach work (audits of Whole School Approach, delivery of assemblies, work to promote positive mental wellbeing in the educational setting, groups etc.)

Are you a qualified Occupational Therapist seeking a new role?We are looking for a full-time Senior Mental Health Practitioner to join our CAMHS Outreach Team, providing specialist community-based support to children and young people experiencing complex and pervasive mental health difficulties. The service works with those who may struggle to engage with traditional clinic-based provision, offering flexible and responsive care within homes, schools, and community settings.
We enthusiastically welcome the knowledge and expertise of an Occupational Therapist to our experienced team of Social Workers, Nurses and Psychologists. The role requires close multidisciplinary working to ensure coordinated, high-quality care throughout the young person's journey in mental health services. You will support and promote young people as occupational beings - utilising Occupational Therapy assessment tools and measures, to consider impact on identity and motivation for change.
You will have the opportunity to develop the role of OT within the CAMHS Outreach team as well as have access to OT specific training including targeted sensory, universal and MOHO modules of human occupations and its relevance to CAMHS.
In addition, there will be the opportunity to attend and participate in the Oxon CAMHS Occupational Therapy Forum and Sensory Supervision as required to support your clinical caseload.
Main duties of the job
- To collaboratively assess, plan and provide evidence-based interventions for children and young people referred to the CAMHS Outreach, paying particular attention to opportunities for functional, creative, educational, work, leisure, interpersonal and life skills.
- Once trained, partaking in the delivery of the Dialectical Behaviour Therapy for Adolescence (DBT-A) provision. This will include being able to participate in all modalities of the therapy, including being part of the on call phone coaching rota and DBT skills group.

Job summary
Are you a recently qualified OT looking to develop your skills and experience across a wide range of physical and mental health settings?We are looking for OT's to join our established and sought after Band 5 Occupational Therapy rotation.
Our rotation offers a unique opportunity to work in a mix of clinical areas across physical health, mental health and various specialist services. The rotation currently includes:
- Adult Mental Health (inpatient)
- Adult Learning Disability (community)
- Forensic Mental Health (inpatient)
- Stroke Rehabilitation (inpatient)
- Older Adult Mental Health (inpatient)
- Older Adult Mental Health (community)
- Community Therapy Service (community)
- Community Hospital rehabilitation (inpatient)
The rotation is focused in and around the city of Oxford, with some rotations based in the community hospitals to the south of the county. Each rotation is for a period of 8 months (subject to change).
This role will require you to travel countywide across the Trusts sites on a regular basis. You must therefore be comfortable with driving for work purposes and able to travel freely within the hours of the role. Main duties of the job

Job summary
The Podiatry Department is responsible for treating foot problems and promoting foot health. We require an enthusiastic and friendly Clinic Assistant/administrator to work with podiatrists in our community clinics in the South East.
This role will be based in Townlands Hospital Henley, but will involve working across our clinics in Wallingford and Didcot. This can be impacted by service need.
The successful candidates will need to work on their own initiative and as part of a small team, be flexible, have a caring attitude and maintain confidentiality at all times. Excellent organisational skills are essential and experience of working with the elderly and in an NHS setting would be an advantage.
On occasion we may require the post holder to work at clinics and attend meetings or training across the wider county of Oxfordshire.

Job summary
This exciting 10 PA post is a replacement post, to be shared with another full-time consultant who will work in the central (outer) area of the city. This post covers the provision of medical leadership to the inner area of the Scarborough Community Mental Health Team (SICT). The division between inner and central (outer) sections is purely for medical cover purposes and follows an equitable division of local GP surgeries. The community team continues to operate as one community team. The job is split into 3 PAs of supporting professional activities (SPA) and 7 PAs of direct clinical care (DCC).
The community team post is based at The Ellis Centre, 18 Dean Road, Scarborough which is a unit to house the facilities to support the delivery of community care to adults.
The community team receives an average of 100 - 120 referrals per calendar month with only a small proportion requiring assessments by the psychiatric team.
The post holder will not have a case load of their own but will work with other members of the multidisciplinary team, to ensure patient safety and appropriate care is delivered.
Main duties of the job
The appointed consultant will be expected to provide key medical leadership to the Community Mental Health Inner Team in Scarborough. The post holder is not expected to hold a care coordination role but may be expected to be the clinical lead for a specific cohort of patients stabilised on Clozapine and Lithium treatments (estimated caseload of 20-30 patients).
The post holder will have an average of 8 patients on CTO/MOJ. They will not have a case load of their own but will work with other members of the multidisciplinary team, to ensure patient safety and appropriate care is delivered.
The post-holder's core clinical duties will involve formulation, review and leadership meetings, MH Act related work, and involvement in complex care planning meetings. Based on Royal College recommendations, Outpatient work will comprise 3,5 PAs; Clinical Admin 1 PA; MDT work 1 PA; Emergencies 1 PA and MHA work 0,5 PAs..Management of patients on CTO, including assessments/reviews and attending tribunals and panels as appropriate.
The post-holder will be expected to provide clinical supervision to a resident doctor and expected to provide clinical placements for Medical Undergraduates form Hull York Medical School.

Main duties of the job
The role is responsible for coordinating formal complaint investigations across the Trust, working closely with clinical and non-clinical staff to ensure all concerns are thoroughly investigated. This includes reviewing medical records and other relevant documentation, gathering evidence, and providing expert guidance throughout the investigation process. The post holder will compile and draft comprehensive, compassionate, and high-quality written responses to complaints on behalf of the Chief Executive, ensuring they are written to an exceptional standard of English and grammar, address all issues raised, clearly explain the findings of the investigation, and outline any learning or actions taken as a result of the complaint.

Bradford City 5 Primary Care Network is looking for an enthusiastic, organised and compassionate Care Co-ordinator to join our growing PCN team.
This is an exciting opportunity to make a real difference to patients across our eight GP practices in inner-city Bradford. Main duties of the job
The Care Co-ordinator will work across Bradford City 5 PCN, supporting patients to navigate health, care and community services and ensuring that agreed actions are followed through.
The postholder will manage an agreed caseload of patients and work closely with GP practices and the wider multidisciplinary team. Key duties will include contacting patients and carers, coordinating appointments, referrals and reviews, helping patients prepare for care conversations and addressing practical barriers that may prevent them from accessing support.
The role will support multidisciplinary team (MDT) working by preparing relevant information, recording agreed actions, identifying responsible professionals and tracking actions through to completion.
