Welcome to FCMS!
We are looking for band 6 practice nurses and non-medical prescribers to join the team on a bank basis, covering all the services within Inclusion Health.We are looking for individuals with a passion and enthusiasm to work with some of the most vulnerable and complex people within our community. The right candidates will be approachable and display excellent communication skills, compassion, tolerance, and a non-judgemental attitude.
Main duties of the job
Day to Day Duties to include, but not exhaustive:
To provide proactive reviews and support for patients with CVD and / or COPD in line with clinical competence and local pathways
To initiate prescribed medication and amend medication when required.
Undertake holistic health assessments, identifying physical, mental and social needs
Deliver health prevention and health promotion interventions including lifestyle advice (alcohol, smoking, diet and physical activity), weight management and wellbeing support, Opportunistic screening and vaccination advice
Identify unmet needs and escalating health concerns, taking appropriate actions
Deliver care within the mobile clinical unit adapting consultations to a community-based environment
Support people who present to the mobile unit by providing assessment and advise where appropriate, signposting and referring to the most suitable service when out the scope of practice and offering reassurance, health information and ongoing support
Promote self-management and empower patients to engage with health services
Accurately record all activity, assessments and outcomes on EMIS using appropriate clinical coding and templates
Maintain confidentiality and data protection standards
Maintain Current NMC registration adhering to the NMC code of conduct
Work autonomously withing scope of practice, escalating concerns appropriately
Participate in supervision, appraisal and continuing professional development

Job description
Job responsibilities
Compass Medical Practice is a GP practice for those who are difficult to manage in mainstream practice. Patients are either consulted via telephone/ video link or face to face. When a face-to-face appointment is required, a risk assessment is undertaken to assess what support the clinician requires. Consultations are supported with the use of patient liaison officers and consultations are carried out in our mobile clinic van or in an agreed prebooked venue.
Compass Medical Practice also provides medical cover to temporary patients residing at Thornton House. These are short term patients that are residing at Thornton house for either rehab, rest or recouperation. There is a weekly ward round and multidisciplinary team meeting. Compass Medical practice also offers medical support throughout the week, outside of the ward round, during opening hours.
Our homeless health service is nurse led and is operates on a Monday, Tuesday, Thursday and Friday from the Salvation Army in central Blackpool. The nursing team take a multidisciplinary approach, often working with wider services to meet the needs of a complex patient cohort who are facing multiple disadvantage.
The Targeted demand management (TDM) service supports people across Blackpool to address clinical and non-clinical factors increasing their risk of an unplanned hospitalisation.
Key aims of TDM are to provide clinical and non-clinical interventions that meet the needs of individuals identified in Blackpool with the following respiratory or cardiovascular long-term conditions. The TDM service, through reducing individual risk, will play a role in relieving pressure on acute services, through preventing health crises.

We are looking for a dynamic and motivated individual to join the dedicated team of Nursing and Medical Staff within Trauma and Orthopaedics.
The Trauma & Orthopaedic Department work across both Whiston and St Helens Hospitals providing day case, in-patient and outpatient services to elective and non-elective patients. The Department is split over 3 wards - 3Alpha, 3B and 3Elective, and our outpatient Fracture Clinic all located in Whiston Hospital.
Ward 3Alpha is an 18 bedded ward dedicated to caring for patients with hip fractures, you with work very closely with the MDT consisting of dedicated Ortho-geriatricians, Orthopaedic Nurse Specialist, Discharge Co-Ordinator and ward based Therapy Staff.
Ward 4F Elective Orthopaedics is a dedicated 22 bedded elective ward caring for patients undergoing joint replacement surgery; the ward also facilitates a pre-operative education programme for patient known as joint school and runs a post-operative follow-up wound care clinic.
Main duties of the job
To work as part of the Nursing Team undertaking household support tasks and direct care tasks as delegated by the Nurse in Charge.
To support the qualified Nursing Staff in the maintenance of high standards of nursing care to patients.
To assist in providing an environment within the ward that is conductive to patient recovery and pleasant for patients and their families.
To accept responsibility for specifically delegated functions within the ward which the Nurse in Charge feels are appropriate for untrained staff to undertake.
To ensure where entries are made in nursing documentation, that these are correct, legible and comprehensive and countersigned by a registered nurse.

Main duties of the job

Our GP Assistant will support the smooth running of clinics by performing routine administration and clinical tasks on behalf of the GP. They could support the GPs/ANPs with urgent admin such as fast tracks, x-rays, scans and referrals generated from our patients with an urgent acute condition but also support with urgent bloods, dressings and ECGs for these patients.

Job summary
The Respiratory Team within Whiston Hospital is looking for Band 5 registered nurses to deliver high quality care
Our units are both acute and fast paced environment. No two days are the same and staff are continuously facing new challenges and experiences that help them grow and develop within their role.
If you join our team, you will have the opportunity to gain experience in acute respiratory nursing. We provide a holistic car to the very acutely unwell and those with chronic respiratory illnesses who require frequent treatment. The wards treat a wide range of thoracic conditions. We care for patients with a variety of conditions, including lung cancer, asthma, COPD, TB, type 1 and type 2 respiratory failure, pleural effusions, pneumothorax, Tracheostomies and PE'S.
The ward team has excellent support from the Consultant Respiratory physicians.
We have strong relationships with our allied health professionals to provide excellent, safe care. Respiratory has a close working relationship with the Respiratory Nurse Specialists and palliative specialist nurses, to deliver high quality patient care.
You will work closely with the therapy team and have fantastic input from the wards pharmacist who will help you gain knowledge in the field of pharmaceuticals. The wards also have a ward-based discharge coordinators, they support with all aspects of the discharge process and provide support for ward staff to ensure effective discharge planning is achieved.
Main duties of the job
As a newly qualified Nurse, we can offer you;
Preceptorship- New starters attend a Trust Induction, where they learn about the Trust, start developing their mandatory skills and begin to understand what will be expected of them in their new role. Staff will then progress onto Preceptorship, an excellent programme designed to facilitate efficient development of competence, confidence and expertise. Support will be given at a clinical level by a preceptor, and from a wider organizational structure including the preceptorship lead, clinical skills team, clinical practice development nurse and learning and development.
The Respiratory Directorate is an excellent environment for experienced nurses also. They may not have previous experience within respiratory, but they would be able to bring other knowledge and skills from other specialties. As described above, the same opportunities are available to experienced nurses. Nurses who may have previous experience within respiratory would have the opportunity to consolidate their knowledge and skills. They would be able to gain experience and develop within certain aspects of respiratory care that may not have been available to them in their previous role. This includes caring for patients in our high dependent units, where our patients may require non-invasive ventilation therapy.
You will be expected to rotate to both days and nights and be flexible in your approach to caring for a group of patients.

This post is for a 2 year fixed term Locum Consultant Haematologist responsible for delivering comprehensive clinical care to patients with a wide range of blood disorders, including haematological malignancy. The role also involves providing specialist advice on all aspects of haematology to colleagues across the Trust and in Primary Care.
The successful applicant will be based at Furness General Hospital, with flexibility to work across Trust sites, including Royal Lancaster Infirmary. Outpatient activity will include general haematology clinics, with the opportunity to develop a sub-specialist interest. Clinics are delivered within oncology units across the three sites, in close proximity to chemotherapy delivery areas.
This is a full-time 10 PA post with 2 additional programmed activities (APA), subject to negotiation and reviewed annually through the job planning process. Main duties of the job

There will be an expectation for the successful candidate to participate in a 1 in 5/6 on-call rota, which attracts an additional 1 PA and a 5% supplement.

We are excited to offer a 1 year fixed-term vacancy for an Assistant Psychologist in the Home-Based Treatment Team in Furness Home Based Treatment Team.

Home Based Treatment Teams provide services to people with complex mental health needs. The postholder will receive weekly clinical supervision from a Clinical Psychologist.
Main duties of the job
The post holder will gain experience in psychological assessment, developing psychological formulations of specific problems, delivering 1:1 and group interventions. The role also involves engaging in indirect work within the team, with family members, carers and external agencies. There will be opportunities to be part of service development projects and to deliver training. An interest in working with individuals with a range of presentations across the lifespan will be essential, as is an interest in developing skills in working within a multidisciplinary framework.
Whilst the base for this role would be Barrow in Furness, work completed may feed into the three Home Based Treatment Teams across the locality (The Bay - South Lakes, Furness and Lancaster & Morecambe). The role requires travelling within the community, including home visits. There is an option for hybrid working to align with service need.The post will sit as part of a wider team of psychological practitioners working within The Bay area. The post holder will work exclusively alongside a visually impaired Cognitive Behavioural Therapist within the Preston Talking Therapies service. An exciting opportunity has arisen for a keen and enthusiastic band 6 Practitioner to join the Forensic Community Mental Health Team (FCMHT). FCMHT is part of the Forensic Community Service within the Specialist Services Network. FCMHT is based at The Parlick Centre at Guild Loge in Whittingham, Preston. FCMHT is a specialist mental health team, providing care designed to support and supervise service users who have a diagnosis of severe mental disorder with a risk to others who have previously received inpatient treatment at low, medium, or high security settings. The FCMHT provides an accessible, responsive and adaptable service, prioritising the needs of service users with complex needs, to maintain the safety of service users, families, carers and the public.
The post of Practitioner sits within a multi-disciplinary team made up of nurses, social workers, doctors, occupational therapists, clinical and forensic psychologists and support workers.
The post holder will support all aspects of the service delivery, in particular supporting the delivery of the clinical model, case holding, engaging in supervision, assessing the needs of those referred, developing collaborative formulations, undertaking specialist risk assessments with the wider MDT. Being part of the 24hour on call provision of the service.

Job summary
This post will facilitate the provision of care to chronic and acute pain patients . The work split will be approximately 6.5 sessions for chronic pain management and 3.5 sessions for the inpatient anaesthetic team. The job plan includes 1.5 SPA.
Clinical experience and training equivalent to Advanced Specialist Training in Pain Medicine is essential for this post and it would be desirable to be a Fellow of Faculty of Pain Medicine of the Royal College of Anaesthetists (FFPMRCA).
The successful postholder will be a currently practicing clinician, holding GMC registration with a license to practice. They will have attained FRCA or equivalent and be included on the GMC's Specialist Register in Anaesthetics.
A commitment to a bio-psychosocial approach to pain management, multi-professional working, evidence-based practice and the principles of shared care and close working with other disciplines and specialties are also essential elements of this role.
The post will be based within the chronic pain management service which is an integral part of the wider Integrated MSK, Pain and Rheumatology Service (IMPReS) in the Division of Diagnostic and Clinical Support and the Anaesthetic Department in the Surgical Division.

We are excited to offer an opportunity for two Salaried GPs to join our friendly, highly supportive inner-city GP training practice. We have up to 8 sessions per week available across the two posts and are happy to be flexible about how these sessions are divided to suit the right candidates.
You will join an established clinical team where you will never feel isolated. Senior, experienced GPs are available in person every day for advice and support, alongside an extensive multidisciplinary team. We offer a genuinely supportive working environment, capped medicine management and Docman workload, dedicated learning opportunities, pro rata study leave. Our commissioned home visiting service means salaried GPs very rarely undertake home visits, other than occasional palliative care visits.

Main duties of the job
The successful candidates will provide high-quality, patient-centred general medical care to our diverse inner-city population, managing a broad range of acute presentations, long-term conditions and ongoing health needs.
Clinical sessions consist of 15 appointments, with a mix of face-to-face and telephone consultations. Standard appointments are 10 minutes, with 20-minute appointments provided where an interpreter is required. We actively encourage continuity of care, with opportunities for GPs to manage and follow up their own patients and develop meaningful ongoing clinical relationships.
You will work closely with our experienced GP and multidisciplinary team, with senior GPs available in person every day for clinical advice and support. We have three full-time, in-house pharmacists who provide medication reconciliation, medication reviews and ongoing medicines support for patients and clinicians.
Salaried GPs have capped medicine management and Docman correspondence workloads. Home visits are very rare due to our commissioned home visiting service, although occasional visits may be required for palliative care patients.
We want our GPs to have the autonomy, support and team around them to provide excellent care safely and sustainably.

We are a friendly, established inner-city GP training practice caring for approximately 14,000 patients from a diverse community. We are proud of our strong team culture, supportive working environment and commitment to providing excellent patient care.
Our clinical team includes 3 full-time GP Partners, 5 Salaried GPs and 6 GP trainees, supported by a comprehensive multidisciplinary team including 3 full-time in-house pharmacists, 1 Advanced Nurse Practitioner, a Physician Associate, Practice Nurse, Nursing Associate, Mental Health Practitioner.
Although we are a busy practice, nobody works in isolation. Senior, experienced GPs are available in person every day, and we place real importance on looking after and supporting one another. Job summary
LTHTR which along with significant investment in our Community Diagnostic centre resulted in us expanding our Consultant Radiologist numbers to support additional work. Specifically, we are looking to appoint a Locum Consultant Radiologist with expertise and interest in Cross-sectional and Non-Vascular Interventional Radiology (NVIR). This will be a 12-month fixed term post. The role will include in-patient CT body reporting sessions, attendance and participation in relevant MDT meetings and participation in the on-call rota. Candidates with additional experience in other areas are encouraged to apply. Where possible we try to encourage development of subspecialty interests that fit with the departmental needs and availability of suitable sessions. The successful candidate will support general Radiology, assist in the smooth running of the Directorate and work with colleagues in the provision of an effective and efficient service. The post will be based at Chorley and Preston Hospital sites as well as sessions at the Community Diagnostic centre depending on subspecialty interest. You should have completed the Radiology training (be within 6 months of CCT or have obtained CESR) and should have sufficient skills to be able to independently report MRI, CT, and US examinations within their areas of subspeciality expertise.
Main duties of the job
You will be expected to provide cross sectional and non-vascular interventional reporting and support for various MDT's. The post will involve 10-programmed activities of imaging support (7.9 DPAs plus 1.5 SPAs and 0.6 for on-call) and will be expected to take part in the management of the service and attend meetings of the Diagnostic Imaging Directorate, the Medical Staffing Committee, and such other committees you may be appointed to by clinical colleagues.

Job summary
An exciting opportunity has arisen for a band 6 mental health practitioner to work in the Home Treatment Team in Lancaster.
The Home Treatment Team cares for people who are experiencing acute episodes of mental illness or emotional distress. The service is an alternative to admission and offers intensive support to service users and their families in the community.
The service works closely with the in-patient wards to support early discharges and with other parts of the mental health services as well as third sector providers, so great communication skills are essential.
The post is a full-time, permanent post covering the service over 7 days per week working long days. The team covers Lancaster, Morecambe and surrounding areas.
Main duties of the job
As a member of the team you would be working with a multi-disciplinary team consisting of Mental Health Nurses; Support Workers; Occupational Therapist; Psychologist and Consultant Psychiatrist.
You will provide a comprehensive assessment and triage service and, where necessary signposting or clear onward facilitation to a mental health pathway. Your referrers will mostly include IRS (initial response service), and Acute Hospital colleagues, trust employed mental health practitioners working in GP service. Service users will be triaged collaboratively and a decision will be made in consultation with the referrer and with the use of a clinical tool, within the response time required.
This comprehensive, collaborative triage system will lead to comprehensive assessment clear onward facilitation of service users to the most appropriate care following assessment
The post holder will provide evidence based mental health assessments and/or signposting to alternative service provision for people who present requiring a specialist urgent mental health assessment. The post holder may offer follow-up appointments within the agreed timescale for intervention as part of the on-going assessment process.

We have an exciting opportunity for Locum Consultant Radiologist with a subspecialist interest in Chest / Cardiothoracic Radiology to join our highly diverse, motivated, and thriving Radiology Department at Lancashire Teaching Hospitals NHS Trust. The new post has become following significant investment in our Community Diagnostic Centre. This is a 12-month full-time fixed term post, with a view to be made a permanent substantive post and will be based at Chorley and Preston Hospital and also our local CDC site, Preston Healthport. The successful candidate will support also general radiology, assist in the smooth running of the Directorate and work with colleagues in the provision of an effective and efficient service. The successful candidate must live within 45 minutes travel time of RPH.
Main duties of the job
You will join a committed and friendly team of Chest / Cardiothoracic Radiologists with various subspeciality interests including cardiac imaging, lung biopsy, ILD, PET/CT and Post-mortem CT.The team currently supports 13 Chest physicians, with whom we have a close relationship, also with a wide variety of clinical interests, including lung cancer, ILD, asthma, interventional bronchoscopy, and pleural intervention. There will be opportunities to develop any of these further as well supporting expanding clinical interests such as the pleural service and innovative interventional bronchoscopy techniques. There are weekly Lung Cancer MDTs, a general chest clinico-radiological MDT and bi-monthly regional ILD MDTs. LTHTR is a cancer centre and is the central hub for the regional ILD service. Duties will include all those duties normally expected of a radiologist in a similar sized Radiology department in the United Kingdom. The majority of the appointee's time will be spent on clinical duties. In addition, the appointee will be expected to take part in the formal teaching and on-going audit programmes within the directorate as well as a full range of management duties commensurate with this role. The successful candidate will also be expected to provide cross sectional imaging reporting and support for various MDT's. The post will involve 10-programmed activities of imaging support (8 DPAs plus 1.5 SPAs and 0.5 for on-call).

Job summary
The post holder will work within a newly established service, Reconnect Care after Custody for Lancashire and Cumbria, covering 6 prisons and also the wider community of Lancashire and Cumbria.
Reconnect is a triage, assessment, referral and support service and starts working with people before they leave prison, where they will have access to a Reconnect practitioners and support practitioners, helping them to make the transition to community-based services that will provide the health and care support that they need. It is offered to people in prison up to 12 weeks prior to release, and up to 6 months support post release.
Main duties of the job
The post holder will work within the Reconnect Care after Custody service for Lancashire and Cumbria, covering 6 prisons and the wider community of Lancashire and Cumbria. Reconnect is a triage, assessment, referral and support service and starts working with people before they leave prison, where they will have access to a Reconnect practitioners and support practitioners, helping them to make the transition to community-based services that will provide the health and care support that they need. It is offered to people in prison up to 3 months prior to release, and up to 6 months support post release.
Please note that hybrid working is necessary due to the area of coverage involving some working from home and utilising other trust sites as a bases as required.

This is a varied role combining clinical coding, document management/scanning and referral administration. We have an opportunity for a Domestics Team member to join our Ancillary Department. The ideal candidate will have experience of cleaning within a professional environment or in a clinical setting, together with good interpersonal skills and the ability to work as part of a small team, but also as an individual.

Pendleside Hospice was founded as a charity in 1988 and is one of the principal providers of palliative and end of life care services to adults in Burnley and Pendle. Pendleside is widely known throughout the area of Burnley and Pendle and is held in deep affection by the local community. It is truly a privilege to work here.
All our care is free of charge to the patients and their families/carers but, of course, it isnt free to provide and most of it is funded through the generosity of the local community in which we serve. CORE TASKS:
Ensure that Hospice policies, procedures and standards are adhered to at all times; with particular regard to those concerning confidentiality, information governance, health and safety, and infection control.
General cleaning of both clinical and non-clinical areas according to Hospice cleaning schedules.
Other cleaning duties including carpet and upholstery shampooing as required.
Dispose of waste including clinical material into designated bins.
Ensure that effective health and safety procedures are implemented and maintained and any risks to patients/staff safety are highlighted for action.
Apply infection control measures ensuring that any risk associated with the prevention and control of infections are reported immediately.
Ensure the correct storage, handling and utilisation of Hospice equipment and supplies.
Process the ordering of cleaning materials.
Maintain patient confidentiality and dignity at all times.
Communicate effectively with service users and Hospice colleagues.
Complete mandatory training.
Contribute to team meetings.

Main duties of the job
What Youll Be Doing
You will support a range of duties overnight, including:
- Support the registered nurse on the night shift on emergency response and personal care as needed
- Carry out administrative tasks such as updating records and data inputting
- You will be ILS (Immediate Life Support) trained by us onsite, as well as NEWS2
- Acting as pharmacy second checker and assisting with medication rounds

What Were Looking For:
