We are pleased announce a new position that has become available within the Community Rehabilitation Team. As a Recovery Worker you will deliver specific components of prescribed care packages under the direct supervision of Care Coordinators and Clinical Leads within the Community Rehabilitation Team. The post holders will provide structured support and practical assistance to service users in order for them to take control over their own lives and engage actively in their own recovery process. We welcome applications from people with lived experience of mental health difficulties, and the service encourages the use of the wisdom gained through personal lived experience - all candidates must have strong values around recovery, social inclusion and diversity. The posts require you to work with service users of all ages (over 18) and to provide a flexible service over 7 days between the hours of 8am and 6pm, Monday to Friday and 9am to 5pm on weekends.
Main duties of the job
The role of the Recovery Worker is to deliver specific components of prescribed care packages under the direct supervision of Senior Rehabilitation Practitioners and Clinical Leads. The post holder will provide formalised support and assistance to service users in order for them to take control over their own lives and engage actively in their unique recovery process.
We welcome applications from people with lived experience of mental health difficulties and the service encourages the use of the wisdom gained through personal lived experience, to inspire hope in others and the belief that recovery is possible for all. Within a relationship of mutuality the peer support model can facilitate and support information sharing to promote choice, self-determination, and opportunities for the fulfilment of socially valued roles with connection to local communities.
The post holder will be part of the Community Rehabilitation Team and will work into the Level 1 and Level 2 inpatient units and in the community with service users that present with complex needs. The post requires effective liaison with service users, carers, and other professionals and agencies as required. It will involve the provision of psychological based care, offering therapeutic time to a defined client group working at all times within the principles of recovery.
The service provision is delivered 7 days per week between the hours of 8 6pm, the post holder will be required to work flexibly over these hour

ROLE DESCRIPTION
- To support the Community Rehabilitation Team and wider Complex Psychosis Pathway workforce in co-facilitating community-based groups accessed by service users across the whole service. To consult with the lead occupational therapists and/or healthy living advisors in the planning, implementation and evaluation of groups.
- To promote social inclusion for clients by facilitating and supporting access to a variety of community-based opportunities including work, social groups, education, spiritual pursuits, leisure activities to ensure holistic wellbeing.
- To participate in the delivery of care plans including attendance and contribution within the meeting and the subsequent care plan.
- To participate in Personalised Care Framework meetings as required.
To carry out the collection, delivery, and monitoring of service users medication under the Trust policy guidelines. Liaising with the team to report any concerns with clients medication and wellbeing as well as the relevant providers i.e. pharmacies.
- To promote the physical health and wellbeing for clients by facilitating and supporting access to relevant screenings and information around medication, diet, nutrition, exercise, sensible drinking and smoking cessation.
- To participate as requested, in multidisciplinary reviews and other clinical meetings to ensure effective communication and a comprehensive approach to service users care.
- Ensure that the recovery goals of the service user you support are integrated into the Trusts Personalised Care Framework process and are reviewed on a regular basis, liaising closely with rehabilitation practitioners as necessary.

The Leeds Mental Wellbeing Service (LMWS) is a city-wide service composed of two elements; NHS Talking Therapies (offering talking therapies to adult NHS patients) and a broader Primary Care Mental Health service (offering mental health and wellbeing support primarily based in primary care settings (GP surgeries, health centres etc).
The service is delivered by a partnership of Third Sector and NHS organisations, comprising Leeds Community Healthcare NHS Trust, Leeds & York Partnership Foundation NHS Trust, Northpoint Wellbeing, Inspire North, Touchstone, Womens Counselling and Therapy Service & Homestart Leeds.
LMWS employs around 260 staff, working in 3 operational Hub teams, each covering a specific geographical area of the city and will support people with common mental health problems and more complex issues.
PURPOSE OF THE JOB
The post-holder will manage and lead a team of mental health practitioners, supported by a Deputy Team Manager and a PCMH Clinical Team Manager, collectively forming the Hub Management team. The role involves supporting service delivery and staff management in a high-volume clinical service, with elements of matrix-management.
KEY RESPONSIBILITIES
1. The PCMH Operations Manager will co-lead a designated locality team in a specific geographic area of Leeds, with responsibility for ensuring a consistent Multi-Disciplinary Team (MDT) approach across their locality area.
2. They will work alongside a Deputy Team Manager, who will support the Operations Manager in working as a Hub Management Team.
3. The Operations Manager will have line-management responsibilities for staff attached to their locality team and maintain operational management oversight for Touchstone employed Primary Care Mental Health Staff.
4. To hold day-to-day responsibility for locality team delivery, by acting as the first or second point of contact for staff on operational and clinical issues, and to act as a duty manager for the whole service on a rota basis (within office hours).
5. To ensure any safeguarding or risk issues are identified quickly and that staff are supported to manage these appropriately within existing policy frameworks.

About us
Leeds Mental Wellbeing Service is a partnership of 11 providers that provides support and psychological therapies for common mental health problems to those aged 17 and over who are registered with a Leeds GP.
The Leeds Mental Wellbeing Service (LMWS) and its partners are committed to ensuring that all service provision is accessible and fairly meets the diverse needs of all people who use the service. All partners in the LMWS recognise the underlying inequalities that exist in the health system and wider society. KEY DUTIES / RESPONSIBLITIES
To ensure implementation of the services Health Equity Action Plan in collaboration with senior leadership team of LMWS.
To support, motivate, develop and coach staff who have a focus of Health Equity within their role across the service.
To ensure the service has robust arrangements in place to capture, analyse and act on equality monitoring data to inform service improvements and employment best practice.
To produce monthly dashboard reports and quarterly impact reports on equality monitoring and workforce data, suggestions recommendations and lessons learnt.
To write and present annual diversity audit reports covering workforce and service delivery.
Where needed, Chair and support working/sub/task & finish groups as directed by the leadership team.
To work closely with the Head of Health Equity, service managers and their teams to ensure consistency of approach and be one point of contact for expert advice including the delivery of briefing sessions.
To draft relevant policies, procedures and support the development of strategies as directed by the LMWS leadership team.
To draft and present performance reports to the leadership team on work related to health equity.
To work in partnership with HR and Strategic Delivery/Operational Delivery colleagues to inform and support inclusive staff recruitment, retention and development strategies.
To deliver and facilitate learning and training sessions as directed by the LMWS Leadership team.
To deliver induction seminars to the LMWS workforce outlining the Health Equity within LMWS.
