As an Advanced Nurse / Clinical Practitioner, youll be part of a dynamic, multidisciplinary team working within a telephone and digital triage environment. Youll help ensure patients receive the right care, in the right place, at the right timereducing unnecessary hospital visits and improving outcomes across the system.
Practice Plus Group is one of the UKs leading independent healthcare providers, and were proud to support the NHS with innovative services that put patients first.
Our OOH service is open from 18.30-08.00 Monday to Friday and 24 hours over the weekend. We can offer a range of sessions to allow you to flexibly choose the right shift pattern for you.

Main duties of the job
Delivering remote clinical assessments via telephone and digital platforms for patients with urgent and unscheduled care needs.
Making safe, evidence-based decisions to direct patients to appropriate services, including self-care, primary care, urgent treatment centres, or emergency services.
Applying advanced clinical knowledge to assess undifferentiated presentations and manage risk effectively.
Supporting clinical governance, contributing to service development, and mentoring colleagues.
Collaborating with GPs, paramedics, mental health professionals, and other clinicians to ensure integrated care across the region.
About us
Registered Nurse or Paramedic with MSc in Advanced Clinical Practice or equivalent
Experience within Out of Hours, Primary Care GP Surgery, or Urgent Treatment Centre
Holder of V300 / Non-Medical Prescribing qualification and minimum 6 months experience post qualification.
Ability to autonomously see and treat patients of all ages
Confident clinical decision-maker with strong assessment skills.
Excellent communicator with a patient-centred approach.
Flexible, self-motivated, and committed to ongoing professional development.
Live within a 1-hour commute to site in Ipswich

Main duties of the job
Delivering remote clinical assessments via telephone and digital platforms for patients with urgent and unscheduled care needs. About us
Fully qualified General Practitioner with GMC registration and relevant UK licence to practice.
Previous experience working in an Out of Hours setting is essential.
Proven experience using the Adastra clinical system for patient record management.
Strong clinical assessment skills and the ability to manage a diverse and often complex caseload.
Excellent communication and interpersonal skills with the ability to work both independently and as part of a team.
Commitment to providing compassionate, patient-centred care aligned with Practice Plus Groups values.
Flexibility with working hours, including evenings, weekends, and bank holidays.
Live within a 1-hour commute to site in Ipswich

The ambition is to deliver specialist, acute mental health care in a setting designed and staffed to meet the specialist environmental and clinical needs of people with a learning disability and or autism where access to mainstream inpatient services cannot be achieved through implementation of reasonable adjustments.
The Cary ward will be a centre of excellence and has been carefully designed to provide services to individuals with autism, learning disabilities and/or mental health disorders. You will be instrumental in the development, implementation and delivery of the clinical and operational model for the inpatient service.
Main duties of the job
To inspire, lead and provide clear leadership to those working on the ward, enabling service delivery of the highest quality, ensuring that people who have Mental Health concerns are provided with a quality, person centred approach to care delivery, which always considers people's safety, privacy and dignity.
To work within a multi-professional team in an inpatient setting, providing, support, care, and treatment to individuals, within guidelines and protocols identified. Managing the development and practice of a set of therapeutic care skills, making a significant contribution to service users' psychological, emotional and social wellbeing. Overseeing that the service provided enables patients to reach their optimum level of independence, take responsibility for ensuring individuals and their relatives receive care that prioritises safety, effectiveness, partnership and hope. To provide enhanced support to our service users who often have complex needs.

Main duties of the job
As a healthcare support worker the majority of your time will be spent engaging and getting know the service users on the ward. You will become able to recognise signs and symptoms of mental health illness and as a key member of the multi professional team which include Nurses, Doctors, Occupational Therapists. You will be able to support in collaborative care planning and implementation.
Although we are a mental health unit we give as much emphasis on physical health care and monitoring. The role of Health care support worker is vital in this area being able to ensure that daily well being needs and care needs are met and monitoring of clinical vital signs.
Successful applicants will also be required to undertake and complete a Prevention and Management of Violence and Aggression course. This enables staff to begin building the skills to de escalate service users in peaks of distress whilst maintaining a safe and therapeutic environment for all.
The ward operates on a 24/7 shift basis and you will be required to work shifts including evenings, nights, weekends and Bank Holidays and will qualify for unsocial enhancements in addition to your basic pay.

An exciting opportunity has arisen for a senior pharmacy assistant to join the Pharmacy Homecare Team at Epsom and St Helier University Hospitals NHS Trust.
We are looking for a proactive and dynamic individual with the requisite skills, knowledge, experience and expertise to join our existing team.
You will mainly be based in the pharmacy offices, within the pharmacy department. Main duties of the job
To be responsible for the creation of official pharmacy homecare orders on the CMM pharmacy system and for placing the orders with the agreed 3rd party homecare providers.
To reconcile homecare delivery notes against purchase orders and approve for payment, invoices and credit notes, in line with departmental policy.
To be responsible for the booking in of homecare items upon receipt of a proof of delivery (POD).
To be responsible for the dispensing of homecare prescriptions under the supervision of the Chief Technician High Cost Drugs/Homecare Services, using the Pharmacy Computer System (CMM), checking proof of delivery andreconciling dispensing records as required, in accordance with departmental policy and procedures.
To assist in the efficient and economical running of the pharmacy homecare service and the management of requests for high cost drugs.
To support the Pharmacy Homecare team by contributing to the development of the Homecare services, including the launch of the CMM homecare module.
Assist in the management of the Homecare Diabetes Service, including patient funding, ordering, invoicing, clinic/patient queries and tracking orders.
To work in the dispensary at the weekend, as part of the 7-day Pharmacy service, as per the rota as well as the Bank holiday rota and lock-up rota.

We are looking for motivated clinician to work 5 days (37.5 hours per week) in South Gloucestershire CAMHS locality team, based in Kingswood and Patchway Hubs. This role has become available as the current post holder has secured internal promotion.
You will be appropriately qualified (Mental Health Nurse, Occupational Therapist, Social Worker, Family Therapist, Accredited CBT therapist) and have appropriate professional registration. You will need to demonstrate that you have experience of holding your own caseload including risk assessment and management of complex and enduring mental health needs. Preferably from working within a community CAMHS service but may also include significant experience within other similar services such as adult mental health.
We are a supportive, experiencedand progressive multi-disciplinaryteam. We offer individual, family and group interventions. South Gloucestershire CAMHS has radically transformed in recent years including broad team skill mix and opportunities for staff development and innovative practice. Main duties of the job

We are seeking a compassionate, motivated psychology assistant to join the Wellbeing Mental Health and Psychosocial Substance Misuse Team atHMP Eastwood Park.

Deliver psychologically informed one-to-one and group interventions to support recovery and wellbeing

An exciting opportunity has arisen for an experienced Health Visitor/midwife to take on a leadership role and join the new Pre and Post Birth Multiagency Team. The team will work jointly with Children's Social Care to offer an enhanced health visiting service throughout the antenatal period until 1yr post birth. Main duties of the job

Job summary
The Research and Development Department have a 12 month fixed term opportunity for a motivated and experienced Clinical Research Practitioner to cover maternity leave.
You will be involved with the delivery of high quality mental health research across a range of studies within the Trust and the Research Delivery Network; North East North Cumbria within a busy and thriving research team that has a can do attitude. Job summary
An exciting opportunity has arisen for an experienced clinical nurse specialist in children's epilepsy to join our well established and supportive epilepsy team based at Chesterfield Royal Hospital.
The successful applicant will join the specialist nursing team in delivering the highest standards of epilepsy nursing care to children , young people and their family within the North Derbyshire area. This will be delivered through a variety of ways including outpatients clinics, inpatient contacts on the children's ward or community settings such as homes and schools.
Main duties of the job
We are looking for an individual who has a wealth of experience in working with children and young people diagnosed with epilepsy. The successful applicant will be registered children's nurse who is passionate about providing high quality epilepsy nursing care and support to children, young people and their family.
You will have the ability to work as part of the epilepsy team and autonomously where required, within the parameters of clinical guidelines/polices and scope of professional practice.
The successful applicant will work collaboratively, providing specialist epilepsy advice to other agencies including social care, education and wider health colleagues. you will be required to organise your own workload, whilst considering epilepsy team and other service demands.
The successful candidate will be a non-medical prescriber or have a willingness to work towards this within an agreed timescale.

Main duties of the job

Main duties of the job
The successful candidate will have a great opportunity to gain new skills and experience, and work in an exciting and dynamic team. They will be working with adults experiencing a range of acquired disorders of communication and/or swallowing. They will work closely with other MDT staff to ensure holistic patient care. The ideal candidate must be able to work independently as well as part of a team and have good time management and prioritisation skills. Adult Speech and Language Therapy are a respected and integrated part of the multidisciplinary team, with a positive record of developing innovative and patient centred services. The service runs weekly community videofluoroscopy and FEES clinics, and a monthly joint voice/swallowing clinic with ENT. The team pioneered a student supported Intensive Comprehensive Aphasia Programme and won a national Advancing Healthcare Award for their development of community laryngectomy care. The team is supporting clinical research projects, including the HARP aphasia study, and collaborating with local system partners to improve communication accessibility of services. CPD is actively supported through regular clinical supervision with senior speech and language therapists, appraisals and excellent training opportunities. Main duties of the job
- Attend regular MDTs and provide clinical leadership and medical input to the team.
- Responsibility for those patients seen by himself/herself, together with those patients seen by medical staff directly supervised by them.
- With other consultants in the team, have responsibility for the clinical governance of the team as clinical leads for the team.
- Work in partnership within the multidisciplinary team for the clinical management of patients. This will include risk assessment, the assessment of the needs of carers, and the development of services to meet the needs of patients and families for whom the team is responsible.
- Clinical leadership responsibility to ensure excellent systems of clinical governance so that their team provides high quality, safe care to patients. Consultants are expected to encourage a Restorative Just and Learning Culture of Learning, participate in safety huddles and investigations and participate in identifying improvements in practice where appropriate.
- Participate in the BSW Consultant on call rota (currently 1 in 11 with prospective cover, paid at category A banded at 3% on call availability supplement. There is anadditional 0.5PA per month (pro rata) paid in recognition of emergency work arising from out of hours).

CAMH Services in BaNES, Swindon and Wiltshire (BSW)
From 1st April 2010 the Trust has been providing CAMHS services in BaNES, Swindon and Wiltshire. This covers a large rural area, with Swindon being the largest town. This area of service delivery is organised around community team bases: Swindon, Marlborough (for East Wiltshire), Melksham (for West Wiltshire), Salisbury (for South Wiltshire) and Keynsham for Bath and North-East Somerset (BaNES). CAMHS has good links with partner agencies.
The BSW CAMH service provides a hospital liaison service to the three district general hospitals in the patch: Salisbury District Hospital, Salisbury, Great Western Hospital, Swindon and Royal United Hospitals (RUH), Bath and provides training to staff on the paediatric ward.
BSW also operates a dedicated eating disorder service (TEDS), with specialist teams in each of the CAMHS bases. Referrals of children and young people with suspected eating disorders are sent directly to this service.
There is a CAMHS Crisis and Home Treatment service embedded in the CAMHS teams with an over arching clinical leadership and operational management senior leadership team.
Oxford Health hosts the Thames Valley New Care Model adolescent inpatient network with a single point of access via Marlborough House, Swindon, which covers B&NES, Berkshire, Buckinghamshire, Gloucestershire, Oxfordshire, Swindon and Wiltshire. The local inpatient service for young people aged 11-18 is provided from Marlborough House, Swindon, which is a general adolescent unit that also includes an eating disorders programme. Oxford Health also runs the Highfield General Adolescent Unit at the Warneford Hospital in Oxford and the Meadow Unit (PICU).
There is also an adolescent Hospital at Home service for Eating Disorders and adolescent Hospital at Home service for young people with Learning Disability and Autism.
The BSW-wide service model has been in place since October 2010 and has improved CAMHS outcomes for children and young people in Wiltshire, Swindon and B&NES. The service was re-commissioned in 2017 on a 7 + 3 year contract.
In addition, Oxford Health has been commissioned to provide Forensic CAMHS services (FCAMHS) for the South West (North) region, covering BSW, Bristol, Gloucestershire and Somerset. The service launched in April 2018.
There are Medical Lead roles for All Age Eating Disorders, Neuro-developmental Conditions, Learning Disability, Non-Medical Prescribing and Crisis, who work alongside the Clinical Leads in these areas.

The BSW CAMH service provides a hospital liaison service to the three district general hospitals in the patch: Salisbury District Hospital, Salisbury, Great Western Hospital, Swindon and Royal United Hospitals (RUH), Bath and provides training to staff on the paediatric ward.
BSW also operates a dedicated eating disorder service (TEDS), with specialist teams in each of the CAMHS bases. Referrals of children and young people with suspected eating disorders are sent directly to this service.
There is a CAMHS Crisis and Home Treatment service embedded in the CAMHS teams with an over arching clinical leadership and operational management senior leadership team.
Oxford Health hosts the Thames Valley New Care Model adolescent inpatient network with a single point of access via Marlborough House, Swindon, which covers B&NES, Berkshire, Buckinghamshire, Gloucestershire, Oxfordshire, Swindon and Wiltshire. The local inpatient service for young people aged 11-18 is provided from Marlborough House, Swindon, which is a general adolescent unit that also includes an eating disorders programme. Oxford Health also runs the Highfield General Adolescent Unit at the Warneford Hospital in Oxford and the Meadow Unit (PICU).
There is also an adolescent Hospital at Home service for Eating Disorders and adolescent Hospital at Home service for young people with Learning Disability and Autism.
The BSW-wide service model has been in place since October 2010 and has improved CAMHS outcomes for children and young people in Wiltshire, Swindon and B&NES. The service was re-commissioned in 2017 on a 7 + 3 year contract.
In addition, Oxford Health has been commissioned to provide Forensic CAMHS services (FCAMHS) for the South West (North) region, covering BSW, Bristol, Gloucestershire and Somerset. The service launched in April 2018.
There are Medical Lead roles for All Age Eating Disorders, Neuro-developmental Conditions, Learning Disability, Non-Medical Prescribing and Crisis, who work alongside the Clinical Leads in these areas.

Responsible for ensuring the provision of effective, efficient and appropriate high quality patient care including diagnosing and treating patients presenting with minor illness, consulting and advising people in respect of their continuing medical and nursing needs and to provide care and support to patients in a community setting.
The nurse practitioner will need to prioritise and triage the needs of patients accordingly making any necessary referrals for investigations in the appropriate manner. Patients with problems needing referral to secondary care should be discussed with the registered GP before making such referral.
The nurse practitioner will support the lead NP and GP Partners with appropriate clinical administrative duties including QOF management, medication reviews, medical reports and referral management within the remit of the role.
Job Responsibilities:
Diagnosing and treating patients presenting with minor illness
- Treat patients once triaged making any necessary referrals to other members of the primary health care team.
- To support the on call doctor assessing/planning/implementing and evaluating patient care.
- To support the practice triage system by performing nurse triage where appropriate.
- Ensure clinical practice is safe and effective and remains within boundaries of competence and to acknowledge limitations.
- Advise patients on general health care and minor ailments with referral to other members of the primary and secondary health care team as necessary.
- Support GPs with medication reviews.
Administration and professional responsibilities
- Participate in the administrative and professional responsibilities of the practice team.
- Ensure accurate and legible notes of all consultations and treatments are recorded in the patients notes.
- Ensure the clinical computer system is kept up to date with accurate details recorded and amended as per practice protocol.
- Ensure appropriate items of service claims are made accurately, reporting any problems to the practice administrator.
- Ensure accurate completion of all necessary documentation associated with patient health care and registration with the practice.
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports and audit.
- Attend and participate in practice meetings as required.
- Restocking and maintenance of clinical areas and consulting rooms.
- Responsible for checking own pathologies and ensuring appropriate action takes place.
- Support GPs with medical reports/insurance reports, QOF management, referral management within own remit.

Training and Personal Development:
- Training requirements will be monitored by monthly supervision and yearly Personal Appraisal and Development Review (PADR) and will be in accordance with practice requirements. Personal development will be encouraged and supported by the practice. It is the individuals responsibility to remain up to date with recent developments.
- Participate in the education and training of junior staff and medical students of all disciplines and the induction of members of the practice staff where appropriate.
- Assist in the development and maintenance of an in house nurse-training programme reporting to the Nurse Practitioner and Practice Manager.
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for professional development ensuring PREP requirements are met.
- Attendance at the North East Lincolnshire Primary Care Programme where appropriate.
- If it is necessary to expand the role to include additional responsibilities, full training will be given.
- Develop and maintain a Personal Learning Plan.
Liaison:
- As well as the nursing team there is a need to work closely with all surgery staff to ensure the smooth running of the practice, reporting any problems encountered to the relevant person and ensuring every one is aware of the different roles within the nursing team.
- There is also the need to establish and maintain good liaison with other surgeries and agencies including secondary care.
