Job summary
We have an exciting opportunity for a Band 6 Community Mental Health Practitioner to join our Adult Community Mental Health Team service, based at Chorley Health Centre, Chorley.
You will join a friendly team who are committed to their own development and who are, outgoing and sociable. The team is also flexible and family friendly where everyone's individualised cases are considered to help manage work-life balance where possible.
We are looking for a Core registered mental health practitioner, with post qualification experience in working clinically with service users experiencing a range of mental health problems, who are able to work as part of a multidisciplinary team, and has experience in assessing, planning, implementing and reviewing care needs and pathways with complex mental health issues.
We require someone who has great knowledge of medications and how to monitor them. Particularly psychotropic medications including Clozaril and Lithium.
You will also be required to have experience and knowledge of risk assessment and care and safety management, the ability to explore issues relating to the impact of health and social needs in mental health and well-being. We take a multi-disciplinary approach, in a holistic way.
You will also be expected to take part in our Duty system on a rotational basis as a back up duty officer.

About Chorley CMHT:
Chorley CMHT is an ambitious and collaborative team that takes pride in supporting both the people who use our services and one another. We are continually developing our pathways and partnerships to provide timely, coordinated care and help people remain well within their communities wherever possible.

Provide comprehensive assessment, advice and treatment to people with highly complex needs referred to Community Services in accordance with care pathways for given specialism
Deliver specialist interventions in accordance with care pathways for given specialism
Work closely with other professionals within and outside the Community Team
Actively supports the Team Leader/Advanced Practitioner in the achievement of KPI / CQUIN and related County Council targets, and maintenance of registration with regulating bodies
Take the lead on specific objectives as directed by the Team Lead and Clinical Team Lead
Provide supervision and direction to practitioners and support staff within community services, undertaking appraisals and supporting the personal development of colleagues
The post holder will have a positive attitude to the care of the service users, be adaptable and resourceful and will be required to work regularly without direct supervision.

Job summary
An exciting opportunity has arisen for a Registered Mental Health Nurse, Social Worker or Occupational Therapist to joint the Pennine West Home Based Treatment Team. We are looking for a highly motivated, innovative and a dedicated practitioner to provide care and treatment for those service users who require support from the Home Based Treatment Team.
This dynamic and driven multi-disciplinary team offer an alternative to psychiatric hospital admission, supporting service users in acute mental health crisis in their homes, as well as supporting loved ones and carers during this period.
The team are passionate about providing person centred care, tailoring care plans to suit the needs of all individuals involved during the care spell and providing a range of psychosocial interventions during their admission.
The team are required to thoroughly risk assess situations and have access to a supportive leadership team to support such complex cases.
Staff wellbeing and development is paramount and well supported in this team.
The post holder will be supported with their professional development as part of their appraisal process, and regular 1:1 supervision is provided.

You will have experience of working with service users who present with complex mental health needs.
Main duties of the job
The post holder will:
Undertake collaborative care planning with service users and their carers.
Undertake risk assessment and record risks in line with the trust policy
Undertake shift co-ordination when allocated to do so.
Participate in Multi-disciplinary Team (MDT) and agree plans with patients in relation to their care.
Maintain their knowledge of evidence based home treatment interventions and deliver these interventions in line with service use care.
Undertake collaborative care planning with services users and their carers.
Undertake gatekeeping assessments to ensure service users are treated in the least restrictive environment.
Undertake allocation of shift co-ordination.
You should be able to demonstrate evidence of care that facilitates a positive culture of patient engagement, a commitment to enhancing the service user and carer experience and of on-going professional development.
You will have experience of implementing effective, evidence based care management / assessment skills and display excellent communication skills.
The post holder will be able to work independently as well as being committed to a multi-disciplinary team Approach (MDT) both of which are essential requirements for the post.

The home is a 5-bed service and is an Edwardian building with lovely spacious rooms including plenty of communal areas where young people and staff can work together. Alongside the house there is a fabulous back garden and large decking area, which are perfect for when the summer months bring the sunny weather to Yorkshire. You will be working with an established, experienced and diverse staff team who all have a passion for supporting the young people to achieve their goals and help them move on to full independence.

We are seeking a part-time Consultant Psychiatrist to join Care in Mind, a unique specialist mental health supported residential care service based in the North of England. Our CQC registered services are all provided in partnership with the NHS and local authorities.

Job summary
The centralised decontamination unit is looking for an enthusiastic and flexible team member to assist in the decontamination and delivery of endoscopes to our user areas within the hospital.

Main duties of the job
The Endoscopy Decontamination Unit decontaminates all clinical and medical devices which can be reprocessed.

We are excited to advertise for 2 x General Adult Consultant to join us working in inpatients at The Harbour, Blackpool. The post is substantive and full time (10 PAs).
You will be expected to participate on the Consultant On Call rota, for which additional remuneration is payable.
Post 1; 18 bedded male acute ward (Churchill)
Post 2; 18 bedded female acute ward (Stevenson)
Both posts have SAS Doctors and Resident Doctors established.

The Harbour MH hospital is a state of the art unit based just off junction 4 on the M55. The M55 connects with the M6 and tends to be a really quiet connecting motorway, with rarely any delays. The Harbour is 2 mins from Junction 4 off the M55.
Main duties of the job
The post holder will be a leading member of the well-established, in-patient, multidisciplinary teams based at the Harbour and will have the following important clinical responsibilities:
o Responsible Clinician for a defined caseload.o Assessment of patients referred by the community teams and responsibility for any of admissions at the Harbour Hospital.o Out of Hours Cover.

He/she will work with a multidisciplinary team including nursing, social work, occupational therapy and psychology staff. The team works with a limited number of GP practices and there is an expectation that there will be close liaison with those practices. The Consultant will be available to give advice to primary care and the multidisciplinary team. The consultant will have a small caseload of patients that he/she will see in outpatients but the majority of patients will have a separate key worker.

We are seeking to recruit a substantive Consultant Psychiatrist to join our Dova Unit at Furness General Hospital, Barrow. The post is full time (10PA).
The postholder will be expected to participate on the Consultant On Call rota, for which additional remuneration will be payable (0.5 PA + 5% supplement)
Main duties of the job
The Dova unit, based at Dane Garth in Furness General hospital, is an 18 bedded mixed gender ward. There is an older adults ward also based at the hospital. FGH is a large district general hospital with around 268 beds, a full range of specialities and an accident and emergency department. The Dane Garth unit has established resident doctor posts including FY1s, FY2s a CT and GPSTs. This post holder would have the 2.5 WTE resident doctors supporting ward duties, along with input from a SAS colleague. There is an active monthly peer group meeting of medical colleagues within the Cumbria locality too.
The post holder will work with a multidisciplinary team including matron, ward manager, clinical leads, nursing staff, occupational therapy HCA and recovery support staff.

Main duties of the job
- Assist the doctor and trained staff in the smooth running of the clinic.

We are delighted to offer a suitably qualified Speciality Doctor the opportunity to work in a substantive position in an exciting, modern and forward looking organisation.
The post is full-time, 10 PA's per week with associated on-call commitments on a 1 in 5 basis.
The post holder is expected to contribute to the Ophthalmology out-patient and surgical activities.
Four years' experience at post graduate level including at least 2 years in ophthalmology is necessary with the ability to practice independently. Starting salary will be commensurate with experience.

Main duties of the job
The successful candidate will be encouraged to develop his/her own area of interest within ophthalmology for which support via the Ophthalmic Local Trainee Scheme can be provided.
The Eye Department, which is staffed by 12 Consultants and 9 Middle Grade Staff, is delivered across the entire Trust Footprint and candidates will be expected to travel between sites.

Job summary
This role provides an exciting opportunity for a newly or recently qualified HCPC Registered Practitioner Psychologist to progress their career through a structured and supported preceptorship scheme. Change Grow Lives Preceptorship Process provides the opportunity for this role to demonstrate competence and confidence across a wide range of domains as you become more established in your career, with a defined pathway through to a Highly Specialist Practitioner Psychologist post. This is covered under a separate job description, but successful completion of the internal preceptorship process will result in you moving to that higher grade within your existing service. Change Grow Live will enable you to further develop your skills and provide ongoing opportunities for professional development.

We are seeking to appoint someone with a caring, compassionate nature, who is innovative, creative, flexible and highly motivated. Primarily you will undertake specialist assessment, therapeutic work and physical health care for people presenting with an eating disorder within the East locality. You should be an excellent communicator and team player, with the successful applicant joining a dynamic multi-disciplinary team of professionals to provide assessment and treatment to children and adult service users and their families/carers. The team includes a range of other psychological professionals, nurses, dietitians and a consultant clinical psychologist/clinical lead.
Main duties of the job
Existing experience in eating disorders across the age range is desirable but not essential, however, a keen interest to develop knowledge and skills in this field and a strong desire to help service users and families is vital. The successful candidate will benefit from regular clinical supervision as well as professional supervision, with encouragement to engage with CPD activities as appropriate.

Tuesday, Wednesday ,Thursday and Friday 12:00 - 20:00. Saturday 08:00 - 16:00.
As the Alliance grows so will the diversity of opportunities and duties within the organisation and this opportunity has become available due to a promotion within the Alliance.
Previous experience of working within a GP Surgery is an advantage but training will be provided for the successful candidates.
The successful applicant must have good attention to detail and ability to operate different IT systems (training given).
There is a requirement for the successful candidate to attend any of the Extended Hours hubs during their working patterns so a full driving license is essential. Travel whilst working will be in one of the Company vehicles but the successful applicant must have access to their own vehicle.

DUTIES AND RESPONSIBILITIES
- To be the first point of contact in providing basic advice and guidance on day-to-day workforce queries, working within the boundaries of the Trusts Policies and Procedures, signposting when necessary and escalating to a senior colleague when appropriate.

- Effective use of the IT systems to collage and produce the workforce management information/metrics, ie. appraisal and mandatory training figures etc.

Job summary
An exciting opportunity has arisen for an inspirational and motivated Health Care Assistant to join the Lytham District Nursing Team.
The successful candidate will be supported by our nursing teamand essential training for the role will be provided.
You must be able to communicate effectively with a wide range of healthcare professionals and be enthusiastic and passionate about the delivery of high-quality care. As a Health Care Assistant, your role will be to assist our patients within their own homes to provide clinical interventions such as taking observations, venepuncture, wound care, pressure area care and health promotion. All necessary training for acquiring these skills will be provided by the Trust.
Working alongside skilled health professionals who work within this multidisciplinary team, the department provides a fantastic place to learn and for you to develop your skills.
Main duties of the job
We are looking to recruit a caring and proactive healthcare assistant to join our team.
You need to have excellent communication skill as youwill be required to liaise with the multi-disciplinary team to ensure patient needs are identified and communicated effectively so that safe decisions are made around patient care.
You will be expected to maintain high standards of nursing care be personally responsible and accountable for your actions following the Trust's Policies and Procedures.
You will be expected to maintain accurate written and/or electronic records ensuring all legal requirements are met.
You will need to be prepared to work flexibly to the needs of the service.

Provide a high-quality administration service to ensure the smooth operation of a diverse range of clinical services from various sites within a business unit. Flexibly work within the administration team to deliver a wide range of services to clinicians, team leaders, and clinical managers as appropriate.Offer a high-quality customer care first point of contact service via telephone, email, written correspondence, and face-to-face interactions.
