Job description
Job responsibilities
The core work of an AP in the PMHTR is delivering low-intensity psycho-educational interventions for service users who are under a community-based court order.
If you are working full time, you can expect to have approximately 15 service users on your personal caseload who will be seen weekly or fortnightly, in sessions that are held in-person, by video, or by telephone (depending on the needs and preferences of the service user and our service).
This work will primarily be CBT-based, with future development possible in adding elements of ACT, CFT, and DBT-based interventions.
In addition to the one-to-one working, we are also in the process of developing our DBT-informed skills group for Emotional Coping Skills. This is a six week protocol with a rolling admission that is currently being administered virtually.
In the future, we will seek to develop a hybrid running of this group, which you would be a part of. We are very excited about our group programme, as it is the first of its kind, and we would be particularly interested to hear from candidates who value group working.
In order to ensure that the right service users--and as many as possible--are being referred into our service, we have to work consistently to ensure that other teams in Probation and Liaison and Diversion services understand who we are and what we do.
Therefore, part of your role will be in making presentations about our work and having useful conversations with our colleagues.
We have recently added a Peer Support Worker to our team and are currently developing a Support Clinic which will provide access to the peer support worker for practical, casual support, as well as an AP for additional skills work.
For a more detailed description, please see attached JD and PS.

You will work alongside individuals, families and multidisciplinary professionals to ensure fair, robust and person-centred decision making for people with significant and complex health needs.
Whether you already have experience within NHS Continuing Healthcare or are looking to develop your career in a new specialist area, we would love to hear from you.

This advert is for NORFOLK work base.
Main duties of the job
Co-ordinate and undertake timely, comprehensive and person-centred assessments, using the appropriate national and local tools for NHS CHC and NHS-funded Nursing Care.
Gather, evaluate and triangulate clinical, functional and contextual evidence from the individual, their representative and relevant professionals, identifying gaps or inconsistencies and taking proportionate action to resolve them.
Prepare clear, accurate and evidence-based assessment reports and eligibility recommendations, demonstrating the application of professional judgement and the relationship between assessed needs, evidence and the relevant national criteria.
Participate in multidisciplinary team meetings, case conferences and decision-making processes, ensuring the individual and their representative are enabled to understand and contribute to the process.

About us
We manage an annual budget of £4.9 billion to commission safe, high quality and accessible health services for 1.7million people living in Norfolk and Suffolk.
We work with hospitals, GP practices, dentists, pharmacies, community, mental health and ambulance services, alongside local councils and voluntary organisations, to improve care. We involve local communities in shaping decisions and use their insights to improve services. As a statutory NHS organisation, we are accountable to NHS England and the UK Government.
Our role is to ensure services meet current and future needs by setting local NHS strategy, allocating funding, maintaining quality and safety standards, improving access, reducing health inequalities and enabling joined up care.
Norfolk and Suffolk include rural, coastal and deprived communities, with an older than average population. Around half of residents live in rural or coastal areas, where access can be harder, and about 219,000 people live in the most deprived areas, where early deaths are significantly higher. These factors shape our planning and focus on reducing inequalities.
Our vision is for people to live longer, healthier lives with access to safe, joined up, patient centred care. We prioritise improving healthy life expectancy, reducing inequalities and ensuring consistent access to high quality services.
We work across five local Places and partner with NHS providers, councils, the VCFSE sector and communities to deliver locally responsive care.

Whether you already have experience within NHS Continuing Healthcare or are looking to develop your career in a new specialist area, we would love to hear from you.
Please note We are one CHC team across Norfolk and Suffolk and our team cover both counties. We have a base in Norfolk (Norwich) and a base in Suffolk (Ipswich.) Please apply for the role where you would prefer your work base to be.
This advert is for SUFFOLK work base.
Complete and review person-centred, outcome-focused care and support plans for people eligible for NHS funding, ensuring that commissioned care is sufficient to meet assessed needs and that identified risks are appropriately managed.
Support individuals to understand and access Personal Health Budgets where applicable, including contributing to safe and lawful support planning and review arrangements.
About us
We manage an annual budget of £4.9 billion to commission safe, high quality and accessible health services for 1.7million people living in Norfolk and Suffolk.
