Job summary
This role is offered on a fixed term basis for up to 14 months to backfill a maternity leave.
We are looking for a registered nurse or midwife to work with children and families in the School Nursing team in Forest of Dean locality based in Lydney. The post is 30 hours per week year round.

Main duties of the job
We are looking to fill a fixed term vacancy in Forest locality.
A School Nurse is someone who is;

- Responsible and accountable for assessing and implementing programmes of care from the Healthy Child Programme, as directed by the Specialist Community Public Health Nurse. This may also involve other evidenced based programmes of care in partnership with clients or other agencies to identify and address health needs.

- Provide leadership and act as a role model to junior members of the team by representing the values and beliefs of the nursing profession in addition to aligning to the GHC Values Framework.
- Assess global health needs of communities, schools, and their children to ensure children are not disadvantaged through ill health or social circumstances.
- Promote and monitor health in school aged children, including identifying physical and mental health needs, disability and problems with development or behaviour.
- Work with schools to provide a healthy environment using data from the Online Pupil Survey, in partnership with the Healthy Schools Programme.
Facilitate the planning and implementation of drop-in sessions, according to the needs of the young people and the whole school community.
We provide a Multi-professional preceptorship programme that is aligned to the National Preceptorship Framework for Nursing and have been awarded the Interim quality mark from NHS England. Our programme is provided to all Newly Qualified; Nurses, Nursing Associates, Internationally Educated Practitioners, Allied Health Professionals and Return to Practice Practitioners. In addition to the programme we also offer all preceptees the opportunity of enrolling and completing the level 7 (Masters) 15 credit validated module Preceptorship for Healthcare (AP7026).
Newly qualified Social Workers are offered the Assessed and Supported Year in Employment programme (ASYE) which is similar to a preceptorship programme.

Job summary
We have an exciting opportunity for a Clinical Pharmacist to join our multi-professional Primary Care Team working within our Health & Wellbeing Hub under the direct supervision of the Clinical Director. The successful candidate will work to deliver safe, effective, and cost-effective use of medicines by applying their pharmaceutical, clinical and technical skills. Job summary
As part of the Provide's multi-disciplinary children and family services, we provide a comprehensive paediatric physiotherapy service for children from birth to 19 years of age who present with neurodevelopmental concerns.
We work with children and their families in their homes, nurseries and schools across Mid-Essex, as well as within our dedicated children's centre.
Our physiotherapists are dynamic, experienced and supportive.
Main duties of the job
To assess children with a variety of neurodevelopmental conditions, identify main therapeutic goals, plan and deliver therapeutic intervention.
To liaise with the multi-disciplinary team within provide and in external settings such as schools, which support the child and family.
To identify safeguarding concerns and contribute to safeguarding procedures when relevant.

The Care Co-ordination Centre (CCC) provides a single access point into the organisation for all customers. UCRT is a 7-day service providing rapid assessment and treatment for patients experiencing a health crisis in their own homes, including nursing and residential care homes. Working closely with partners across Mid and South Essex--including ambulance services, acute hospitals, GPs, social care, community services and the voluntary sector--we provide holistic assessments within two hours of referral and deliver timely, coordinated care.
As a clinical lead you will be expected to provide senior clinical leadership within the URCT team and undertake a range of clinical duties and nursing procedures along with supporting the team to manage more complex patients within their own home.
You will need to demonstrate expert knowledge surrounding community care and a commitment to provide high quality, safe and effective care for patients requiring rapid nursing intervention within their own home to prevent hospital admission.

To improve outcomes for people with frailty and complex co-morbidities and reduce the need for these people to attend or be admitted into hospital.

We are very lucky to have a fantastic nursing team.
We have 2 Advanced Nurse Practitioners with a wealth of experience, Linda (our clinical nurse lead, who works 4 days per week) and Karen, who works 3 days per week. They predominantly have same-day, face-to-face appointments. Additionally, we have Elaine, our Asthma and COPD nurse.
Our Treatment Room nurses include Marie, working 4 days per week, maintaining a very high standard of care for our 766 patients with diabetics. We are very proud that she was awarded Doctor Magazine Practice Nurse of the year award in 2012 and has continued to be amazing ever since! Bea works 5 days providing our main treatment room services, performs learning disability checks and home visits for long term condition assessments and frailty checks.
Alice works as our Healthcare Assistant and is passionate about promoting health and wellbeing. She completes multiple long-term condition reviews, including CHD, AF, stroke plus MMSEs, ECGs and dressings.
We have additional support for our COPD patients with remote clinics run by 2 specialist respiratory nurses.

Our Business Manager, Chamila Perera joined us in January 2023, bringing a wealth of experience with 20 years managing in General Practice. Our Operations Manager, Andrew Poole joined in August 2022 and has modernised the way we work, bringing new innovative ideas. We have a fantastic team comprising 3 medical secretaries and 12 part-time administrative staff (including team-leaders, care navigators, coders and a care-coordinator). Out of Hours & Home Visits
Our patients have access to Out of Hours through 12 Point Care, at the nearby Sollershot GP surgery. There is also an Urgent Care Centre at Lister Hospital, Stevenage, which includes Mental Health Urgent Care.
The Hertfordshire Acute In Hours Visiting service see our patients for any acute issues, which hugely reduced the number of home visits performed by our GPs, usually to 0 or 1 per week. Job summary
An excellent opportunity has arisen for a Long-term condition nurse or ACP.
This role will play a key role in delivering anticipatory, personalised care for patients living with long-term conditions and frailty across community and care home settings. With a clinical background in managing conditions such as diabetes, COPD, heart failure, and multimorbidity, the postholder will reduce avoidable deterioration, support independent living, and improve quality of life through holistic assessment, care coordination, and early intervention. Working within an integrated multidisciplinary team, the nurse will champion continuity, equity, and proactive planning particularly for vulnerable and housebound cohorts.
One of the main aims of the proactive care team is to help reduce unplanned hospital admissions and A&E attendances.
As an autonomous practitioner, the successful candidate will ideally be a non-medical prescriber, able to demonstrate critical thinking and decision-making relating to long term conditions.

- Identify and stratify patients with moderate to severe frailty using validated tools (e.g. eFI, Rockwood)
- Conduct comprehensive biopsychosocial assessments, including cognition, mobility, nutrition, and social support
- Apply specialist knowledge in managing long-term conditions, such as Diabetes, Respiratory Disease and/or cardiovascular conditions.
- Develop and review personalised care and support plans in collaboration with patients, families, and MDTs

- Facilitate advance care planning, DNACPR discussions, and end-of-life care coordination
Coordination & Collaboration

The role combines leadership, service development, partnership working and ensuring high-quality, person-centred interventions that improve employment outcomes.
90% of people with Severe mental illness want to work, but currently only 8% are in work. Getting into employment can be a critical part of a recovery journey, so as a Community Manager, you will be supporting a team of Lead Employment Specialists and Employment Specialists, providing supervision, training and role modelling of the IPS approach. This is a dynamic and diverse role, where you will also work as part of mental health teams to promote employment and raise expectations around the ability of service users to retain and access paid employment. The ability to network in the local community and build strong relationships is essential.
Main duties of the job
You will be required to:
-Provide effective management, leadership and clinical guidance for the IPS Service
-Lead on service development and continued expansion to meet ambitious referral/access targets set by NHS England.
-Be responsible for managing the budget and take accountability for service KPIs
-Lead the team during IPS Fidelity Reviews and Guided Self Assessments and help build adherence to the IPS model
-Promote and nurture a positive team environment with open communication, collaboration and a shared goal
-Work within the principles of the IPS model; to monitor the team's performance and collated and submitted data monthly (this will be shared with NHS England, IPS Grow and other stake holders).
-Liaise closely with key internal/external stakeholders

Using your experience, you will be duty clinician and supervisor to a shift that consists of Band 7 Clinical Leads, Band 6 Senior Mental Health Practitioners, Band 5 Mental Health Practitioners Band 3 Call Handlers, and students, supervising, mentoring and coaching to develop their skills and playing a key role in the development of the service.
Our First Point of Contact Centre (FPCC) is a priority one urgent response telephone service launched in 2019. Since then it has consistently demonstrated itself to be an integral part of our mental health services. We have improved access to mental health services for individuals who need it, whilst reducing demands on care and treatment delivery services.
We are continuing to evolve and build our team and the support we offer as part of the development of the National Mental Health NHS 111 response model. Which involves 24/7 working, including night shifts.
Main duties of the job

In the FPCC your skills and expertise will be in triaging patients at the point of contact, providing mental health support and identifying what service will best meet their needs moving forwards.
The spectrum of mental health needs varies significantly; you will have the ability to offer advice, guidance and support to those in mental health distress. This will have a routine, urgent or crisis response. Equally, you will have the ability to respond to urgent referrals with more acute needs and higher risks in the community.
You will be handling a wide range of calls, and thriving amid the challenges this presents. This requires an outstanding amount of resilience and emotional strength as you will be supporting unwell and distressed patients at their time of need, their families, carers, emergency services, and referrers who may need support. Your support will make a difference to lives and requires people who can be there for those that need it, sometimes in the face of adversity and trauma.
We're currently recruiting and staff can chose to work short shifts or long shift.

Job summary
An exciting opportunity has arisen for a Research Support Worker, to support the Research Delivery Team within Aneurin Bevan University Health Board. The role will mainly be based in our new Clinical Research Centre in the Royal Gwent Hospital, but travel may be required across our other sites in Nevill Hall Hospital and The Grange Hospital.

Main duties of the job
Assist research team in administrative duties for research project start up, initiation, maintenance and study closure.
Arrange patient clinic appointments with research team and study participants as required.
Assist the research team in the preparation and conduct of clinic visits for patients enrolled in research
Arrange couriers for safe and timely transport of clinical samples as required
Arrange monitoring visits for external monitors and assist in the preparation and follow up of these visits. This includes coordinating diaries, requesting notes, booking venues, communicating action plans.

Our service has an exciting opportunity for a CYP Psychological Trainings: Therapy Trainee Autism Spectrum Condition/Learning Disabilities (ASC/LD) to join our team as part of the NHS England (NHSE) funded Children and Young People (CYP) Psychological Training Programmes.
This government-funded initiative promotes the growth and skill of the child mental health workforce to meet much needed demand. NHSE covers salary and training costs for a one-year full-time position, allowing services to employ trainees to support specialist pathways to bridge the gaps in CYP mental health support.

Training Information
This full-time training will begin in January 2027 with around 2-3 days a week for academic work (teaching and personal study), and 2.5 days based at the service seeing clients (children, young people, and carers) related to training assignments. Trainees will be both an employee of the service and an enrolled student with UCL/Anna Freud. Upon completion, qualified trainees will receive a UCL Postgraduate Diploma.
Main duties of the job
Main duties of this training role include:
- With the assistance of their clinical supervisor accept appropriate referrals;
- Assess clients for suitability for the treatment for which modality specific training is provided;
- Make decisions on suitability of new referrals, adhering to the department's referral protocols, and refer unsuitable clients on to the relevant service or back to the referral agent as necessary;
- Formulate, implement and evaluate therapeutic intervention for clients in line with their training modality and experience;
- Involve the system around the child in treatment as appropriate, e.g. parent/carer, school, conveying therapy formulations with sensitivity in easily understood language;
- Attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate;
- Complete all requirements relating to data collection within the service;
- Keep coherent records of all clinical activity in line with service protocols;
- Work closely with other members of the team ensuring appropriate step-up and step-down arrangements are in place to maintain a stepped care approach.

The department has UKAS accreditation to ISO151589:2022. It processes and reports, per year, approximately 23,000 surgical specimens (approximately 18,000 for the NHS with 16% from general practitioners), 1200 diagnostic cytopathology specimens and 1000 semenology samples.
Main duties of the job
The laboratory was designed in collaboration with the staff and has modern equipment including height adjustable dissection benches and a recently introduced Sakura Tissue-Tek Prisma Plus automated slide stainer and coverslipper, and a Dako Omnis immunohistochemistry immunostainer. Another similar immunostainer is being brought in to meet the increasing demands of immunohistochemistry and molecular pathology. The laboratory currently operates with two digital scanners which are used on a daily basis.
