Reviewing stopping criteria
Monitor homecare deliveries to reduce waste, including patients in a partnership
Help in moving appropriate patients to subcutaneous formulations as they become available
To advise and develop biologic pathways in collaboration with the ICB when/ if new developments are made and/ or new biologics are introduced
Conduct audit on patients due to start on biologics are instead started on MTX S/C (if appropriate) and the financial savings this could make vs starting patients on biologics
Start biologic weaning clinic for stable patients with regular monitoring to wean patients off biologic safely resulting in reduced medication burden to patient and financial saving for trust
Start moderate pathway initiating clinic as per NICE - starting patients early on biologics will reduce the health-economic impact.
Counselling patients starting biologics and DMARD's thoroughly will help reduce calls to helpline and improve patient safety and compliance
Safety checks for patients initiated on biologic can be done by nurse/ pharmacist as a telephone clinic - increasing clinic capacity in doctors clinic for new patients
Combined counselling sessions for patients being started on DMARD's/ Biologics - having presentations to run through the information
1. Service Provision
1.1 To lead on the implementation of the biosimilar switching as appropriate to ensure patients are switched as smoothly as possible & within the Trust timeframe of the biosimilar implementation
1.2 To work with the specialist nurses to keep a database of all switches & ensure patients are followed up
1.3 To keep a record of failures to switch & the reasons why
1.4 To produce financial reports for the Family, Diagnostic and Community and Planned Pathways Divisions where appropriate
1.5 To improve mechanisms within Rheumatology to minimise ‘waste’ with Homecare biologics. Examples of mechanisms include: monitoring dose changes; monitoring prescription requests and monitoring drug supplies.
1.6 To ensure the smooth running, day to day management of the Homecare pharmacy service to the rheumatology patients, as well as ensuring the switch happens with no disruption to patients.
1.7 To optimise funding applications for biological therapies that are used outside of national guidelines
1.8 To assist the existing CNS Rheumatology team in setting up a Biologics Review Service to optimise the use of biological therapy within the Rheumatology Service
1.9 To ensure that service provision is flexible and responsive to patient and service needs
1.10 To ensure the safe, clinically effective and cost-efficient use of drugs within rheumatology
1.11 To attend MDTs as part of the multi-disciplinary team
1.12 To meet any medication-related needs of rheumatology patients
1.13 To ensure that all healthcare professionals working in rheumatology are trained appropriately and are aware of all medicine related policies and procedures relating to the service
1.14 To provide drug expenditure reports and identify potential cost savings within the service as requested
1.15 To design and carry out audits as required
1.16 To attend outpatient clinics as deemed appropriate
1.17 Undertake risk management, including investigating clinical incidents in rheumatology and ensuring compliance with medicine legislation.
1.18 The post holder will be expected to identify, where necessary, areas that require service changes and develop a strategy to carry out the change through consultation with senior colleagues within and outside the pharmacy including commissioners.
1.19 Communication with ICB and NHS England about medicines policies or guidelines specific for their area of practice and for funding requests
1.20 Review and deal with high cost drug challenges from ICB and NHS England
1.21 Undertakes research into homecare medicines management services, which may include the trial of novel methods of providing homecare medicines management, or services for medicines previously not managed through homecare.
1.22 Pharmacy lead in the achievement of short, and long term, objectives within rheumatology in relation to medicine optimisation, especially regarding DMARDS
1.23 Develop and manage biologic review meetings to discuss all biologic initiations and yearly reviews
1.24 Counselling patients and answering concerns on their medication including telephone support clinics
1.25 To manage non biologic patients through drug switching DMARD management
1.26 To implement NICE guidance
1.28 Write or respond to business cases for service development to secure additional funding.
1.29 Attend departmental meetings to advise on medicine use and financial implications
1.30 Horizon scan for new drugs and advise the rheumatology meetings on the financial implications
1.31 Co-ordinate Medicines Use Savings Projects

Domestic Assistants are important members of the team who assist with maintaining a clean and welcoming environment over a 24/7 period for patients, staff and visitors within a busy acute hospital.The successful applicant should preferably have previous cleaning experience in a healthcare environment however full training will be provided and all successful post holders will be expected to work towards gaining a NVQ Level 2 Certificate in Cleaning and Support Services .
Successful post holders will be expected to work shift pattern which includes weekends. All post holders will be subject to a 6 month probationary period.
Main duties of the job
Successful post holders will be required to carry out specific cleaning and other related duties following set procedures and standards to maintain the levels of cleanliness throughout the environment.
They will be required to work as part of a team and have the ability to use a degree of initiative to prioritise and organise the daily duties, whilst maintaining a professional approach at all times regardless of the situation.
They will ensure that communicaton channels are maintained between appropirate staff, visitors and patient groups with regard to departmental cleaning hygiene.

As a Transport Driver you will keep blood and related products moving safely and on time, 24/7. Main duties of the job

- Drive a range of vehicles up to and including 7.5 tonne (except in South London (Tooting), Basildon, Lancaster and Southampton, where you will drive light goods vehicles up to 3.5 tonne).
- Use digital hand-held devices to track work and complete accurate electronic and paper records.
- Load and unload products and equipment, including assembling and dismantling kit, and safely move items up to 20kg with or without lifting aids.

Applications are invited for a Senior Clinical Fellow (ST3+ level) in General Surgery.
This post is suitable for doctors with prior core surgical training and/or registrar level experience in emergency and elective General Surgery, who are either seeking registrar-level experience before applying to a Higher Surgical Training programme, or who wish to gain experience that may contribute to an application for Article 14 entry onto the Specialist Register. It is also suitable for doctors who wish to pursue a non-training grade career at registrar level. For doctors who are new to working in the NHS/UK, a period of induction without on call commitments may be appropriate, and can be offered.
Main duties of the job
Previous and current post-holders have gained considerable clinical and non-clinical experience, with many either securing Higher Surgical Training posts or successfully passing the FRCS examination ahead of Article 14 applications. This post has become available due to current post-holders attaining National Training Numbers in General Surgery through the National Selection process in 2023, reflecting the skills and experience gained in these posts and support during the selection process.
A named educational supervisor and clinical supervisors will be provided, and training opportunities will be equivalent to those available to our higher surgical trainees on the All-Wales general surgical training scheme.
Clinically this post includes commitments to emergency and elective general and GI surgery including emergency vascular surgery, as well ongoing inpatient and outpatient care. Team rotas within sub-specialties ensure equal access to training opportunities, alongside higher surgical trainees, core surgical trainees and Foundation doctors. Clinical commitments will be primarily in the Grange University Hospital, with elective commitments on other sites within the Health Board, including Royal Gwent Hospital, Newport and Nevill Hall Hospital in Abergavenny.
For this reason, post-holders will be expected to travel independently between sites; travel expenses will be reimbursed in line with Health Board policy.

ELFT are excited to be advertising for a Peer Support Worker with lived experience of 'complex emotional needs' (often diagnosed as 'personality disorder') to join the countywide Bedfordshire and Luton Complex Needs Service. Our service aims to work with service users, colleagues, and carers to establish a learning culture that improves the care and treatment for people with complex emotional needs and their families/carers in the community. The successful applicant will use their lived experience of 'complex emotional needs' and local services to demonstrate that recovery is achievable. They will share their story of recovery in a way that conveys a sense of connection, hope and optimism to those who are earlier on in their journeys or experiencing challenging times, and will provide time-limited support to enable service users to progress towards their own recovery goals. The successful applicant will be well supported by colleagues in the Complex Needs Service and the Peer Support Lead. This post will be based at Mountbatten House, Dunstable and the post-holder will be required to travel to CMHT bases, inpatient units and other sites across Bedford, Central Bedfordshire and Luton.

Job summary
We are looking for an enthusiastic and experienced Systemic Family Psychotherapist to join our innovative Systemic Family Therapy Service in the Forensic Directorate. The post holder will join a team of Systemic Family Psychotherapists who work across Medium and Low Secure units and Forensic Outreach team, ELCFS (The East London Community Forensic Service).
The post holder will deliver specialist systemic interventions to service users referred to the Family Therapy Service. This will involve providing assessment and psychotherapy to service users, their families, and to other non-professional carers. The post involves offering in-reach services to service users approaching discharge, working with inpatient wards, community teams, service users and their families.
This is a part-time post and the successful candidate will be working across in-patient services and the community outreach team, ELCFS (The East London Community Forensic Service). ELCFS is multidisciplinary service, for service users in the process of and following discharge from the Forensic inpatient service.

We are looking to recruit an Assistant Psychologist to join our High Intensity User Team (HIUT).
The HIUT exists to identify frequent attenders to the Emergency Department (ED) at the Royal London Hospital, Whitechapel. To consider any unmet needs that may be increasing their attendance and seeks to ameliorate this unmet need. Job summary
This new 10 PA post is based within the male low-secure part of the Forensic Directorate of the East London Foundation NHS Trust. It is suitable for doctors who have completed their Foundation training. The post will be based at Wolfson House ], a forensic low secure unit, (311-315 Green Lanes, London N4 2ES), with Quality Improvement as a special interest. The successful candidate will provide day-to-day care for patients on two wards, and be provided with shared office and support services at Wolfson House. The post will be divided between 8 PAs of Direct Clinical Care, and 2 PAs of Special Interest, with particular training and experience provided in Quality Improvement methodology and Change Leadership. This is a full-time position (9am-5pm Monday to Friday) with no on-call commitment.
Main duties of the job
The post-holder will work at the level of a Junior Clinical Fellow, working on Loxford Ward and Woodberry Ward, and to join another Clinical Fellow and Speciality doctor in covering the unit. Clinical supervision will be given weekly. It is expected that the post holder will comply with the statutory requirements for annual appraisal, drawing up personal development plans and revalidation as set out by the General Medical Council and the Trust.

Clinical duties will include day-to-day management of inpatients, including mental state examinations, physical health reviews and reviews of medications; reviews of enhanced observations; admission assessments, reports and discharge summaries. On occasion, there will be the opportunity to liaise with prisons and courts. Attendance at ward rounds and CPA review meetings is expected, although this may be limited because of the two ward role. Job summary
The post holder will be an expert in psychiatric therapeutics and provide a specialist referral point for all health and social care professionals working within the crisis and liaison pathways.
The successful candidate will have a strong clinical background, as well as a postgraduate clinical pharmacy diploma and ideally a non-medical prescribing qualification. Good knowledge of physical health conditions is essential in this role. They must be able to work and make decisions autonomously in an acute hospital environment and integrate with the acute trust pharmacy teams. They will be enthusiastic, innovative, flexible, proactive and have knowledge of how operations/pathways in this service support patient care.

An exciting opportunity has arisen for an experienced OT, Social Worker or Nurse to work within the Newham Early Intervention Service (NEIS). The service supports 18-65 year olds experiencing a First Episode of Psychosis. It aims to reduce the impact of psychosis by reducing the duration of untreated psychosis and engaging clients in biopsychosocial interventions.
NEIS is a progressive and evolving team, looking to recruit candidates who are similarly interested in creative approaches to supporting individuals with First Episode Psychosis. It is important that our values align with that of any successful candidate.

You would be working in a highly motivated multidisciplinary team, which values the unique contribution of each member. Main duties of the job

The service has led the way in the design of adolescent mental health facilities nationally.
The client group is varied and challenging. We have an eclectic and dynamic staff group and are seeking a candidate who has an interest in developing our current service, and delivering our clinical care in new ways. From the assessment, time limited interventions are offered both directly and through consultation with members of the wider multi-disciplinary team. A range of therapeutic orientations is represented in the wider team including psychodynamic, systemic and cognitive behavioural perspectives. Psychological input begins at admission and is focussed on enabling young people to return to their Community as soon as possible.

1. Ensure that clients receive a high standard of clinical care, while also mentoring junior staff to provide a comprehensive and suitable psychology service for clients, families, and caregivers.

Ocean is offering a 0.8 WTE fixed term 12 month maternity cover for a band 7 psychologist or psychotherapist. Occupational Therapy Assistant
The Forensic Community Learning Disability and Autism Service is part of the ELFT Forensic Service, with an overarching aim of reducing the risk of offending or reoffending of adults with a diagnosis of Learning Disability and/ or Autism.
The team provides a range of support across North East London, including complex case management and joint work with and consultation to numerous partner agencies such as other areas of the ELFT Forensic Service, criminal justice partners and Community Learning Disability Services, and providing a substantial programme of training. The team will also offer in reach to inpatients at The John Howard Centre and other low secure sites who are approaching discharge.
A strengths-based approach will underpin work with service users and carers so that their individual assets can be optimised and positive life choices and opportunities can be enabled in partnership with individuals and the services around them.
Main duties of the job
- Under the supervision of an Occupational Therapist, to implement agreed occupational therapy treatment plans by carrying out practical individual and group activities with service users.
- To collaborate with senior OT staff to review and modify specialist treatment programmes when necessary.
- To assist senior OT staff in the assessment and treatment of service users in a specialist mental health service.
- To work and liaise appropriately with the multi-disciplinary team, including attendance at service user-related meetings as directed.
