Job summary

The Patient Safety workstream sits within the Trust's Clinical Governance Team.

The team is required to set the strategic direction for patient safety. This includes delivering reactive and proactive aspects of patient safety improvement work including education and training, to enable and support front line staff to keep patients and staff safe.

The Deputy Patient Safety Manager will work with, and deputise for the Patient Safety Manager in delivering the Trust's patient safety portfolio. That includes patient safety incident management and the Patient Safety Incident Response Framework (PSIRF), management and oversight of duty of candour, inquest management, and management of the Central Alerting System notifications.

They will support trust-wide patient safety learning events by working closely with clinical senior leadership teams, nominated safety experts, external stakeholders and agencies to deliver successful and engaging events that promote learning and improvement.

There will be a requirement to analyse patient safety data and produce reports at a high standard for Board and the Senior Executive Team.

Overall, the role will support the delivery of the Trust's evolving patient safety agenda and includes supporting the day to day activities of the Patient Safety Team.

Main duties of the job

Deputise for the Patient Safety Manager.

Support the organisation with trust wide patient safety learning events (virtual or in person).

Participate in continuously promoting a culture of learning, patient safety and quality improvement.

Utilise clinical training and experience to influence patient safety (NMC/HCPC/GMC).

Communicate effectively with colleagues at all levels.

Produce high level reports relating to patient safety events and learning responses for trust wide quality reporting. This includes advanced use of Datix.

Manage responses for multiple ongoing safety alerts.

Coordinate and support spoke student placements within the team.

Be flexible to meet the needs and demands of the service by providing cross cover in the Clinical Governance Team when required.

Work on own initiative, organising and prioritising own and others' workloads to meet changing and often tight deadlines.

Job summary

We are pleased to announce a fantastic opportunity for a full-time Band A Dental Officer to join the Leeds Community Healthcare NHS Trust based in the Community Dental Service, for a full-time 1.0 WTE (37.5hrs) permanent post. We are a consultant-led, referral only service which provides NHS dental care for children & adults who have treatment needs which cant be met in general dental practice. You will be working with patients with complex needs, including physical, mental health, sensory, medical, or social impairment. Therefore, all our dental suites have a range of facilities to make sure they are fully accessible. Our service is proud to be a partner within the West Yorkshire Community Dental Service Collaborative, and aims to be the leading provider of dental care in Leeds for children and adults of Tier 2 complexity and above. The Dental Officer will provide dental care in our clinics located throughout the city (currently Chapeltown, Middleton & Yeadon). The post may also involve supporting in general anaesthetic theatres within Leeds Teaching Hospitals NHS Trust and domiciliary visits. Clinical facilities within our service include: surgeries equipped with ceiling track hoists, a wheelchair recliner and bariatric bench, the provision of inhalation and intravenous sedation, and access to comprehensive care under GA. Other duties will include working in partnership to provide clinical support to the full dental team. The post is available for job share or on a less than full-time basis.

Main duties of the job

The Dental Officer will be working with a supportive and compassionate team comprising dentists, dental therapists, dental nurses and administrative team members. They will be accountable to the Clinical Director, Consultant in Special Care Dentistry and Consultant in Paediatric Dentistry. They will be expected in their duties to perform dental care for both children and adults in clinics located throughout the city. Domiciliary visits and assisting in GA theatres may be required. The assessment, treatment planning, and provision of treatment for children and adults of Tier 2 complexity is expected. The service has all equipment available to facilitate appropriate patient care. Where a longitudinal, core, or mid-year dental trainee is present within CDS, the Dental Officer may be asked to support and clinically supervise the trainee. Contribution to the clinical and service development of the CDS is also a key component of this post. This will include participation in team meetings, peer review, and clinical governance and audit activities as required. An exciting opportunity has arisen for an enthusiastic and motivated practitioner to join Leeds Childrens and Young Persons Mental Health Service. The post holder will work as part of the CORE team, a dynamic and busy multidisciplinary team. CORE stands for: Children & Young People Outreach & Home treatment Risk assessment & Formulation Enhanced Support We are a citywide team from a range of backgrounds, including psychiatry, mental health and paediatric nursing, psychology and social work. We provide specialist assessment and formulation and intervention for young people up to the age of 18 and their families. We are committed to keeping the needs of the children and young people we see at the heart of what we deliver. The team actively participates in clinical and other forms of supervision. To complement our existing multidisciplinary team, we are seeking either a registered Mental Health Nurse, or induvial with relevant professional registration who has previous experience of working with Children and Young People with risk and complex mental health needs.
